A Study on Nutritional Status, Biochemical Parameters, Lipid and Electrolytes Concentrations According to the Duration of Enteral Nutrition Tube-feeding (경장영양 기간에 따른 영양상태, 생화학적 지표, 지질 및 전해질 농도에 관한 연구)

  • The objective of this study was to investigate the nutritional status, biochemical parameters, lipid and electrolytes concentrations of the enteral nutrition patients according to the duration of enteral nutrition. Eighteen neurosurgery patients in the intensive care unit (ICU) at K University Hospital were subjected in this study. The duration of enteral nutrition was classified into under or over six month of period. Anthropometric, biochemical, clinical, and dietary assessments were performed. Patients' intakes of energy and protein were insufficient, from 82% to 95% of their requirements. Mid-arm muscle circumference (MAMC) and mid-am muscle area (MAMA) were significantly lower in patients over six months of enteral nutrition than those in patients under six months. The subjects were malnourished as indicated by nutrition-related parameters such as hemoglobin, albumin, total lymphocyte count (TLC), tricep skinfold thickness (TSF), mid-arm circumference (MAC), MAMC, and MAMA. Serum chloride level of the patients eve, six months of enteral nutrition was lower (94.7 $\pm$ 3.4 mmo1/1) significantly as compared to that of patients (99.3 $\pm$ 3.5 mmol/ 1) under six months. Urinary sodium and chloride levels were lower in the longer time of enteral nutrition patients than those of shorter period of enteral nutrition patients (p < .05). While serum phospholipid level was higher in the patients over six months of enteral nutrition, other blood biochemical parameters and electrolyte concentrations did not show any differences with the duration of enteral nutrition. Neurosurgery patients in the ICU undergoing long-term enteral nutrition tube-feeding were malnourished and had a variety of metabolic complications. The duration of enteral nutrition could affect the patients' nutritional status, biochemical parameters, and electrolytes balance. The patients who require nutritional support over an extended time need the continuous follow-up care and monitoring by the nutrition support team for laboratory, clinical, and nutritional assessments.

Chinical and Nutritional Examination in Examination in Obese Children and Adolescents (일부 비만아동 및 청소년에 대한 임상영양학적 조사연구)

  • The purpose of this study was to investigate the relationship between body fat and risk factors of chronic diseases in obese children and eventually to contribute to the prevention and treatment of childhood obesity. Anthropometric parameters such as height, weight, skinfold thickness, circumferences, body fat content were measured in 55 obese subjects of age 5-20. Blood pressure, serum components, daily food intakes were also considered. The average age of the subjects was 12.5$\pm$3.3yrs and the mean obesity index was 64.7$\pm$21.3%. The average percentage of body fat was 35.6$\pm$5.4% and the mean weight of lean body mass was 45.3$\pm$13.9kg. Mean total muscle weight of the subjects was estimated to be 34.11$\pm$1.3kg. The mean systolic and diastolic blood pressures of the obese were 134.8$\pm$15.8mmHg and 69.6$\pm$11.6mgHg respectively. The concentrations fo serum triglyceride, total cholersterol, HDL-cholesterol, LDL-cholesterol were estimated to be 246.0$\pm$136.4mg/dl, 257.0$\pm$54.3mg/dl, 48.2$\pm$13.1mg/dl and 158.9$\pm$53.5mg/이 respectively. The concentrations of serum GOT and GPT were 34.6$\pm$17.1U/L and 24.7$\pm$15.3U/L. The mean content of uric acid is serum was 6.2$\pm$1.9mg/dl. Of these 55 children, 82% has hyperlipidemia, 26% was abnormal liver function and 24% was hyperuricemia. One patient was diabetic. Eighty seven% of them have developed more than one complications.

