Sensory Neural Hearing Loss after Concurrent Cisplatin and Radiation Therapy for Nasopharyngeal Carcinoma (비인강암 환자에서 Cisplatin과 방사선 병합치료시 발생하는 감각신경성 청력손실)

  • Objectives: It is expected that the combined cisplatin and radiation therapy for nasopharyngeal carcinoma produces more sensory neural hearing losses compared to radiation therapy alone. The purpose of this study was to evaluate the incidence of sensory neural hearing losses after concurrent cisplatin and radiation therapy for nasopharyngeal carcinoma. Materials and Methods: From Jun. 1994 to Mar. 1997, 10 patients were available for this study with the following eligibility criteria: 1) The patients received concurrent cisplatin and radiation therapy for nasopharyngeal carcinoma. 2) There was no pre-existing auditory disease except serous otitis media due to nasopharyngeal carcinoma. 3) They had normal sensorineural hearing function on the pretreatment pure tone audiogram. 4) Pure tone audiograms were performed at least one time after treatment between 6months to 1 year follow-up without local recurrence. Results: At 1 year follow-up, 3 patients complained of decreased hearing capacity. Sensorineural hearing losses were developed in 15 ears out of 20(75%) and were more frequent and severe in high frequency area on pure tone audiogram. Conclusion: The concurrent cisplatin and radiation therapy for nasopharyngeal carcinoma may produce sensorineural hearing losses more frequently compared to historical data of radiation therapy alone and close evaluations with regular audiograms are necessary even in patients without symptoms and signs of hearing impairment.

A Tie-Over Dressing Using a Silicone Tube to Graft Deep Wounds

  • Background The most common cause of skin graft failure is the collection of blood or serous fluid underneath the graft. In our study, we describe the use of silicone tube for tie-over dressing to secure the skin graft margins with the aim of decreasing loss of the skin graft, particularly in grafting of deep wounds. Methods Between March 2008 and July 2011, we used this technique in 17 patients with skin defects with depths ranging from 3.5 to 8 mm (mean, 5.5 mm). First, the skin graft was sutured with 3/0 silk suture material from its corners. Then, a silicone round drain tube was sutured with 3/0 absorbable polyglactin 910 over the margins of the graft. Finally, long silk threads were tied over the bolus dressing, and the tie-over dressing was completed in the usual fashion. Results The mean follow-up was 7 months (range, 2-10 months) in the outpatient clinic. Graft loss on the graft margins due to hematoma or seroma was not developed. The results of adhesion between the graft and wound bed peripherally was excellent. Conclusions In our study, we suggest that use of a silicone tube for additional pressure on the edges of skin grafts in case of reconstruction of deep skin defects.

Resonance Characteristics of the Pears for Exporting for Optimum Packaging Design (적정 포장설계를 위한 수출용 배의 공진특성)

  • Shock and vibration inputs are transmitted from the vehicle through the packaging to the fruit. Inside, these cause sustained bouncing of fruits against each other and container wall. These steady state vibration input may cause serous fruit injury, and this damage is particularly severe whenever the fruit inside the package is free to bounce, and is vibrated at its resonance frequency. The determination of the resonance frequencies of the fruit and vegetables may help the packaging designer to determine the proper packaging system providing adequate protection for the fruit, and to understand the complex interaction between the components of fruit when they relate to expected transportation vibration inputs. Instrumentation and technologies are described for determining the vibration response characteristics of the pears for exporting with frequency range from 10 to 200 Hz, sweep rate of 1 octave/min, sweep method of logarithmic up and down and acceleration levels of 0.2, 0.4, 0.6, 0.8 and 1.0 G considering the domestic transportation environment. The resonance frequency of the pears ranged from 49.04 to 87.16 Hz and the amplitude at resonance was between 0.96 and 4.02 G in test frequency band and acceleration level. The resonance frequency and amplitude at resonance frequency band of the pears decreased with the increase of the sample mass. The multiple nonlinear regression equations for predicting the resonance frequency of the pears were developed using the independent variables such as mass, input acceleration.

