Comparative Study on the Perception of the Neighborhood Environment and Experience of Out-of-School Activities between Typical Students and Attention-Deficit Hyperactivity Disorder Patients (ADHD 학생의 동네환경과 특별활동의 경험에 대한 비교연구)

  • Objectives The purpose of this study is to investigate the differences between a attention deficit hyperactivity disorder (ADHD) group and a general student group in terms of the perception of the residential environment and periodic activities outside the classroom. Methods The patient group consisted of 62 outpatients or inpatients at the Department of Pediatric Psychiatry, and 49 patients diagnosed with ADHD were selected. For the control group, 3727 youths in elementary schools, middle schools, and high schools of one city were asked to fill out questionnaires. Of the 1717 respondents who responded to the questionnaires, 245 were selected for a patient-controlled study. The differences between the patient group and the control group were compared using the chi-squared test (${\chi}^2$ test). Results There was a significant difference between the patient group and the control group in areas where students responded by "My neighborhood is dirty and littered (p < 0.05)". There was also a significant difference between the patient group and the control group in terms of having more than one club activity per month (p < 0.05). Conclusions It was found that the patients with ADHD are more aware of perceived negative aspects of the area in which they reside, thus reflecting their anxiety. In addition, it was also found that the patient group did not partake in as many social activities as the control group.

A Study on Developing Patient Clothing of Free-Size (Free-Size 환자복 개발에 관한연구)

  • Hospitals in Korea are having trouble with economic matters because of the low cost of medical insurance and the consumer's demand for high-quality care. The management of patient clothing is very difficult Especially for the management of size is very difficult because the mix of patients in the wards is very different from the stocked clothing sizes. Because of economic matters the ward dose not prepare enough clothing for every size. The nurses in the wards have trouble managing the clothing. Therefore the nurses want free-size patient clothing for all patients. The problem of managing the patient's clothing were surveyed by nureses in the wards. It was known that the free-size patient clothing was needed. The clothing designed from 150cm, to 180cm by 5cm were made. The different sizes of clothing were pretested. The experimental clothing were selected. The experimental patient clothing are designed for between from 165cm and 170cm in height which refers to Korean average. The fitness and sensory test were made by medical personnel outpatient who will be future client for hospital and by specialist in clothing. Finally The free-size patient clothing was made. It was said that there is no difference between the C-type (170cm) and the D-typer(165cm) clothing by specialists in clothing. The outpatient future client for hospital liked the C-type was selected. The questionnaires of the C-type patient clothing was tested by outpatient patients and by medical personnel. Eighty-four point-eightprecent(84.8%) of evaluators liked free-size clothing 75.8% liked unisex use.

Antioxidant Activity of Medicinal Plant Extracts Used as Folk Remedies by Diabetic Patients

  • The aim of this study was to investigate the antioxidant effect of medicinal plants used by diabetic patients. Fourteen medicinal plants were selected and antioxidant activity such as 1,1-diphenyl-2-picrylhydrazyl (DPPH) radical scavenging activity, inhibition effect of linoleic acid autoxidation and low density lipoprotein (LDL) oxidation, thiobarbituric acid (TBA) value were measured. The Cornus officinalis had DPPH radical scavenging activity of 84.79%, which was higher than the 78.79% for $\alpha$-tocopherol. Rosa rugosa Thunberg, Pueraria thundergiana Bentham, Artemisia princeps var. orientalis and Sasamorpha purpurascens Nakai also had high values. Extracts with higher DPPH radical scavenging activities had higher total phenol concentrations, and positive correlations between these parameters were found. Inhibitory activities of linoleic acid autoxidation, LDL oxidation and TBA value, used as indices of oxidative stress, were observed in most of the selected medicinal plants. The highest inhibitory activity for TBA value was observed in the extract of Pueraria thundergiana Bentham. Rosa rugosa Thunberg (75.50 %), Sasamorpha purpurascens Nakai (74.00 %), and Cornus officinalis (73.00 %) all had high antioxidant activity against linoleic acid autoxidation, similar to that of $\alpha$-tocopherol. The same 3 plants also had high in inhibitory activity against LDL oxidation. Therefore, we demonstrated that medicinal plants used as folk remedy by diabetic patients had antioxidant activity. Especially, Rosa rugosa Thunberg, Cornus officinalis, Pueraria thundergiana Bentham, Arlemisia princeps var. orientalis, and Sasamorpha purpurascens Nakai had high phenol concentrations which resulted in high values for DPPH radical scavenging. These same plants exhibited high values for inhibitory activities for TBA value, linoleic acid autoxidation and LDL oxidation, indices associated with lipid peroxidation.