1226-0983 A Study on Prevalence of Obesity, Eating Habits and Life Styles of 5th Grade Students in Inchon (인천시내 초등학교 5학년생의 비만실태와 식습관 및 생활습관에 관한 연구)

  • The purpose of this survey was to investigate the prevalence of obesity, eating habits and life styles of children. This survey was carried out anthropometry and questionnaires in 180 male and 141 female students of fifth-grade at Dongmak elementary school. The results are summarized as follows : 1)18.2% of the male and 19.3% of female students were obese. Underweight group was 10.5% of the male and 15.0% of the female students. 2) Average height of male and female students were 142.0㎝, 144.0㎝ and weight of those were 35.8㎏ and 37.1㎏ respectively. The height and skinfold thickness of the triceps in female students were significantly higher than those of male students and body fat content in female students was significantly lower than that of male students. There was no difference in BMI, RW and RI between male and female students. The average BMI, RW, RI were 17.7, 25.4, 124.0, respectively. 3) The student's height was positively related to parent's height and moth's weight. There were significantly positive correlation between mother's and subject's weight. BMI, RW and RI were positively related to parent's BMI. 4) 78.3% of male and 51.8% of female students had prejudice for special food, and 45.5% of male and 34.5% of female students had an overeating habit. Male students had significanthy higher rate than female in both cases. Average eating time per meal was 18.9 minutes for male and 21.0 minutes for female students. Experience for taking nutrient supplement in male student was more than that in female. Obese group had prejudice for special food, overeating habit and rapid eating rate more than any other groups. The prevalence of obesity among the children is gradually increasing. Therefore, in order to prevent progress into adulthood obesity, nutrition education including the right perception for obesity and eating behavior modification was needed.

  • This study was to evaluate the anthropometry, dietary attitudes and the other factors of 201 children in day-care centers according to their parents' income levels. The evaluation was conducted using anthropometric measurements such as height, weight, skinfold thickness, blood pressure, and questionnaire survey which was completed by the children's mothers. The results are summarized as follows: Overall growth of children was normal according to Korean standard. There were no significant differences in anthropometry according to the income levels. The scores of eating habits of children were different significantly according to income levels. That is, the score was the lowest in the low-income group. Children generally prefer one-dish meal or snacks, while the preference levels of kimchi, seasoned vegetable and beans broiled in soysauce were lower than those cf other foods. In the questions to know mothers' nutrition knowledge, we could see that they didn't know well about protein, breast-feeding, weaning and balance in nutrients. Nutrition knowledge score of mothers was significantly lower in the low income group, which shows close relationship with low-score eating habits of their children. The score of nutrition knowledge was affected by the mother's education level. The higher the mother's nutrition knowledge and education level, the better children's eating habits. These findings emphasize the planning and application of nutrition education programs for mothers especially in the low income group.

Nutritional Status of the Elderly Living in Cheongju - II. Anthropometric, Biochdemical and Clinical Assessment - (청주지역 노인의 영양실태조사 - II. 신체계측, 생화학적 검사 및 건강상태 조사 -)

  • To assess the nutritional and health status of the elderly, anthropometric measurements and blood test for analyzing biochemical indices were carried out from August to September in 1996. Data on the incidence of a specific disease and clinical symptoms were also collected by interviews with a questionnaire. Among the total of 169 free-living elderly interviewed(91 men, 78 women), 86 elderly(57 men, 29 women) participated in the anthropometric measurements, and 73 elderly(52 men, 21 women) in blood tests. Average heights and weights were much lower than the standards established in the Korean Recommended Dietary Allowances. While average triceps skinfold thickness of men and women were above the 50 percentile of the reference data, mid-arm muscle circumferences were lower than the 50 percentile. Prevalences of anemia, assessed by hemoglobin using WHO definition, were 38.5% for men and 33.3% for women. Blood levels of total protein and albumin were above the normal limit for all participants. Mean blood cholesterol levels of men and women were 163.9mg/dl and 185.8mg/dl, and triglyceride levels were 138.2mg/dl and 161.9mg/dl, respectively. Women older than 75 years generally had high levels of BMI, total cholesterol and triglyceride. Health status assessed by clinical symptoms generally showed no significant difference by age, and their smoking, and drinking habits. Males and the elderly who exercised regularly had better health status. It was suggested that exercise was a good way to improve health status. (Korean J Community Nutrition 2(4) : 568-577, 1997)

Determining Obesity Frequency of Rural Children by Skinfold Thickness, Analyzing Their KAP Related ti Obesity and Obesity Camp Strategy (농촌아동의 피부두겹두께 의한 비만도, 이에 따른 비만관련 KAP 비교 및 비만캠프 전략)

