A Case Report of Korean Medicine Treatment for a Patient with Left Facial Paraesthesia, Temporomandibular Joint Pain, and Post-traumatic Neck Pain induced by Cerebral Infarction (뇌경색으로 유발된 좌측 안면부 이상감각, 턱관절 및 후경부 통증 환자에 대한 한의학 치료 1례)

  • Objectives: The aim of this case report is to present the effects of Korean medicine treatment in a patient with left facial paraesthesia, temporomandibular joint (TMJ) pain, and post-traumatic neck pain induced by cerebral infarction. Methods: A 26-year-old male patient was treated with Cheongpa-Jeon H, acupuncture, pharmacopuncture (Shinbaro2), and Chuna Therapy for 24 days of hospitalization. Left facial paraesthesia, TMJ pain, and post-traumatic neck pain were assessed with a Numeric Rating Scale (NRS) and EuroQoL-5 Dimension (EQ-5D). Post-traumatic neck pain was assessed using the Neck Disability Index (NDI). Results: After 24 days of treatment, the patient's left facial paraesthesia and left arm tingling sensation were relieved from NRS 6 to NRS 2. Left TMJ pain was reduced from NRS 5 to NRS 2. The EQ-5D score increased from 0.138 to 0.73. The NDI score decreased from 71.11 to 37.78. Conclusion: These results show that left facial paraesthesia, TMJ pain, and post-traumatic neck pain can be relieved with Korean medicine treatment. However, further well-designed studies are required to confirm these findings.

A Short form of the Gray-Wheelwright Test (단축형 그레이-휠라이트 검사)

  • We investigated whether the 81 items of the Gray-Wheelwright test correctly measure the concept of Jung's typology and aimed to refine the test. Participants (n=431) completed the Gray-Wheelwright test, and the results were analyzed using factor analysis with the varimax rotation and the maximum likelihood extraction method. A pair of opposing attitudes, introversion/extroversion, or one of the two pairs of opposing functional types, thinking/feeling or intuition/sensation, was labeled to the extracted factor according to the majority type of the items in the factor. The minority items or items not included in any factors were excluded from making a short form of the Gray-Wheelwright test with 45 items. We used intraclass correlation (ICC) coefficient and Cronbach's alpha for the test-retest reliability and internal consistency of the test, respectively. The newly developed short form of the Gray-Wheelwright test measured the Jung's personality types well, which was comparable to the original one while reducing time and effort required for the testing.

The Effect of the Cicadas' Songs on the Psychological Responses in Adolescents (매미과(科) 노랫소리가 청소년의 심리적 반응에 미치는 영향)

  • This experiment was performed to find out the effects of the cicadas' songs on the psychological responses in adolescents. As a basic coulee, the experiment to set up 'Acceptable & Unacceptable' was performed. As a further coulee, five kinds of the cicadas' songs heard frequently were analyzed and adjectives expressing the feeling to cicadas' songs were factor-analyzed, and psychological responses to auditory sensations were analyzed through regression equations. As a result, the effect of the Cryptotympana atrata's song, the Meimuna opalifera's song and the traffic noise are similar in the degree of disturbing the meditation but they are less disturbing than the white noise. The experiment for adjectives expressing was performed, because it is possible that cicadas' songs affect adolescents as a noise. Cicadas' songs can be expressed with three kinds of factors. First factor is [Annoyance], second factor is [Strength] and third factor is [Rhythm]. The first factor dominates in the songs of the Cryptotympana atrata and the Platypleura kaempferi who generate steady sound, and the third factor dominates in the songs of the Meimuna opalifera, the Leptosemia takanonis and the Oncotympana fuscata who generate fluctuating sounds. The loudness of sound didn't affect on the third factor but the emotional values of the fist and the second factors are linearly proportional to the loudness. The analysis results of the first factor associated with noise showed that the annoyance of adolescents is increased in the order of white noise - the Platypleura kaempferi - the Cryptotympana atrata, if the loudness of sounds are generated equally.