Clinical Features of Acute Subdural Hematomas Caused by Ruptured Intracranial Aneurysms

  • Objective : Spontaneous acute subdural hematomas (aSDH) secondary to ruptured intracranial aneurysms are rarely reported. This report reviews the clinical features, diagnostic modalities, treatments, and outcomes of this unusual and often fatal condition. Methods : We performed a database search for all cases of intracranial aneurysms treated at our hospital between 2005 and 2010. Patients with ruptured intracranial aneurysms who presented with aSDH on initial computed tomography (CT) were selected for inclusion. The clinical conditions, radiologic findings, treatments, and outcomes were assessed. Results : A total of 551 patients were treated for ruptured intracranial aneurysms during the review period. We selected 23 patients (4.2%) who presented with spontaneous aSDH on initial CT. Ruptured aneurysms were detected on initial 3D-CT angiography in all cases. All ruptured aneurysms were located in the anterior portion of the circle of Willis. The World Federation of Neurosurgical Societies grade on admission was V in 17 cases (73.9%). Immediate decompressive craniotomy was performed 22 cases (95.7%). Obliteration of the ruptured aneurysm was achieved in all cases. The Glasgow outcome scales for the cases were good recovery in 5 cases (21.7%), moderate disability to vegetative in 7 cases (30.4%), and death in 11 cases (47.8%). Conclusion : Spontaneous aSDH caused by a ruptured intracranial aneurysm is rare pattern of aneurysmal subarachnoid hemorrhage. For early detection of aneurysm, 3D-CT angiography is useful. Early decompression with obliteration of the aneurysm is recommended. Outcomes were correlated with the clinical grade and CT findings on admission.

Preparation and Hypoglycemic Effect of Reconstituted Grain Added with Selected Medicinal Herb Extract (한방 생약재 추출물을 첨가한 재성형 당뇨쌀의 제조 및 혈당강하 효과)

  • Hot-water extract from selected medicinal herbs of diabetes prescription was mixed with brown rice and pearled barley flours and extruded to prepare a reconstituted grain for diabetes millitus patients. Even though the cooked reconstituted grain containing medicinal herb extract was somewhat different from cooked ordinary rice in textural properties measured by texture analyzer, it was estimated to be organoleptically acceptable in sensory parameters. The reconstituted grain added with medicinal herb extract reduced the rate of dialysis of glucose evaluated by in vitro dialysis experiment. The effects of reconstituted grain diet on blood glucose levels in diabetes patients were studied during 3-week period. Compared to normal diet, the reconstituted grain diet for 2 weeks significantly decreased the fasting and 2-hr postprandial blood glucose levels in diabetes patients by 14% and 10%, respectively. These results suggested that the reconstituted grain could be used as an effective therapeutic diet for the control of diabetes mellitus.

