Evaluation on the Usefulness of Filter in Sentinel Lymphoscintigraphy Using $^{99m}Tc$-Phytate (Phytate를 이용한 감시림프절 검사 시 Filter의 유용성 평가)

  • Purpose: The aim of this study was to investigate distribution of particle size in phytate kit and compare filtered method with non-filtered method using 200 nm filter for sentinel lymphoscintigraphy (SLS). Materials and Methods: Five phytate kit of having the same available period was measured by particle size analyzer. For in-vivo experiment, $^{99m}Tc$-phytate was injected intradermally at both foot to perform lymphoscintigraphy. Imaging was acquired at 1hour after injection. Region of interest (ROI) was drawn in inguinal and background area for analysis. RAW 264.7 cells (Murine macrophage cell) were prepared for measurement of celluar uptake as a representative of macrophages. Paired t-test was performed using SPSS (SPSS Inc, USA) for statistical analysis. Results: The size of most particle in Techne phytate kit was distributed in 130~650 nm(90.5 %). In-vivo study, the ROI analysis showed similar result between filtered and non-filtered sample, and the numerical value of count/pixel were $58.3{\pm}5.97$ and $60.2{\pm}4.88$. In-vitro study, cellular uptake study also showed no difference between filtered and non-filtered sample by gamma counting. Conclusion: The present study demonstrates that there was no meaning of 200 nm filtered method for SLS using $^{99m}Tc$-phytate.

The Optimization of Reconstruction Method Reducing Partial Volume Effect in PET/CT 3D Image Acquisition (PET/CT 3차원 영상 획득에서 부분용적효과 감소를 위한 재구성법의 최적화)

  • Purpose: Partial volume effect (PVE) is the phenomenon to lower the accuracy of image due to low estimate, which is to occur from PET/CT 3D image acquisition. The more resolution is declined and the lesion is small, the more it causes a big error. So that it can influence the test result. Studied the optimum image reconstruction method by using variation of parameter, which can influence the PVE. Materials and Methods: It acquires the image in each size spheres which is injected $^{18}F$-FDG to hot site and background in the ratio 4:1 for 10 minutes by using NEMA 2001 IEC phantom in GE Discovey STE 16. The iterative reconstruction is used and gives variety to iteration 2-50 times, subset number 1-56. The analysis's fixed region of interest in detail part of image and compute % difference and signal to noise ratio (SNR) using $SUV_{max}$. Results: It's measured that $SUV_{max}$ of 10 mm spheres, which is changed subset number to 2, 5, 8, 20, 56 in fixed iteration to times, SNR is indicated 0.19, 0.30, 0.40, 0.48, 0.45. As well as each sphere's of total SNR is measured 2.73, 3.38, 3.64, 3.63, 3.38. Conclusion: In iteration 6th to 20th, it indicates similar value in % difference and SNR ($3.47{\pm}0.09$). Over 20th, it increases the phenomenon, which is placed low value on $SUV_{max}$ through the influence of noise. In addition, the identical iteration, it indicates that SNR is high value in 8th to 20th in variation of subset number. Therefore, to reduce partial volume effect of small lesion, it can be declined the partial volume effect in iteration 6 times, subset number 8~20 times, considering reconstruction time.

  • Purpose: Recently, South Korea has seen a rapidly increased incidence of both breast and thyroid cancers. As a result, the I-131 scan and lymphoscintigraphy have been performed more frequently. Although this type of diagnostic imaging is prominent in that visualizes pathological conditions, which is similar to previous nuclear diagnostic imaging techniques, there is not much anatomical information obtained. Accordingly, it has been used in different ways to help find anatomical locations by transmission scan, however the results were unsatisfactory. Therefore, this study aims to realize an imaging technique which shows more anatomical information through the fusion of gamma and realistic imaging. Materials and Methods: We analyzed the data from patients who were examined by the lymphoscintigraphy and I-131 additional scan by Symbia Gamma camera (SIEMENS) in the nuclear medicine department of the National Cancer Center from April to July of 2009. First, we scanned the same location in patients by using a miniature camera (R-2000) in hyVISION. Afterwards, we scanned by gamma camera. The data we obtained was evaluated based on the scanning that measures an agreement of gamma and realistic imaging by the Gamma Ray Tool fusion program. Results: The amount of radiation technicians and patients were exposed was generated during the production process of flood source and applied transmission scan. During this time, the radiation exposure dose of technicians was an average of 14.1743 ${\mu}Sv$, while the radiation exposure dose of patients averaged 0.9037 ${\mu}Sv$. We also confirmed this to matching gamma and realistic markers in fusion imaging. Conclusion: Therefore, we found that we could provide imaging with more anatomical information to clinical doctors by fusion of system of gamma and realistic imaging. This has allowed us to perform an easier method in which to reduce the work process. In addition, we found that the radiation exposure can be reduced from the flood source. Eventually, we hope that this will be applicable in other nuclear medicine studies. Therefore, in order to respect the privacy of patients, this procedure will be performed only after the patient has agreed to the procedure after being given a detailed explanation about the process itself and its advantages.

