향요법 발반사마사지가 복지시설노인의 스트레스, 우울 및 수면에 미치는 효과 (Effects of Aromatherapy and Foot Reflex Massage on Stress, Depression, and Sleep Pattern of the Institutionalized Elderly)

  • The purpose of this study was to investigate the effects of aroma therapy and foot reflex massage on stress, depression, and sleep pattern of institutionalized elderly. The design of this study was non-equivalent control group pre-posttest design. The data were collected from February 24 to April 9 of 2005. Fifty-one elderly were divided into two groups, 26 institutionalized elderly for the experimental group and 25 institutionalized elderly for the control group. The experimental group underwent aromatherapy and foot reflex massage two days per week(40 minutes per session} over five weeks period. Stress(subjective stress and serum cortisol level), depression, and sleep pattern were assessed in both groups before and after intervention. Five weeks after intervention were spended, the experimental group was again assessed. Five weeks without intervention were spended, the control group was again assessed. The data were analyzed by the statistical program SPSS WIN 12. The results of this study were as follows; $\cdot$ Stress of the experimental group with the intervention was significantly lower than that of the control group. $\cdot$ Depression of the experimental group was significantly lower than that of the control group. $\cdot$ Sleep pattern of the experimental group was significantly higher than that of the control group. In conclusion, the aroma therapy and foot reflex massage program had positive effect on Institutionalized elderly's on stress(subjective stress and serum cortisol level), depression, and sleep pattern.

완골(完骨) 신문(神門) 삼음교(三陰交) 자가지압(自家指壓) 수면장애(睡眠障碍)와 피로(疲勞)에 미치는 효과(效果) (The Effects of Self-Acupressure of Wan-gol(G12), Shinmun(H7), Samumgyo(Sp6) on Clinical Nurses' Sleep pattern disturbance and Fatigue)

  • Objective : To examine the effects of self-acupressure which is usually used in reducing clinical nurses sleep pattern disturbances and fatigue. Methods : The research involved a nonequivalent control group with a pre test and post test design. The study subjects studied included 48 clinical nurses who work in C hospital located in Pusan, and they were classified into 2 groups : 25 nurses in the experimental group who took self-acupressure, and 23 nurses in the control group who did not. Self-acupressure of G12, H7 and Sp6 was administered over a period of 3 days with 1 demonstration and 4 frequencies. Results : 1. The sleep pattern disturbance score of the experimental group who took self-acupressure was significantly lower than that of the control group. (t= 7.00, p= .00) 2. The fatigue score of the experimental group who took self-acupressure was significantly lower than that of the control group. (t= 6.78, p= .00) Conclusions : Self-acupressure of G12, H7, Sp6 was effective for relieving clinical nurses sleep pattern disturbances and fatigue. The relevance of this study involved the use of Meridian self-acupressure in nursing intervention and was based on the theoretical foundations of Oriental Medicine.

이압요법이 항암화학요법을 받는 유방암 환자의 불안 및 수면에 미치는 효과 (Effects of Auricular Acupressure on Anxiety and Sleep among Patients Undergoing Chemotherapy for Breast Cancer)

  • Objectives: This study aimed to investigate the effects of auricular acupressure on anxiety and sleep among patients undergoing chemotherapy for breast cancer. Methods: A nonequivalent control group nonsynchronized design was employed. The experimental group received auricular acupressure on specific acupoints (shenmen, heart, kidney, subcortex), and the control group received auricular acupressure on helix 1, helix 2, helix 3, and helix 4 three times a day for three weeks. A total of 60 women were divided into an experimental (n=30) and control (n=30) group. Patient recruitment occurred between May and August 2019. The collected data were analyzed by a chi-square test, paired t-test, and independent t-tests using the SPSS 21.0 program. Results: There was a significant decrease in anxiety(t=4.61, p=<.001) and increase in sleep(t=3.81, p=<.001) in the experimental group compared to the control group. Conclusions: The findings confirm that auricular acupressure is an effective nursing intervention to decrease anxiety felt by patients undergoing chemotherapy and to increase the quality of their sleep.

