A Study on Self-medication Beharior of Four Cautious Drugs. (특정의약품의 자가투약행위에 관한 연구)

  • The survey with questionnaire were conducted to grasp the actual condition of four cautious drugs-antibiotics, sedative hypnctics, mincr tranquilizer, analeptics-self -medication behavior through drugstore use and to analyze the primary factors connected with it. The following six objectives were established. 1) To catch the reason why the man who medicate himself buy four cautious drugs at a drugstore, and to know the choice motive of each drug purchased. 2) To grasp the distribution of four cautious drug taken by the man who medicate himself and to analyse the reason of taking the drug. 3) To find out whether the buyer has taken the same drug past and to know the duration and the frequency. 4) To grasp the degree of recognition about four cautious drugs and the attitude toward continuous taking them. 5) To analyze the degree of recognition about the influence on human body and the attitude of medication behavior. 6) To know "the experience of side-effects" and to grasp the kind of the side-effects. The data were collected from 15 drugstores with 677 respondents in seoul from August 17 to september 21, 1987. The following results were obtained. 1) The reasons for drugstore use were first, "for easy access and convenience" (53.7%) second, "for the slightness of illness" (19.9%) third "for the cheap price" (13.2%). According to that result, the factor of "the easy access of drugstore" was most influential. In case of the poor, the factor "for the cheap price "was revealed second (37.3%). And "for the slightness of illness" was second reason in the medically insured (22. 9%), "for the cheap price" was second reason in the others (29%). 2) The kind of drugs purchased were antibiotics (62.8%), analeptics(17.2%), minor tranquilizer(13.3%) and sedative hypnotics (6.7%) on the whole. In teenagers, besides antibiotics the percentage of taking analepits came second (42.4%) and it was revealed that the percentage of sedative hypnotics, minor tranquilizer increase with age. But in proportion to the increase of age, the taking of analeptis decreased. 3) The choice motives of each drug purchased were all different. In case of antibiotics "recommendation of pharmacist" was 39.6%, sedative hypnotics "recommendation of pharmacist" and "my own judgement", 28.9%, respectively, minor tranquilizer "my own judgement", 35.2%. and in case of analeptics "my oun judgement" was most common with 53.5%. 4) The reasons for taking drugs were as follows. antibiotics was taken for the inflammation mitigation of a wound and a swelling (38.5%), sedative hypnotics for the removal of insomnia (97.5%), minor tranquilizer because of restlessness and qloominess(39.3%), and analeptics for the shortening of sleeping hours (35.1%). 5) Those experienced in taking four cautious drugs were 78.2% on the average. It was revealed that antibiotics use duration was "from one week to one month" (38.9%), the frequency was "rarely" (62.1%), sedative hypnotics and minor tranquilizer; "More than three years", (35.7%, 30.4%), respectively, "Somtimes", (43.8%, 35%), respectively. analeptics ; "from one year to three years", (27.6%), "Sometimes", (42.7%). 6) In regard to the source of information in taking drugs, 35.3 percent of male were relied on "recommendation of pharmacist", 32.6 percent of female "my own judgement", There was a difference between below the middle school graduates and over the high school graduates Thease were relied on "recommendation of pharmacist" first, those "my own judgement" first. And "my own judgement" was the first source of specialist(40%), labours(41.4%), salesman(43.5%), and jobless men(36.8%), "recommendation of ralatives on friends" was the first source of students (30.4%), "recommendation of pharmacist" was the first soure of teachers, administrative office workers (39%) and housewives (40.7%). 7) The degree of recognition about four cautious drugs was as follows. "know a little" was (43.0%), "don't know" (30.2%), "know" (26.8%), respectively. In regard to the attitude toward continuous taking them, it was revealed that "I will take it according to circumstamces" was first (56.2%), " I will not take it as possible" was first(56.2%), "I dont know well" (12.9%), "I will take it continuously" (8.3%). 8) About the influence on human body, "if proper, it is effect" was common attitude toward antibiotics (43.6%), "Probably bad influence on the health" was first toward the other drugs (46.2%), In terms of the degree of observance to taking-time and dose of drugs, "suitably" was first (37.3%), "relatively correctly" second (27%), "correctly" (17%). 9) The percentage of four cautious drugs side-effects was 27.5 on the average. The kind of side-effects were eruption and urticaria (28.5%), heart acceleration and so forth. (25.4%), headache and Giddiness, dyspepsia (23%), respectively.

