Effects of Aspirin on the Pathogenesis of Acute Lung Injury in Rats Subjected to Hemorrhage (출혈성 쇼크로 인한 급성 폐손상의 발병기전과 아스피린의 효과)

  • Background : For unknown reasons, the serum ferritin concentrations are higher in patients with acute lung injury. A pretreatment with aspirin reduces the acute lung injury in rats subjected severe hemorrhage, and increases the rate of ferritin synthesis in vitro. This study investigated the effect of aspirin on the serum ferritin changes in rats subjected to severe hemorrhage. Methods : Hemorrhagic shock was induced by withdrawing blood (20 ml/kg of B.W.) through the femoral artery for 5 min. The rats were pretreated with aspirin (10 mg/kg, i.v.) 30 min before hemorrhage. Results : The protein content and leukocyte count in the bronchoalveolar lavage fluid, lung tissue myeloperoxidase activities were significantly higher after hemorrhage. The aspirin pretreatment prevented these changes. The serum and lavage fluid ferritin concentrations were elevated higher after hemorrhage. These were also attenuated by the aspirin pretreatment. Conclusion : The changes in the serum and lung lavage ferritin level might be closely related to the severity of hemorrhage-induced acute lung injury. Therefore, the serum and lavage ferritin concentrations can be a useful biomarker for patients with precipitating conditions.

Iron Nutriture and Related Dietary Factors in Apparently Healthy Young Korean Women(1) : Comparison and Evaluation of Blood Bichemical Indices for Assessment of Iron Nutritional Status (우리나라 젊은 성인 여성의 철분영양상태와 이에 영향을 미치는 식이요인 분석(1) : 혈액의 철분 영양 상태 평가 지표의 비교 및 분석)

  • To evaluate iron nutritional status and to compare biochemical indices used for evaluation of iron nutriture, fasting blood sample was taken from 96 Korean female college student and hemoglobin(Hb) concentration, hematocrit(Hct), serum iron, total iron binding capacity(TIBC) and serum ferritin concentration were measured and transferrin(TF) saturation was calculated. Prevalence of iron deficiency varied from 4.2% when judged from Hb concentration and Hct to 38.5% and 40.6% when TIBC and serum ferritin concentration were used. TIBC was negatively correlated with serum ferritin(r=0.4561, p<0.001), while other indices showed positive but less significant correlations(p<0.05). Regarding iron status judged by serum ferritin was regarded as true iron status, the present cutoff points of Hb, Hct, serum iron, TF saturation were very insensitive in identifying the subjects with iron deficiency. Only TIBC was suitable for detecting iron deficiency in the study subjects. It is concluded from the study results that iron deficiency in young Korean adult women would be significantly underestimated with presently used biochemical measurements except TIBC and new cutoff points of these indices were proposed based on sensitivity, specificity measured prevalence, and positive and negative predictive values.

  • Iron deficiency anemia (IDA) is the world's most common nutritional problem. It is characterized by a low hemoglobin (Hb) level and low iron status. A study was conducted to investigate the incidence of iron deficiency anemia in day scholar girls belonging to different socioeconomic strata at Punjab University, Lahore. Iron status of the subjects was estimated by measuring Hb, hematocrit (Hct), red blood cell (RBC) count, mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCR), serum iron, serum ferritin, total protein and albumin. Results indicated that females belonging to low socioeconomic strata had lower values for Hb, Hct, RBC count, total protein and albumin. Serum iron, serum ferritin, MCV and MCH values fell within the normal range for all of the socioeconomic groups. However, serum iron and ferritin varied with socioeconomic status and higher-income groups had significantly higher serum iron and ferritin. It was concluded that anemia may develop due to poor intake and absorption of iron and that those in the low-income bracket are the most affected group.

