소아치과 내원 아동의 진정요법에 대한 보호자의 인식도와 만족도 (On the awareness and satisfaction of parents with sedation therapy of children visiting pediatric dentistry)

  • In pediatric dentistry, sedation therapy is one of methods for managing children's behavior, and various types of sedation therapy are being used. Thus, this study surveyed the awareness of and satisfaction with sedation therapy in 204 parents who visited the Pediatric Dentistry using a self-administered questionnaire during the period from June to December 2008 in order to get information necessary in behavior management for pediatric patients, and obtained results as follows. 1. The most common path of learning about sedation therapy was dentistry (49.5%), and the most common reason for choosing sedation therapy was 'The child was young'(42.3%), which was followed by 'The child is too afraid'(18.8%), 'There are many teeth to be treated'(16.0%), 'To adjust the number of visits'(11.3%), and 'For behavior management'(3.1%). 2. Most of the parents felt anxiety about sedation therapy, and the reasons for the anxiety were 'The child was young'(33.0%), 'Worry about side effects'(31.3%), 'Worry about recovery of consciousness'(25.7%), 'Worry about the blunting of intellectual abilities'(7.6%), and others (2.4%). 3. Satisfaction with sedation therapy was higher in men than in women, and was high in order of 'Stationing of an anesthetist', 'One time completion of treatment', and 'Cautions'. Statistically significant difference was observed between men and women only in question "Have you ever heard about sedation therapy?"(p=0.018). 4. According to age, satisfaction with sedation therapy was highest in parents whose child was less than 48 months old. Satisfaction was high in order of 'Stationing of an anesthetist', 'One time completion of treatment', and 'Cautions', 'Choice of sedation therapy', and statistically significant difference was observed according to age only in question "Have you ever heard about sedation therapy?"(P=0.005). 5. Positive correlation was observed among questions on satisfaction with sedation therapy. In the survey on the awareness of and satisfaction with sedation therapy in parents visiting the dentistry, awareness and satisfaction were generally high with regard to the parents' choice of sedation therapy, but many of the parents answered that they chose sedation therapy and felt anxiety because their child was young. The results of this study need to be considered for behavior management in pediatric dentistry and there should be systematic education and promotion of sedation therapy.

의식하진정요법을 이용한 치과치료가 치과치료 만족도에 미치는 영향 (The effect of dental treatment using conscious sedation therapy on patient satisfaction)

  • Objectives: This study aimed to investigate the effects of conscious sedation on patient satisfaction with dental treatment. Methods: The survey included questions on the patients' general characteristics, dental treatment fear, anxiety, and satisfaction, and patient evaluation by an observer. Statistical analyses were performed using SPSS 20.0 ver. and data were analyzed using frequency analysis, independent t-test, Pearson's correlation coefficient, and multilinear regression analysis. Results: Patients who received conscious sedation therapy showed significantly lower levels of dental fear and anxiety, whereas their dental treatment satisfaction was significantly higher than that of patients who received regular dental treatment (p<0.05). Dental treatment fear, anxiety, satisfaction, and conscious sedation depth were significantly correlated in patients who received conscious sedation therapy (p<0.05). Factors influencing dental treatment satisfaction included age, weight, use of medication, smoking habits, use of conscious sedation therapy, dental treatment fear and anxiety, and conscious sedation depth (p<0.05). Conclusions: Conscious sedation therapy can be an effective method to reduce dental treatment fear and anxiety and improve patient satisfaction.

침자 보사에서 자침 자극 강도의 특성 (Characterizing the Stimulation Intensity in Acupuncture Manipulation Techniques for Tonification and Sedation Therapy)

  • Objectives : The purpose of this study was to characterize the stimulation intensity in acupuncture manipulation techniques for tonification and sedation therapy. Methods : To describe the level of stimulation used in acupuncture manipulation techniques for tonification and sedation therapy, we reviewed the acupuncture manipulation techniques described in classical medical textbooks. Results : Based on the patients' conditions and pattern identification, acupuncture manipulation strategies for tonification and sedation therapy were chosen. For example, the excess condition was treated with sedative therapy, and the deficiency condition was treated with tonification therapy. For tonification therapy, weak to modest stimulation was applied in acupuncture manipulation techniques, whereas intense stimulation was applied for sedative therapy. Even though the intensity of acupuncture stimulation was chosen based on the clinical examination, deqi sensation is a crucial component of acupuncture treatment, and during acupuncture practice, the practitioner should choose the right intensity of acupuncture stimulation based on deqi response in each patient. Conclusions : We concluded that the tonifying and sedative effects of acupuncture treatment are related to the stimulation intensity of acupuncture manipulation techniques. For individualized medicine, the right amount of acupuncture stimulation should be administered based on the patients' conditions and responses, such as deqi responses.

