Drug Consumption and Nutritional Status of the Elderly in Chung-Buk Area - I. Diseades and Drug Consumption- (충북지역 노인들의 약물복용 및 영양상태 - I. 질병 및 약물복용실태-)

  • Three hundred sixty-two(male 131, female 231) elderly aged over 65 in Chungb- uk area were interviewed to determine the disease states and drug usage patterns. The prebalence of disease was 78% and women reported more chronic diseases(83%) than men(71%). Elderly who live with spouse and have an occupation have a lower rate of disease. Average number of diseases of the elderly was $1.8\pm{1.1}$, and women$(2.1\pm{1.3)}$ have significantly higher average number of diseases than that of men$(1.4\pm{0.7)}$. Also the elderly in urban areas$(2.1\pm{1.4)}$ have significantly higher number of diseases than that of the elderly in rural areas$(1.6\pm{0.9)}$. Arthritis, hypertension, cardiovascular and gastric diseases were the most frequently listed chronic diseases in order for both men and women. Anemia and fracture of bone were relatively higher in women than in men. Particularly, the arthritis of the urban elderly have a rate of 1.5 times higher than that of the rural elderly. Fifty-two percent of the elderly were currently using drugs ; among drug users 71.2% used prescription drugs and 20.5% used nonprescription drugs. The average number taken per person was 2.1$\pm$1.4 and there was no sex or age difference. However, the elderly in rural areas $(2.7\pm{1.7)}$ consumed a significantly higher number of drugs than those in urban areas$(1.7\pm{0.7)}$. The average number of prescripti- on drugs taken was 2.0$\pm$1.4 while the average of nonprescription drugs taken was $(1.3\pm{0.6)}$. Analgesics and antihypertensive drugs were most commonly used. Vitamin and analgesics were the most frequently used self-prescribed drugs. It was noted that potential adverse drug interaction by concominant drug consumption for arthritis and antihypensive drug, abuse of digestants and antiacid without treatment of the underlying disease, and misuse of quick-acting bowel medications were problematic for the elderly. In addition drugs used for the elderly have some adverse effect on the digestive system. The types and composition of drugs used by the elderly were identified and presented. Medication compliance was poor and 13.5% reported adverse reactions such as edema, heartburn, nausea, and difficulty with eating. Seventeen percent of the elderly obtained drugs arranged by those other than medical staff. Also, even among those elderly who obtained drugs prescribed by a doctor, 69.1% of subjects had not receive instruction about potential adverse reactions. These results suggest that nutritional problems related to drug usage might exist and so dietitians, either individually or as members of health teams, need to have a better understanding of drug-nutrient interaction and closer supervision, and drug information/education service should therefore be provided to prevent or minimize adverse drug reaction in elderly users of medication.

Mutual Maturity Process between Client-Therapist in Music and Imagery Session: Therapist's Point of View (음악과 심상 세션에서 내담자-치료사간 상호 성장의 과정: 치료사의 관점에서)

  • The purpose of the study was to prospect for mutual maturity between therapist-client in Music and Imagery music therapy session from therapists' points of view. Qualitative data has been collected by in-depth interview of therapist-client mutual maturity process, then analysed with grounded theories. The questions of the study were to ask process of mutual maturity experienced by music and imagery therapist and meaning of the experience itself. Therapists ware to answer the moment of mutual maturity, realization and meaning of mutual maturity process through semi-structured in-depth interview. The four therapists who participated in the interview are certified MIT(Music & Imagery Therapist) by Lisa Summer and currently in training GIM. From the in-depth interview, 81 concepts have been collected and have been divided into 20 categories in open coding, then rearranged into 9 categories in axis coding. To explain music and imagery therapists' maturity process through grounded theory paradigm, as a result, the main phenomenon of experience was 'to deal with counter-transference'. When dealing their counter-transference, therapists experienced their maturity process. Supervision and musical/non-musical self-reflection are also used for interactive intervention medium. So, maturity of therapists leads to knowing and accepting herself and mature therapists are able to help clients mature.

Effects of School-Based Oral Health Programs among Schoolchildren : Focus on the Oral Health Knowledge and Behaviors (학교구강보건실 운영이 아동들의 구강보건지식 및 행동변화에 미치는 영향)

