Comparison of one-jaw and two-jaw orthognathic surgery in patients with skeletal Class III malocclusion using data from 10 multi-centers in Korea: Part I. Demographic and skeletodental characteristics

  • Objective: To investigate demographic and skeletodental characteristics of one-jaw (1J-OGS) and two-jaw orthognathic surgery (2J-OGS) in patients with skeletal Class III malocclusion. Methods: 750 skeletal Class III patients who underwent OGS at 10 university hospitals in Korea between 2015 and 2019 were investigated; after dividing them into the 1J-OGS (n = 186) and 2J-OGS groups (n = 564), demographic and skeletodental characteristics were statistically analyzed. Results: 2J-OGS was more frequently performed than 1J-OGS (75.2 vs. 24.8%), despite regional differences (capital area vs. provinces, 86.6 vs. 30.7%, p < 0.001). Males outnumbered females, and their mean operation age was older in both groups. Regarding dental patterns, the most frequent maxillary arch length discrepancy (ALD) was crowding in the 1J-OGS group (52.7%, p < 0.001) and spacing in the 2J-OGS group (40.4%, p < 0.001). However, the distribution of skeletal pattern was not significantly different between the two groups (all p > 0.05). The most prevalent skeletal patterns in both groups were hyper-divergent pattern (50.0 and 54.4%, respectively) and left-side chin point deviation (both 49.5%). Maxillary spacing (odds ratio [OR], 3.645; p < 0.001) increased the probability of 2J-OGS, while maxillary crowding (OR, 0.672; p < 0.05) and normo-divergent pattern (OR, 0.615; p < 0.05) decreased the probability of 2J-OGS. Conclusions: In both groups, males outnumbered females, and their mean operation age was older. The most frequent ALD was crowding in the 1J-OGS group, and spacing in the 2J-OGS group, while skeletal characteristics were not significantly different between the two groups.

군집분석을 통한 한국인 성인 골격성 제 II급 부정교합의 아분류 (Subclassification of Skeletal Class II Malocclusion of Korean Adults Using Cluster Analysis)

  • Skeletal malocclusion is the result of abnormal dimension and alignment of each skeletal component. Understanding on these mechanisms may help to elucidate the etiology of skeletal malocclusion and to establish population-oriented treatment plans. Attempts to subdivide the Angle's classification have been performed for Class III malocclusion, while few studies have been conducted for Class II malocclusion despite recent growing interests in Class II malocclusion. 200 adults (88 male, 112 female) with skeletal Class II malocclusion were collected and subdivided using cluster analysis, using the measurements representing the dimension and the alignment of each facial skeletal component. The properties of each cluster was grouped within the subjects and a comparison between the subjects and the control group (38 male, 35 female) with normal occlusion was performed. Six clusters were finally recognized in each male and female groups. The clusters in both genders were mainly characterized by the cranial base alignment, dimension of the posterior cranial base, dimension of the mandibular ramus and the degree of mandibular rotation. The results implicate that active treatment of mandible rather than the nasomaxillary complex may be primarily considered for the correction of Korean Class II skeletal pattern.

골격성 3급 부정교합자시 악교정 수술후 골격이동량에 따른 설골의 위치와 상기도 변화에 관한 연구 (A study on relation of position of hyoidbone and upper airway dimensional change according to chin movement in persons with skeletal class III facial pattern after orthognathic surgery)

  • The goal of this study is the comparison of upper airway size and change of skeletal Class I group and skeletal Class III group (before operation, within 2 weeks after operation, 6 months after operation) respectively. At first, we measured the lines between selected upper air way landmarks on lateral cephalometric x-ray film of skeletal Class I 40 persons whoes age were 23-26 years old, ,and did the same lines of landmarks of skeletal Class III 44 persons who had not been operated yet, were within 2 weeks after operation, were 6 months after operation. And we compared it respectively and analyzed it with paired t-test. We studied the relationship of those on produced data. 1. Skeletal Class III group was narrower in nasopharyngeal air way space than that of skeletal Class I group, and increased in thickness of oropharyngeal, hypopharyngeal wall within 2 weeks after operation, and reduced in nasopharyngeal, oropharyngeal air way space, and did in thickness of nasopharngeal, hypopharyngeal wall 6 months after operation. 2. Skeletal Class III group reduced in nasopharyngeal, oropharyngeal air way space, and increased in thickness of nasopharyngeal, oropharyngeal, hypopharyngeal wall within 2 weeks after operation, restored the thickness of nasopharyngeal, oropharyngeal wall, but did not restored nasopharyngeal, oropharyngeal, hypopharyngeal air way space. 3. Vertical length from hyoid bone to mandibular plane did not have signifacant difference from Class I group but after operation, it increased more than Class I group significantly. 4. The size of airway reduced after operation. Among this, oropharyngeal airway most reduced.

