Purpose: The purpose of this study was to analyze effects of a community-based case management program for clients with hypertension living in the community. Methods: The research design was a one group pre and post-test design with 30 participants with hypertension who agreed to participate in the 8-12 week case management program provided by case managers from the National Health Insurance Corporation in 2002. Data were collected three times, before and after the case management services, and 6 months later. Outcomes included changes in blood pressure, knowledge of hypertension and daily life practices, including alcohol consumption, smoking, exercise, and medication adherence. Results: Repeated-measures ANOVA and post-hoc tests of means revealed significant differences before and after service for systolic blood pressure, daily life practices (monitoring body weight and BP, low salt and cholesterol and high vegetable diet, and stress-relief practices), and exercise. The goal for medication adherence was attained after service. Significant improvements from baseline to 6 months after service were observed in measures of salt and vegetables in diet. There were no significant differences on hypertension knowledge, alcohol consumption or smoking behavior between before service and after, and at 6 months. Conclusion: The findings provide preliminary evidence that case management intervention can have positive outcomes on BP control, daily life practices, exercise, and medication adherence for clients with hypertension. However, additional interventions are needed to sustain long-term effects.
Purpose: The purpose of this study was to investigate the level of clinical image quality of panoramic radiographs and to analyze the parameters that influence the overall image quality. Materials and Methods: Korean dental clinics were asked to provide three randomly selected panoramic radiographs. An oral and maxillofacial radiology specialist evaluated those images using our self-developed Clinical Image Quality Evaluation Chart. Three evaluators classified the overall image quality of the panoramic radiographs and evaluated the causes of imaging errors. Results: A total of 297 panoramic radiographs were collected from 99 dental hospitals and clinics. The mean of the scores according to the Clinical Image Quality Evaluation Chart was 79.9. In the classification of the overall image quality, 17 images were deemed 'optimal for obtaining diagnostic information,' 153 were 'adequate for diagnosis,' 109 were 'poor but diagnosable,' and nine were 'unrecognizable and too poor for diagnosis'. The results of the analysis of the causes of the errors in all the images are as follows: 139 errors in the positioning, 135 in the processing, 50 from the radiographic unit, and 13 due to anatomic abnormality. Conclusion: Panoramic radiographs taken at local dental clinics generally have a normal or higher-level image quality. Principal factors affecting image quality were positioning of the patient and image density, sharpness, and contrast. Therefore, when images are taken, the patient position should be adjusted with great care. Also, standardizing objective criteria of image density, sharpness, and contrast is required to evaluate image quality effectively.

Purpose: The objectives of this study were to investigate the prevalence of the metabolic syndrome (MetS) and to identify associated factors with MetS among rural residents. Methods: Data were collected from 1,196 subjects over aged 30 years by a self-administered questionnaire, physical measurement, and blood test in a rural area. The prevalence of MetS was determined by the criteria of the American Heart Association/National Heart, Lung, and Blood Institute (AHA/NHLBI) and 2005 the Korean society for the study of obesity. Results: The prevalence of MetS was 40.5% for men, 49.2% for women. The prevalence of risk factors of MetS was 57.4% for elevated blood pressure, 49.0% for low HDL-cholesterol, and 48.6% for abdominal obesity. Unemployment and higher Body mass index (BMI) were associated factors for MetS regardless of gender. And higher age and physical inactivity in women only increased the odds of the MetS. Especially, BMI was a strong risk factor of MetS in both men and women. Conclusion: The prevalence of metabolic syndrome was higher in a rural area. Therefore, health care providers should develop lifestyle modification program to increase physical activity level and to prevent the obesity among rural residents in order to decrease the prevalence of MetS.
Rapid process of industrialization and modernization in Korea has caused earning differentials between the rich and the poor, the feeling of alienation followed by excessive competitions among the members of community as well as the conflict between generations. Because of those factors, Koreans have come to undergo several mental problems such as anxiety, melancholy and suicide. However, scarce concern or effort has been given to solve those mental problems in the professional field of mental health. Social work service for mental health in Korea has been delivered on the basis of the Law of Mental Health, most of which are focused on treatment or care of mental patient himself or his family. Therefore ordinary people who have no mental disorder but have desire for mental health have no way to take service in the field of public mental health except for counselling programs given by some of the private social welfare institutions. In this context, the process of clarifying the concept of mental health is demanded before we develop the mental health program. As social welfare is the practical science that helps people seek after happiness and meaningful life, so clarifying the concept of mental health is needed not only for the field of mental health itself but also for every field of social welfare. In order to achieve this purpose, this study firstly arranged concepts of mental health in Korea using Q methodology. As the result, we found 21 dimensions including spiritual one and self-wellbeing one which newly identified in this study as well as 19 dimensions of Jahoda. We also found Koreans perceive concepts of mental health as independently as 5 types of Communalist, Rationalist, Individualist, Humanist, and Religionist. Secondly, on the basis of the result, we suggested that mental health programs in social welfare should be oriented to the positive concept of mental health. This study could furnish fundamental data that enable the concept of mental health to be spread to that of the growth for a better life not limiting on that of the prevention or the treatment of the mental disorder.

