  • The effect of radiation on the steroidogenesis of rat ovary was investigated. For this purpose, female rats aged 7-8 weeks were exposed to a single dose of ${\gamma}$ radiation(320 rad or 800 rad) by the cobalt-60. At fourth(day of the first estrus) and eighth(day of the second estrus) days after irradiation, the concentrations of serum steroid hormones were determined by radioimmunoassays (RIA). The correlation between survival rate(Y) and radiation dose(X) was Y=-0.06X+100(r=0.89, n=10). Lethal dose$(LD)_{50(30)}$ and $LD_{20(30)}$ were 833.33rad and 333.33rad respectively. The weights of body and ovary was decreased significantly by the $LD_{50(30)}$ irradiation during the 4 days, but both weights were recovered at day 8. The serum levels of 17a-hydroxyprogesterone(170HP) and estradiol($E_2$) in the irradiated rats were not generally different from those of control. However, the levels of testosterone(T) and 19-norterstosterone(19NT) in the irradiated rat at $LD_{50(30)}$ and $LD_{20(30)}$ radiation doses were decreased, while progesterone levels in serum of the irradiated rats were slightly increased. These results suggest that irradiation may inhibit the first step of steroidogenesis, especially the conversion of P to T, in the rat ovary.

A high response to controlled ovarian stimulation induces premature luteinization with a negative impact on pregnancy outcomes in a gonadotropin-releasing hormone antagonist cycle

  • Objective: The goal of this study was to investigate the relationship between serum progesterone (P4) levels on the day of human chorionic gonadotropin (hCG) administration and the pregnancy rate among women undergoing controlled ovarian stimulation for in vitro fertilization (IVF) or intracytoplasmic sperm injection-embryo transfer (ICSI-ET) using a flexible antagonist protocol. Methods: This prospective study included 200 IVF and ICSI-ET cycles in which a flexible antagonist protocol was used. The patients were divided into five distinct groups according to their serum P4 levels at the time of hCG administration (0.80, 0.85, 0.90, 0.95, and 1.00 ng/mL). The clinical pregnancy rate (CPR) was calculated for each P4 interval. Statistically significant differences were observed at a serum P4 level of 0.9 ng/mL. These data suggest that a serum P4 concentration of 0.9 ng/mL may represent the optimal threshold level for defining premature luteinization (PL) based on the presence of a significant negative impact on the CPR. Results: The CPR for each round of ET was significantly lower in the PL group defined using this threshold (25.8% vs. 41.8%; p=0.019), and the number of oocytes retrieved was significantly higher than in the non-PL group ($17.3{\pm}7.2$ vs. $11.0{\pm}7.2$; p=0.001). Elevated serum P4 levels on the day of hCG administration were associated with a reduced CPR, despite the retrieval of many oocytes. Conclusion: Measuring serum P4 values at the time of hCG administration is necessary in order to determine the optimal strategy for embryo transfer.

Monitoring the Reproductive Status of Dairy Cows by Urinary Pregnanediol Glucuronide

  • This study was undertaken with the aim to establish a reliable radioimmunoassay (RIA) system for urinary pregnanediol glucuronide (PdG) and to employ it for monitoring the reproductive status of dairy cows. Urine and blood samples were collected from the Holstein cows both pregnant and non-pregnant. The samples were then investigated for evaluating the relationship between progesterone ($P_{4}$) in blood and PdG in urine adjusted with or without urinary creatinine basis. Biweekly urine collection was employed for three cows in estrous and those artificially inseminated, while urine from pregnant cows was collected on a monthly basis. P_{4}$ and PdG levels were measured by enzymeimmunoassay (EIA) and RIA techniques, respectively. Our results indicated the sensitivity of PdG for RIA being 35 pg/tube and the recovery rate of 100%. Urinary creatinine concentrations also fluctuated within a day, but change at midday was not noteworthy. Regardless of the time of urination the change in concentrations of PdG was relatively smaller and did not vary significantly. The urinary PdG concentration showed periodic changes as that with serum P_{4}$ levels during the cow's estrus cycle. The correlation coefficient rose when creatinine level in urine was adjusted but the change was also not significant. The concentrations of PdG during the luteal phase were detected between 8.2 and 17.4 ng/ml, three to five times higher than that in the follicular phase. The concentration of PdG from pregnant cows (21 days after conception) was three to four times higher than in the nonpregnant cows. Our finding suggests that the determination of urinary PdG could be reliably employed for early pregnancy detection. The urinary PdG level continued to raise until 30 days pre-partum while the concentration reached its peak at 30 ng/ml, after which it started to fall 18 to 30 days before parturition and finally fell to its nadir value one week after parturition. As the correlation coefficient between the urinary PdG and serum P_{4}$ was higher than that corrected by urinary creatinine it can be suggested that the adjustment is not needed. The concentrations of urinary PdG could be maintained stably for 2 days in urine samples stored at room temperature and extended to 8 days when the samples were pretreated by boiling for 30 minutes. In conclusion urinary PdG concentration even without the need for creatinine basis adjustment can be used directly for monitoring the reproductive status of dairy cows.

