혈액투석환자의 수면장애에 영향을 미치는 요인 (Study on the Factors Affecting the Sleep Disorder of Hemodialysis Patients)

  • Purpose: The purpose of this study is to investigate the degree of sleep disorder and its related factors in hemodialysis patients. Method: The subjects of this study consisted of 210 hemodialysis patients. Data were analyzed by descriptive statistics, t-test, ANOVA, pearson's correlation coefficient, or stepwise multiple regression analysis by using SPSS/PC+ program. Result: Sleep disorder degree averaged 2.24. Stress of hemodialysis patients averaged 2.52. Depression degree of hemodialysis patients averaged 1.14. There was significant difference in sleep disorder according to age, marriage status, education, family income and help-givers. Sleep disorder was significantly associated with stress and depression. Stress, depression and marriage status explained 31.3% of the variance in the sleep disorder. Conclusion: In conclusion, the sleep disorder of hemodialysis patients is the nursing problem. The sleep disorder degree relates with stress, depression. Especially, it is strongly affected by stress among them.

부부갈등과 유아의 수면문제 간의 관계에서 어머니 우울의 매개효과 (The Mediating Effects of Mothers' Depression Between Marital Conflict and Preschoolers' Sleep Problems)

  • Objective: The goals of this research were (1)to examine the relationship among parental conflict, mothers' depression and preschoolers' sleep problems, and (2) to examine the mediating effects of mothers' depression between marital conflict and preschoolers' sleep problems. Methods: The participants were 1,639 mothers with preschoolers taken from the 5th year Korean Children Panel. Date were analyzed using descriptive statistics, partial correlation analysis, and multiple regression analysis. Results: The results were as follows: First, preschoolers' sleep problems were positively related to marital conflicts. Second, preschoolers' sleep problems were positively related to mothers' depression. Third, marital conflict was positively related to mothers' depression. Lastly, mothers' depression fully mediated the relationship between marital conflict and preschoolers' sleep problems. That is, marital conflict had an effect on preschoolers' sleep problem, only through mothers' depression. Conclusion/Implications: Based on the results, intervention programs should be developed to prevent preschoolers' sleep problems such as marital relationship improvement programs and mothers' depression improvement programs.

An update on the cause and treatment of sleep disturbance in children and adolescents with autism spectrum disorder

  • Autism spectrum disorder (ASD) is a neurodevelopmental disorder characterized by abnormalities in social communication/interaction and restrictive, repetitive patterns of behavior. ASD is a relatively common psychiatric disorder, with a prevalence of approximately 1.7% in children. Although many children and adolescents with ASD visit the hospital for medical help for emotional and behavioral problems such as mood instability and self-harming behavior, there are also many visits for sleep disturbances such as insomnia and sleep resistance. Sleep disturbances are likely to increase fatigue and daytime sleepiness, impaired concentration, negatively impact on daytime functioning, and pose challenges in controlling anger and aggressive behavior. Sleep disturbance in children and adolescents with ASD negatively affects the quality of life, nothing to say the quality of life of their families and school members. In this review, sleep disturbances that are common in children and adolescents with ASD and adolescents are presented. The developmental and behavioral impacts of sleep disturbances in ASD were also considered. Finally, non-pharmacological and pharmacological treatments for sleep disturbances in children and adolescents with ASD and adolescents are reviewed.

뇌졸중 후 수면-각성장애 (Sleep-Wake Disturbance in Post-stroke Patients)

  • Purpose: The purpose of the present study was to investigate the prevalence and related factors of sleep-wake disturbance (SWD) in Korean post-stroke patients. This study was the first to address post-stroke SWD in Korea using a structured questionnaire. Method: We investigated the prevalence of SWD and related factors including lesion location, stroke severity, presence of depression and fatigue. We assessed sleep-wake pattern including quality of sleep, sleep latency, wake episodes, wake time after sleep onset, daytime sleep episode and sleep time at daytime. Ninety stroke patients admitted to a university affiliated hospital in Seoul between the period September 2008 and January 2009 were included in the study. Result: Thirty five patients (38.9%) complained insomnia and 32 (35.6%) complained excessive daytime sleepiness (EDS). Quality of sleep (p=.000), sleep latency (p=.000) and total sleep time (p=.001) were significantly poorer in 16 patients with both insomnia and EDS than in the others. The related factors to insomnia were level of education (p=.030), depression (p=.007) and fatigue (p=.034), though related factors to EDS were stroke onset time (p=.049), stroke severity (p=.005), motor dysfunction (p=.035), dysphasia (p=.018), fatigue (p=.001) and lesion location (p=.019). Conclusion: Sleep-wake disturbance is a common problem in Korean stroke patients. Strategies to improve quality of sleep are urgently needed in the post-stroke patients.

  • Disorders of intitating and maintaining sleep(DIMS : insomnia) may be a symptom of some other disease or the basic, root problem in some patients. DIMS is usually treated by private physicians or psychiatrists, initially. Many physicians can do to improve the quality of life of the sleep disordered patients by appropriate evaluation and management For the effective management of DIMS patients, the accurate differential diagnosis is essential. In polysomnographic evaluation for insomnia, the physician should consider the aspects of cost-effectiveness to provide the maximal benefits for the patients. For the purpose of this, evaluation of DIMS complaints outside the sleep disorder clinic, indication for referral to a sleep disorder clinic, the role of polysomnopraphy in evaluating DIMS, and polysomnographic findings of different types of DIMS are discussed, together with possible way to diagnose them.