  • Demand for appropriate health care has gradually increased in Korea. In addition, developments of community- and school-based oral health programs have also focused oral health care for the oral health promotion. Especially, school-based oral health programs are the underpinnings of promoting oral health and preventing oral diseases among schoolchildren. School-based oral health programs have had three major components: oral health education, oral health services, and a healthful environments. These included oral health education(one-to-one communication, group communication, and use of mass communication), oral examination, fluoride mouthrinsing, pit-and-fissure sealants, fluoride gel application, mechanical plaque control, and chewing xylitol candy. In this study, we evaluate the effects of oral health programs among primary schoolchildren by comparing the oral health knowledge, oral health behaviors, and perception of caries prevention procedures. Data for this study were obtained from 699 primary schoolchildren at the two primary school in Daegu, Korea. One is experimental group, N primary school, that was established school-based oral health center under supervision of Nam-gu Public Health Center, the other is control group, N' primary school, that was yet to establish school-based oral health center. We surveyed children's oral health knowledge and behaviors, and perception of caries prevention procedures using self-administrated questionnaire and then analyzed differences of each item among two groups. The brief findings of this study were summarized as follows. There are several advantage to a comprehensive school-based oral health program. (1) School-based oral health programs facilitate and increase the effectiveness of teaching oral health subjects. (2) Schoolchildren are available for prevention or treatment procedure. (3) School-based oral health center may be less threating than private dental clinic. (4) With comprehensive school-based oral health programs the decayed, missing, and filled teeth(DMFT) of schoolchildren should demonstrate a substantial and steady decrease over time(Choi et al, 2004). In conclusion, treatment is not the answer to solving children's oral health programs; rather primary prevention is the key. Many countries and communities are focusing on hoe millions of underprivileged children can be provided with health care. Schoolchildren gain the knowledge and behaviors to attain and maintain good oral health in schools. For these reasons, the role of school-based oral health center is not only important but also a necessity.

Relationship between the Intake of Children's Favorite Foods and Policy based on Special Act on Safety Control of Children's Dietary Life (아동의 기호식품섭취와 어린이기호식품정책과의 관련성 분석)

  • Objectives: This study examined the status of children's favorite foods intake and the relationship with the policy environment based on the Special Act on Safety Control of Children's Dietary Life for suggesting a supportive policy strategy. Methods: The subjects were 4th grade students (n=1,638) in elementary school from 45 schools collected from seven areas (Seoul, Daegu, Daejeon, Gyeonggi, Chungnam, Jeonbuk, and Gyeongnam). The children participated in a self-administered questionnaire survey in class under the supervision of the teacher. The questionnaire consisted of items, such as social demographic characteristics, frequency of intake of the children's favorite foods, and policy cognition. A t-test and ANOVA were applied to explore the relationship between the frequency of children's favorite foods intake and policy cognition. The survey was implemented from August 2016 to September 2016. Results: For the boys, the frequency of 'high-calorie low nutrient foods intake' (HCLN) was significantly higher than that of the girls (p<0.01). For the children who received information on their favorite foods from the internet, the frequency of HCLN was higher than the other sources (p<0.01). The time of TV viewing and computer usage, and smartphone usage was associated with a higher frequency of HCLN, and a lower healthy favorite food intake (all p<0.001). The intake frequency of healthy favorite foods indicated a positive correlation with the policy cognition, including policy perception, usefulness, necessity and buying intention, and educational experience. Conclusions: This study showed a correlation with the frequency of children's favorite foods intake and policy. In particular, the frequency of children's healthy favorite foods intake indicated a meaningful relationship with the policy than the frequency of HCLN. This study also found that the consumption of children's healthy favorite foods was positively correlated with the educational experience. To develop a supportive policy for a good dietary environment for children, there is a need to focus on how to collaborate with multiple levels of influences, such as the national level, school level, and family.

A Study on the Special Needs of the Hearing-Impaired Person for Disaster Response (청각장애인 재난대응 욕구에 관한 연구)

  • This study evaluated the actual status of special needs of the hearing-impaired person for disaster response. The analysis revealed a significant level of unmet needs in disaster response for hearing-impaired person. The 5 special needs in disaster response include: 1) communication needs, which involve securing the means to make an emergency rescue request and communicating information during the rescue process; 2) transportation needs, which indicate the effective evacuation capacity and the level of training; 3) medical needs, which address the degree of preparedness for physical and mental emergency measures and the delivery of health information for rescue and first aid process; 4) maintaining functional independence needs, which refer to the level of self-preparedness to minimize damage in disaster situations, and; 5) supervision needs, which correspond to a personalized support system provided to disaster-vulnerable groups.

A Meta-analysis on the Variables Related with Case Management Performance (사례관리수행에 영향을 미치는 변인에 관한 메타분석 연구)

  • This study conducted a meta-analysis on the variables related with case management performance in Korea. In order to do a meta-analysis, a total of 27 journals, theses and dissertations published between 1998 and 2020 were reviewed systematically. Through this process, this study calculated average effect size of each variable and explored moderator variables. This study results were as follows. First, this study identified a total of 14 individual, enviroment of institution, population variables related with case management performance. Second, the results indicated that case management performance variables which showed large effect sizes included professional capability, supervision, network, case management significance recognition, manual, agency support. Third, variables with medium effect sizes included self-efficacy, case management education, autonomy, motivation, number of conference and variables with small effect sizes included work experience, academic ability, age. Fourth, Moderator effects were identified in the field, target, publication of case management. Finally, the implications of the study findings were discussed.