The Evaluation of Quantitative Accuracy According to Detection Distance in SPECT/CT Applied to Collimator Detector Response(CDR) Recovery (Collimator Detector Response(CDR) 회복이 적용된 SPECT/CT에서 검출거리에 따른 정량적 정확성 평가)

  • Purpose Recently, with the spread of SPECT/CT, various image correction methods can be applied quickly and accurately, which enabled us to expect quantitative accuracy as well as image quality improvement. Among them, the Collimator Detector Response(CDR) recovery is a correction method aiming at resolution recovery by compensating the blurring effect generated from the distance between the detector and the object. The purpose of this study is to find out quantitative change depending on the change in detection distance in SPECT/CT images with CDR recovery applied. Materials and Methods In order to find out the error of acquisition count depending on the change of detection distance, we set the detection distance according to the obit type as X, Y axis radius 30cm for circular, X, Y axis radius 21cm, 10cm for non-circular and non-circular auto(=auto body contouring, ABC_spacing limit 1cm) and applied reconstruction methods by dividing them into Astonish(3D-OSEM with CDR recovery) and OSEM(w/o CDR recovery) to find out the difference in activity recovery depending on the use of CDR recovery. At this time, attenuation correction, scatter correction, and decay correction were applied to all images. For the quantitative evaluation, calibration scan(cylindrical phantom, $^{99m}TcO_4$ 123.3 MBq, water 9293 ml) was obtained for the purpose of calculating the calibration factor(CF). For the phantom scan, a 50 cc syringe was filled with 31 ml of water and a phantom image was obtained by setting $^{99m}TcO_4$ 123.3 MBq. We set the VOI(volume of interest) in the entire volume of the syringe in the phantom image to measure total counts for each condition and obtained the error of the measured value against true value set by setting CF to check the quantitative accuracy according to the correction. Results The calculated CF was 154.28 (Bq/ml/cps/ml) and the measured values against true values in each conditional image were analyzed to be circular 87.5%, non-circular 90.1%, ABC 91.3% and circular 93.6%, non-circular 93.6%, ABC 93.9% in OSEM and Astonish, respectively. The closer the detection distance, the higher the accuracy of OSEM, and Astonish showed almost similar values regardless of distance. The error was the largest in the OSEM circular(-13.5%) and the smallest in the Astonish ABC(-6.1%). Conclusion SPECT/CT images showed that when the distance compensation is made through the application of CDR recovery, the detection distance shows almost the same quantitative accuracy as the proximity detection even under the distant condition, and accurate correction is possible without being affected by the change in detection distance.

The Effects of Image Quality due to Scattering X-ray according to increasing Patient Thickness (피사체 두께에 따른 산란선 발생이 화질에 미치는 영향)

  • In this study, scattering factors affecting the quality of medical images were quantitatively analyzed and investigated. MCNPX simulation was conducted by using ANSI phantom, made of tissue equivalent materials, to calculate the scattering ratio occurred by the increase of the object thickness. Then, the result of the simulation was compared with the result of actual radiation measurement. In addition, we evaluated the image quality by the RMS evaluation, RSD and NPS analysis using X-ray images acquired with increasing object thickness. Furthermore, the scattering ratio was analyzed by increasing the thickness of acrylic phantom on chest phantom. The result showed that the scattering ratio was increased to 57.2%, 62.4%, and 66.8% from 48.9%, respectively, when the acrylic phantom thickness was increased by 1 inch from 6.1 inches. The results of MCNPX simulation and the actual measured scattering dose showed similar results. Also, as a result of RMS measurement from acquired x-ray images, the standard deviation decreased as the object thickness increased. However, in the RSD analysis considering the average incident dose, the results were increased from 0.028 to 0.039, 0.051, 0.062 as the acrylic phantom thickness was increased from 6.1 inches to 7.1 inch, 8.1 inch, and 9.1 inch, respectively. It can be seen that the increase of the scattering effect due to the increase of the object thickness reduces the SNR. Also, the NPS results obtained by measuring scattered radiation incident on the detector resulted in the increase of the noise as the object thickness increased.