Relationship among Egg Productivity, Steroid Hormones (Progesterone and Estradiol) and Ovary in Korean Native Ogol Chicken

  • There exists considerable evidence that steroid hormones are involved in the regulation of ovulation rate and oviductal development in poultry. However, the effect of steroid hormones on egg productivity of Korean Native Ogol Chicken (KNOC) has yet to be studied. Therefore, this study was performed to relate the expression of steroid hormones, especially progesterone ($P_4$) and estradiol ($E_2$), with egg productivity during the laying period. Egg production and egg weight of 70 KNOC were recorded from 20 to 60wk. Blood was taken every 10 wk and serum $P_4$ and $E_2$ were measured by radioimmunoassay. Based on egg productivity and steroid hormones levels up to 60 wk, chickens were divided into two groups, high and low. Compared to the low egg production group, a significantly higher expression of $P_4$ at 30 wk was detected in the high group. Moreover, egg production in the high $P_4$ group significantly differed from that in the low group at 30 wk. On the other hand, a Significant difference (p<0.05) in $E_2$ expression was found between high and low egg weight groups at 30 wk. Although a significant difference in egg weight between two groups by $E_2$ was not detected, the high $E_2$ group showed a higher level of egg weight than the low $E_2$ group except for 25 wk. In the comparison of ovary weight and small yellow follicle number, the group with high egg productivity and steroid concentration showed greater levels than the low group. Taken together, the results indicate that $P_4$ is related to egg productivity whereas expression of $E_2$ is associated with egg weight in KNOC.

Reproduction-Nutrition Relationship in Dairy Buffaloes. I. Effect of Intake of Protein, Energy and Blood Metabolites Levels

  • Fifty one Nili-Ravi dairy buffaloes in their last two months of gestation were selected. After parturition, rectal examination of reproductive organs was carried out until the occurrence of the first oestrus (PEI). Milk samples were analyzed for milk progesterone levels (MPL). Ovulation (POI) was confirmed by rectal palpation and MPL. Feed and blood samples were collected fortnightly and analyzed. Body condition score (BCS) was recorded on a scale of 0 to 5. Crude protein (CP) intake varied among different seasons and correlated positively with serum urea levels, POI (p<0.01) and PEI (p<0.05). Excess CPI was lower in the group showing oestrus as compared to those remaining as anoestrus (p<0.05). The dietary ratio of crude protein - metabolizable energy (CP:ME) in the oestrus animals was narrow and constant, while the anoestrus animals had a widely fluctuating one. In normal breeding season (NBS) calvers, mean serum urea level (SUL) was lower than the low breeding season (LBS) calvers. SUL was positively correlated with PEI and POI (p<0.01). Up to six months postpartum, SUL were constantly higher in anoestrus than oestrus buffaloes. Mean metabolizable energy (ME) intake was lower in the NBS calvers than the LBS calvers (p<0.01). BCS and postpartum ovulation interval were correlated with ME intake (p<0.01). Prepartum ME intake was higher in oestrous as compared to anoestrous animals (p<0.05). Higher and lower ME intakes were associated with anoestrus, while a moderate energy intake was associated with a PEI of less than 75 days. Buffaloes with poor BCS belonged to the LBS calving group and most of the NBS calving buffaloes had good BCS. BCS was negatively correlated with PEI (p<0.01) and was higher in oestrous buffaloes than anestrus. It was concluded that excess intake of crude protein, associated with higher serum urea levels and low energy intake, associated with poor body condition, are the key factors for low reproductive efficiency. It may be corrected by adopting a proper feeding strategy.