Background: As of 2016, average Koreans sleep 7 hours and 42 minutes, the lowest figure among Organization for Economic Cooperation and Development(OECD) countries, and the number of people with sleep disorders reached 561,000. Accordingly, the government has promoted the provision of 'Multiple Sleep Test' to strengthen the diagnosis service for patients with 'sleep disorder' in july 2018. As a result, healthcare costs for patients with sleep disorder is on the rise every year. In this study, we utilized 'Appropriate Sleep' criteria of United States's National Sleep Foundation(NSF) then investigated Korean's sleep pertinence using 「7th National Health and Nutrition Survey for 2016-2018」 by different occupational type, demographic characteristics, socio-economic characteristics, and health behaviors. Methods: We performed descriptive analysis to examine differences of sleep appropriateness by various sample characteristics. Multivariate logistic regression models were used to examine sleep appropriateness by occupational type and other variables. We also analyzed subgroup models to investigate. Results: As a result, a total of 1,948 (18.37%) study subjects experienced in-appropriate sleep. Results of the Multivariate logistic regression analysis revealed that blue color group had a higher odds ratio (OR) for experiencing in-appropriate sleep (OR=1.179). In addition, the odds ratio of experienced in-appropriate sleep among the elderly aged 70 and over was 2.698, and the odds ratio of the overstressed group was 1.299. Furthermore, sub-group analysis showed that blue color job of female(Or=1.334), high school or below(OR=1.404), divorce/death/separation(OR=2.039), 25%ile-50%lie income group(OR=1.411) more likely experienced in-appropriate sleep. Conclusion: Growing sleep disorder patients and related health care costs are expected. Government should apply detailed 'total periodic sleep disorder management policy' including pre-consultation, examination, diagnosis, treatment, post-consultation, self-management especially to vulnerable population that this study found.

The rapid increase of the elderly living alone is a critical issue in worldwide as it leads to a rapid increase of a social support costs (e.g., medical expenses) for the elderly. In early stages of dementia, the activities of daily living (ADL) including self-care tasks can be affected by abnormal patterns or behaviors and used as an evidence for the early diagnosis. However, extracting activities using non-intrusive approach is still quite challenging and the existing methods are not fully visualized to understand the behavior pattern or routine. To address these issues, this research suggests a model to extract the activities from coarse-grained data (spatio-temporal data log) and visualize the behavioral context information. Our approach shows the process of extracting and visualizing the subject's spaceactivity map presenting the context of each activity (time, room, duration, sequence, frequency). This research contributes to show a possibility of detecting subject's activities and behavioral patterns using coarse-grained data (limited to spatio-temporal information) with little infringement of personal privacy.

This study was designed to identify the relationship between the level of sense of coherence(SOC) and coping resources of nurses working in hospital. The subjects of this study were 248 nurses working at C University hospital in Seoul. The data was collected during the period from July 1. to July 14. 2001. The insruments for this study were SOC scale by Antonovsky(1987) and Coping Resources Inventory by author. The analysis of data was done by simple statistics, ANOVA and Pearson correlation coefficient with SAS program. The results of this study were as follows: 1. Total mean SOC was 132.1/203.0 with a range 96.0 to 171.0. The mean score of this item was arranged according to three areas. The meaningfulness was 4.9, managability was 4.7, and comprehensibility was 4.2. 2. Total mean coping resources was 144.5/225.0 with a range 93.0 to 190.0. The highest mean score of item according to the area was social support 4.0 and the lowest mean score was self care coping 2.7. 3. A significant statistical difference between age, religion, spending money and SOC was found. 4. Significant statistical difference between age, educational level, spending money, working period and coping resources were found. 5. Coping resources by the level of SOC was high in the highest group of SOC(F=6.19, p=0.00). With respect to the area of coping resources, higher group of SOC was using much coping resources generally except area of leisure. 6. SOC was positively related to age(r=0.17), working periods(r=0.16), each SOC area, coping resources and all area of them except leisure. 7. The coping resources were positively related to age(r=0.21), working periods(r=0.21), SOC (r=0.49), each coping resources area, and each SOC area. In conclusion, we confirmed that the level of SOC of the subjects was low relatively and the high level of sense of coherence was related to the high score of coping resources. Therefore, it could also contribute to select suitable nurses in their fields. In the future, it's necessary to develop strategies improving SOC.
