대학생의 기록관 인식현황 및 이용 활성화 방안 연구 (A Study on a Method to Use Activation and Awareness on Archives of University Student)

  • 기록관은 대중에게 친근하게 다가가기 위한 노력으로 견학, 전시, 교육 등 다양한 기록정보서비스를 제공하고 있다. 하지만, 여전히 기록물과 기록관의 인식 부족 문제가 늘 제기되고 있다. 기록관의 이용을 활성화하기 위해서는 기록관의 이용자를 이해할 필요가 있다. 본 연구는 기록관의 잠재 이용자 중 대학생을 한 범주로 설정하고 기록물 및 기록관의 인식 조사를 통하여 대학생 집단을 이해하고자 하였다. 대학생 집단은 대학도서관과 대학기록관 등의 연구에서 특정한 이용자로 연구대상이 되어 왔는데, 물리적으로도 대학 기록관과 가장 가까이에 있기 때문에 향후 기록관의 적극적 이용자로 편입될 가능성이 높다고 판단하였다. 따라서 본 연구의 목적은 대학생 집단의 기록물 및 기록관의 인식 특징을 도출하여 기록관의 이용 활성화 방안 개발에 따른 특징을 반영한 고려사항 및 이용 활성화 방안을 모색하는 것이다. 이를 위하여 문헌조사 및 전북지역 J 대학교 대학생 182명을 대상으로 설문조사를 실시하였다. 설문 문항은 2003년도에 영국 MLA(Museums Libraries Archives Council)에서 진행한 프로젝트 일부로 MORI(Market & Opinion Research International)(2003)에 의해 진행된 설문조사 보고서와 조윤희(2008)의 연구에서 사용되었던 설문 문항을 참고하였다. 설문지 문항은 기록물 및 기록관에 대한 인식, 기록물 및 기록관 이용경험과 비이용 요인, 기록관 이용 요구사항, 기록관의 효율적인 홍보 방법 등 크게 4분야로 구성하였다. 조사결과 대학생 집단은 대부분 기록물에 대하여 무관심하며, 기록관의 인지도가 부족한 것으로 나타났다. 하지만 기록물을 관리하는 것은 매우 중요한 일로 인식하고 있었으며, 기록물은 역사적 가치에, 기록관은 정보적 가치에 의미를 두고 있었다. 또한, 향후 기록관의 이용에 대하여는 긍정적인 의향을 가지고 있으므로 적절한 서비스를 통하여 적극적인 이용자로 전환될 가능성이 높은 집단임을 알 수 있었다. 이를 바탕으로 대학생 집단의 기록관 이용을 활성화하기 위한 고려사항을 알아보았고, 이용자교육 측면, 프로그램개발 측면, 이용자 세분화 측면에서 기록관 이용 활성화 방안을 제언하였다.

암 환자의 발생 초기 의료비와 이에 영향을 미치는 요인 (Costs of Initial Cancer Care and its Affecting Factors)

  • Objectives : The purposes of this study is to estimate the cost of cancer care after its diagnosis and to identify factors that can influence the cost of cancer care. Methods : The study subjects were patients with an initial diagnosis one of four selected tumors and had their first two-years of cancer care at a national cancer center. The data were obtained from medical records and patient surveys. We classified cancer care costs into medical and nonmedical costs, and each cost was analyzed for burden type, medical service, and cancer stage according to cancer types. Factors affecting cancer care costs for the initial phase included demographic variables, socioeconomic status and clinical variables. Results : Cancer care costs for the initial year following diagnosis were higher than the costs for the following successive year after diagnosis. Lung cancer (25,648,000 won) had higher costs than the other three cancer types. Of the total costs, patent burden was more than 50% and medical costs accounted for more than 60%. Inpatient costs accounted for more than 60% of the medical costs for stomach and liver cancer in the initial phase. Care for latestage cancer was more expensive than care for early-stage cancer. Nonmedical costs were estimated to be between 4,500,000 to 6,000,000 won with expenses for the caregiver being the highest. The factors affecting cancer care costs were treatment type and cancer stage. Conclusions : The cancer care costs after diagnosis are substantial and vary by cancer site, cancer stage and treatment type. It is useful for policy makers and researchers to identify tumor-specific medical and nonmedical costs. The effort to reduce cancer costs and early detection for cancer can reduce the burden to society and improve quality of life for the cancer patients.

  • Park Tae Won is one of the physicians that participated in the treatment of King Injo in "承政院日記(Seungjeongwonilgi, official records of incidents during the Joseon Dynasty)". The purpose of this study is to look into the forms of actual medical treatment, social status and activities of the physicians at that time by organizing Park Tae Won's records as a physician and his other works. Park Tae Won was originally an acupuncturist for the royal medical department but he performed other activities such as actual diagnosis and drug prescriptions. More concretely it seems that he used methods mentioned in "鍼灸經驗方(Chimgugyungjeonbang)". Park Tae Won held concurrent positions as royal physician as well as provincial governor. It is suspected that physicians were named provincial governors of metropolitan areas so that they could be brought in immediately when in need and so that the capital centered medical service could be distributed to other provinces. We can also presume that the appointment of physicians as provincial governors was part of the government policy to efficiently distribute aid to the common people. From this study we can conclude that the physicians of that time did not commit solely to their jobs but also held important social positions that led the Joseon society.