Iron Nutritional Status of Female Students in Kangnung National University (강릉대 일부 여대생의 철분영양상태에 관한 연구)

  • To evaluate iron nutritional status of female college students, fasting blood samples were taken from 76 female students of Kangnung National University. Hemoglobin(Hb), hematocrit(Hct), serum iron(Fe), total iron binding capacity(TIBC) and serum ferritin concentrations were measured and transferrin saturation was calculated. Mean values for Hb, Hct, Fe, TIBC, TS and serum freeitin were 13.64$\pm$1.42g/dl, 40.99$\pm$4.31%, 103.0$\pm$33.3$\mu\textrm{g}$/이, 395.3$\pm$9.07$\mu\textrm{g}$/dl, 26.58$\pm$9.07$\%$and 26.76$\pm$17.5ng/ml, respectively. Prevalence of iron deficiency greatly varied by indices from 6.8% when judged by Hct to 26.0$\%$ by serum ferritin concentration. The Hb concentration was positively correlated with hematocrit (r=0.5402), serum iron(r=0.2819) and transferrin saturation(r=0.2777)(p<0.05). on the other hand, serum ferritin concentration showed significantly negative correlation with TIBC(r=-0.3196). Two-day dietary intake records were collected from subjects to estimate mean daily iron intake and bioavailability of dietary iron. Mean daily intake of iron was 13.15mg and heme iron intake was 0.83mg which was 6.4% of total iron intake. Total absorbable iron calculated by the method of Monsen was 1.27mg and bioavailability of dietary iron was 9.6%. In the light of high prevalence of iron deficiency based of serum ferritin concentration and low bioavailability of iron in the diet, guidelines about diet should be made to increase the content and bioavailability of iron in the diet if female college students.

Significance of Serum Ferritin in Multiple Trauma Patients with Acute Respiratory Distress Syndrome (다발성 외상 환자에서 발생되는 급성 호흡 곤란 증후군의 예측 인자로서 혈청 페리틴의 의의)

  • Purpose: Clinically, acute respiratory distress syndrome (ARDS) occurs within 72 hours after acute exposure of risk factors. Because of its high fatality rate once ARDS progresses, early detection and management are essential to reduce the mortality rate. Accordingly, studies on early changes of ARDS were started, and serum ferritin, as well the as injury severity score (ISS), which has been addressed in previous studies, thought to be an early predictive indicator for ARDSMethods: From March 2003 to March 2005, we investigated 50 trauma patients who were admitted to the intensive care unit in Dongguk University Medical Center, Gyeongju. The patients were characterized according to age, sex, ISS, onset of ARDS, time onset of ARDS, serum ferritin level (posttraumatic $1^{st}\;&\;2^{nd}$ day), amount of transfused blood, and death. Abdominal computed topography was performed as an early diagnostic tool to evaluate the onset of ARDS according to its diagnostic criteria. The serum ferritin was measured by using a $VIDAS^{(R)}$ Ferritin (bioMeriux, Marcy-1' Etoile, France) kit with an enzyme-linked fluorescent assay method. For statistical analysis, Windows SPSS 13.0 and MedCalc were used to confirm the probability of obtaining a predictive measure from the receiver operating characteristics (ROC) curve. Results: The ISS varied from 14 to 66 (mean: 33.8) whereas the onset of ARDS could be predicted with the score above 30 (sensitivity: 90.0%, specificity: 60.0%, p<0.05). On the posttraumatic $1^{st}$ day, the serum ferritin levels were measured to be from 31 mg/dL to 1,200 mg/dL (mean: 456 mg/dL), and the onset of ARDS could be predicted when the value was over 340 mg/dL (sensitivity: 80.0%, specificity: 65.0%, p<0.05). On the posttraumatic $2^{nd}$ day, the serum ferritin levels were measured to be from 73 mg/dL to 1,200 mg/dL (mean: 404 mg/dL), and the onset of ARDS could be predicted when the value was over 627 mg/dL (sensitivity: 60.0%, specificity: 92.5%, p<0.05). The serum ferritin levels and the ISS were significantly higher on the posttraumatic $1^{st}$ and $2^{nd}$ day in the ARDS group, suggesting that they are suitable indices predicting the onset of ARDS, however relationship between the serum ferritin levels and the ISS was not statistically significant. Conclusion: In this study, we discovered increasing serum ferritin levels in multiple- trauma patients on the posttraumatic $1^{st}$ & $2^{nd}$ day and concluded that both the serum ferritin level and the ISS were good predictors of ARDS. Although they do not show statistically significant relationship to each other, they can be used as independent predictive measures for ARDS. Since ARDS causes high mortality, further studies, including the types of surgery and the methods of anesthesia on a large number of patients are essential to predict the chance of ARDS earlier and to reduce the incidence of death.