수지요법이 노인의 견비통, 일상생활활동 및 수면에 미치는 효과 (The Effects of Hand Acupuncture.Moxibustion Therapy on Elders' Shoulder Pain, ADL/IADL and Sleep Disorders)

  • Purpose: The purpose of this study was to evaluate the effects of hand acupuncture moxibustion therapy on elders' shoulder pain, ADL/IADL and sleep disorder. Methods: This is quasi-experimental with none equivalent control group pretest posttest design. The subjects were randomly assigned to one of Experimental Group A (Ceramic Seoammoxa therapy) (n=20), Experimental Group B (Seoampellet therapy) (n=18), Experimental Group C (Ceramic Seoammoxa and Seoampellet therapy) (n=20), and a control group (n=20). The intervention was applied 3 times per week for 6 weeks. NRS, Song's ADL/IADL scale, Oh, Shin and Kim's sleep disorders scale were used. Results: Hypothesis "Both shoulder pain and the level of sleep disorders of the experimental group A, B and C would be lower than the control group" was supported (p<.001). Hypothesis "ADL/IADL of the experimental group A, B and C would be higher than the control group" was supported (p<.001). In 3 weeks after the experiment, the experimental group A, B and C showed significant difference in change of ADL/IADL (p=.013). In 6 weeks after the experiment, the experimental group A, B and C showed significant difference in change of ADL/IADL (p<.001) and sleep disorders (p<.001). Conclusion: Consequently, the hand acupuncture moxibustion was an effective therapy in relieving shoulder pain and sleep disorders and improving ADL/IADL among elders.

소아 폐쇄성 수면무호흡증후군 1례 (A Case of Obstructive Sleep Apnea Syndrome in Childhood)

  • 페쇄성 수면무호흡증후군은 다양한 원인에 의하여 발생할 수 있으며, 특히 편도 및 아데노이드 비대증은 소아에서 가장 흔한 원인이다. 편도 및 아데노이드 비대증에 의하여 발생한 폐쇄성 수면무호흡증후군은 다양한 증상과 함께 행동 장애, 야뇨증, 성장 및 발달장애, 폐성심, 고혈압과 같은 다양한 합병증을 초래할 수 있다. 이러한 이유로 임상적으로 소아에서 폐쇄성 무호흡증상이 수면 중에 관찰되면 적절한 진단과정 후에 상태에 따라서 적극적인 치료가 필요하다. 소아에서의 치료는 편도 및 아데노이드 비대증에 의한 경우 수술적인 제거를 통하여 80% 이상에서 호전을 관찰할 수 있다. 그러나 편도 및 아데노이드 제거 후에도 증상이 남아있거나 수술적인 치료가 불가능한 환아에 대해서는 체중조절, 수면자세의 변화와 같은 생활습관의 조절 및 지속적 기도양압호흡기를 이용하여 추가적인 치료를 시행할 수 있다. 저자들은 수면다원검사상 심한 폐쇄성 수면무호흡이 관찰되어 편도 및 아데노이드 절제술을 시행한 후 증상의 호전이 있었으나, 장기간 추적관찰 후 재발한 수면무호흡과 코골이를 조절하기 위하여 생활습관의 개선교육과 자동화 기도양압호흡기로 치료한 1례를 경험하였기에 보고한다.

  • Purpose: The purpose of this study is to investigate the effect of eye-shields and sleep-inducing music application on sleep quality and incidence of delirium for intensive care unit (ICU) patients. Methods: This study was a untreated control group design with pretest and posttest. The data was collected in the ICU of D University Hospital in Busan, from June to September 2011. Thirteen subjects were assigned to eye-shields and sleep-inducing music intervention and 15 subjects were in the control group. Results: Eye-shields and sleep-inducing music application was effective for improving the quality of sleep in ICU patients. There was no statistically significant difference in the incidence of delirium between the two groups. Conclusion: The sleep improvement intervention with eye-shields and sleep-inducing music would be a meaningful part of nursing intervention programs to enhance the quality of sleep in ICU patients.