The First Neonatal Case of Panton-Valentine Leukocidin-Positive Staphylococcus aureus Causing Severe Soft Tissue Infection in Korea

  • Staphylococcus aureus (SA) is a common cause of skin and soft tissue infections. Panton-Valentine leukocidin (PVL) toxin-producing strain of SA has been discovered worldwide and is known to cause serious infections. However, reports of neonatal infections caused by PVL-positive SA are rare. Here, we report a case of severe skin and soft tissue infection caused by PVL-positive SA in a 7-day-old neonate. The patient was admitted to the emergency room with a history of fever for one day, tenderness, and sensation of buttocks heating. The infant presented with fever, tachycardia, poor general health, progressive tenderness, and edema of the buttocks on the day of admission. Ultrasonography and magnetic resonance imaging revealed necrotizing fasciitis involving the skin, soft tissue, and muscles. Specimens drained from the buttock lesions confirmed the presence of PVL-positive methicillin-resistant SA (MRSA), and there was no bacteremia. She recovered after one month of intravenous antibiotics and surgical drainages. One month after discharge, she was rehospitalized for otitis externa and was infected with MRSA again. Considering the PVL-positive strain, the patient was treated with intravenous linezolid and dressing. The patient underwent decolonization therapy in a 0.5% chlorhexidine bath and recovered completely without sequelae. This case suggests that aggressive drainage and antibiotic treatment are essential for PVL-producing MRSA infections, and additional decolonization is needed to prevent recurrence and community spread.

A Study on the Meaning of Shengmaisan -Focusing on the argument of Zhangyuansu and Lidongyuan- (생맥산(生脈散)의 방의(方義)에 대한 고찰(考察) - 장원소(張元素)와 이동원(李東垣)의 논설을 중심으로 -)

  • Objectives : This paper aims to examine the meaning of Shengmaisan in relation to original qi based on the discussion of Zhangyuansu, Lidongyuan and other doctors. Methods : Discussion of Shengmaisan made by Zhang, Li and others were examined. Afterwards, the medicinal properties of renshen, maimendong, and wuweizi as written in the Bencaogangmu, Donguibogam, Benjingshuzheng and texts of Zhang and Li were studied in relation to original qi. Related concepts such as '肺中伏火'·'補肺中元氣不足'·'補天眞元氣'·'夏食寒' were analyzed as well. Results & Conclusions : 1. Various pathology related to deficiency in original qi could act as internal pathogen for Shengmaisan application with which external factors such as summerheat and dampness heat meet results in most damage in the Lung metal. Therefore the upper body Lung is emphasized as pathogen. 2. The pathology of Shengmaisan application is depletion of humor, which should be refined, due to summer or dampness heat in a state of original qi deficiency which damages the Lungs, disabling its function of managing clear and pure qi. 3. The disease pattern for Shengmaisan in relation to original qi, according to other doctors, includes symptoms related to the Lungs such as dryness in the mouth due to excessive perspiration, blankness, loss of consciousness, shortness of breath, coughing, fast breathing, along with general bodily symptoms such as heavy sensation of the body, lethargy, difficulty walking, limping. These general bodily symptoms are due to either qi deficiency of the Lung, or humor deficiency due to failure of qi convergence in the lower body, or symptoms caused by Jing deficiency in severe cases. Symptoms related to problems in the lower body could manifest as dysfunction in urination and defecation, weakness in the limbs caused by original qi deficiency. 4. The Lungs circulate the clear and pure qi, converging qi and creating humor to assist the Triple Burner's Qi-Humor metabolism, enabling smooth communication of original qi throughout the body. With the original qi well tonified, the shen mechanism which is rooted in original qi allows for autonomous and consistent qi circulation within the body. Therefore, both Zhang and Li emphasized the importance of original qi. The property of Shengmaisan is explained as 'supplement qi/create liquid, convergence of yin/stop perspiration'. Zhang and Li explained its property as managing the circulation of original qi, to tonify Lungs' original qi and origin of water-humor.

