Perceived Causes of Work-Related Sick Leave Among Hospital Nurses in Norway: A Prepandemic Study

  • Background: Although sick leave is a complex phenomenon, it is believed that there is potential for prevention at the workplace. However, little is known about this potential and what specific measures should be implemented. The purpose of the study was to identify perceived reasons to take work-related sick leave and to suggest preventive measures. The study was completed before the COVID-19 pandemic emerged, and the risk factors identified may have been amplified during the pandemic. Methods: An in-depth cross-sectional survey was conducted across a randomly selected sample of hospital nurses in Norway. The national sample comprised 1,297 nurses who participated in a survey about their sick leave during the previous 6 months. An open-ended question about perceived reasons for work-related sick leave was included to gather qualitative information. Results: Among hospital nurses, 27% of the last occurring sick leave incidents were perceived to be work-related. The most common reasons were high physical workload, high work pace, sleep problems, catching a viral or bacterial infection from patients or colleagues, and low staffing. Conclusions: Over a quarter of the last occurring sick leave incidents among Norwegian hospital nurses are potentially preventable. To retain and optimize scarce hospital nursing resources, strategies to reduce work-related sick leave may provide human and financial benefits. Preventive measures may include careful monitoring of nurses' workload and pace, optimizing work schedules to reduce the risk of sleep problems, and increasing staffing to prevent stress and work overload.

종합병원 근무자의 병가율 (Analysis of Sick Leave Rates of Employees in General Hospitals)

  • The objective of this study was examine sick leave rates of hospital employees. The sick leave data of 2,123 employees in three(3) general hospitals located in Seoul during the period from January 1, 1992 to December 31, 1992 was analyzed to achieve the study objective. The sick leave rates were computed in compliance with the standards recommended by the International Association on Occupational Health. Univariate analysis methods($X^2$-test and ANOVA) were used to assess the sourse of variance in the rates. The results were as follows : 1. The total annual rates of sick leave were 4.8% in frequency(persons), 0.23% in lost time, 0.68 days in duration and 14.0 days in severity. 2. The sick leave rates of frequency(person). duration and lost time were significantly higher in female than male, in groups of 40-49 years than in the other age group, the married than the unmarried and in the long employment of 8 years or above than the short employment. But there was no significant difference in the rate of severity. Only the sick leave rate of frequency(person) was significantly related to the educatial status, but there was no significant difference in other analytical factors of sick leave rate. 3. The main causes of sick leave were injury and poisoning(24.3%), and disease of the digestive system, disease of the nervous system and sense organs, and complications of pregnancy, childbirth and puerperium(respectively 11.6%). The severity rate was the highest in neoplasms(32.2 days), and followed by endocrine, nutritional and metabolic disease and immunity disorders, injury and poisoning, and infections and parasitic disease in descending order. 4. The sick leave rates of frequency(person), duration and lost time were the highest in nutritional workers followed by registered nurses. However, severity rate was the highest in doctors and pharmacists and followed by in nutritional works. 5. The main cause of sick leave was complication of pregnancy, childbirth and puerperium in registered nurses(26.3%), injury and poisoning in nutritional workers(78.6%) and disease of respiratory system and digestive system in other workers.

운항승무원의 생활습관과 건강검진 결과가 병가에 미치는 영향 (Impact of Lifestyle and Health Examination Results on Sick Leave in Commercial Airline Pilots)

  • Purpose: This study aims to compare the general characteristics, life-style, health examination results, and sick leave days by airmen medical examination decision and to investigate factors affecting sick leave days. Methods: We obtained data from 2,361 Korean pilots who worked for a commercial airline. Comparison of the results by airmen medical examination decision (Fit or waver) was conducted using the 𝝌2 test or Fisher's exact test. Factors affecting sick leave days were analyzed using logistic regression. Results: Age, smoking history, blood pressure, obesity, and fasting blood sugar level were significantly different between the Fit and Waver groups. Rate of using sick leave long-term was higher in the Waver than in the Fit. Sick leave days were significantly associated with age, habits of drinking, and smoking in the Fit group. Conclusion: This study demonstrated the health risk factors that affect the number of sick leave days. By providing basic data for the health care of workers, it is expected to be applicable to the provision of health promotion and disease prevention programs for workers.