Antimicrobial effects of natural extracts on bacteria isolated from the uterus of Hanwoo (Korean native cattle) (한우의 자궁 내 분리 세균에 대한 천연물질 항균효과)

  • Bacterial infection of the uterus in Hanwoo can kill embryos. Therefore, many antibiotics have been used to treat this infection in the uterus of repeat breeders. Incorrect use of antibiotics has led to resistance in bacteria. Natural compounds have used as substitutes for antibiotics because they are safe and have very mild side effects. This study was conducted to examine the antimicrobial effects of five extracts from medicinal plants including Humulus japonicas (Hj), Phelledendron amurense (Pa), Viola mandshurica (Vm), Carthamus tinctorius (Ct), and Chelidoni herba (Ch) on bacteria isolated from the uterus of Hanwoo using the paper disc diffusion method. Hj and Pa extracts had potent antimicrobial effects against Staphylococcus lentus, Streptococcus infantarius subsp. coli, and Bacillus pumilus. Pa had the greatest antimicrobial effect among the five medicinal plants and was effective against 19 types of bacteria from bovine uterus. Compared to Pa, Hj showed weaker antimicrobial effects on all the bacteria tested except Pseudomonas aeruginosa. Vm, Ct, and Ch also showed weak antimicrobial effects on the tested bacteria. The results obtained suggest that Hj and Pa are natural compounds suitable for treating bacterial infection in repeat breeders and improving conception rates of Hanwoo.

  • For recent years, use of autologous fat injection has increased significantly in facial contouring surgery. Along with such increase in use, complications like atypical mycoplasma infection have been also on the increasing trend. The authors report two cases of Mycobacterium chelonae infection that occurred after autologous fat injection. Patients were treated as infection that resistant to common antibiotics and results were negative to routine culture and Gram staining. Acid-fast bacillus stain, polymerase chain reaction (PCR) test and mycobacterial cultures were conducted for diagnosis under suspicion of atypical mycoplasma infection. Then, combination antibiotics therapy, surgical treatment, and steroid injection were performed for treatment. Both patients were diagnosed with Mycobacterium chelonae in PCR test. They were positive to mycobacterial cultures. Combination antibiotics therapy was repeated to improvement of symptom. However, they could not be free from side effects such as deformation in facial contour, scar and pigmentation even after full recovery. When chronic wound infections after autologous fat injection, we must suspect atypical or mycobacterial infection and conduct examinations for a early diagnosis and proper antibiotic therapy that is effective to the nontuberculous mycobacteria.

5 Cases of Acute Pyelonephritis (급성(急性) 신우신염(腎盂腎炎)의 임상적(臨床的) 고찰(考察))

  • Purpose: In general, Acute Pyelonephritis is effectively treated with the use of antibiotics. However, some antibiotics are reported to cause side effects, and the abuse of antibiotics results in the increase of the disease's tolerance to antibiotics. Recently, I have effectively treated five cases of Acute Pyelonephritis by using only Constitution - Acupuncture and Herb, and therefore I would like to report about these cases. Methods: I diagnosed Acute Pyelonephritis of these five patients by confirming symptoms and employing a urine analysis with reagent strip(Multi $stix{\circledR}$). I used Kuon's method of constitutional diagnosis for the purpose of the diagnosis of the constitutional 8 morbidities. I relied on Sungjeong(性情) and Chehyungkisang(體刑氣像) in diagnosing Sasang Constitutions(四象人). I performed acupuncture on the left and right sides, depending on 8 constitutions, by employing Chang - temperament Inflammation Formula(臟系炎症方) that is used for the treatment of all kinds of chang-temperament inflammation diseases, as well as Bactericidal Formula(殺菌方) that is used for the treatment of all kinds of bacterially caused diseases. I prescribed by consulting the appearance of disease and general symptoms of each case with Dongyi Soose Bowon(東醫壽世保元)'s prescription symptoms. Result: Two of them showing severe symptoms were hospitalized, while three others took OPD treatment. The patient who was PANCREOTONIA and Soyangin improved through hospitalization for three days, another patient who was PULMOTONIA and Taeyangin with severe symptoms, improved through hospitalization for seven days, and completely recovered through OPD treatment later. The three others took only OPD treatment, and improved within 5-7 days. Conclusion: I confirmed that each of 8-constituions and Sasang Constitutions were all treated effectively without antibiotics.