위해사건 확인을 위한 증례검토지 개발 (Development of Case Review Form for Detecting Adverse Events)

  • Objectives: The purpose of this study is to develop a case review form for detecting adverse events through a medical records review in hospitalized patients in South Korea. Methods: To develop the case review form, several literatures were reviewed, first. Through the clinical expert meeting, screening criteria were selected and case review form was developed. Result: The Korean version of case review form consisted of the review form-1 for adverse event screening and form-2 for adverse event identification. The applied methodology for the case review form is determined according to the previous studies. For example, the method used in the first stage review is nurse review. Furthermore, the National Coordinating Council for Medication Errors Reporting and Prevention index is used to categorize disability, and a scale of 1 to 6 was used in the causation scores and preventability scores, respectively. Through the clinical expert meeting, a total of 41 screening criteria were selected. Conclusion: The Korean specific case review form was developed for detecting adverse events in hospitalized patients. The results from this study can be used in a large-scale study regarding the nationwide incidence of adverse events.

부산·경남 지역에서 전원 의뢰된 중증외상 분석을 통한 119 구급대의 환자 분류와 이송병원 선정에 대한 고찰 (Field Triage of Severely Injured Patients and Transportation by the EMS Rescue Group of Busan and Kyungnam, Korea: Is It Appropriate?)

  • Purpose: The objective of this study was to determine the appropriateness of Emergency Medical Service's (EMS's) triage and transport of severely injured patients in Busan and Kyungnam, Korea. Methods: The medical records of the Emergency Medical Information Center were retrospectively reviewed from January 1, 2010 to December 31, 2010. We identified the number of patients that should have been transferred from a secondary to a tertiary hospital according to the EMS field triage protocol. Results: In a total of 472 cases requests to be transferred to a third hospital were made through the Emergency Medical Information Center. Of these, 207 patients(43.9%) should have been transferred to a tertiary hospital according to the EMS field triage protocol. Among them, thirty-three(15.9%) patients satisfied step 1, 15(7.0%) satisfied step 2, and 117(56.5%) satisfied step 3. Twenty-three(11.1%) patients satisfied both steps 1 and 3. Conclusion: We found the triage by the EMS in the transfer of severely injured patients to a tertiary hospital to be inappropriate and re-education of EMS personnel regarding the EMS field triage protocol is needed. Because many patients are transferred from a secondary to a third hospital, we suggest changing the EMS field triage protocol to expand the severe injury criteria. A need exists to authorize secondary hospitals to transfer severely injured patients directly because there are no trauma centers in Korea.

The Role of Major Donors in Health Aid to the Democratic People's Republic of Korea

  • We investigated the major trends in health aid financing in the Democratic People's Republic of Korea (DPRK) by identifying the primary donor organizations and examining several data sources to track overall health aid trends. We collected gross disbursements from bilateral donor countries and international organizations toward the DPRK according to specific health sectors by using the Organization for Economic Cooperation and Development creditor reporting system database and the United Nations Office for the Coordination of Humanitarian Affairs financial tracking service database. We analyzed sources of health aid to the DPRK from the Republic of Korea (ROK) using the official records from the ROK's Ministry of Unification. We identified the ROK, United Nations Children's Fund (UNICEF), World Health Organization (WHO), United Nations Population Fund (UNFPA), and The Global Fund to Fight AIDS, Tuberculosis and Malaria (GFATM) as the major donor entities not only according to their level of health aid expenditures but also their growing roles within the health sector of the DPRK. We found that health aid from the ROK is comprised of funding from the Inter-Korean Cooperation Fund, private organizations, local governments, and South Korean branches of international organizations such as WHO and UNICEF. We also distinguished medical equipment aid from developmental aid to show that the majority of health aid from the ROK was developmental aid. This study highlights the valuable role of the ROK in the flow of health aid to the DPRK, especially in light of the DPRK's precarious international status. Although global health aid from many international organizations has decreased, organizations such as GFATM and UNFPA continue to maintain their focus on reproductive health and infectious diseases.