Does Paid Sick Leave Induce Welfare Burden?

  • Purpose: The purpose of this study is to empirically evaluate the unintended welfare losses induced by paid sick leave, examine the severity of the unintended moral hazard loss caused by paid sick leave, and evaluate how much moral hazard cost society can accept to obtain paid sick leave benefits. Research Design, Data and Methodology: We examine the Medical Expenditure Panel Survey data collected in 2013 and 2014 by employing a panel probit analysis to control for individual heterogeneity. Results: The estimation result shows that the probability of absence due to paid sick leave increases from 4.91% to 7.84%. Among them, excluding the probability of increasing absence from 1.29% to 2.69% due to the actual disease, the probability of absence due to the moral hazard was estimated to be 2.41% to 6.49% in the proposed models. Based on the result, if we evaluate the increase in absence caused by moral hazard as a social cost, the estimated cost is approximately $174 to $297 per worker per year. Conclusion: Considering these expected costs, our society can obtain the access benefit from paid sick leave if we are willing to accept the moral hazard cost.

병원 및 환자특성에 따른 수지절단 산업재해환자의 요양기간과 진료비 (The Patient Days and Medical Care Benefits of Finger-Amputated Patients due to Industrial Accident by Hospital and Patient Characteristics)

  • This study was conducted to analyze patient days and medical care benefits of finger-amputated patients due to industrial accident. The 161 personal data on medical care for finger-amputated patients due to industrial accident(88 in 1994, 73 in 1995) of Regional Labor Office and hospital characteristics were analyzed. The major results of this study were as follows : According to stepwise multiple regression analysis of patient days, number of amputated finger, location of hospital, bed capacity of hospital, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, sick leave benefits per day were the major significant variables in order. In stepwise multiple regression analysis with medical care benefits as a dependent variable, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, number of amputated finger, sick leave benefits per day, age, bed capacity of hospital were the major significant variables in order. The minimum optimal size with the lowest medical care benefits was a hospital with 300 beds. This shows that the economy of scale is also applicable for hospital industry. In summary, presence of plastic surgery in hospital, number of orthopedic specialist per 100 beds, number of amputated finger, sick leave benefits per day, bed capacity of hospital were the major significant variables in both patient days and medical care benefits.

다중흐름모형을 적용한 서울형 유급병가 정책 도입과정에 관한 연구 (A Study on the Policy Decision Making Process of Seoul-Type Paid Sick Leave: Applying Kingdon's Multiple Streams Model)

  • In 2019, the Seoul metropolitan government established its own 'Seoul-type paid sick leave project'. Although the central government had to introduce such a system, which is also called sickness benefits, it was not implemented. In order to understand the process by which the Seoul government has implemented such a policy, this study used Kingdon's multiple streams framework. As a result, in the problem stream, it was found that the economic burden of sickness has been considered only in terms of medical expenses in the past of Korea. Then Songpa's three women and Middle East respiratory syndrome incidents raised awareness of the necessity of the sickness benefit system in 2014 and 2015. In the political stream, several social affairs such as national health insurance huge surpluses and the 2017 presidential election opened policy window. At that time, Seoul Mayor actively promoted sickness benefits as a policy entrepreneur. In the policy stream, the sickness benefit system has gained new attention through political events. To summary, these three streams flowed separately and then they assembled around huge political affairs. As a result, it was confirmed that Kingdon's model is the most effective theory than any other models in analyzing the health care policy decision process in Korea.

Demographic, Lifestyle, and Physical Health Predictors of Sickness Absenteeism in Nursing: A Meta-Analysis