  • In 105 patients with the past history of poor response to the previous controlled ovarian hyperstimulation(COH) due to poor follicular growth or premature LH surge, the effectiveness of pituitary suppression with gonadotropin-releasing hormone agonist(GnRH agonist) in IVF/GIFT program was evaluated in 112 cycles of COH using a combination regimen of Leuprolide acetate (Lupron TAP Pharmaceuticals, USA) and FSH/hMG or pure FSH from May to December, 1989 at SNUH. Starting on day 21 of the menstrual cycle(MCD #21, Day 1), Lupron (1.0mg/day, subcutaneous) was administered once a day till next MCD #3(suppression phase). After the confirmation of pituitary suppression, ovarian follicular growth was stimulated with FSH/hMG or pure FSH from MCD #3(Day + 1), and Lupron was continued with hMG or FSH until hCG administration (D 0) (stimulation phase). After suppression phase, serum E2 level decreased from 183.7${\pm}$95.1(Day 1) to 17.4${\pm}$12.3pg/ml (Day +1), and serum progesterone level from 19.17${\pm}$8.67 to 0.12${\pm}$0.05ng/ml. But there was no decresas in serum LH and FSH levels; LH from 12.74${\pm}$6.21 to 15.49${\pm}$4.93mIU/ml,FSH from 7.60${\pm}$3.84 to 8.58${\pm}$3.15 rnlU/ml. There was no occurrence of premature LH surge during COH. Eleven cycles(9.8%) were cancelled due to poor follicular growth during stimulation phase, and 3 cycles (3.0%) failed in the transvaginal oocytes fretrieval. Serum E2 level was 1366.8${\pm}$642.4 on D 0 and 1492.3${\pm}$906.9pg/ml on D+1. 7.00${\pm}$3.32 follicles(FD${\geq}$12mm) were observed on D 0, and 6.11${\pm}$4.15 oocytes were retrieved, with the oocyte retrieval rate per follicle of 95.0%. 3.59${\pm}$2.57 oocytes were fertilized and cleaved with the oocyte cleavage rate of 55.7%. In 83 IVF patients, 4.08${\pm}$2.39 embryos were transferred, and 16 pregnancies were obtained with the pregnancy rate per ET 2.39 mebryos were transferred, and 16 pregnancies were obtained with the pregnancy rate per ET of 19.3%. In 6 GIFT patients, 7.83${\pm}$3.31 oocytes were retrieved and transferred with maximum number of 6, but no pregnancy was obtained. When compared with the previous 108 cycles of COH using FSH/hMG or pure FSH regimen, the cancellation rate during COH was significantly decreased, and all the parameters of the outcome of COH including the pregnancy rate were increased. These data suggest that GnRH agonist therapy for pituitary suppression is an effective adjunct to the current gonadotropin regimens for COH in IVF/GIFT and can increase the probability of oocytes retrieval and pregnancy, especially in the previous poor responders.

Urinary Estrone Sulfate for Monitoring Pregnancy of Dairy Cows

  • The purpose of this study firstly was conducted to establish a radioimmunoassay (RIA) of estrone sulfate ($E_1S$), secondly to monitor the reproductive status of dairy cows using their urine samples. Urine and blood samples were collected in series within a day from four pregnant Holstein-friesian cows to evaluate the relationship between $E_1S$ levels in blood and urine with or without urinary creatinine basis. The urine was then collected biweekly from three cows in estrous and those artificially inseminated; collection from pregnant cows was made on a monthly basis. Results indicated that sensitivity for the $E_1S$ RIA was 5 pg/tube and the recovery rate was 100%. The daily urinary creatinine concentrations fluctuated within a day, but changes were slighter in midday, whereas the changes of concentrations of $E_1S$ in urine were relatively smaller. The concentrations of serum $E_1S$ during the estrous cycle were undetectable due to the limitation of assay, but the urinary $E_1S$ level could be measured with no obvious changes during the cycle. The urinary $E_1S$ levels increased remarkably around 7.7 to 8.3 ng/ml, 80 to 100 days after pregnancy but the serum $E_1S$ levels did not elevate until 120 to 150 days. The level of $E_1S$ increased gradually during pregnancy and eventually reached its peak before parturition at around 40 ng/ml and finally decreased to its basal level 2 days postparturition. During pregnancy, $E_1S$ concentrations of urine increased earlier than those in blood. The correlation coefficients between urinary and serum $E_1S$ concentration during pregnancy and postparturm were higher than those adjusted with creatinine (creatinine ratio). The concentrations of $E_1S$ in urine could be maintained unchanged for 8 days storing the samples in room temperature, which was extended to 8 days when the samples were pretreated by boiling for 30 minutes or treated with autoclave. In conclusion urinary $E_1S$ concentrations can be used directly for monitoring the pregnant status and fetal viability of dairy cows and can assist accurate confirmation of pregnancy in cows at least 80 to 100 days after insemination much earlier than by serum $E_1S$.