A nationwide study of children and adolescents with pneumonia who visited Emergency De­partment in South Korea in 2012

  • Purpose: Acute respiratory infection, particularly pneumonia, is the most common cause of hospitalization and death among children in developing nations. This study aimed to investigate the characteristics of children and adolescents with pneumonia who visited Emergency Department (ED) in South Korea in 2012. Methods: We analyzed National Emergency Department Information System (NEDIS) records from 146 EDs in South Korea for all pediatric patients aged ${\leq}18years$ who were diagnosed with pneumonia between January and December 2012. Results: Among 38,415 subjects, the male-to-female ratio was 1:0.8. Patients aged <12 months comprised 18.0% of the study population; those aged 1 to 3 years, 54.4%; those aged 4 to 6 years, 16.8%; those aged 7 to 12 years, 7.4%; and those aged 13 to 18 years, 3.4%. Presentation rates were highest in April, followed by January, March, and May. The hospital admission rate was 43.5%, of which 2.6% were in intensive care units. The mortality rate was 0.02%. Based on the International Classification of Diseases, 10th Revision, diagnostic codes, the types of pneumonia according to cause were viral pneumonia (29.0%), bacterial pneumonia (5.3%), Mycoplasmal pneumonia (4.5%), aspiration pneumonia (1.3%), and pneumonia of unknown origin (59.3%). Conclusion: Despite the limited data due to the ED data from the NEDIS lacking laboratory results and treatment information, this study reflects well the outbreak patterns among children and adolescents with pneumonia. Our results provide a basis for future studies regarding ED treatment for children and adolescents with pneumonia.

개인형 이동수단별 헬멧 착용 유무가 두부 손상에 미치는 영향: 일개 권역응급의료센터에 교통사고로 내원한 환자를 대상으로 (The effect of wearing a helmet on head injury risks among personal mobility vehicle riders: A study of patients who visited a regional emergency medical center due to traffic accidents)

  • Purpose: This study aimed to identify the factors that contribute to head injuries among drivers of personal mobility devices and provide insights into safety perceptions. Methods: This retrospective study analyzed data of 221 trauma patients obtained from electronic medical records and the National Emergency Department Information System (NEDIS) over one year, from August 1, 2021, to July 31, 2022. The patients, all in their 20s and 30s, presented to a single emergency medical center following personal mobility device accidents (motorcycles, electric scooters, and bicycles). Results: Among motorcycle riders, 18.2% were not wearing helmets, while the percentage of e-scooter riders not wearing helmets was 87.5%. Wearing a helmet was associated with a 71.2% lower likelihood of head injuries (OR=0.288, CI=0.163 to 0.509, p=0.000). Of the personal mobility devices, motorcycles had a 0.431 times lower odds ratio for head injury compared to e-scooters (p=0.009), and there was no significant difference between e-scooters and bicycles (p=0.776). Conclusion: It is imperative to strengthen safety regulations by mandating helmet use for riders of personal mobility devices. A system to enhance driving enforcement for electric scooters, which are increasingly popular among young adults, should also be established.

Evaluation of the Quality of Care among Hospitalized Adult Patients with Community-Acquired Pneumonia in Korea

  • Pneumonia is an important cause of morbidity and mortality. Since 2014, the Health Insurance Review and Assessment Service (HIRA) has assessed the overall quality of care among hospitalized adult patients with community-acquired pneumonia (CAP) provided by all medical institutions in Korea. A committee of the Korea Academy of Tuberculosis and Respiratory Diseases developed the hospital inpatient quality measures set for CAP consisting of eight core measures and five monitoring measures. The composite measure score was calculated. The medical records of hospitalized adult patients ages 18 years or more with CAP from October to December 2014 were evaluated. The data of 523 hospitals (42 tertiary hospitals [8.0%], 256 general hospitals [49%], and 225 hospitals [43.0%]) and 15,432 cases (tertiary hospitals, 1,673 cases [10.8%]; general hospitals, 8,803 cases [57.1%]; hospitals, 4,956 cases [32.1%]) were analyzed. We found large variations among institutions in terms of performance of care measures for CAP. For the composite measure score, the mean value was 66.7 (tertiary hospitals, 98.5; general hospitals, 79.2; hospitals, 43.8). Despite significant differences in measure scores between tertiary, general hospitals and hospitals, no significant differences were found in mortality between hospitals. Further studies are needed to determine the care measures appropriate for CAP.

서울시 1개 구 구립 보육시설의 영유아 건강관리 실태 (Health Problems and Health Services in Child Day Care Centers)

  • Purpose: To investigate the health problems and health services in child day care centers. Methods: Data were collected from 115 teachers at 16 child day care centers in Seoul city. A questionnaire was used to collect data, which were analyzed with the SPSS 12.0 program. Results: Most teachers had experienced various child health problems such as colds, hand․foot․mouth diseases, chicken pox, skin injuries, nasal bleeding, vomiting and diarrhea. Furthermore, they even experienced some serious ones including dysentery, measles, asthma and seizures, which demand professional skill. However, there were no registered nurses and most teachers requested that parents take a child home when these health problems happened. Only 31.3% of the child care centers had a teacher with CPR training. Approximately half of the centers kept child health records which included reports on allergic substances, and medical history but only 18.7% of the child care centers offered regular immunizations for the children. Conclusion: Various health problems were found in child day care centers. To maintain the children's health, there is a need to develop and make provisions for health services and programs in child day care centers.