Usefulness of Abdominal Compressor Using Stereotactic Body Radiotherapy with Hepatocellular Carcinoma Patients (토모테라피를 이용한 간암환자의 정위적 방사선치료시 복부압박장치의 유용성 평가)

  • Purpose: We evaluated usefulness of abdominal compressor for stereotactic body radiotherapy (SBRT) with unresectable hepatocellular carcinoma (HCC) patients and hepato-biliary cancer and metastatic liver cancer patients. Materials and Methods: From November 2011 to March 2012, we selected HCC patients who gained reduction of diaphragm movement >1 cm through abdominal compressor (diaphragm control, elekta, sweden) for HT (Hi-Art Tomotherapy, USA). We got planning computed tomography (CT) images and 4 dimensional (4D) images through 4D CT (somatom sensation, siemens, germany). The gross tumor volume (GTV) included a gross tumor and margins considering tumor movement. The planning target volume (PTV) included a 5 to 7 mm safety margin around GTV. We classified patients into two groups according to distance between tumor and organs at risk (OAR, stomach, duodenum, bowel). Patients with the distance more than 1 cm are classified as the 1st group and they received SBRT of 4 or 5 fractions. Patients with the distance less than 1 cm are classified as the 2nd group and they received tomotherapy of 20 fractions. Megavoltage computed tomography (MVCT) were performed 4 or 10 fractions. When we verify a MVCT fusion considering priority to liver than bone-technique. We sent MVCT images to Mim_vista (Mimsoftware, ver .5.4. USA) and we re-delineated stomach, duodenum and bowel to bowel_organ and delineated liver. First, we analyzed MVCT images to check the setup variation. Second we compared dose difference between tumor and OAR based on adaptive dose through adaptive planning station and Mim_vista. Results: Average setup variation from MVCT was $-0.66{\pm}1.53$ mm (left-right) $0.39{\pm}4.17$ mm (superior-inferior), $0.71{\pm}1.74$ mm (anterior-posterior), $-0.18{\pm}0.30$ degrees (roll). 1st group ($d{\geq}1$) and 2nd group (d<1) were similar to setup variation. 1st group ($d{\geq}1$) of $V_{diff3%}$ (volume of 3% difference of dose) of GTV through adaptive planing station was $0.78{\pm}0.05%$, PTV was $9.97{\pm}3.62%$, $V_{diff5%}$ was GTV 0.0%, PTV was $2.9{\pm}0.95%$, maximum dose difference rate of bowel_organ was $-6.85{\pm}1.11%$. 2nd Group (d<1) GTV of $V_{diff3%}$ was $1.62{\pm}0.55%$, PTV was $8.61{\pm}2.01%$, $V_{diff5%}$ of GTV was 0.0%, PTV was $5.33{\pm}2.32%$, maximum dose difference rate of bowel_organ was $28.33{\pm}24.41%$. Conclusion: Despite we saw diaphragm movement more than 5 mm with flouroscopy after use an abdominal compressor, average setup_variation from MVCT was less than 5 mm. Therefore, we could estimate the range of setup_error within a 5 mm. Target's dose difference rate of 1st group ($d{\geq}1$) and 2nd group (d<1) were similar, while 1st group ($d{\geq}1$) and 2nd group (d<1)'s bowel_organ's maximum dose difference rate's maximum difference was more than 35%, 1st group ($d{\geq}1$)'s bowel_organ's maximum dose difference rate was smaller than 2nd group (d<1). When applicating SBRT to HCC, abdominal compressor is useful to control diaphragm movement in selected patients with more than 1 cm bowel_organ distance.

Applying QFD in the Development of Sensible Brassiere for Middle Aged Women (QFD(품질 기능 전개도)를 이용한 중년 여성의 감성 Brassiere 개발)

  • Quality Function Deployment(QFD) is a product development tool which ensures that the voice of the customer needs is heard and translated into products. To develop a sensible brassiere for middle-aged women QFD was adopted. In this study the applicability and usefulness of QFD was examined through the engineering design process for a sensible brassiere for middle-aged women. The customer needs for the wear comfort of brassiere was made by one-on-one survey of 100 women who aged 30-40. The customer competitive assessment was generated by wearing tests of 10 commercial brassieres. The subjective assessment was conducted in the enviornmental chamber that was controlled at $28{\pm}1^{\circ}C,\;65{\pm}3\%RH.$ As a results, we developed twenty-one customer needs and corresponding HOWs for the wear comfort of brassiere. The Customer Competitive Assessment was generated by wearing tests of commercial brassiere. The subjective measurement scale and dimension for the evaluation of sensible brassiere were extracted from factor analysis. Four factors were fitting, aesthetic property, pressure sensation, displacement of brassiere due to movement. The most critical design parameter was wire-related property and second one was stretchability of main material of brassiere. Also, wearing comfort of brassiere was affected by the interaction of initial stretchability of wing and support of strap. Engineering design process, QFD was applicable to the development of technical and aesthetic brassieres.