PULP TREATMENT OF PRIMARY TEETH USING A COMBINATION OF ANTIBIOTICS (항생제를 사용한 유치의 치수치료)

  • The purpose of this study was to observe the potentiality of a mixture of antibiotics to relieve the endodontic and periapical infections in primary teeth. A mixture of metronidazole, ciprofloxacin, and minocyclin in distilled water or in a root canal sealer were used to disinfect the infected endodontic and periapical lesions. For application of this mixture, medication cavities were prepared at orifices of root canals by using the $Fissurotomy^{(R)}$ burs(S S White, USA). The cavities and bottom of pulp chamber were filled with antibiotics mixture. In this cases, clinical symptoms were improved following a single application of that mixture. Gingival swelling, pain, and fistulae were disappeared within a week. More long term evaluation will be needed. Although there were no reports of side effects, care should be taken if children are sensitive to antibiotics. Within the limitations of this case, the mixture of 3 antibiotics provided an excellent improvement in treatment of infected primary teeth under the physiologic root resorption.

Antibacterial Activity and Synergism of Hydnocarpi Semen Extracts with Ampicillin or Oxacillin against Methicillin-resistant Staphylococcus aureus

  • Methicillin-resistant Staphylococcus aureus (MRSA) is a serious clinical and an urgent problem worldwide. Few new drugs are available against MRSA, because MRSA has the ability to acquire resistance to most antibiotics, which consequently increases the cost of medication. In the present study, the antibacterial activity of Hydnocarpi Semen was investigated. The most effective method is to develop antibiotics from the natural products without having any toxic or side effects. Therefore, there is a need to develop alternative antibacterial drugs for the treatment of infectious diseases. Five Clinical isolates (MRSA) were obtained from five different patients at Wonkwang University Hospital (Iksan, South Korea). The Other 2 strains were ATCC 33591 (Methicillin-resistant strain) and ATCC 25923 (Methicillin-susceptible strain). Antibacterial activity (Minimal Inhibitory Concentrations, MICs) was determined by broth dilution method, disk diffusion method, MTT test, and checkerboard dilution test. Antibacterial activity of n-hexane fraction was remarkable, and had a MICs ranging from $31.25-125{\mu}g/m{\ell}$. FICI values for HFH+AM and HFH+OX were 0.13-0.19 and 0.04-0.29, showing the increase of synergistic effect. When combined together, these antibacterial effects were dramatically increased.

A Clinical study of Recurrent Acne in the Oriental Medicine (여드름에 관한 임상적 연구)

  • Acne is a common chronic disease which most frequently affects the face, appearing in both males and females near puberty, and in most cases becomes less active as adolescence ends. The intensity and duration of activity vary for each individual. The disease may be minor, with only a few comedones or papules, or it may occur as the highly inflammatory and diffusely scarring acne conglobation. Permanent scarring of the skin and psyche can result from such inaction. The disease, however, has implications far beyond the few marks that may appear on the face. The lesions cannot be hidden under clothing; each is prominently displayed and detracts significantly from one's personal appearance and self-esteem. Recently many of acne patients are visiting to Oriental Medicine Hospitals. They want to receive the treatment of acne related another disease and causes, or worry about side effects of antibiotics and hormone therapy. This study was performed at Pundang Cha Oriental Medicine Hospital Department of Dermatology from March 1, 1997 to February 29, 2000 in 31 patients with recurrent acne. We investigated the clinical findings, included distribution of age & sex, severity of acne, sites on face, related causes, duration of disease and causes of visiting Oriental Medicine Hospital and so on. The results were as follows : The peak age at onset was 20-29 years old in females with 16 cases reported. The 22 total cases patients had severe signs with most frequently sites being forehead & chin. They visited Oriental Medicine Hospital because they want to take the treatment of acne related with another body problems(12 cases) and worry about side effects of antibiotics. Twenty cases were diagnosed 肺熱血熱(20 cases). And given acupuncture treatment, herb-medication and sometimes we used external applications. Relatively, their satisfaction grade was high. Dermatologists are trying to treat new methods in order to remove scar and decrease sebaceous gland secretion,. And we, Oriental Medicine doctors, have to develop and offer to patients the most effective management and successful treatment of this disease.