  • A survey was conducted on 222 students in two elementary schools in rural areas, one was a rural small city and the other was in the countryside. By measuring their skin-foldness in 4 spots by caliper, the body fatness of 5 grades(BFGr) of the subjects was resulted as follows ; 28.0% of boys and 6.7% of girls were classified as obese the and 34.1% of boys and 31.1% of girls as overweight. Comparing by region, children in the small city seemed to be fatter than the ones in the countryside(21.7% vs. 17.2% as obesity). Comparing by gender, boys were fatter than girls(28.0% vs. 6.7% as obesity). When recategorizing body fatness from 5 grades(BFGr) into 3 groups(BFGp) as overweight group(obesity+overweight), normal and underweight group(underweight+severe-underweight), we observed no significant difference in childrens dietary habits and food intake frequencies by BFGp. Overweight and underweight groups enjoyed snacking before dinner more frequently than the normal group(42.1-59.3% vs. 31.5%). The subjects showed poor nutrition knowledge score with relatively high attitude score about weight control and dietary behavior score was a little lower than the attitude score. Nutrition education programs, like camps, should stress on childrens nutrition knowledge. BFGp related to obesity attitude positive, a very low level and the latter related to diet, exercise and fasting attitudes at positive levels. Therefore overweight children seemed to be motivated easily. Also the higher the in exercise-value score, the more increased was shown in exercise attitude at positive, very low level. Emphasizing exercise-value in camp programs might make childrens exercise attitude more positive. As strategies for a rural obesity camp program are being developed by obese and non-obese children. The camp would be held over summer vacation for 3 days(2 nights) at the participants expence not exceeding 50,000 won. Contents of the program cover nutrition knowledge, and exercise-value as well as cooking lessons, exercise practice, and self-esteem enhencement.

Eating behavior and life habits of kindergarten children in Chuncheon area (춘천지역 일부 유치원 원아의 식습관 및 생활습관에 관한 조사)

  • The purpose of this study was to investigate the eating behavior and life habits of kindergarten children 5 to 7 years old in Chuncheon area. This study was conducted in December, 1996. Anthropometric indices(height, weight, skinfold thickness, mid-arm circumference) were measured and body mass index(BMI), Rohrer index, arm fat area were calculated to estimate children’s body fatness. Also the eating behavior and life habits were determined tv using questionnaire method. The results were as follows: Body fat rate of the children was almost normal according to Rohrer index and BMI. There was significant difference in birth weight between male and female. The problems of the eating behavior were frequent skipping breakfast and eating-out. The rate of the children who took snacks 2 times a day was 46.9%. Most children tended to take sweet snacks such as ice-cream, chocolates, cookies, and they tended to prefer meats and fishes to vegetables. The rate of breast feeding was 28.3% and the rate of weaning foods prepared at home was 61.1%. Many children preferred indoor activities such as watching TV, toy furniture play, building block to outdoor activities such as roller-skating, cycling, jogging in their spare time. The rate of the children who took exercise regularly was 15%. Children tended to eat kimbap, fried chicken, ramyeon more frequently in various instant foods. The intake rates of fruit juice and barly tea were relatively high and children's favorite fruits were watermelon and mandarin orange. From this results, skipping breakfast, frequent eating-out and the monotony of food preference appears to cause imbalance in nutrient intakes of the children and to have bad influence on children’s health. Accodgingiy, the comprehensive nutrition education and proper modification program should be needed for the children and their parents to correct the eating behavior and life habits. The reasonable exercise programs are needed to motivate outdoor activities of the children, and the importance of breast feeding and weaning foods should be emphasized to lactating women and pregnant women.

Comparative Analysis and Evaluation of Health and Nutritional Status of Male Industrial Workers in Korea (사무직과 생산직 근로자의 영양실태 비교 및 건강 관련 요인 분석)