Effect of scaling and root planing combined with systemic doxycycline therapy on glycemic control in diabetes mellitus subjects with chronic generalized periodontitis: a clinical study

  • Purpose: The purpose of study was to compare glycemic control using glycated hemoglobin levels ($HbA_{1c}$) in diabetic patients with chronic generalized periodontitis (CGP) undergoing scaling and root planing (SRP) with and without systemic doxycycline. Methods: Fifty subjects with type 2 diabetes mellitus ($T_2DM$) and CGP receiving antidiabetic therapy were selected for study. The selected subjects were randomly assigned to two groups (test group [TG] and control group [CG]) comprising 25 patients each. The TG received SRP followed by systemic doxycycline. The CG received treatment with SRP only. The periodontal parameters were recorded at baseline (day zero), and every 1 month for 4 months and included probing depth, clinical attachment level, plaque index, gingival index, and $HbA_{1c}$ level were recorded at baseline (day zero) and at the end of 4 months. Results: A statistically significant effect was demonstrated for the periodontal parameters for both the TG and CG. $HbA_{1c}$ values did not show a statistically significant difference in the treatment group as compared to the CG. Conclusions: The authors concluded that nonsurgical periodontal therapy improved glycemic control in patients with $T_2DM$ in both groups, but no statistical difference was observed with adjunctive systemic doxycycline therapy. A further study with a larger sample size is required.

Comparison of Lumbar Stabilization Exercises and Gluteal Strengthening Exercises on Pain, Disability and Psychosocial Factors in Low Back Pain Patients with Lumbar Instability (요추부 불안정성을 가진 요통환자의 요추부 안정화 운동과 둔근 강화 운동이 통증, 기능장애 및 심리사회수준에 미치는 효과 비교연구)

  • Background: Lumbar stabilization exercise and gluteal muscle strengthening exercises are widely used to treat for lower back pain patient. The present study aimed to compare the effects of lumbar stabilization exercise and gluteal muscle strengthening exercises on chronic lower back pain with lumbar spine instability, with regard to pain intensity, disability, and psychosocial factors. Methods: Among 53 patients with chronic lower back pain, those with spine instability were selected using 5 examination tests. The selected 28 patients were randomly assigned to lumbar stabilization exercise group (LSE, n=15) and gluteal strengthening exercise group (GSE, n=13). Each group performed the corresponding exercise for 40 minutes, twice a week for 4 weeks. To analyze and compare the effects, pain intensity, the level of low back disability, and psychosocial factors were assessed before and after intervention. Results: There was significant difference in lower back pain intensity between the two groups before and after intervention. The change in low back disability was significant in the GSE group alone following intervention (p<.05), but no other significant difference was found between the groups. Among psychosocial factors, the changes in the fear-avoidance beliefs questionnaire (FABQ)-physical activity and FABQ-total were significant in the LSE group alone following intervention (p<.05). However, no significant difference were found in these factors between the two groups before and after intervention. Conclusions: LSE and GSE for lower back pain with lumbar spine instability showed no significant difference for pain intensity, physical disability, or psychosocial functioning.

The Effect of Exercise Using Grasping on Shoulder Muscle Activity and Muscle Thickness of Patients with Rotator Cuff Repair

  • Purpose: This study intended to mediate shoulder exercise using grasping and investigate examine how its effects on affects activities of the shoulder surrounding muscles of the shoulder and thickness of shoulder muscle to suggest effective exercise for patients with rotator cuff repair to return to daily life. Methods: This study has been done targeteding male patients more than 6 weeks after having En-masse Suture Bridge Technique as rotator cuff repair of the right shoulder, aged 40 to 55 in a medical institution located in Jeollanam-do. Experimental group I (n=15) was selected for a conducting common exercise therapy program and Experimental group II (n=15) was selected for an conducting exercise therapy program using grasping. %RVC was measured by surface electromyography and muscle thickness was measured by ultrasound before mediation to for analysisze of before and after results within group and between groups. Results: In comparison of change of %RVC, Tthere were meaningful differences in only posterior deltoid and infraspinatus in comparing a change of %RVC within experimental group I (p<0.001),. and Tthere were meaningful differences in both supraspinatus and infraspinatus in muscle thickness within experimental group I (p<0.001). In comparison of change of %RVC, Tthere were meaningful differences in anterior deltoid, posterior deltoid and infraspinatus in comparing a change of %RVC within experimental group II (p<0.05)(p<0.01) (p<0.001), and there were meaningful differences in both supraspinatus and infraspinatus in muscle thickness within experimental group II (p<0.001). In comparison of change of %RVC between groups, Tthere was a meaningful difference in only posterior deltoid in comparing a change of %RVC between groups (p<0.001). Conclusion: We found that exercise therapy using grasping has a positive effect on shoulder stability muscles such as supraspinatus muscle and infraspinatus muscle and activity of deltoid muscle.

