ST714-SCCmec type IV CA-MRSA에 의한 피부 연부조직 감염증으로 내원한 소아 증례 (ST714-SCCmec type IV CA-MRSA isolated from a Child with Recurrent Skin and Soft Tissue Infections in South Korea: A Case Report)

  • 최근 전세계적으로 지역사회 기원 메티실린내성 황색포도알균(CA-MRSA)에 의한 피부연부조직 감염증이 증가하고 있다. 16개월의 한국 여아가 생후 6개월부터 시작된 반복되는 피부연부조직 감염증을 주소로 내원하였다. 환아의 가족들도 환아와 비슷한 시기에 피부연부조직 감염증의 병력이 있었다. 환아의 혈중 면역글로불린 및 NBT 검사는 정상이었고, 피부 병변 외에는 가족 모두 건강하였다. 환아의 감염증 부위의 고름 배양 및 비강 내 보균 검사시 모두 PVL genes을 생성하는 CA-MRSA ST714 SCCmec type IV가 확인되었고, 이는 ST30의 single locus variant로서 국내에서는 보고된 적이 없는 MRSA의 유전형이다. 환아와 함께, 환아의 균주와 동일 항생제 감수성 양상을 보이는 MRSA 보균자인 환아부에게 재발 방지를 위해서 비강내 mupirocin 5일 요법을 시행하였고 이후 10개월간 추가적인 재발은 없었다.

소아 피부 및 연조직 감염 환자에서의 황색포도알균의 항생제 감수성의 변화: 2010년부터 2018년까지 단일기관에서의 경험 (Changing Susceptibility of Staphylococcus aureus in Children with Skin and Soft Tissue Infections: a Single Center Experience from 2010 to 2018)

  • 목적: 피부 및 연조직 감염(skin and soft tissue infections [SSTIs])의 가장 흔한 원인은 황색포도알균(Staphylococcus aureus)이다. 본 연구는 19세 미만의 SSTI 환자에서 분리된 황색포도알균의 항생제 감수성의 변화추이를 알아보고자 하였다. 방법: 황색포도알균이 분리된 소아청소년 지역사회관련 SSTI 환자에서 후향적 관찰연구를 시행하였다. 미생물학 및 인구학적 자료를 수집하고, 항생제 감수성의 변화추이를 관찰하였다. 결과: 2010년 1월부터 2018년 12월까지 총 807개의 지역사회관련 황색포도알균이 연구에 포함되었다. Oxacillin에 대한 감수성은 전반적으로 증가하여(P<0.001) 2018년에는 75.0%였다. Trimethoprim/sulfamethoxazole과 tetracycline에 대한 감수성은 매우 높게 유지되어 2018년 각각 97.6%와 95.2%였다. 만 1-5세 연령군에서 만 6-18세 연령군에 비해 oxacillin 감수성이 유의하게 낮았다(53.4% vs. 75.0%, P<0.001). 결론: 소아청소년의 지역사회관련 SSTI의 원인이 된 황색포도알균 중 메티실린내성 황색포도알균의 비율이 감소하고 있는 것으로 보인다. 임상의는 경험적 항생제를 선택할 때 지역적 항생제 감수성 양상을 확인해야 한다.

Allosteric Probe-Based Colorimetric Assay for Direct Identification and Sensitive Analysis of Methicillin Resistance of Staphylococcus aureus

  • The accurate and rapid detection of methicillin-resistance of Staphylococcus aureus (SA) holds significant clinical importance. However, the methicillin-resistance detection strategies commonly require complicated cell lysis and gene extraction. Herein, we devised a novel colorimetric approach for the sensitive and accurate identification of methicillin-resistance of SA by combining allosteric probe-based target recognition with self-primer elongation-based target recycling. The PBP2a aptamer in the allosteric probe successfully identified the target MRSA, leading to the initiation of self-primer elongation based-cascade signal amplification. The peroxidase-like hemin/G-quadruplex undergo an isothermal autonomous process that effectively catalyzes the oxidation of ABTS2- and produces a distinct blue color, enabling the visual identification of MRSA at low concentrations. The method offers a shorter duration for bacteria cultivation compared to traditional susceptibility testing methods, as well as simplified manual procedures for gene analysis. The overall amplification time for this test is 60 min, and it has a detection limit of 3 CFU/ml. In addition, the approach has exceptional selectivity and reproducibility, demonstrating commendable performance when tested with real samples. Due to its advantages, this colorimetric assay exhibits considerable potential for integration into a sensor kit, thereby offering a viable and convenient alternative for the prompt and on-site detection of MRSA in patients with skin and soft tissue infections.

