A Variation of Health Service Utilization in Wonju City (시군통합 원주시 내의 의료서비스 이용의 변이)

  • This study intends to examine the behavioral pattern and small area variations of health service utilization within Wonju city. We selected three small areas in Wonju city as the study site: Haksung-dong(central area of the city), Moonmak-myun (industrial area which is located 25km away from the center of the city), and Gure-myun (agricultural area which is located 32km away from the center). The data were collected by administering questionnaire interviews with 526 people in three areas. The questionnaire include the items on health service utilization behaviors, sociodemographic characteristics, health status, and perceptions. The statistical methods used for the analysis were ANOVA and hierarchical logistic regression. From the analysis, it was found that there was a variation of health service utilization by areas. Compared to those of other areas, the respondents from agricultural area showed a high probability of using health services. When respondents' personal characteristics were taken into account, the effect of dummy variables representing areas disappeared. Instead, the perceived health status became the prime factor of health service utilization. This result showed that the small area variations of health service utilization is due to the demand factor rather than the supply factor.

A Study on Married Female Immigrants' Life Style and Marriage Satisfaction in Terms of Preparing Their Old Age in Chungcheongnam-do (여성결혼이민자의 라이프스타일, 부부관계 만족도, 노후준비의식에 관한 연구: 충청남도지역 여성결혼이민자를 중심으로)

  • The purpose of the present study was to examine married female immigrants' life style and marriage satisfaction on the perception of preparing their old age. We also investigated how socio-demographic factors influence their Preparation for old age. For the study, -data were collected from 350 questionnaires obtained from female immigrants living in Chungcheongnam-do. Cronbach's ${\alpha}$ tests were conducted for reliability, and regressional analysis was conducted to test our hypotheses. The results of this study shows that there is evidence of relations among the sub-variables. However, we found attitudes to safety and material pursuit are not related to married female immigrants' preparations for their old age. On the other hand, the results indicate that their life style and marriage satisfaction influences their perception of preparing their old age in a positive way. Finally, sociodemographic factors such as gender, age, education, marriage status, occupation and income are shown to affect the women's perception of preparing their old age positively.

Maternal Factors Affecting Delivery Mode of the Previous Cesarean Delivery Mothers (선행 제왕절개분만 산모의 제왕절개 후 질식분만과 반복 제왕절개분만 관련 요인)

  • Purpose: To examine delivery type of mother who have had a previous cesarean and identify maternal factors related to type of delivery. Methods: The study sample included 60,504 mothers who had delivered through cesarean section. Related variables were categorized as sociodemographic factors (age, residence, health insurance type, income level) and clinical characteristics (14 maternal factor, 4 fetal factor and pre-term). For data analysis, $x^2$ and multivariate logistic regression were conducted. Results: Among the 60,504mothers, 3,075 were delivered through Vaginal Birth After C-Section (VBAC) and the VBAC rate was 5.1%. Underage 34, the VBAC rate increased according to age increases up to 3%. Mothers residing in urban areas had VBAC more frequently than mothers in rural area. Mothers in the high and middle income levels had a greater possibility of having VBAC than mothers in lower income levels. A greater likelihood of increase in repeated cesarean section were found in mothers with maternal and fetal factors. Conclusion: Evidence based nursing practice guidelines and education programs for previous cesarean section mothers and health policy are needed to increase VBAC.

Factors associated with the Patterns of Alcohol Use in Korean Adults (한국 성인의 음주 유형에 대한 영향요인)

  • Purpose: This secondary data analysis study was aimed at identifying the factors related to the patterns of alcohol use (normal alcohol use, hazardous alcohol use, and alcohol dependence) in the Korean adult. Methods: This study analyzed the cross-sectional data from 230,715 Korean adults aged 19 year and older who participated in the 2009 Korean Community Health Survey. Multinomial logistic regression was used to uncover associations between demographic, health related factors and the patterns of alcohol use. Results: The patterns of alcohol use was differentiated from the sociodemographic variables (residential area, gender, age, marital status, educational level, monthly income, and occupation) and with health related factors (smoking status, hours of sleep, body mass index, pain or discomfort, subjective health, stress level, liver disease, cardio/cerebro vascular disease, and depression). Hazardous alcohol use was lower in rural areas and lower among women, but higher among people who smoke, reported severe stress, and/or depression. Similarly, alcohol dependence was lower among women and among those in rural areas, and was more likely associated with those who smoke, have severe stress and depression. Conclusion: A number of factors are associated with patterns of drinking. Smoking, severe stress and depression seems to confound the problems of alcohol use. Additional research is needed to isolate those factors that are the most important influencing the use of alcohol.

Time Allocation of Farm wives:A Household Production Approach (농가주부의 시간배분연구: 가정생산이론적 접근)

  • This study examined the time allocation pattern of farm wives. In particular it addressed i)the differences in extent of time spent in farming housework child care and leisure between on-farm and off-farm seasons ii)differences in the extent of time spent in such activities among farm wives and urban wives and iii) the determinants of time spent in such activities. To achieve these goals a system of time allocation equations based on household production theory was employed. On hundred and eight farm wives wiere selected in 1993 by the methods of judgement sampling and cluster sampling. Two major questions in the survey elicited sociodemographic information and time-use of farm wives using the time-diary method, The results indicate that farm wives different decisions about how to allocate their time to farming housework child care leisure during on-farm and off-farm seasons. In addition it was that farm wives are more likely to have dual responsibilities than are urban wives. Finally in the off-farm season education number of younger children and region were significant variables in explaining the time allocation decisions of farm wives. However only age was a significant factor in their time allocation decisions during the on-farm season. This leads one to conclude that time allocation is not a choice for farm wives in the on-farm season.