음악요법이 척추마취 하 수술 대상자의 불안, 진정상태 및 스트레스 반응에 미치는 효과 (The Effects of Music Therapy on Anxiety, Sedation, and Stress Responses of Patients Undergoing Surgery with Spinal Anesthesia)

  • Purpose: This study was conducted to examine the effects of music therapy on anxiety, sedation, and stress responses of patients during surgery with spinal anesthesia. Methods: A quasi-experimental design with a non-equivalent control group pre-post test was used. There were 55 adults over 19 years of age scheduled for a lower leg surgery with spinal anesthesia: 27 in the treatment group and 28 in the comparison group. Each subject in the treatment group identified their music preference which was used during the period of surgery which usually lasted, 61 minutes. Data were analyzed using chi-square, Fisher's exact test, independent t-test and repeated measured ANOVA. Results: There were significant reported differences in levels of anxiety, sedation, and stress responses which were measured by plasma cortisol levels, heart rate, and respiratory rate. The findings of all the variables were effectively decreased among those in the treatment group than the comparison group. Conclusion: The preference based music therapy may be useful as a non-pharmacological intervention.

Dexmedetomidine for Repeated Sedation in Pediatric Sedation During Consecutive Radiation Therapy

  • External beam radiotherapy (EBRT) causes anxiety and claustrophobia in pediatric patients. To provide exact figures and radiation amounts, an appropriate sedation or anesthesia can be supplied. Alpha 2 agonist dexmedetomidine has been used for sedation and anesthesia in pediatric imaging. Dexmedetomidine has an advantage because it has minimal respiratory depression and no direct effects on myocardial function. We report repeated sedation with dexmedetomidine for 33 consecutive radiation therapies in 5 years old children.

  • Objectives : Acupoints of LR1(Dadun)${\cdot}$HT9(Shaochong), KI10(Yingu)${\cdot}$HT3(Shaohai) are used as controlling the diseases from heart problems. Especially, when they are used, tonification or sedation techniques method is taken as a controlling the medication for an early stroke in the Korean medicine. So the aim of this study is to investigate the anti-apoptotic and neuroprotective effects of Acupuncture techniques of tonification or sedation at LR1${\cdot}$HT9, KI10${\cdot}$HT3 on the focal ischemia induced by intraluminal filament insertion in rats. Materials and Methods : The focal ischemia was induced by intraluminal filament insertion into middle cerebral artery. The animals were divided into five groups (n=7 in each group) : Control group, no therapy group after ischemia induced; AT-1; acupuncture therapy group at LR1${\cdot}$HT9, KI10${\cdot}$HT3 after ischemia-induced. AT-2; acupuncture therapy at right LR1, HT9(forward direction), KI10${\cdot}$HT3(opposite direction) inserting to the direction of route of the each meridian. AT3; acupuncture therapy at right LR1, HT9(twirling forward with the thumb of right hand 9 times), KI10${\cdot}$HT3(twirling forward with the forefinger of right hand 6 times) AT-4; acupuncture therapy at right LR1, HT9(forward direction, twirling forward with the thumb of right hand 9 times), KI10${\cdot}$HT3(opposite direction, twirling forward with the forefinger of right hand 6 times) inserting to the direction of route of the each meridian. The anti-apoptotic and neuroprotective effects of Acupuncture techniques of tonification or sedation at LR1, HT9, KI10${\cdot}$HT3 were observed by Bax, Bcl-2, mGluR5, Cytochrome c, Cresyl violet and ChAT-stain. Results : The intensity of Bax and Bax/Bcl-2 ratio were increased in ACU-2 and ACU-4 group but decreased in ACU-3 group. The intensity of mGluR5 was increased in ACU-1, ACU-2, ACU-3 and ACU-4 group. The intensity of cytochrome c was decreased in ACU-3 and ACU-4 group. The density of neurons stained by Cresyl violet and ChAT were increased in ACU-1, ACU-3 and ACU-4 group. Conclusions : Our study suggests that acupuncture therapy of tonification at LR1, HT9 by twirling forward with the thumb of right hand 9times, sedation at KI10${\cdot}$HT3 by twirling forward with the forefinger of right hand 6 times after perpendicularly inserting needle shows anti-apoptotic and neuroprotective effects on cholinergic neuron in focal cerebral ischemia of the stroke in rats.