  • Background: Sickness absenteeism is an area of concern in nursing and is more concerning given the recent impacts of the COVID-19 pandemic on healthcare. This study is one of two meta-analyses that examined sickness absenteeism in nursing. In this study, we examined demographic, lifestyle, and physical health predictors. Methods: We reviewed five databases (CINAHL, ProQuest Allied, ProQuest database theses, PsycINFO, and PubMed) for our search. We registered the systematic review (CRD de-identified) and followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. Additionally, we used the Population/Intervention/Comparison/Outcome Tool to improve our searches. Results: Following quality testing, 17 articles were used for quantitative synthesis. Female employees were at higher risks of sickness absenteeism than their male counterparts (OR = 1.73; 95% CI: 1.33-2.25). Nursing staff who rated their health as poor had a greater likelihood of experiencing sickness absence (OR = 1.38; 95% CI: 1.19-1.60). Also, previous sick leave predicted future leaves (OR = 3.35; 95% CI: 1.37-8.19). Moreover, experiencing musculoskeletal pain (OR = 2.41 95% CI: 1.77-3.27) increased the likelihood of sickness absence with greater odds when it is a back pain (OR = 3.05; 95% CI: 1.66-5.62). Increased age, physical activity, and sleep were not associated with sick leave. Conclusion: Several variables were statistically associated with the occurrence of sickness absenteeism. One primary concern is the limited research in this area despite alarming rates of sick leave in healthcare. More research is required to identify predictors of sickness absence, and thereby, implement preventative measures.

Reflections on Public Holidays and Productivity in Nigeria

  • Public holidays are periods set aside by nations or states to observe particular events like national days, independence anniversary or religious festivals. These days are different from the usual periods of statutory leave or vacation provided for workers in the collective agreement or rule of engagement at the point of entry. These periods of leave enjoyed by workers include annual vacation, casual leave, sick leave, maternity leave, paternity leave and others as may be recognized within an establishment. Individual worker's period of leave is personal and at the discretion of the worker or the organization as the case may be, but public holidays involve the stoppage of work by the entire workforce within the period it would last. Nigeria observes many public holidays some of which, at times, are unplanned for by the people. This has attracted arguments for and against on the basis that these holidays are too many and detrimental to productivity and the national economy. Using secondary data, this paper examines the issue and opines that there is the need to review these public holidays with a view to halting their negative effects on the nation's economy. Also, measures to manage these holidays in line with constitutional provisions are espoused.

민간 항공기 운항승무원의 건강문제 분석(2016~2018) (Analysis of Health Problems among Airline Pilots in Korea (2016~2018))

  • The medical fitness of pilots is part of the civil aviation safety scenery. This study aimed to analyze the health problems occurring among Korean commercial pilots. Three data sources were used to identify the health problems: 7,574 aviation medical examination data, 5,400 sick leaves and 39 cases of a decrease in medical fitness during flight duty period and layover of Korean commercial pilots who have been working for K airline from Jan. 1, 2016 to Dec. 31, 2018. This study was retrospectively analyzed using SPSS 22 statistical program. Waiver of medical certificates was an average 11.9% of total issuance for 3 years, with a denial of 0.1%. The leading cause of denial of medical certificates was predominantly of cardiovascular cause (55.6%). Mild respiratory and digestive disease accounted for 82% of total sick prevalence and 68% of total sick days. The psychiatric and cardiovascular disease were ranked high according to the number of days lost per case. The most common cause of decrease in medical fitness during flight duty period was acute abdominal pain (36.4%) and musculoskeletal disorder (40%) when staying abroad. Aeromedical emphasis on minimizing cardiovascular risk remains appropriate. Major pilot health problems identified in this study should be considered in establishing pilot health promotion programs for aviation safety.

한국 상병수당제도 및 전달체계 설계연구: 주요 선진국과의 제도 비교를 중심으로 (Designing the Sickness Benefit Scheme in South Korea: Using the Implication from Schemes of Advanced Nations)

  • Currently, the South Korean Government does not provide sickness benefits from the National Health Insurance, which is different from most other Organization for Economic Cooperation and Development countries. The sickness benefit guarantees a part of lost income due to injuries or diseases. The purpose of this study is to propose a sickness benefit scheme for South Korea. To this end, we compare health care systems, sickness benefit schemes, and delivery systems of those in Germany, Japan, and Sweden, focusing on the seven categories: management authority, object, level of payment, duration of payment, qualification requirements, connection with paid sick leave of workplace, and financial resources, and as to delivery system, the six categories: the number of procedures, transferring document between institutions, whether or not utilizing electronic reporting system, applicant, and administrative convenience. Based on the implications derived from the case study, we propose the sickness benefit scheme and its delivery pathway and other details for South Korea. This study is first to propose the sickness benefit for health insurance in Korea with its level of details. More studies should follow with case studies of other countries, as well as productive debates to build a feasible and sustainable sickness benefit system in South Korea.