다약제내성 균주 감염에 대한 Tigecycline의 치료 (Tigecycline Treatment for Infections Caused by Multidrug-Resistant Pathogens)

  • Background: Tigecycline (TIG), a new broad-spectrum glycylcycline with anti-multidrug-resistant-(MDR)-pathogen activity, was launched in March 2009 in South Korea, but there are insufficient clinical studies on its use in the country. As such, this study was performed to analyze cases of severe MDR-pathogen-caused infections treated with TIG. Methods: Patients treated with TIG within the period from May 2009 to June 2010 were enrolled in this study. Their clinical and microbiologic data were reviewed retrospectively. Results: Twenty-one patients were treated with TIG for complicated skin and soft-tissue infections (cSSTIs) (42.9%), complicated intra-abdominal infections (cIAIs) (38.1%), or pneumonia (19.1%) caused by MDR pathogens like carbapenem-resistant $Acinetobacter$ $baumannii$ (76.2%), methicillin-resistant $Staphylococcus$ $aureus$ (61.9%), extended-spectrum beta-lactamase-producing $Escherichia$ $coli$ and $Klebsiella$ $pneumoniae$ (38.1%), and penicillin-resistant $Enterococcus$ species (33.3%). Thirteen patients (61.9%) had successful clinical outcomes while five (23.8%) died within 30 days. The rate of clinical success was highest in cSSTI (77.8%), followed by cIAI (50%) and pneumonia (50%), and the mortality rate was highest in pneumonia (50%), followed by cIAI (25%) and cSSTI (11.1%), Conclusion: Tigecycline therapy can be an option for the treatment of severe MDR-pathogen-caused infections in South Korea, Due to its high risk of failure and mortality, however, prudence is required in its clinical use for the treatment of severe infections like nosocomial pneumonia.

황흑산(黃黑散)의 메티실린 내성 황색포도상구균에 대한 항균 활성과 옥사실린과의 시너지 효과 (The antimicrobial activity of Hwangheuk-san and synergy effect with oxacillin against methicillin-resistant Staphylococcus aureus)

  • Objectives : Hwangheuk-san is a complex prescription composed of oriental traditional medicine and has been reported for antioxidant, antimicrobial and anticancer effects in the recent study. Methicillin-resistant Staphylococcus aureus (MRSA) is one of important causes of fatal infectious diseases such as septicemia, endocarditis, toxic shock syndrome, pneumonia, skin and soft tissue infections (SSTIs). S. aureus is reported as being for a variety of human diseases and its epidemiological relevance is mainly due to their ability of becoming highly resistant to common antimicrobials such as tetracycline, penicillin, cphalosporin and aminoglycoside. The objective of this study is to determine the antimicrobial effect of Hwangheuk-san ethanol extracts (HHS) and synergistic effects with antibiotics oxacillin against MRSA.Methods : The antimicrobial activity of HHS was measured by the disc diffusion method, broth microdilution method and the checkerboard dilution test, time-kill curve assay was performed to investigate synergistic effects with antibiotics oxacillin against MRSA.Results : HHS showed antimicrobial activity against MRSA with a MIC value of 125 ㎍/㎖. In the checkerboard test, the interaction of HHS with antibiotics oxacillin produced almost synergy or partial synergy against MRSA. This study showed that HHS reduced the MICs of oxacillin tested, and a remarkable antibacterial effect of HHS, with membrane permeability enhancers.Conclusions : These results suggest that HHS has the antimicrobial effect and synergistic effects with antibiotics oxacillin against MRSA. This study thus can be a valuable source for the development of a new drug with low MRSA resistance.