성호르몬과 안질환 (Sex Steroid Hormone and Ophthalmic Disease)

  • 성호르몬은 lacrimal system, corneal anatomy and disease, aqueous humor dynamics and glaucoma, crystalline lens and cataract, retinal disease를 포함한 안과영역의 다양한 부위에 영향을 끼칠 수 있다. Androgen은 전신에 걸친 sebaceous glands의 발달, 분화, 지질생성 등에 관여하므로 androgen deficiency는 meibomian gland의 기능장애와 건성안에서 증발을 더욱더 심화시킬 수 있는 것으로 알려져 있다. 반면에 estrogens은 sebaceous glands의 크기, activity, 지질생성을 감소시켜 meibomian gland에서는 androgen과 반대되는 작용을 할 가능성이 있다. 성호르몬은 또한 각막의 구조와 질병경과에도 영향을 줄 수 있는데, 각막이 월경주기 제2일째 및 배란주위기에 두꺼워지며, 폐경 여성에서 여성호르몬 치료 시 각막두께가 증가한다는 사실은 estrogen levels과 각막 두께와의 연관을 시사하는 소견이라고 하겠다. Fuch's dystrophy 역시 폐경 후 여성에서 더 흔히 관찰되어, 나이에 따른 호르몬 변화가 그 원인 중 하나로 지목되고 있으며, vernal keratoconjunctivitis 환자의 각막조직에서는 estrogen과 progesterone 수용체의 overexpresson이 발견되어 역시 이들 질환에서의 성호르몬의 역할을 시사한다고 할 수 있겠다. 성호르몬은 안압에도 영향을 미칠 수 있는데, intraocular pressure와 혈중 luteinizing hormone, follicular stimulating hormone (FSH), estrone, estradiol, progesterone, testosterone과는 연관이 없는 것으로 보고한 결과도 있으나, 혈중 progesterone의 경우 특히 임신중인 여성에서 안압과의 연관관계가 보고되었으며, 또한 폐경 여성에서 혈중 남성호르몬 농도가 높을수록 안압이 상승하는 경향이 있음이 보고되었다. 한편 'Female sex'는 모든 종류의 백내장의 위험요소인 것으로 알려져 있다. 백내장으로 진단받은 여성의 경우 호르몬 측정 시 연령등을 보정하고도 폐경기에 가까운 호르몬 수치를 보였으며, 이들 백내장 환자군내에서는 estradiol과 FSH 농도 사이에 유의한 음의 상관관계가 보고되었다. 또한 DHEAS의 경우 백내장 위험도를 감소시키는 것으로 보고되었고, 그 기전으로 전구호르몬인 DHEA의 역할을 제시되고 있는데. 즉 백내장의 주요 기전 중 하나인 oxidative stress를 감소시키는 antioxidant activity를 DHEA가 보이기 때문일 것으로 추정되고 있다. 성호르몬은 망막 내 혈관의 흐름에도 영향을 줄 수 있는 것으로 알려져 있는데, 안구 내 혈류에서 여성호르몬의 긍정적인 효과와 남성호르몬의 부정적인 효과가 보고되었고, age-related macular degeneration (AMD)의 경우, estradiol이 ER-$\beta$ 수용체를 통해 보호효과가 있음이 보고되어, ER-$\beta$ 수용체가 향후 AMD 치료에서 치료적 target이 될 수도 있을 것으로 알려지고 있다. 또한 DHEAS의 경우 AMD 위험도를 증가시킬 가능성이 있으며, 당뇨로 진단받은 환자에서는 혈중 testosterone 농도는 당뇨병성 망막병증의 진행과 연관이 있는 것으로 보고되었다. 성호르몬은 안과영역의 다양한 질환과의 연관관계는 아직 추가연구가 필요한 실정이나 다양한 성호르몬이 안질환의 발생 및 경과에 영향을 줄 가능성이 있으며, 이와 같은 연관관계가 규명된다면 추후 호르몬을 통해 질환의 경과 및 예방의 가능성이 제시될 수도 있을 것으로 사료된다. 특히 폐경이후 다양한 안질환이 증가한다는 기존의 연구결과들을 고려 시, 안과영역에 있어 폐경기 여성호르몬 치료의 긍정적인 역할에 관해서는 지속적인 관심이 요구된다고 하겠다.