Changes in Human Gene Expression After Sleep Deprivation

  • Objectives Sleep is fundamental to maintaining homeostatic control and has behavioral and psychological effects on humans. To better understand the function and pathophysiology of sleep, specific gene expressions in reference to sleep deprivation have been studied. In this study, we investigated the gene expression of peripheral blood mononuclear cells after sleep deprivation to better understand the functional consequence of sleep. Methods In eight healthy men, 24 h sleep deprivation was induced. Blood was sampled at 14:00, before and after sleep deprivation. mRNA was isolated and analyzed via microarrays. cDNAs before and after sleep deprivation were coupled to Cy3 or Cy5, respectively, and normalized cDNAs were selected with a ratio greater than two as a significant gene. Results are expressed as mean. Results Among 41174 transcripts, 38852 genes were selected as reliable, and only a small minority (< 1%) of the genes were up-or down-regulated. Total six and eleven genes were selected as significant upregulated and downregulated genes, respectively. Protein tyrosine phosphatase receptor type O was most upregulated (6.9-fold), and low-density lipoprotein receptor-related protein 5-like protein showed the most substantial inhibition (0.06-fold). Conclusions This study showed significant associations between sleep deprivation and the immune system. Acute sleep deprivation affects pathways in proinflammatory cytokines as well as metabolic pathways of glutamate and purine, neurotransmitters related to sleep and wake cycle.

Influence of general anesthesia on the postoperative sleep cycle in patients undergoing surgery and dental treatment: a scoping review on the incidence of postoperative sleep disturbance

  • General anesthesia may influence the postoperative sleep cycle; however, no clinical studies have fully evaluated whether anesthesia causes sleep disturbances during the postoperative period. In this scoping review, we explored the changes in postoperative sleep cycles during surgical procedures or dental treatment under general anesthesia. We compared and evaluated the influence of general anesthesia on sleep cycles and sleep disturbances during the postoperative period in adult and pediatric patients undergoing surgery and/or dental treatment. Literature was retrieved by searching eight public databases. Randomized clinical trials, observational studies, observational case-control studies, and cohort studies were included. Primary outcomes included the incidence of sleep, circadian cycle alterations, and/or sleep disturbances. The search strategy yielded six studies after duplicates were removed. Finally, six clinical trials with 1,044 patients were included. In conclusion, general anesthesia may cause sleep disturbances based on alterations in sleep or the circadian cycle in the postoperative period in patients scheduled for elective surgery.

자하거(紫河車) 약침(藥鐵)이 수면장애(睡眠障碍)에 미치는 효과(效果) (The Effects of the Hominis Placenta Herbal acupuncture on Sleep pattern disturbance)

  • Objectives : This study has been designed and performed to identify the effect of Hominis Placenta herbal acupuncture which is usually used in reducing sleep pattern disturbances. Methods : The study subjects studied included 48 patients who were admitted in hospital located in Pusan, and they were classified into 2 groups : 25 patients in the experimental group who injected Hominis Placenta herbal acupuncture and 23 patients in the control group who were treated by acupuncture. The both group injected on $GB_{20}$, $GB_{12}$ and $GB_{20}$ for 5 days without medicine. The sleep pattern disturbance score was measured by using 15 questions according to Korean Sleep Scale A(Oh, Jin Joo. Song, Mi. Soon. Kim, Shin Mi. 1998). Results & conclusions : The sleep pattern disturbance score of the experimental group who injected Hominis Placenta herbal acupuncture was significantly lower than that of the control group(t=7.00 p=.00). These results provided that Hominis Placenta herbal acupuncture of $GB_{20}$, $GB_{12}$ and HT, was effective for relieving sleep pattern disturbances, it is need mere sample's number and more treatment's duration.

등마사지가 위암 수술 환자의 통증 정도, 상태불안 및 수면의 질에 미치는 효과 (The Effect of Back Massage on Degree of Pain, State Anxiety and Quality of Sleep of Postoperative Patients with Gastrectomy)

  • Purpose: Most of postoperative patients experience pain, state anxiety and sleep disturbance. These problems negatively influence the recovery of postoperative patients. So alleviating these problems has been one of the nurses' central roles. The purpose of this study was to examine the effects of back massage on pain, state anxiety and quality of sleep of postoperative gastrectomy patients. Methods: A non-synchronized non-equivalent control group pre and post-test design was used. The research instruments used in this study were the Numerical Rating Scale (NRS) for pain, the State-Anxiety Inventory (STAI) for anxiety and the Verran and Synder-Halpern scale for quality of sleep. The subjects were patients admitted to a university hospital located in D city. Twenty-nine patients in the experimental group had a 10 minute manual back massage stimulation for 5 days from the 1st day to the 5th day after their operation, and 25 patients in the control group did not. Results: The degree of pain was significantly reduced according to post operation day and quality of sleep was significantly increased. However state anxiety was not significantly reduced. Conclusion: Back massage is a partially effective nursing intervention for postoperative patients with gastrectomy who experience pain and sleep disturbance.