A Study on Serum Ferrum, TIBC, and Ferritin's Circadian Rhythm in Normal Adults (정상 성인의 혈청 철과 TIBC 및 Ferritin의 Circadian Rhythm에 대한 연구)

  • Purpose: The purpose of this study was to identify characteristics of serum ferrum, TIBC and ferritin's circadian rhythm in normal adults and to prepare a standard to determine the examination material extraction time. Method: Nine women and ten men made up the convenience sample for this study they were from the staff of D university hospital and students in D medical School located in K city who met the qualifications for inclusion in the sample. The value of serum ferrum, TIBC and circadian rhythm were calculated as follows : First. each variable's amplitude. the acrophase and average were measured for a 24 hour cycle using the cosinor method, and then each person's rhythm was analyzed. Results: There were significant serum iron circadian rhythm for both men and women (p<.05). For the men, mesor was $105.91{\mu}g/dl$. amplitude was $29.52{\mu}g/dl$, and the acrophase was 9.76 hour. For the women, mesor was $108.17{\mu}g/dl$, amplitude was $28.09{\mu}g/dl$, and the acrophase was 11.42 hour The rhythm change of TIBC was only significant for the women (p<.05), mesor was 383.39mg/dl, amplitude was 60.29mg/dl. and the acrophase was 14.93hour. As for the circadian rhythm of the ferritin, there are no diurnal variation in either sex, men were between 134.0ng/ml and 137.4ng/ml, and women, between 29.1ng/ml and 30.1ng/ml. Conclusion: To help diagnose the boundary line between normal or deficiency in iron, measurement should be carried out at a fixed time in the morning and evening, or a more proper time would be in the afternoon at the time when the width of amplitude is the least.

Serum Iron Concentration of Maternal and Umbilical Cord Blood during Pregnancy (임신기 모체 혈청과 신생아 제대혈청의 철분함량)

  • Anemia diagnosed early in pregnancy is associated with increased risks of low birth weight and preform delivery. The purposes of this study were to assess the maternal iron status during pregnancy and to evaluate the relationships between the iron indices of maternal-umbilical cord serum iron and ferritin levels and pregnancy outcomes. Dietary intakes of the pregnant women were estimated by 24 hour-recall (3 times). Serum iron and ferritin levels in maternal blood and umbilical cord were measured at 1st-, 2nd-, 3rd- trimester and delivery, respectively. The mean of maternal se겨m iron levels of the trimester and delivery were $124.27\;{\mu}g/dl,\;97.03\;{\mu}g/dl,\;94.32\;{\mu}g/dl,\;and\;145.53\;{\mu}g/dl$. Those maternal levels were significantly lower than that of umbilical cord blood ($222.59\;{\mu}g/dl$). Serum ferritin levels of maternal trimester and delivery were 22.68 $22.68\;{\mu}g/l,\;11.09\;{\mu}g/l,\;14.18\;{\mu}g/l,\;and\;\;24.54\;{\mu}g/l$, which were significantly lower than those of umbilical cord blood ($184.35\;{\mu}g/l$) (p < 0.0001). This prevalence of anemia of total subjects was $30.3\%$ by WHO criteria (Hb < 11.0 g/dl, Hct < $33\%$). Iron levels of 2nd-trimester was significantly higher in the normal group than in the anemia group. And ferritin levels of 3rd-trimester and delivery was significantly higher in the normal group than in the anemia group. Therefore, we suggest for successful pregnancy outcome and delivery differential iron supplementation programs will be carried out with individual Pregnant women on the basis of pre-Pregnancy nutrition. Study on the Prevalence of Iron-deficient Anemia in Korean Preschool Children (학령 전 아동의 철 결핍성 빈혈 영양 상태 판정에 관한 연구)