Efficacy of Neoadjuvant Chemotherapy and Radiotherapy for the Histology-confirmed Intracranial Germinoma - Preliminary Report (조직학적으로 확진된 두개내 배아종의 전보조화학요법 후 방사선치료의 성적 - 예비적 결과)

  • Purpose : We intended to decrease late CNS reaction after radical radiotherapy for an intracranial germinoma by using combined neoadjuvant chemotherapy and involved-field radiotherapy. The efficacy in terms of its acute toxicity and short-term relapse patterns was analyzed. Materials and Methods : Eighteen patients were treated with combined neoadjuvant chemotherapy and radiotherapy between 1995 and 2001. The chemotherapy regimen used was the Children's Cancer Group (CCG) 9921A (cisplatin, cyclophosphamide, VP-16, vincristine) for 5 patients younger than 16 years, BEP (bleomycin, VP-16, cisplatin) for 12 patients, and EP (VP-16, cisplatin) for 1 patient. The radiotherapy covered the whole craniospinal axis for 5 patients, the whole brain for 1, and the partial brain (involved field) for 12. the primary lesion received tumour doses between 3,960 and 5,400 cGy. Results : The male to female ratio was 16:2 and the median age was 16 years old. The tumors were located in the pineal gland in 12 patients, in the suprasellar region in 1, in the basal ganglia In 1, in the thalamus in 1. Three patients had multiple lesions and ventricular seedings were shown at MRI. In 3 patients, tumor cells were detected in the cerebrospinal fluid and MRI detected a spinal seeding in 2 patients. The response to neoadjuvant chemotherapy was complete remission in 5 patients, partial remission in 12, and no response in 1. However, after radiotherapy, all except 1 patient experienced complete remission. The toxicity during or after chemotherapy greater than or equal to grade III was remarkable; hematologic toxicity was observed in 11 patients, liver toxicity in none, kidney toxicity in none, and gastrointestinal toxicity in one. One patient suffered from bleomycin-induced pneumonitis. Radiotherapy was therefore stopped and the patient eventually died of respiratory failure. The other 17 are alive without any evidence of disease or relapse during an average of 20 months follow-up. Conclusion : A high response rate and disease control was experienced, which was the same as observed other studies and the morbidity from chemotherapy-induced toxicity was similar. With these results, the results from adjuvant chemotherapy and involved-field radiotherapy cannot be concluded to be equal to those from extended-field radiotherapy. The long term follow-up study on later complications are required in order to draw definite conclusions on the optimal management with minimum side effects.

A Study on the Knowledge and Attitude about Sex in One Commercial Girls' High school in Seoul (서울지역 일 여자 상업고등학교의 성에 대한 지식 및 태도에 관한 연구)