  • This experiment was performed to investigate apoptosis during undergoing pathogenesis of avian infectious bronchitis virus(IBV)-infected chicks. Sixteen days old chicks were infected with IBV, Massachusetts-41 strain(M-41, 10$^4$-10$^{5}$ EID$^{50}$ ) experimentally, they were autopsied to remove trachea, lung, kidney and cloacal bursa at 6hr, 12hr, 1 day, 3 day and 7 day post infection(PI) respectively for H-E and TUNEL staining. Grossly, mild serous, catarrhal exudate was observed in the trachea, nasal passages and sinuses nasal from 4 day PI. The cloacal bursa was swollen from 3 day PI. Histopathologically, the trachea was seen mild cellular infiltration, edema of the mucosa and submucosa, vascular congestion and mild hyperplasia of the epithelium from 6 hr PI and the changes were seen a little more severely on 7 day PI. It was observed that the cloacal bursa was getting more and more hyperplasia through the experiment. The nuclei degeneration were shown in the kidney on 7 day PI. No specific changes were seen in the lung. In TUNEL analysis, apoptotic cells showed sharp increasing at 12 hr PI and reaching a maximum on 1 day PI in the trachea, lung, kidney and cloacal bursa. And then apoptotic cells decreased gradually returning to a level of the control by 7 day PI in all the removed organs.

TREATMENT OF THE CHILD WITH ROBINOW SYNDROME UNDER GENERAL ANESTHESIA : A CASE REPORT (Robinow 증후군 환아의 전신마취를 이용한 치료증례보고)

  • Robinow syndrome or fetal face syndrome is a rare inherited disorder characterized by short stature, mesomelic brachymelia, hypoplastic genitalia, and a typical facial appearence ("fetal face") with frontal bossing, hypertelorism, ear abnormalities, a short upturned nose, long philtrum, micrognathia, and macrocephaly. Intraoral features have included quite a few dental cavities, crowding, hypoplastic uvula, cleft lip or/and cleft palate, gingival hyperplasia, alveolar hyperplasia, enamel hypoplasia, delayed eruption, and congenital missing of the permanent teeth. We report on a 10 years old girl with Robinow syndrome. The patient had most of the typical anomalies of the syndrome and negative family history but, in addition, had mental retardation, hearing loss, and serous otitis media. Intraoral findings included dental cavities, crowding, hypoplastic uvula, repaired cleft palate, and mouth breathing. Dental treatment and V-tube insertion(by dept. of ENT) were performed under general anesthesia. In all cases of Robinow syndrome, thorough evaluation and united treatments with medical specialists should be performed.

Intra-Peritoneal Cisplatin Combined with Intravenous Paclitaxel in Optimally Debulked Stage 3 Ovarian Cancer Patients: An Izmir Oncology Group Study

  • Background: The advantage of intra-peritoneal (IP) chemotherapy (CT) in the initial management of ovarian cancer after cytoreductive surgery is well known. The feasibility and toxicity of a treatment regimen with an IP + intravenous CT (IPIVCT) for optimally debulked stage III ovarian cancer were here evaluated retrospectively. Materials and Methods: A total of 30 patients were treated in our institution between October 2006 and February 2011. Patients received IV paclitaxel $175mg/m^2$ over 3 hours followed by IP cisplatin $75mg/m^2$ on day 1; they also received IP paclitaxel $60mg/m^2$ on day 8. They were also scheduled to receive 6 courses of CT every 21 days. Results: The median age of the patients was 55 years (35-77), and the majority had papillary serous ovarian cancer (63.3%). The patients completed a total of 146 cycles of IPIVCT. Twenty-eight were able to receive at least three cycles of IPIVCT and 18 (60%) completed the scheduled 6 cycles. Two patients discontinued the IPIVCT because of toxicity of chemotherapy agents and 6 had to stop treatment due to intolerable abdominal pain during IP drug administration, obstruction and impaired access. Grade 3/4 toxicities included neutropenia (6 patients; 20%), anemia (2 patients; 6.7%) and nausea-vomiting (2 patients; 6.7%). Doses were delayed in 12 cycles (8%) for neutropenia (n=6), thrombocytopenia (n=3) and elevated creatinine (n=3). Drug doses were not reduced. The median duration of progression-free survival (PFS) was 47.7 months (95%CI, 38.98-56.44) and overall survival (OS) was 51.7 months (95%CI, 44.13-59.29). Two and five-year overall survival rates were 75.6 % and 64.8%, respectively. Conclusions: IPIVCT is feasible and well-tolerated in this setting. Its clinically proven advantages should be taken into consideration and more efforts should be made to administer IPIVCT to suitable patients.

