Discussion on Expansion and Establishment of Home Nursing Care (가정ㆍ방문간호사업의 확대 및 정착을 위한 시론(時論))

  • The purpose of study is to develop a home nursing care project model suitable for Korea's heath care system and policy directions for expansion and establishment of home nursing care. Method: The first, status of home nursing care program in medical institution and public health center was evaluated respectively in view of structure, process and performance. And then, issues of the program were analyzed in view of accessibility, safety, and sufficient supply. The second, demand projection of home nursing care according to income level and technical level of service that is needed to the subject was tried. In addition, the level of supply for home nursing care at present was estimated. The third, home nursing care program in advanced countries (Japan and U.S.A.) was examined. Result and Conclusion: Community-based home nursing care program is developed into public-operation model and private-operation model from above the sub-subject result. Functional network for referral system among related institutions is built up to meet various needs, regardless of accessibility to distance and economy. And prior settlements and policy directions for expansion and establishment of home nursing care are suggested.

An Analysis of Effects of Differential Coinsurance Policy and Utilization of Outpatients Care by Types of Medical Institutions (종별 의료기관 외래 경증질환 약제비 본인부담 차등정책 효과분석)

  • Background: The purpose of this study is to analyze the effects of differential coinsurance policy on prescription drug coverage of outpatients by types of medical institutions. Methods: In this study, we used a sample cohort database of the National Health Insurance Service and frequency analysis and marginal logistic regression model using generalized estimating equation were used for statistical analysis. Results: The summary on the major research is followed. First, about 16% of patients who used only tertiary or general hospital due to 52 ambulatory care sensitive conditions before policy implementation moved to hospitals and clinics. However, about 57% of them still use tertiary or general hospital. Second, the factors influencing the utilization of hospitals and clinics after the implementation of the policy were gender, age, and income level. As a result, the policy is effective to reduce the medical use of outpatient mental patients in tertiary or general hospital, but the effect is not significant. Conclusion: Therefore, in order to achieve the purpose of the policy for establishing the health care delivery system, it is necessary to adjust the co-payment so as to feel the burden on the co-payment when the outpatient for 52 ambulatory care sensitive conditions is used at the tertiary or general hospital.

Application of Cost-Volume-Profit Analysis in Decision-Making by Public Universities in Vietnam

  • This paper aims to examine the application of cost-volume-profit (CVP) analysis by public universities in Vietnam. In the context where Vietnam is gradually transferring financial autonomy to public universities, the conduct of a CVP analysis in relation to these public universities is particularly urgent. Research samples were collected in 2018 and 2019 by surveying Vietnamese public universities. After collection, the data is synthesized by excel file, conformity check, data cleansing and data analysis on SPSS software by tools such as Frequency statistics, price statistics, and means. The results show that: (1) universities used the CVP analysis in decision-making, (2) information related to the CVP analysis used for decision-making by administrators remained simplistic and lacked cost-control details, and (3) the application of the CVP analysis by university administrators for decision-making was neither comprehensive nor coordinated. The findings also show that, given the current conditions in Vietnam, increasing the governance in public universities is essential, as is contributing to reducing costs, increasing universities'income, providing the best service to students, and improving the quality of training. The study calls for the flexible application of the CVP analysis, which will provide information to help managers at Vietnamese public universities make the best decisions.

Prescription of antibiotics after tooth extraction in adults: a nationwide study in Korea

  • Objectives: This study aimed to understand the nationwide patterns of antibiotic prescription after tooth extraction in adult patients. Materials and Methods: This study analyzed dental records from the National Health Insurance Service-National Sample Cohort (NHIS-NSC) database on 503,725 tooth extractions performed in adults (≥19 years) during 2011-2015. Patient sex, age, household income, systemic disease (diabetes mellitus and hypertension), type of dental institution, region of dental institution, year of prescription, and type of tooth extraction procedure were considered. The antibiotic prescription rate and broad-spectrum antibiotic prescription frequency were analyzed using chi-squared tests. Factors affecting the prescription of broad-spectrum antibiotics were evaluated using multivariate logistic regression analysis. Results: The rate of antibiotic prescription after tooth extraction was 81.85%. Penicillin was most commonly prescribed (45.25%), followed by penicillin with beta-lactamase inhibitors (18.76%), metronidazole (12.29%), and second- to fourth-generation cephalosporins (11.52%). The proportion of broad-spectrum antibiotics used among all prescribed antibiotics was 45.88%. Conclusion: The findings of this study demonstrate that the rate of antibiotic prescription after tooth extraction is higher in Korea than in other countries. Furthermore, broad-spectrum antibiotics are used more frequently, which may indicate unnecessary drug prescription, an important contributor to antibiotic resistance.

Research on a Component-Based Method for Developing Electronic Documents for the Effective Reuse of XBRL Taxonomies (XBRL 택사노미의 효과적인 재활용을 위한 컴포넌트 기반 전자문서 개발에 관한 연구)

  • As XBRL applications for sharing the contents and semantics of business reports get activated, it is increasingly necessary to reuse components systematically by the standardization in the national level. The current standardization of Korean electronic documents follows the "Guideline for XML Documents Development", which aims to maximize the reusability of components by providing a business library as well as the UN/CEFACT core components methodology. XBRL takes the same approach with the standardization of electronic documents in that their components (concepts and their structures) are defined using XSD. In XBRL, however, concept are restricted to specific types and diverse relationship types can be defined for associating the concepts to themselves and other additional information. As a result, there are some issues that may not be solved by just applying the guideline to XBRL. This paper presents a basic method for applying the methodology to XBRL applications in order to realize the systematic reusing and standardization of XBRL documents in the national level. For case analysis, FSS (Financial Supervisory Service) income statement is employed to demonstrate the possibility of the method.