  • This study was conducted in order to grasp the condition of the student's knowledge and attitude about sex. And to offer the basic materials for sex education of school health service. The objects were comprised of 464 volunteered students who study in commercial girls' high school. The material of this study was the questionaire suited to the purposed of this research which has been made through studying references. All the questionaire written by students. The self reported questionaires were collected immediately without explanation on supervision of school nurse. The data was collected from 28th to 30th of June on 1990. Analysis of the data was done utilizing SPSS for percentage, mean, ANOVA and Pearson Correlation Coefficients. The Results are as follows; 1. General features of the objects of study School grade distribution was similar. Fathers of 41-50 years were the highest(58.3%), mothers of 41-50 years were the highest(64.3%), family of living together were the highest(87.5%), fathers of graduated high school were highest(60.7%), mothers of graduated middle school were the highest(43.0%) and neuclear families were highest(91.5%). 2. The Conditions of Knowledge about sex When 5 Point was given to "Well known" and 1 point was given to "Never known", the total average was 2.97, Especially the mean of Female physiology was shown 3.93, 73.4% of students have known. But the mean of male physiology was shown 2.23, 17.2% of students only. And Family planning item was 3.54, hymen item was 3.38, female genitalia item was 3.35, abortion item was 3.25, Intercourse and pregnancy item was 3.24, Ovulation item was 3.02, Contraception item was 2.97, Veneral disease and masturbation item was 2.82, maintenance of pregnancy item was 2.76, Anatomical differences between male and female item was 2.59, male genitalia item was 2.31, ejaculation item was 2.27. 3. The conditions of attitude about Sex When 5 point was given to "Very affirming" and 1 point was given to "Very deny" the total average was 3.20. Especially the mean of social intercourse between other sex was shown 3.92, 73.4% of students have affirmed. But the mean of psychic response on menstruation was shown 2.24, 8.8% of students only. And baby birth item was 3.72, the fact that I am a woman item was 3.53, marriage item was 3.49. Secondary body change item was 3.38, puberty item was 3.31, delivery and sexual intercourse item was 3.05, pregenancy item was 3.02, psychic condition on menarche item was 2.50. Also present counsellors about sex were teachers in charge (44.9%), friends(21.6%), mothers(20.6%), elder sisters (10.6%), mass-communications (1.5%), fathers (0.4%), school nurses and elder brother(0.2%). In addition to, future counsellors about sex were friends (37.7%), mothers(30.6%), elder sister (18.4%), school nurses (4.6%), mass communication (3.8%), teachers in charge (2.5%), elder brothers (1.4%) and fathers(1.0%). 4. Correlation between the general features and knowledge variables School grade and knowledge condition has relationship to female genitalia(P<0.05), female physiology (P<0.00), male physiology (P<0.05),ovulation (P<0.00), and femily planning (P<0.005). Fathers age and knowledge condition has relationship to male physiology(P<0.05), and abortion (P<0.05). Marrital status and knowledge condition has relationship to female physiology (P<0.01), masturbation (P<0.05). Fathers educational background and knowledge condition has relationship to masturbation (P<0.00). Mothers age and knowledge condition has relationship to family planning (P<0.05). 5. Correlation between the general futures and attituded variables Fathers age and attitude condition has relationship to psychic response on menstruation (P<0.05). Mothers age and attitude condition has relationship to fact that I am a woman (P<0.00). Mothers educational background and attitude condition has relationship to social intercourse between other sex (P<0.05). Type of family and attitude condition has relationship to puberty (P<0.01). 6. Correlation between knowledge condition and attitude condition Those who had more knowledge about sex have more affirmative response (P<0.001).

Oncologic Outcome of Chondrosarcomas (연골육종의 종양학적 결과)

  • Purpose: We evaluated oncologic outcomes of chondrosarcomas and analyzed the disease-free survival rate of chondrosarcomas according to the various factors. Materials and Methods: We performed a retrospective study for the disease-free survival rate of 48 chondrosarcomas, 44 of which underwent surgical treatment and followed up more than 18 months since 1993, and in the remaining 4 cases, the patients died before 18 months after surgery. The vsariables were location, tumor volume, histologic grade, stage, age at presentation and treatment performed. The mean follow up period was 43.8 months (1-196 months). Results: The overall disease-free survival rate was 77.1% at mean 43.8 month follow up. The 5 year- and 10 year disease-free survival rates were 64% and 58% respectively. The histologic grade, stage, age at presentation revealed statistical significance on disease-free survival. All 9 patients treated with extended curettage for grade 1 central chondrosarcomas revealed disease-free survival with excellent functional outcome. Conclusion: The disease-free survival rate of chondrosarcomas mainly depended on histologic grade, stage and age at presentation. Local recurrence and distant metastasis also revealed statistically significant differences of disease-free survival rate. Comparing to wide resection, extended curettage for low-grade central chondrosarcomas in extremities were efficient methods with similar survival rate and less functional losses and complications.