A Study on LED Lighting Control according to Sleep Stage using PPG Sensor of Wearable Device

  • Recently, as the sleep disorder problem of modern people deepens, the interest towards quality of sleep is increasing. To increase the quality of modern people's sleep. This paper has suggested an LED lighting control system according to the sleep stage using PPG sensors of wearable devices. The pulse of the wrist radial artery was measured using a wearable device mounted with PPG sensor, which enables heart rate-measuring, and by using the point that heart rate lowers during stable sleep than non-sleeping, the LED lighting of indoors was controlled, which is the disturbing element when sleeping. For the performance evaluation, a 10-Fold cross analysis was conducted for performance evaluation, and a result of an average accuracy 87.02% was obtained as a result. Therefore, the LED lighting control system according to the sleep stage using a wearable device of this paper is expected to contribute to raise the quality of the user's life.

수면 단계에 따른 심전도 신호의 상관관계 분석 (Correlation Analysis of Electrocardiogram Signal according to Sleep Stage)

  • There is a problem to measure neutral bio-signals during sleep because of inconvenience of attaching lots of sensors. In this study, we measured single electrocardiogram(ECG) signal and analyzed the correlation with sleep. After R-peak detection from ECG signal, we extracted 9 features from time and frequency domain of heart rate variability(HRV). Mean of HRV, RR intervals differing more than 50ms(NN50), and divided by the total number of all RR intervals(pNN50) have significant differences in each sleep stage. Specially, the mean HRV has an average of 87.8% accuracy in classifying sleep and awake status. In the future, the measurement ECG signal minimizes inconvenience of attaching sensors during sleep. Also, it can be substituted for the standard sleep measurement method.

일 대학 간호학과 학생의 수면의 질과 우울의 상관관계 (Relationship between Sleep Quality and Depression in Nursing Students)

  • 본 연구의 목적은 간호학과 학생의 수면의 질과 우울 실태를 파악하고 이들의 상관관계를 파악하기 위함이다. 구조화된 설문지를 이용하여 1개 종합대학 간호학과 재학생 268명을 임의표집하여 자료를 수집하였으며, 수집된 자료는 SPSS 19.0 프로그램을 이용하여 빈도와 백분율, 평균과 표준편차, t-test, ANOVA, Pearson's correlation coefficiency로 분석하였다. 이에 따른 연구 결과는 다음과 같다. 대상자의 수면의 질 평균 점수는 6.65점이었고, 대상자의 86.6%가 수면의 문제를 가지고 있었다. 대상자의 우울 평균 점수는 16.89점이었고, 대상자의 25.4%는 경한 우울, 20.5%는 중증 우울 상태인 것으로 나타났다. 일반적 특성에 따른 수면의 질 차이는 없었고, 우울은 학년, 학과 만족도와 학교 만족도에 따른 유의한 차이가 있었다. 수면의 질과 우울의 상관관계 분석 결과, 두 변수는 통계적으로 유의한 양의 상관관계 있었다. 본 연구 결과를 통해 상당수의 간호학과 학생들이 수면의 질과 우울의 문제를 가지고 있음을 알 수 있으며, 간호학과 학생의 신체적 및 심리적 건강 향상을 위한 중재 방안에 이러한 요인에 대한 고려가 필요하다.

한국인의 침실환경과 침실 사용자그룹 세분화에 관한 연구 (A study on sleep environment in bedroom and segmentation of sleepers in Korea)

  • This paper reports on the results of a questionnaire survey on actual conditions of sleep environment, sleep problems and sleep patterns in bedroom of house. The objectives of survey are: 1) classifying Korean people into several groups by sleep environment condition, sleep problem and life pattern, 2) describing the characteristics of user groups, and 3) searching needs of support for each user group. Despite of several studies about sleep condition that researched in advance, they weren't considered about user's respective characters. So it seems necessary for more studies about users' needs, and segment users to grasp their each detail needs. Therefore, this study conducted questionnaire survey gathering relevant information to classify user groups and to investigate the current bedroom condition. In the result, 702 respondents were divided into six clusters by extracted eight factors. Four clusters of them had some problems for sleep, so it could be seen that they need environmental solution for their good sleep. As the further objective, it will be connected to develop the good sleep system.

전립선암환자의 수면장애 (Sleep Disturbance in Prostate Cancer Patients)

  • Purpose: This study was done to investigate clinical characteristics and risk factors for sleep disturbance in patients with prostate cancer. Method: Participants were recruited from P hospital outpatient clinic from March 23 to April 20, 2006, and 101 participants completed a questionnaire assessing general and clinical characteristics, sleep quality, physical symptoms and psychological symptoms such as anxiety and depression. The data was analyzed using the SPSS 12.0 program. Results: In this study, 29.7% of the patients reported sleep disturbance. Cancer diagnosis related factors which affected sleep disturbance were onset (55.3%) and aggravation (83.3%). Habitual sleep efficiency of patients with sleep disturbance was as follows: bedtime was 10 PM, wake-up time was 6AM, sleep duration was six hours and twenty minutes. Risk factors for the presence of sleep disturbance included metastasis, the presence of intestinal symptoms, depression and anxiety. Conclusion: Sleep disturbance is a frequent problem associated with prostate cancer and seems to be influenced by aggravation of illness and the presence of physical and psychological symptoms.