Comparison of the Clinical Effect after Wearing Conventional and 2-Weeks Cosmetic Tinted Contact Lens (일반 미용 칼라 콘택트렌즈와 2주 교체용 미용 칼라 콘택트렌즈 착용에 따른 임상성능 비교)

  • Purpose: To compare and analyze the ocular changes after wearing conventional and 2-weeks cosmetic tinted contact lens, which were made of different material and fabrication method. Methods: Thirty five university students, age of twenties, participated in this study. With daily wearing schedule, they had worn temporary cosmetic tinted lens for 2 weeks and conventional tinted lens for two month in their right and left eye, respectively. Subjective symptoms and objective signs were graded by using CCLRU scales and corneal thickness were measured after 1 day and 2 months wearing, and under or over 5 hours wearing in a day, respectively. Results: Major subjective symptoms found in the student wearing cosmetic tinted contact lens for a 1 day and 2 months were dryness, redness, foreign body sensation and itchiness. It was also found that those symptoms were more severe in left eye (conventional cosmetic tinted contact lens) than right eye (2-weeks cosmetic tinted contact lens). However, symptoms of redness in both eyes showed statistically significant difference in the case of 1 day and 2 months wearing and under or over 5 hours wearing in a day (p=0.000). After 2 months wearing, left eye with conventional cosmetic tinted contact lens showed limbal redness (p=0.000) and bulbar conjunctival redness (p=0.012) with statistically significance, whereas there was no significant difference in both eyes before lens wear. Left eye showed statistically significant changes in all objective signs, but right eye showed palpebral conjunctival redness, corneal staining and conjunctival staining. Therefore, it was concluded that conventional cosmetic tinted contact lens may be more effective on ocular changes after wearing of lens. A statistically significant changes in both eyes after wearing lens were found that corneal thickness increased about $5{\mu}m$ (p=0.001) in right eye whereas $7{\mu}m$ (p=0.004) in left eye. Conclusions: A statistically significant number of participants in this study preferred to wearing of high Dk/t temporary cosmetic tinted contact lens made by sandwich method which resulted in less ocular changes and may give more healthy and safe ocular conditions.

A study on oral discomfort in gynecological cancer patients undergoing chemotherapy (화학요법을 받는 부인암환자의 구강불편감에 관한 연구)