우울증을 수반한 폐경기 여성환자에서의 항우울제와 경피 에스트라디올 호르몬의 병합치료 효과 : 우울증상 및 신체증상과 혈장 Gonadal Hormone, 5-Hydroxyindoleacetic Acid 농도를 중심으로 (Effects of Addition of Transdermal Estradiol to Antidepressants Treatment in Postmenopausal Women with Depression : On the Depression, Physical Symptoms and Plasma Hormonal Status, 5-Hydroxyindoleacetic Acid Levels)

  • A on-off study was designed to evaluate the effects of addition of transdermal esrtradiol to tricyclic antidepressants on depression level, vasomotor symptom(hot flush), sexual functions and hormonal status, plasma 5-hydroxyindoleascetic acid(5-HIAA) level in postmenopausal women with depression. Plasma level of estradiol, progesterone, LH, FSH, prolactin and 5-HIAA was measured by Time-resolved fluoroimmunoassay and HPLC(High Performance Liquid Chromatography). To asses their symptoms, the BDI(Beck Depression Inventory) and modified symptom scale, extracted from women's health questionnaire were used. Depression score, sexual function score were decreased by the last 4-weeks of addition of transdermal estradiol to antidepressant treatment, not Significant, but vasomotor symptom (hot flushes) score was decreased significantly(p<0.05) by the last 4-weeks of the given treatment. Thus, during addition of transdermal estradiol to antidepressants treatment, only vasomotor symptom(hot flushes) was improved significantly, but depression level was not changed in postmenopausal women with depression. Plasma FSH, estradiol and prolactin level was not changed in postmenopausal women with depression. Plasma FSH, estradiol and prolactin levels were increased by the last 4-weeks of the treatment. There were not significant correlations between clinical symptoms and plasma hormonal status and 5-HIAA level in baseline. After the last 4-weeks of transdermal estradiol treatment, the change of depression score was correlated significantly with change of serum prolactin and 5-HIAA level and the change of vasomotor symptom score was correlated significantly with the change of plasma prolactin level.

Prognostic Significance of Circulating Tumor Cells and Serum CA15-3 Levels in Metastatic Breast Cancer, Single Center Experience, Preliminary Results

  • Background: Breast cancer is the second leading cancer causing death in women. Circulating tumor cells are among the prognostic factors while tumor markers are of diagnostic value and can be used for follow-up. The aim of this study was to investigate the correlation between the prognostic significance of the serum CA15-3 levels, number of circulating tumor cells and histopathological tumor factors. Materials and Methods: Thirty patients recently diagnosed with breast cancer were included in the study. Number of circulating tumor cells and serum CA15-3 level were assessed when metastasis was detected and diagnostic value was assessed. Presence of associations with estrogen and progesterone receptors, c-erbB2, Ki-67 proliferation index and histological grade were also evaluated. Results: Median overall survival of the patients with serum CA15-3 levels of >108 ng/dl was 19 months whereas for those with a low serum level it was 62 months. Median overall survival for CTC ${\geq}5$ vs CTC<5 patients was 19 months and 40 months respectively. The difference between the two groups was statistically significant. Conclusions: Prognostic significance of the CTC count and CA15-3 levels in metastatic breast cancer patients was demonstrated.