  • The objective of this study was to identify the nutritional risk factors by blood analysis, in 1,083 preschool children of age 3 to 6 years. The frequency of anemic children was 7.3% with Hb<11.1 g/dL, 29.9% with ferritin<20 ng/mL, and 16.7% with transferrin Fe saturation(%)<15%. The prevalence of anemia in these children were 12.8% for those with MCV<79 fL, and it was 71% for those with TIBC> $400{\mu}g/dL$. Serum ferritin concentration was 20 ng/mL in the normal children. Thirty two percent of the children had anemia with Hb<12 g/dL, which is below the normal range of Hb. Exactly 15.4% of the children had serum Fe concentration of $60{\mu}g/dL$. The transferrin Fe saturation of the children (16.3%) was >15%. The serum ferritin concentration showed low correlations with Hb, Fe, transferrin Fe saturation, and MCV. The transferrin Fe saturation, higher Hb concentration, MCV, and Hct values were increased significantly. Consequently, iron-deficiency anaemia was thus defined as having Hb concentration <12 g/dL accompanied by ferritin concentration <20 ng/mL or Hct <33%.

Effects of Kimchi Consumption on Iron Status in Adult Male Volunteers (김치의 섭취가 성인 남성의 철분영양상태 지표에 미치는 영향)

  • The aim of this was to investigate whether the regular consumption of kimchi influences the iron status (RBC , Hb, Ht, MCH, MCV, MCHC, transferrin , serum iron, and ferritin) in volunteers. Healthy male adults(n=12) took part in the study subdivided into the control Ⅰ-phase(for 2weeks), kimchi-phase (for 4 weeks), and control Ⅱ-phase(for 2 weeks). In addition to their normal diet, participant consumed 300g of lactic acid fermented Chinese cabbage kimchi daily for four weeks. In the control Ⅰ and control Ⅱ phases, the participants kept up their normal diets without consuming any fermented foods. Dietary intakes were recorded for 3 consecutive days in each phase, with the aid of household measures. Every two weeks. blood specimens were analysed. Significant differences(p<0.05) between the phases were found in MCHC, and transferrin in blood were not significantly changed during kimchi consumption. However, serum iron and ferritin levels were significantly increased(p<0.05) during kimchi consumption, achieving the highest levels in the fourth week of the kimchi components(ascrobic acid, sulfer compound, organic acid, capsaicin, gingerol , allicin). Because of lacticacid fermented kimchi's potential to prevent anemia , the consumption of this food can be recommended.

A Study on the Quality Control of 2-site Immunoradiometric Assay of Serum Ferritin (혈청(血淸) ferritin 측정(測定)의 정도관리(精度管理)에 관(關)한 고찰(考察))

  • A 2-site immunoradiometric assay for serum ferritin was evaluated with commercially available kit. The assay required 6 hours. The slope of the standard curve kept up ideal range with the calculation of maxium binding instead of total dose until expire date. The stage II washing was more important than the stage I washing on the modified washing procedure as the bead keeping to remain in the tube. With this modified mothod, three times of tube. washing was sufficient to reduce the significant errors The measured values of serially diluted sample with standard diluting buffer was proportional to the predicted values. In the experiment of serum effect on the assay. a linear relationship from 5 to 50% serum, but beyond 50% there was reduction in measured ferritin concentration. It has a sensitivity of 2.77 ng/ml, within-assay precision (CV) of 8.0%, and between-assay reproducibility(CV) of 7.4% (mean 174.8 ng/ml).