유치열기 3급 부정교합 환아에서 facemask의 효과와 재발 양상 (THE EFFECT AND RELAPSE PATTERN OF FACEMASK THERAPY FOR CLASS III MALOCCLUSION CHILDREN)

  • 본 연구의 목적은 유치열기 3급 부정 교합 환아에서 facemask를 이용한 악정형 치료 후 골격적 변화와 치료 후 나타나는 재발 양상을 평가하는 것이다. 유치열기 3급 부정교합 환아 15명을 대상으로 구내장치로 bonded expander, 구외장치로 facemask를 이용한 악정형 치료를 평균 12 개월 동안 시행하였으며, 1년 간의 follow-up 기간 동안 유지 장치는 사용되지 않았다. 치료 시작 전, 치료 직후, 치료 1년 후에 측면두부방사선사진을 촬영하고 전후방 및 수직적 골격관계와 연조직의 변화를 비교분석하였다. 모든 환아에서 치료 직후, 유의할 만한 골격적 전후방 관계의 변화를 보였고 1년 간의 follow-up 기간 동안 재발되는 경향을 보였으나 치료 시작 전과 비교하여 치료 효과는 유지되었다. 수직적 골격적 변화는 치료 직후 증가되었으나 1년 간의 follow-up 기간 동안 다시 감소하여 치료 시작 전과 비교하여 차이를 보이지 않았다. 연조직의 변화는 facial convexity 및 상순의 위치가 치료 직후 개선됨을 보였고 1년 간의 follow-up 기간 동안에도 치료 전과 비교하여 치료효과는 유지되었다. 하순의 위치는 치료 직후에 유의할 만한 변화를 보이지 않았다. Facemask는 유치열기 3급 부정교합에 있어서 효과적인 치료 방법이며, 안정적인 치료 결과를 위해서는 적절한 형태의 유지장치가 고려되는 것이 바람직하다.

Cone-beam computed tomography based evaluation of rotational patterns of dentofacial structures in skeletal Class III deformity with mandibular asymmetry

  • Objective: The purpose of this study was to assess rotational patterns of dentofacial structures according to different vertical skeletal patterns by cone-beam computed tomography (CBCT) and analyze their influence on menton deviation in skeletal Class III deformity with mandibular asymmetry. Methods: The control group consisted of 30 young adults (15 men, 15 women) without any severe skeletal deformity. The asymmetry group included 55 adults (28 men, 27 women) with skeletal Class III deformity and at least 3-mm menton deviation from the midsagittal plane; it was divided into the hyperdivergent and hypodivergent subgroups using a mandibular plane angle cutoff of $35^{\circ}$. Fourteen rotational variables of the dental arches and mandible were measured and compared among the groups. Correlations between menton deviation and the other variables were evaluated. Results: The asymmetry group showed significantly larger measurements of roll and yaw in the mandible than the control group. The hypodivergent subgroup showed significant differences in maxillary posterior measurements of yaw (p < 0.01) and maxillary anterior shift (p < 0.05) compared with the hyperdivergent subgroup. All the mandibular measurements had significant correlations with menton deviation (p < 0.01). Most measurements of roll were positively correlated with one another (p < 0.01). Measurements of yaw and roll in the posterior regions were also positively correlated (p < 0.05). Conclusions: Menton deviation in skeletal Class III deformity with mandibular asymmetry is influenced by rotation of mandibular posterior dentofacial structures. The rotational patterns vary slightly according to the vertical skeletal pattern.

Effects of exhaustive exercise on ER Stress of skeletal muscle and adipose tissue in rats

  • The purpose of this study was to identify the effects of gene expression of endoplasmic reticulum (ER) stress in skeletal muscle and adipose tissue on acute exhaustive exercise. Thirty-five Sparague Dawley male rats were divided into a control group (CON, n = 7) and a exhaustive exercise group (n = 28), immediately after exhaustive exercise group (n = 7), after 30 minutes exhaustive exercise group (n = 7), after 60 minutes exhaustive exercise group (n = 7), after 180 minutes exhaustive exercise group (n = 7). As a result, changes in the composition of the blood serum triglyceride concentration increased significantly in immediately after exhaustive exercise group, On the contrary, blood glucose showed a significantly decreased (p < .05). Homeostasis of energy metabolism due to exhaustive exercise as a result of the mechanism of action of skeletal muscle in the glycogenolysis and absorption, which indicates that the process of means. On the other hand, a result of examining changes in endoplasmic reticulum stress-related proteins in skeletal muscle and adipose tissue, JNK1 except in skeletal muscle BiP, ATF4, CHOP, GRP78 mRNA increased significantly immediately after exercise, and after 30 minutes returned to normal levels that could be confirmed (p < .05). BiP mRNA in adipose tissue show a similar pattern and skeletal muscle increased significantly immediately after exercise, but other changes in the specificity of the endoplasmic reticulum stress-related proteins also did not appear. In conclusion, Exercise applies and exercise training duration and exercise intensity as well as research on the interaction of the endoplasmic reticulum stress-related genes should be study continuously, to be more clear.

하악 전돌 환자의 하악지 시상분할 골절단술 후의 골격성 회귀 양상 (SKELETAL RELAPSE PATTERN AFTER SAGITTAL SPLIT RAMUS OSTEOTOMY OF MANDIBULAR PROGNATHIC PATIENT.)