Measurement of competency through self study in basic nursing lab. practice focused on cleansing enema (기본간호학 실습에 있어 자가학습을 통한 능숙도 측정 - 배변관장을 중심으로 -)

  • This study was conducted to provide the basic data necessary for the improvement of the teaching method for basic nursing practice as well as the effectiveness of the practice by examining the students' competency in cleansing enema after doing the self study instead of the traditional education. To examine the competency in cleansing enema after the self study, this study is an one group pretest-posttest design that subjects did the enema practice through the self study. The subjects were 89 sophomore students at Y University. College of Nursing. In basic nursing lab practice class, cleansing enema self study module was given to the students which was developed by the researcher based on the literature review and asked them to finish doing the pre study and checking the self study evaluation criteria after reading the goal, learning activities and theoretical guideline. After watching the video tape, students practiced the process in the module by themselves. For the competency in cleansing enema. repeated autonomous practices were done during the open lab other than the regular class. Whenever the practice was done, the frequency and time were measure and documented. When the student felt confident through repeated practices, the competency was evaluated by the researcher and two assistants based on the evaluation criteria. And the process was repeated till the student could perform all the items on evaluation criteria completely. The data were collected for 42 days from Oct. 15 to Nov. 26 in 1996. Collected data were analyzed by frequency, percentage, Pearson correlation coefficient and variance analysis. The results are summarized as follows : 1. 43.2% of the students were favorable to nursing and 63.6% like lecture, but 71.6% like practice. So they were more interested in practice than in lecture. 2. 62.3% of the students scored high in written test, 97.8% scored high in practice. So the practice score was better. 3. The frequency of repeated practice to pass the test ranged from 1 to 4 and the average is 2.2. 4. The average time needed in preparation and the performance was nearly the same regardless of the frequency. It took 5 to 38 minutes for those who passed the test after practicing once and the average was 16 minutes. 5 to 60 minutes were taken for those who practiced twice to pass the test and the average was 21 minutes. Those who passed the test after three practices needed 8 to 30 minutes and the average was 15 minutes, which was similar to the time that the students who passed the test for the first trial. Only one student passed the test after 4 practices and it took 10 minutes. 5. 64% of the students agreed that the context and the content of the module were appropriate for the self study and 68.2% were satisfied. And 71.9% said that the module helped them to practice the enema self study 6. Though only 42% of the students were satisfied with the video. 50.6% said that it was helpful for the self study. 7. 52.3% of the students were satisfied with the self study method, and 86.6% obtained self-confidence when performing the enema. 8. The lower the student's practice score was, the more practices were needed for them to pass the test(r=-.213, P<.05). As a result, for performing the enema practice competently, two or more practice opportunities were needed to be given. And it is possible to obtain the less complex nursing skills through the self study, when enough learning resources and assistance such as learning guidance or video tapes are provided. Based on this study. I want to suggest that. 1. There must be college policy that can support the new method instead of the traditional learning method for the students to attain the proficiency in basic nursing skills. 2. The assistant materials should be developed as soon as possible to promote the self study of basic nursing skills.

Evaluation to Purification Capacity of Pollutants by Column Test with the tidal flat sediment (통수실험에 의한 갯벌의 오염물질 정화능력 평가)

  • The purpose of this study is to evaluate the purification capacity of pollutants by column test with the tidal flat sediment. Sediment materials were taken from Chunjangdae tidal flat which located in Chungnam Seochungun. The column tests were conducted on four conditions(R1 : raw sewage filtered by G2 filter, R2 : sterilized sewage after filtered by GF/C filter, R3 : R2 /filtered(membrane) seawater (1:1), R4 : R2 /filtered(membrane) seawater(1:2)). The results of this study may be summarized as followed ; The removed COD by column tests were increased according to increasing the quantity of sewage. During the column tests of 580min, the total removed ammonia nitrogen were 90.1mg for R1, 81.0mg for R2, 27.6mg for R3 and 4.1mg for R4. The result was similar to COD experiment. During the 580min, the total removed total-phosphates were 3.4mg for R1, 4.2mg for R2, 5.6mg for R3 and 2.0mg for R4. The removal efficiency of Pb and Cd for R3 and R4 reactor were higher than R1 and R2 reactor. The remove of heavy metal by the column test was high in sample with seawater. But in the initial 20min, the adsorbed Pb and Cd showed about 3% of the total adsorbed Pb and Cd during 580min.