  • The frequency with which administration of chemotherapy for gynecological cancer treatment is used has increased along with the use of surgery and radiotherapy Among the various side effects of chemotherapy, stomatitis causes a problem of function and sensation in the oral cavity. This oral discomfort can be categorized into two components ; perceived oral symptoms and observed oral symptoms. If the oral problem continues, it may cause infection, bleeding and nutritional deficiencies. As a result of this condition, compliance with the treatment process can be affected as well as the prognosis for the cancer patients. But as the oral discorrfort usually appears after chemotherapy, it is often not reported to the health care personnel as a patient problem. Without problem identification of the oral discomfort and ability to assess the problem, effective intervention cannot be planned. Therefore, this study was conducted to identify the pattern and the degree of oral discomfort due to cancer chemotherapy and thus to provide data for identification of the patient problem and for nursing assessment. The design of this study was a longitudinal de-scriptive study The subjects were in - patients who received chemotherapy under the diagnosis of gynecological cancer between Mar. 15, 1994 and May 15, 1994 at a general hospital in Seoul, Korea. The number of subjects was 64 and they were divided into two groups, one of 41 (A : 5FU & Neoplatin), the other of 23(B : Neoplatin, Cytoxan, Adriamycin), according to the treatment regimen. The data were collected for 24 days using self-re-port instruments. The instruments were the 「Perceived Oral Symptom Assessment Tool」 and 「Observed Oral Symptom Assessment Tool」 developed by this researcher. Data were analyzed using the SPSS-PC program, ANOVA, t-test, paired t-test and the Pearson Correlation Coefficient were applied. The results of this study are as follows : 1. In A regimen the peak time for perceived oral symptom scores was the fifth day after chemotherapy, and the tenth day for observed oral symptom scores. Both of the problems started on first day of chemotherapy and were not resolved completely until the 24th day after treatment. 2. In B regimen, the peak time for perceived oral symptom scores was on the seventh day after chemotherapy, and the eighth day for observed oral symptom scores. It was noted that perceived oral symptom scores were higher than observed oral symptom scores consistently for 24 days. Both also started on first day of chemotherapy, and were not resolved completely until the 24th day after chemotherapy. 3. There were no differences statistically in perceived oral symptom scores between A and B regimen. The loss of appetite and xerostomia caused the most severe discomfort in both of these two groups. 4. The were no differences statistically in observed oral symptom scores between the A and B regi moil. In the A regimen, the highest observed symptom scores were the lips, gingiva, tongue and buccal membrane in that order. But in the B regimen, the highest observed symptom scores were tongue, lips, buccal membrane and gingiva in that order. 5. In A regimen, the patients who had gingival edema and dentures had significantly higher perceived oral symptom scores. And those who had gingival edema and bleeding, foul odor and aphthous stomatitis had significantly higher observed oral symptom scores. 6. In B regimen, the patients who had the experience of stomatitis in the last course of chemotherapy had significantly higher perceived oral symptom scores. Those who had gingival edema had significantly higher observed oral symptom scores. 7. In the A regimen there was no correlation between lab values for lymphocytes and albumin with perceived oral symptom scores and observed oral symptom scores. In the B regimen, there was a significant negative correlation between lymphocytes and albumin with the observed oral symptom scores, but not between perceived oral symptom scores and lymphocytes and albumin values. In conclusion, the nurse should expect that the patient undergoing chemotherapy will complain severely about subjective discomfort and before objective physical change is observed. Also the patients who have chronic oral problems such as dentures, gingival edema and bleeding, foul odor, aphthous stomatitis will complain of severe oral discomfort due to chemotherapy.

MORPHOLOGY OF THE TERMINAL ARBORS FROM THE MASSETERIC MUSCLE SPINDLE AFFERENTS IN THE TRIGEMINAL MOTOR NUCLEUS (삼차신경 운동핵에서 교근 근방추 구심성 신경섬유 종말지의 미세구조)

  • Muscle spindle afferents from masseter muscle were labelled by the intra-axonal HRP injection and were processed for light microscopic reconstruction. Regions containing terminal arbors scattered in the central portion of the masseteric motor neuron pool (type I a) and those restricted to 2-3 small portion of it (type II) were selected and processed for electronmicroscopic analysis with serial sections. The shape of the labelled boutons was dome or elongated shape. Scalloped or glomerulus shape with peripherial indentation containing pre or postsynaptic neuronal propiles, which is occasionally found in the trigeminal main sensory nucleus and spinal dorsal horn, was not observed. Both type Ia and type II boutons had pale axoplasm and contained clear, spherical vesicles of uniform size(dia : 49-52nm) and occasionally large dense cored vesicles(dia : 87-118nm). The synaptic vesicles were evenly distributed throughout the boutons although there was a slight tendency of vesicles to accumulate at the presynaptic site. The average of short and long diameter(short D. + long D./2) of type I a bouton was smaller than that of type II bouton. All the labelled boutons, which showed prominent postsynaptic density, large synaptic area and multiple synaptic contact, made asymmetrical synaptic contact with postsynaptic neuronal propiles. Most of the type Ia and type II boutons made synaptic contact with only one neuronal propile and boutons which shows synaptic contact or more neuronal propiles was not observed. Most of the type Ia boutons(87.2%) were presynaptic to the soma or proximal dendrite and a few remainder(12.8%) made synaptic contact with dendritic shaft or distal dendrite. In contrast, majority of type II boutons showed synaptic contact with dendritic shaft and remainder with soma or proximal dendrite. In conclusion, terminal boutons which participate in the excitatory monosynaptic jaw jerk reflex made synaptic contact with more proximal region of the neuron, and showed very simple synaptic connection, compared with those from the primary afferenst in the other region of the central nervous system such as spinal dorsal horn and trigeminal main sensory nucleus which assumed to be responsible for the mediating pain, tactile sensation, sensory processing or sensory discrimination.