Maternal Efficacy Influenced by the Internet - Based on environment & actual condition of internet used by the mother - (어머니의 인터넷 이용 실태 및 인터넷 이용이 양육 효능감에 미치는 영향)

  • The purpose of this study was to research the actual state of mother's internet use and to explore the possibility of the internet use as a social support. The affect of mother's internet use on maternal efficacy is also examined. The Subjects were 250 mothers, who have their first child under 8 years old, in Seoul and Ilsan. Data were gathered through questionnaires. As predicted, mother's internet use was popular among young mothers. And, the main reason of internet use of mothers was to find some information on parenting and nurturing child. Moreover, mothers believed that they could get some social support from using internet in terms of informational, instrumental, and emotional service. However, it should be noted that mother's perceived level of support was significantly different, depending on mother's age, level of mother's education, household income, and the age of the first child. It was also found that mothers who received social support from internet showed higher material efficacy. Thus, we believe that internet use could be a new source of social support for the mothers of young children.

Attitudes and Preferences for Elderly Assisted Living according to Socio-economic Status (사회계층별 노인생활지원주택에 대한 태도와 선호)

  • The objectives of this study were 1) to analyze the attitudes about Elderly Assisted Living(EAL), 2) to consider the policy implications for elderly communal housing according to socio-economic status(income, occupation and education). The survey was undertaken from August to September, 2000 in Seoul and Kyunggi-Do(Ilsan, Bundang). The data were analyzed by SPSS Window program. The major findings were as follows: Most of the respondents prefered to live in EAL(over 56%), under 10 units and desired to be serviced(housework.leisure medical program.meal service). The upper class respondents preferred high-rise apartment style inner city or suburban and prefered paid services in EAL. They would willingly to pay over 800,000 won for living expenses and more than 200 million won for buying their own EAL. The middle class respondents prefered 5 stories multi-family house style with common facilities as a number. And they prefered to common facilities opening to public if the quality of facilities were better. The low class respondents prefered 5 stories multi-family house style and prefered to live within 30 minutes apart from their families and friends. They would pay under 300,000~600,000won for monthly living expenses and under 100 million won for their own EAL.

An Analysis of Economic Effect for Women-farmer's Center (여성농업인센터 운영사업의 사회적 편익 추정)

  • The purpose of this study was to calculate the economic effect of Women-Farmer's Center. Since 2001, The Ministry of Agriculture and Forestry has run Women-Farmer's Centers in which women-farmers can receive the care for their pre-schooling children, after-school learning service, and city-farm exchange, education, and counseling. In other words, Women Farmer's Center provides not only improvement of ease and quality of life of women-farmer's, but also spreading economic effect to community and country. By calculation based on business plan of 14 centers that run centers, total economic income effects are 2,784 million won, which consist of 1,265 million won for counseling, 146 million won for the care of infants and children, 139 million won after-school learning, 1,020 million won for education, and 214 million won for city-farm exchange program. The Women-Farmer's Center should be managed reasonably with government support so that Women-Farmer's Center will become as a base camp for young women farmers to participate in agriculture and rural community and increase its economic effect for the nation in the future.

  • Based on Smart Phone, this study added and implemented the mobile App to the existing IT agriculture system, which system automatically controlled green house's growth environment. Because the mobile App characterfully possesses high portability, limited resource, mobile wireless network support, enterprise mobile computing support, etc., the hybrid-App should be inevitably adopted. The hybrid App can minimize resource consumption for performing related duties and also, in spite of the mobile device's limits, it can derive high performance, in which facts the system can ensure the service-based architecture's reliance and also, implements the highly sophisticated functionality as well. Through the proposed system, the study expects to contribute to improve quality of life as well as to increase farmer's income through IT-based agriculture system development.

The Effect of Consumer Factors on the Use of Mobile Internet (소비자 특성이 무선인터넷 이용에 미치는 영향에 관한 연구)

  • At the end of 2002, about 91% of mobile telecommunications service subscribers have mobile phones with the mobile internet access function. However, despite the rapid spread of mobile internet phones, the number of real mobile internet users is very small. In this situation, this paper focuses on the effect of consumer demographics such as sex, age, job, etc. and consumer life-style on the use of mobile internet. This study tries to find the answers about the following questions ; 1) Which consumer demographic variables affect the use of mobile internet\ulcorner 2) How can we categorize the consumers with the mobile internet phones\ulcorner 3) What are the characteristics of categorized groups and is there any difference in using the mobile internet\ulcorner For this purpose, an off-line survey was conducted over 1,500 consumers with the mobile internet phones. This study concludes as follows ; The important demographic factors of the use of mobile internet are age, job, marriage, academic career and personal spending money. Totally viewed in the consumer demographics, the typical type of mobile internet users can be described as the young student. On the other hand, sex and family income variables do not significantly affect the use of mobile internet. And the mobile internet users can be categorized by the life-style into four distinct groups, which are named as the innovation oriented group, the practicality oriented group, the conservation oriented group, the ostentation oriented group. These findings show that the consumer life-style have various effects on the use of mobile internet.