The Clinical and Prognostic Significance of Command Hallucinations in Schizophrenic Patients (정신분열병 환자에서 명령환각의 임상적 및 예후적 의의)

  • Background : Patients with command hallucinations are commonly assumed to be at high risk for dangerous behavior. However the issue of whether command hallucinations hold any clinical relevance in schizophrenic patients has not been established. Method : The author analyzed the clinical and research records of schizophrenic patients with auditory hallucinations who participated in outpatient research follow-up for 9 months after discharge. Patients with auditory hallucinations were classified as experiencing or not experiencing command hallucinations based on clinical psychiatric assessment. Results : Of 63 patients with auditory hallucinations, 29(46%) reported the command hallucinations and these hallucinations often were violent in content(44.8%). Patients with command hallucinations were not significantly different from patients without command hallucinations on sociodemographic and clinical characteristics, and clinical or prognostic course variables, but patients with command hallucinations had significantly more short hospitalizations(less than one month) than patients without command hallucinations. 3 of the patients with command hallucinations who committed suicide during the follow up periods were died. Conclusion : Command hallucinations may be frequent, and in most cases they have minimal influence on the outcome of schizophrenia, but if the patients with command hallucinations have a history of suicide attempts before admission, the possibility of suicide attempts by command hallucinations should be considered.

A Study on Psychosocial Well-being and Health Promoting Lifestyle Practices of University students (대학생의 사회심리적 건강과 건강증진 생활양식 이행)

  • The purpose of this study was to find out the relationship between psychosocial well-bing and health promoting lifestyle practices of university students in Korea. The subjects were 282 students of one university in Chung-ju. The data were analyzed by the SAS program using mean, frequency, t-test, ANOVA and pearson correlation coefficient. The major results were as follows: 1. The average score for psychosocial well-being and health promoting lifestyle practices were low at 55.97, 103.5. In the subcategories of health promoting lifestyle practices, the highest degree of performance was interpersonal support(2.77), and the lowest degree was health responsibility(1.49). 2. There weren't statistically significant differences for the Psychosocial well-being according to sociodemographic variables. The performance of health promoting lifestyle practices was significant different according to gender and school year. 3. The Psychosocial well-being was negatively correlated with health promoting lifestyle practices. Also it was negatively correlated with subscale of health promoting lifestyle practices except health responsibility. So, significant correlation between psychoscial well-bing and self-actualization, exercise, nutrition, interpersonal support, stress management was found. Based upon this results, health promoting behavior will be clues for developing a interventional programs and strategies for the health promoting lifestyle practices in university students

Factors Influencing Quality of Life and Its Measurements in Patients with Hypertension: A Systematic Review (고혈압 환자의 삶의 질 영향요인 및 측정도구에 대한 체계적 문헌고찰)

  • Purpose: The purpose of the study was to comprehensively explore the factors influencing quality of life and its measurements in patients with hypertension. Methods: Based on the PRISMA statement and NECA systematic literature review guideline, a systematic literature review was conducted in this study. To search studies related to quality of life in patients with hypertension, multiple electronic databases were used using a combination of key words 'hypertension' and 'quality of life'. Throughout this process, a total of 983 studies were identified. Then, the second selection processes and quality assessment were conducted by four investigators independently. Finally, a total of 19 studies were included for the analysis. Results: Results showed that quality of life was focused on the health-related quality of life, and the SF-36 was one of the most frequently used measurements. The factors influencing quality of life were categorized into sociodemographic, health-related, health behavioral, and psychosocial variables. Conclusion: Researchers need to consider these multiple factors to promote quality of life in patients with hypertension.

  • Objectives: The purpose of this study is to analyze basic data on socio - demographic characteristics, occupational careers, and desire for employment of job seekers in their Fifties and Sixties using a portal site. Methods: For this research, secondary data were collected by the researcher from 704 job seekers and were statistically analyzed using the SPSS package. Results: The results of the analysis are as follows: Most job seekers want a job that suits their competence and career trajectory, but the reality is that as their age increases, jobs suitable for those in their fifties and sixties are hard to find; thus these job seekers accept simple labor or functional jobs. Conclusions: Differences were found in work conditions such as occupation level, job position, total career years, desired employment type, and expected salary depending on socio - demographic variables. Consequently, improvements should be made for various job searching paths for older adults as well as increasing the accessibility of job searching information.

The influence of adolescents' life satisfaction, and perceived parental rearing style on adolescents' depression: Verification of mediating effect of resilience (청소년의 삶의 만족도 및 인지된 부모양육방식이 청소년의 우울에 미치는 영향: 자아탄력성의 매개효과 검증)

  • Objectives: This study explored the effect of adolescents' sociodemographic variables, parenting rearing styles, resilience, and life satisfaction on adolescents' depression. The resilience as mediating effects of parental rearing styles and life satisfaction on depression was examined as well. Method: The data were from 2,108 middle school freshmen panel of a national sample at the forth year, Korea Children and Youth Panel Survey(KCYPS). Hierarchical multiple regressions and mediation regression analyses, steps for testing mediation by Baron and Kenny, were conducted. Results: Adolescents' resilience had both direct and mediating significant effects, showing that resilience partially mediated the effects of affective parental rearing style and life satisfaction on depression, respectively. Life satisfaction had the most significant impact on adolescents' depression. Conclusions: While some parental rearing styles were significant, adolescents' personal properties including resilience and life satisfaction revealed the most significant explanatory power of adolescents' depression. Life satisfaction was the most powerful predictor for lessening adolescents' depression.