A Systematic Review and Meta-analysis on the Effect of Delirium Prevention Intervention in Korean Intensive Care Units (국내 중환자실 섬망 예방 중재에 관한 체계적 고찰 및 메타분석)

  • Purpose : This study aimed to systematically review the preventive interventions for delirium in Korean intensive care unit (ICU) patients and evaluate their efficacy. Methods : For this systematic review and meta-analysis, we searched the literature and selected studies from data sources that included the RISS, KISS, National Central Library, National Assembly Library, DBpia, Science on, MEDLINE, and Cochrane Library. We used Cochrane's revised tool for risk of bias in randomized trials and non-randomized studies of intervention tools to assess the quality of the selected studies. The effect size of the intervention was calculated as odds ratio (OR) and standardized mean difference (SMD). Results : Preventive interventions reported in 23 studies with a total of 4,799 ICU patients were effective in reducing the occurrence of delirium (OR=0.64, 95% CI : 0.49~0.91, p=.011), but not the duration (SMD=-0.22, 95% CI : -0.51~0.08, p=.148). As a result of a subgroup analysis, non-pharmacological interventions were effective in reducing the occurrence of delirium (OR=0.66, 95% CI : 0.47~0.94, p=.020), while pharmacological interventions had no effect (OR=0.68, 95% CI : 0.33~1.40, p=.295). Among the non-pharmacological interventions, multi-component intervention had the largest effect size (OR=0.38, 95% CI : 0.26~0.55, p<.001). Conclusion : Non-pharmacological interventions were effective in reducing the occurrence of delirium. We recommend the development and application of multi-component interventions to prevent delirium in the Korean ICU patients.

Normal Range of Humeral Head Positioning on the Glenoid on Magnetic Resonance Imaging: Validation through Comparison of Computed Tomography and Magnetic Resonance Imaging

  • Background: To determine the normal range of humeral head positioning on magnetic resonance imaging (MRI). Methods: We selected normal subjects (64 patients; group A) to study the normal range of humeral head positioning on the glenoid by MRI measurements. To compare the MRI measurement method with the computed tomography (CT), we selected group B (70 patients) who underwent both MRI and CT. We measured the humeral-scapular alignment (HSA) and the humeral-glenoid alignment (HGA). Results: The HSA in the control group was $1.47{\pm}1.05mm$, and the HGA with and without reconstruction were $1.15{\pm}0.65mm$ and $1.03{\pm}0.59mm$, respectively, on MRI. In the test group, HSA was $2.67{\pm}1.47mm$ and HGA with and without reconstruction was $1.58{\pm}1.16mm$ and $1.49{\pm}1.08mm$, on MRI. On CT, the HSA was $1.72{\pm}1.01mm$, and HGA with and without reconstruction were $1.54{\pm}0.96mm$ and $1.59{\pm}0.93mm$, respectively. HSA was significantly different according to image modality (p=0.0006), but HGA was not significantly different regardless of reconstruction (p=0.8836 and 0.9234). Conclusions: Although additional CT scans can be taken to measure decentering in patients with rotator cuff tears, reliable measurements can be obtained with MRI alone. When using MRI, it is better to use HGA, which is a more reliable measurement value based on the comparison with CT measurement (study design: Study of Diagnostic Test; Level of evidence II).