Forest Fire Area Extraction Method Using VIIRS (VIIRS를 활용한 산불 피해 범위 추출 방법 연구)

  • The frequency and damage of forest fires have tended to increase over the past 20 years. In order to effectively respond to forest fires, information on forest fire damage should be well managed. However, information on the extent of forest fire damage is not well managed. This study attempted to present a method that extracting information on the area of forest fire in real time and quasi-real-time using visible infrared imaging radiometer suite (VIIRS) images. VIIRS data observing the Korean Peninsula were obtained and visualized at the time of the East Coast forest fire in March 2022. VIIRS images were classified without supervision using iterative self-organizing data analysis (ISODATA) algorithm. The results were reclassified using the relationship between the burned area and the location of the flame to extract the extent of forest fire. The final results were compared with verification and comparison data. As a result of the comparison, in the case of large forest fires, it was found that classifying and extracting VIIRS images was more accurate than estimating them through forest fire occurrence data. This method can be used to create spatial data for forest fire management. Furthermore, if this research method is automated, it is expected that daily forest fire damage monitoring based on VIIRS will be possible.

Bacterial Contamination in Disposable Wet Wipes in General Restaurants 2nd (대중음식점 일회용 물티슈의 세균 오염도 조사(2))

  • Disposable wet wipes provided by general restaurants in the Jeju Special Self-Governing Province were evaluated for compliance with the labeling requirements and the degree of bacterial contamination set by the Ministry of Food and Drug Safety. Of the 32 types of disposable wipes, five types of disposable wipes did not indicate the name and location of the business office of the manufacturer. Only 12 types of wet wipes were marked with the date of manufacture and expiration date. Bacterial contamination was seen in 9 types of wet wipes through bacterial culture but these were absent in the remaining 23 types (71.9%). The results of the antimicrobial susceptibility test for the 9 identified strains showed no methicillin-resistant coagulase-negative staphylococci (MRCNS) or multidrug-resistant bacteria. Through this study, it is recommended that the date of manufacture and expiration date must be indicated on the surface of disposable wet wipes designated as hygiene products, and clean uncontaminated wet wipes must comply with storage standards. In addition, the results are expected to contribute to the enhancement and improvement of hygiene management supervision, personal hygiene management, and an improvement in national health.

Continuation and Rebirth: A Study on the Changing Mechanism of Customary Law - Based on the fieldwork on the main ethnic minority areas in South China (续造与重生:习惯法变迁机制研究 --基于南方主要少数民族聚居区的田野调查)

  • From the field survey of the main ethnic minority areas in the south China, it is found that political, economic, cultural and natural environmental changes are the main reasons leading to the change of customary law. The power mechanism of the customary law change include the three aspects, such as the promotion of country elite, the dominance of grassroots government, and the daily demands of the villagers, which promote the change of customary law alone or together. Through the application of customary law, the country elites can adjust and refine the rules of customary law in order to make out the new customary law and promote its development. In the current pattern of rural governance, grassroots self-government is actually the "official supervision of people's autonomy". The executive power of the grassroots government often intervene the practice of customary law and other informal rules. This is another mechanism of customary law change. Customary law arises from the practice of the daily life of the villagers. If the villagers think that the norms of customary law cannot meet the actual needs of daily life practice, the customary law will be promoted in the form of collective consultation. This is the most important dynamic practice mechanism of customary law change. Transformation and abandonment are the two ways to change customary law. No matter what kind of change does not lead to the demise of the customary law system, the demise of the customary law is only an outdated result, which is made by the universality, nature and objectivity of customary law. The procedure of customary law change is the process of continuation and rebirth about customary law. The result of the change is to produce the new customary law of keeping pace with the times, and the customary law will be presented with new content and form after the change. The continuation of customary law means the inheritance of traditional customary law, but it is based on the transformation of traditional customary law. The rebirth of customary law means that the traditional customary law is completely discarded. But it will produce new customary law rules and be based on the needs of social life practice. Customary law occupies a pivotal position in the normative system and the national law cannot be replaced. The purpose of customary law change will let the customary rules better adapt to the development of modern society, adjust the social relations more reasonably and better meet people's needs of production and life, which is decided by the character of customary law.

A Study on Actual Conditions of Preschool Education for Activation of Public Education (공교육 활성화를 위한 유아교육 현황 분석)

  • This study intended to activate public education, researched and analyzed real conditions of preschool education to know desirable directions of public education and help parents recognize public education. The final purpose is to offer basic materials to reestablish preschool education in the public education system. First, parents' recognition concerning preschool education in the public education system was much different from that of teachers. As for definitions of preschool education as a public education, parents understood that it means 'the nation and self-governing bodies manage aid both the public and private kindergartens', and teachers recognized that it means that 'the nation and self-governing bodies not only found operate public kindergartens but also aid manage educational funds of private kindergartens. Second, the examination of educational environments of kindergartens told that teachers and parents thought that opportunity to enter the kindergarten is not equal for all children in our country, and quality of kindergarten teachers is satisfactory. Also, they thought that curriculums of preschool educational organs have been set well according to children's developmental stages, education quality is different according to preschool education organs' foundation-types. They recognized that facilities and apparatus of preschools are satisfactory, security and neatness of preschools are satisfactory, too. Third, the examination of developmental directions of preschool education as a public education proved following facts; as for foundation-types of kindergartens in the future when promoting preschool education as a public education, parents answered financial support and supervision by the government, teachers answered aid and management of operational fees as well as support for teacher personnel expenses.