Correlation between the actual sleep time 24 hours prior to an examination and the time to achieve chloral hydrate sedation in pediatric patients in South Korea: a prospective cohort study

  • Purpose: This study investigated correlations between the actual sleep time 24 hours prior to an examination and the time to achieve chloral hydrate sedation in pediatric patients. Methods: With parental consent, 84 children who were placed under moderate or deep sedation with chloral hydrate for examinations from November 19, 2020 to July 9, 2022 were recruited. Results: Patients' average age was 19.9 months. Pediatric neurology patients and those who underwent electroencephalography took significantly longer to achieve sedation with chloral hydrate. There was a negative correlation between the time to achieve sedation and actual sleep time within 24 hours prior to the examination. Positive correlations were found between the actual sleep time 24 hours prior to the examination and the second dose per weight, as well as between the sedation recovery time and awake hours before the examination. Conclusion: Sleep restriction is not an effective adjuvant therapy for chloral hydrate sedation in children, and sedation effects vary according to pediatric patients' characteristics. Therefore, it would be possible to reduce the unnecessary efforts of caregivers who restrict children's sleep for examinations. It is more important to educate parents about safe sedation than about sleep restriction.

수술 중 음악청취가 척수마취 환자의 불안, 진정 및 활력징후에 미치는 효과 (The Effect of Listening to Music on Anxiety, Sedation, and Vital Signs of Patients Undergoing Spinal Anesthesia)

  • Purpose: The aim of this study was to examine the effect of listening to music on the level of anxiety, sedation, and vital signs of patients undergoing surgery with spinal anesthesia. Methods: A convenience sample of 70 adult patients, ASA physical status I-II, scheduled for elective urologic or orthopedic surgery with spinal anesthesia, was included in this study. They were assigned to either an experimental group (n=35), listening to music during surgery, or a control group (n=35), not listening to music. Their anxiety was measured with the Spielberg's Trait and State Anxiety Inventory (STAI-KYZ). Sedation level was monitored with the Bispectral Index, and the vital signs at beginning, 10, 20, 30, and 45 min after operation. Results: The music group reported significantly lower state anxiety level during surgery as compared with the control group (t=3.91, p<.001). Repeated measures of ANOVA indicated a significant by group interaction on BIS index scores of sedation (F=4.23, p=.006). Among the vital signs, only heart rate was a significant by group interaction (F=5.529, p=.004). Conclusion: These findings indicate that listening to music during surgery with spinal anesthesia is a useful and effective nursing intervention to reduce anxiety and to maintain proper sedation.

A Comparison of the Effects of Concomitant Analgesics with Midazolam for Sedative Dental Therapy

  • Purpose: Intravenous sedation with midazolam is common in contemporary dentistry. That is effective for anxious patients, but additional analgesic agent needs to be used, because midazolam alone doesn't have an analgesic effect. This study was performed to select an analgesic agent between an opioid agent, and nonsteroidal anti-inflammatory drugs as adjunctives in intravenous sedation with midazolam. Methods: The subjects were 60 patients who visited the Department of Oral and Maxillofacial Surgery, Sacred Heart Hospital, Hallym University, between August 2009 and February 2010. Conscious sedation was performed on 20 patients of 3 groups (control group, ketorolac group, and fentanyl group), who were divided randomly. The analgesic agent was administrated preoperatively. For sedation, vital signs were recorded. After sedation and operation, subjective questionnaires of the patient and operator were implemented. Results: All of the $SPO_2$, blood pressure, and heart rates stayed within the normal range for sedation. The sedation depth and analgesic effect of the ketorolac group and fentanyl group were similar. In the case of sedation depth, 12 patients in the ketorolac group and 14 patients in the fentanyl group had no memory of surgery. In the case of analgesic effect, the visual analogue scale of pain scored 2~3 in 13 patients in the ketorolac group, and 0~2 in 12 patients in the fentanyl group. The satisfaction of patients and doctors was also similar. Conclusion: Considering the management and complication of an opioid agent, non-steroidal anti-inflammatory drugs is more effective than an opioid agent.

기계환기기 치료 환자들의 진정상태유지를 위한 투약양상 및 사정내용에 대한 일 조사연구 (The Administration Patterns of Sedatives for Sedation of Mechanically Ventilated Patients and Patient Assessment by Nurses)

  • Purpose: The purpose of this descriptive study was to investigate the administration patterns of sedatives and paralytic agents for mechanically ventilated patients. Method: The participants were 18 adult patients who received mechanical ventilation therapy for more than 1 day in the MICU. Collected data were type of injected medications, administration interval, injected amount, administration purpose and patient assessment by nurses. Result: Mean age of the patients was 61 years old. Seventy-two percent were male. Only 2 drugs (midazolam and norcuron) were administered to most of subjects. The type of medication administered was less heterogeneous than in previous studies. Most of the patients received sedatives and paralytics intermittently. Six different methods were used by nurses to administer sedatives and paralytics. The mean injection frequency was 9.3 times/day for sedatives and 10.8 times/day for paralytic agents. The most common purpose for administration of the drugs was full sedation of the patient (57%). Conclusion: Since a few patients received sedatives and paralytic agents higher than usual dose and a few Patients received those drugs lower than usual dose, nurses need to use proper protocols and guidelines for sedation to avoid oversedation and undersedation.