An Experimental Study on Artificial Infection of Olive Flounder Paralichthys olivaceus by Streptococcus parauberis Using Different Injection Sites (넙치(Paralichthys olivaceus)의 Streptococcus parauberis 인위감염을 위한 공격실험 방법에 관한 연구)

  • Streptococcosis in the olive flounder Paralichthys olivaceus can be caused by Streptococcus parauberis. We compared three routes of administration for experimental injections of the S. parauberis 19FBSPa0003 strain in the olive flounder. Pathological changes were observed during the experimental infection. Inflammation of the serous membrane in the liver, intestine, spleen and heart was the major pathological change found in the infected olive flounder. No mortality was observed in fish that received intraperitoneal (IP) injection at less than 1×104 colony-forming unit (CFU)/fish. The lethal dose 50 for olive flounder, given an intravenous (IV) injection, was 7.94×104 CFU/fish. Fish with a higher concentration of IV injected S. parauberis (1×108 CFU/fish) died within a maximum of two days. However, serious necrosis and bacterial proliferation in ellipsoidal cells of the spleen and heart tissues were found in moribund or dead fish, 1-2 days after IV injection. Similar histopathological signs were observed in olive flounder inoculated by subcutaneous (SC) infected and naturally infected. In addition, SC was also strongly associated with bacteria concentration and cumulative mortality rate. Based on these results, SC is the recommended method for artificial infection by S. parauberis in the olive flounder.

Survival Outcomes of Recurrent Epithelial Ovarian Cancer: Experience from a Thailand Northern Tertiary Care Center

  • To assess survival outcomes in a retrospective study, recurrent epithelial ovarian cancer patients were divided into three groups according to the platinum free interval as follows: platinum refractory that included the patients with tumor progression during treatment; platinum resistant and platinum sensitive that included the patients with tumor progression less than or more than six months, respectively. Clinical data for tumor progression in epithelial ovarian cancer patients treated at Chiang Mai University Hospital between January, 2006 and December, 2010 were reviewed. Thirty-nine patients were in the platinum refractory group while 27 were in the platinum resistant group and 75 in the platinum sensitive group. The mean age, the parity, the administration of neoadjuvant chemotherapy and the serous type did not significantly different across groups while the mean total number of chemotherapy regimens, the early stage patients, the patients with complete surgery and the surviving patients were significant more frequent in the platinum sensitive group. Regarding subsequent treatment after tumor recurrence, 87.2% underwent chemotherapy. With the median follow up time at 29 months, the median overall survival rates were 20 months, 14 months and 42 months in platinum refractory, platinum resistant and platinum sensitive groups, respectively (p<0.001). In addition, when the platinum sensitive patients developed the next episode of tumor progression, the median progression free interval time was only three to four months. In conclusion, the outcomes for platinum refractory the and platinum resistant groups was poorer than the platinum sensitive group. However, subsequent progression in the platinum sensitive group was also associated with a poor outcome.

Analysis for Vibration Characteristics of the Watermelon for Optimum Packaging Design in Domestic Distribution (국내 유통 수박의 적정 포장설계를 위한 진동특성 분석)

To analyze the vibration properties of the watermelon for optimum packaging design during transportation, sinusoidal sweep vibration tests were carried out. The resonance frequency of the watermelon ranged from 19 to 32 Hz and the amplitude at resonance was between 1.6 and 2.9 G. The resonance frequency and amplitude at resonance frequency band of the watermelon decreased with the increase of the sample mass. The multiple nonlinear regression equation for predicting the resonance frequency of the watermelon were developed using the independent variables such as mass, input acceleration and sphericity.

Herpes Zoster Ophthalmicus in Minor Facial Burn (눈대상포진이 병발된 경미한 얼굴의 화상)

  • Purpose: Many conditions can mimic the presentation of burns. Herpes zoster is one of them. The characteristic features of herpes zoster such as vesicles, pustular lesions and crusts can also be found in burns. Herpes zoster ophthalmicus is a disease caused by recurrent infection of varicella - zoster virus in the ophthalmic division of the trigeminal nerve. This virus frequently affects nasociliary branch and serious ocular complications can occur. Thus, early diagnosis and proper treatment of this disease is important to prevent further ocular manifestations. We report a man who sustained minor facial burn injury that was complicated with herpes zoster ophthalmicus. Methods: A 66 - year - old man visited emergency room with multiple whitish vesicles with serous discharge on right forehead, right medial canthal area and nose. At first he was thought to have a secondary infection of facial burn injury. The vesicles on his face began to form crusts on the next day. Since his skin lesion was located on the ophthalmic division of trigeminal nerve, we also suspected herpes zoster ophthalmicus. He was referred to dermatologist and ophthalmologist. Results: We used antiviral agent (Acyclovir) and NSAIDs for treatment. The patient had no ocular complications. His skin lesion was almost healed after 1 month and remained scars. We treated a patient with minor facial burn complicated with herpes zoster ophthalmicus with antiviral agent. Conclusion: In this work, we describe a case of old patient with herpetic infection and emphasize the need for careful examination to diagnose accurately.