  • This study was carried out to collect information to establish a framework for meal management and nutritional service for prevention of chronic degenerative disease in the industrial workers. We investigated the health concerned life-style, nutrient intakes, anthropometry and biochemical parameters in the male workers in Korea by work condition. Anthropometric parameters of height, weight, waist, hip and triceps skinfold thickness were measured and biochemical parameters including Urine pH, hemoglobin, blood glucose, total cholesterol, GOT, GPT, $\gamma$-GPT and blood pressure were determined for 101 subjects(50 office workers : 33.9yr, 51 laborers : 34.4yr). To assess the nutrient intakes and diet quality of workers, dietary intake was measured by one day 24-hour recall method. Result of anthropometric parameters of height(p<0.01), weight(p<0.05), hip(p<0.05), TSF(p<0.01) were significantly higher in office workers than in laborers. Biochemical parameters were not significantly difference in both of them. Average daily nutrient intakes of both groups was higher than the Korean RDA and report on 2001 National Health & Nutrition Survey. The calcium(p<0.01), iron(p<0.05), vitamin A(p<0.01), vitamin B1(p<0.01) and niacin(p<0.05) intakes in laborers were significantly higher than in office workers. Nutrition adequacy ratio(NAR), mean adequacy ratio(MAR) and index of nutritional quality(INQ) of laborers were higher than those of office workers. This results indicated that the onset possibility of chronic degenerative disease appeared higher in office workers. Therefore, meal menagement and nutritional service for prevention of chronic degenerative disease of industrial works needs to be more variant guidelines.

The Effect of Education Program on Weight Control in Taeumin Childhood Obesity (태음인 비만아를 위한 체중조절 프로그램의 실시효과)

  • Objective; Childhood obesity has become one of the most common health problems facing children in the world. Although a number of regimens have been developed and distributed for the treatment of adult obesity, few studies have focused on therapeutic programs for obese children. The objective of this study was to develop and measure the effects of Taeumin childhood obesity by a sasang constitution­based education program. Methods ; A total of 14$(\%RBW\geq20)$ among 18 taeumin with childhood obesity from June to August 2003 participated in the constitution-based education program. It was designed to examine the effects of a constitution-based education program through anthropometric measurements, body composition indicates, biochemical indicates and serum level of taeumin childhood obesity. Results; The results of this study were as follows: 1. The anthropometric measurement of subscapular skinfold thickness was reduced after 4 weeks of the constitution-based education program. 2. The body composition indicates reduction in degree of obesity, percent body fat, and fat distribution. The other side increased in height, total body water, soft lean mass, and lean body mass. 3. For biochemical indicates, HDL-cholesterol was increased significantly after 4 weeks of the constitution-based education program. The other side decreased in AST and ALT. 4. There was a significant reduction in BMI and $\%RBW$ after the constitution-based education program. Conclusion : A constitution-based education program for weight control results in reduced degree of obesity among taeumin children.

The relationship between % Body Fat and Blood Pressure, Total Cholesterol, and Serum Lipoprotein Ratios in College Students (대학생의 체지방 수준과 혈압, 총콜레스테롤 및 혈청 지단백비율과의 관계)

  • The purpose of this study was to develop cardiovascular health related percent body-fat standards that may be applied to epidemiologic investigations of the prevalence and incidence of obesity in adolescents, pediatric health screenings, and youth fitness tests. The subjects included 102 males and 80 females aged 19~22years. All subject were Honam University Students Total body fat was derived from body density which was estimated from age and the triceps and subscapular skinfold thickness measured with Lang calipers to the nearst l.0mm. Serum total cholesterol and lipoprotein cholesterol fraction(HDL-CLDL-C) were measured from blood obtained from fore arm vein after blood pressure measurement. In analyses to determine critical fat levels associated with elevated CDD(Chronic Degenerative Disease) risk factors;male and female were grouped by level of percent fat as follows: male, 〈 10%, 10-14.9%, 15-19.9%, 20-24.9%, and $\geq$ 25%;female, 〈 20%, 20-24.9%, 25-29.9%, 30-34.9%, and $\geq$ 35%. As the results of the data, the conclusions were as follows: 1. A dose respones effect was observed between blood pressure and percent body fat in males and females; in contrast, total cholesterol and lipoprotein ratios were relatively independant of percent fat among the lower four fatness group in males and the lower three fatness groups in females. 2. The percentage of subjects in the uppermost quintile for S-Bp, D-Bp, TC, LDL/H was significantly(P〈.05) greater than expected by change alone(20%) in males with $\geq$ 25% fat and in females with $\geq$ 30% fat females with $\geq$35% had even greater representation in the uppermost quintile of all CDD risk factors compared to females with 30-34.9% fat.