An overview of Acne Vulgaris (Busoor Labaniya)

  • Acne vulgaris is a common dermatological condition affecting the pilosebaceous units and having a multifactorial etiology. In Unani terminology, acne vulgaris is referred as Busoor Labaniya, characterized by white lesions on the face, nose, and cheeks; on squeezing, release cheesy material. In conventional medicine, mild cases are best addressed with topical regimens, but more severe cases require systemic medications. Retinoids (retinoic acid, adapalene, isotretinoin, tazarotene), benzoyl peroxide, clindamycin, erythromycin, and azelaic acid are a few examples of topical medications, whereas systemic drugs include antibiotics (Doxycycline, minocycline, erythromycin, azithromycin). In the Unani System of Medicine, numerous single and compound drugs have been used to treat Busoor Labaniya. These drugs are considered harmless and do not have any major side effects. In Unani system of medicine, acne vulgaris is effectively managed with natural medicines as well as therapeutic regimens with minimal side effects even after long-term usage. Acne vulgaris is usually treated with systemic blood purifiers along with topical Unani drugs. Hence, clinical studies with proper scientific parameters are needed to be conducted to establish and validate their efficacy in the prevention and control of acne vulgaris.

Transport of Selected Veterinary Antibiotics (Tetracyclines and Sulfonamides) in a Sandy Loam Soil: Laboratory-Scale Soil Column Experiments (토양컬럼을 이용한 테트라사이클린계 및 설폰아마이드계 항생물질의 이동특성 평가)

  • Antibiotics are biologically active substances and commonly used for therapeutic treatment of infectious disease in humans and for treating and protecting the health of animals. In recent years, antibiotics have attracted worldwide attention because of their side effects on the environment. Consequently, efforts have been made to monitor the residual of antibiotics in the environment. This study tested the mobility of tetracyclines and sulfonamides in soil and leachate through column experiments. The three tetracycline antibiotics showed higher mass recovery rates in all kinds of soils(28.00~44.11%) than in leachate(10.54~27.43%). This seems attributable to the high adsorption coefficient values($K_d$) of tetracyclines representing strong and active adsorbability to organic and mineral phases in soil, ending up relatively small amount being detected in surface water. By contrast, the sulfonamides(sulfamethazine and sulfathiazole) showed higher mass recovery rates in leachate(23.19~26.20%) compared to in soil(10.41~14.21%) due to lower adsorption coefficient values and higher mobility of sulfonamides, enabling easier movement to surface water through the runoff in the environment.

The effects of Torilis fructus extracts against enteropathogenic Escherichia coli in Piglets (사상자 추출물의 자돈 병원성 대장균 억제 효과)

  • The antibiotic chemotherapy produces sometimes side effects and fails to eliminate bacterial infection. The occurrence of strains resistant to antibiotics would be expected to increase, and it is nowadays important to search for non-antibiotic substances. We are aimed to evaluate the effects of Torilis fructus extracts against enteropathogenic Escherichia coli (E. coli) in Piglets. The piglets were divided with three groups; Negative control group, E. coli-infected positive control group, and the Torilis fructus extracts treated group with E. coli infection. During the study period, we compared clinical signs, weight increase rate, fecal scores, gross findings between the treated group and non treated group. After necropsy, necropsy findings and histopathological findings were conducted with the comparison between the groups. As the results of this study, the Torilis fructus extracts additive showed the effects on the suppression of E. coli-induced lesions. On the basis of this study results, our data suggest that the Torilis fructus extracts additive have the antimicrobial effects. The Torilis fructus extracts additive could be used as the alternative material for antimicrobial feed additives.