  • The purpose of this study was to evaluate the skeletal relapse pattern of the mandibular prognathic patients after mandibular set back surgery by sagittal split ramus osteotomy. The horizontal and vertical position of the cephalometric points were measured before, after surgery and after one-year follow up period. The next, the positional change of the proximal and distal mandibular segment were evaluated respectively. The obtained results were as follows; 1. The horizontal and vertical position of Cd was not changed before and after surgery, and it was maintained its original position during the observation periods. 2. As the mandibular prognathism of the patients was severe before surgery, the more skeletal relapse tendency was observed during follow-up period(p<0.05). 3. As the horizontal positional change of the mandible which was obtained by mandibular set-back surgery was large, the more horizontal relapse tendency was observed during follow-up period(p<0.05). 4. The corpus axis angle decreased by sagittal split ramus osteotomy(p<0.01), but it was kept its reoriented position during follow-up period. 5. During the follow-up period after mandibular set-back by sagittal split ramus osteotomy, the forward relapse of mandible correlated with not only the forward rotation of the proximal segment but also the forward movement of the distal segment(p<0.05).

성인 환자에서 구치부 압하를 통한 개방교합의 치료와 유지 (Non-surgical treatment and retention of open bite in adult patients with orthodontic mini-implants)

  • 성장이 종료된 치성 또는 골격성 개방교합 환자를 치료하는 것은 매우 어려운 일이다. 전치부 개방교합 환자는 증가된 전안면고경, 상악 구치부의 과도한 수직성장, 큰 하악각 등의 특징을 지닌다. 이러한 증례에서 구치의 압하를 통한 개방교합의 해소는 좋은 치료전략이다. 본 연구는 교정용 미니임플란트를 이용하여 개방교합을 치료하고 성공적인 유지를 얻은 두 명의 환자에 대한 보고이다. 적절한 진단이 시행되고 교정용 미니임플란트 등의 장치를 이용한 치료기법을 적용하면 수술치료 없이 교정치료만으로 개방교합의 치료가 가능하며, 하악구치부에 posterior bite block을 추가한 가철식 유지장치 및 SFR이 개교환자를 위한 유지장치 또는 보조유지장치로 유용하게 사용될 수 있다는 결론을 얻었다.

골격고정원을 이용한 Pendulum 장치가 두개 안면의 수직적 요소에 미치는 효과 분석 (Analysis of effects from usage of skeletal anchorage-assisted Pendulum appliance on vertical component of craniofacial structure)

  • 목적: 골격성 고정원의 사용으로 고정원이 보강된 Pendulum 장치가 환자의 두개 안면 요소의 수직적인 부분에 미치는 영향을 분석하고자 하였다. 연구 재료 및 방법: 골격성 고정원과, pendulum을 이용하여 비발치 치료를 시행한 환자 중, 발산형 안모(SN-MP > $37^{\circ}$)와 중간형 안모($29^{\circ}$ < SN-MP < $37^{\circ}$) 각각 10명씩 분류하여 치료의 단기적 효과에 대해 분석하였다. 결과 및 결론: 골격 형태에 따른 차이로 발산형 안모군(HG)은 구치부 후방이동과 관련하여 하악 전치의 순측 경사가 유의하게 나타났다(P < 0.05). 중간형 안모군(MG)은 구치부의 유의한 이동만 나타났다(P < 0.05). 전치의 순측 이동은 유의성이 없었다. 본 논문에서 Pendulum 장치는 수직적인 안모 형태에 유의한 영향을 주지 않으며, 악화시키지 않았다.

Cerebral Infarction Mimicking Skeletal Metastases on Tc-99m MDP Bone Scintigraphy

  • A 6000-year-old male with carcinoma of the prostate and cerebral infarction underwent a Tc-99m MDP bone scintigraphy for the evaluation of skeletal metastases. Bone scintigraphy (Fig. 1) showed multiple areas of increased uptake of Tc-99m MDP in the skull, spine, and ribs representing skeletal metastases. Two different patterns of uptake occurred in the skull region (Fig. 1A-C); one represents bony metastasis and the ether represents cerebral infarction. The shape, size, location, intensity, and border of the increased uptake differed between the two lesions. An oval-shaped pattern with smaller size, greater intensity and more sharply defined border in the frontal region was consistent with bony metastasis. A rectangular-shaped pattern with larger size, lesser intensity and relatively indistinct border in the temporo-parieto-occipital region was consistent with cerebral infarction. Increased uptake of bone-seeking radiotracers in cerebral infarction has been reported previously.$^{1-4)}$ A suggested mechanism by which bone-seeking radiotracers accumulate in the necrotizing cerebral tissue is an alteration of the blood-brain barrier induced during cerebral infarction, which results in entry of the radiotracers into the extracellular space of the brain.$^{4)}$ Brain CT (Fig. 2) performed 7 days before and one month after the bone scintigraphy revealed lesions on the right temporo-parieto-occipital region consistent with acute hemorrhagic and chronic cerebral infarction, respectively.
