Tobacco smoking and secondhand smoke exposure are linked to a number of adverse health outcomes. There is a new concept of thirdhand smoke that is residual tobacco smoke contamination remained after the cigarette is extinguished. This paper reviews published studies examining the residual tobacco smoke. Tobacco smoke can be attached to various surfaces and reemitted to air for long period of time. The dynamic process can allow thirdhand smoke exposure to tobacco smoke without direct exposure to secondhand smoke. One critical evidence of the thirdhand smoke exposure was significantly high urinary cotinine level of infant who did not have direct secondhand smoke exposure. Potential exposure to new and more potent chemicals generated from chemical reactions between residual tobacco smoke and ambient air pollutants can get more attention. Considering toxicity and exposure route, children are uniquely susceptible to thirdhand smoke exposure. The review provides strong background information for thirdhand smoke but warrant more researches on this issue.

Background: The aim of this study was to increase knowledge regarding the dangers associated with tobacco use, and decrease secondhand smoke exposure and tobacco use behaviors with an antitobacco messaging campaign among rural, medically underserved, blue-collar workers. Methods: A quasiexperimental study was conducted with employees at two worksites. One worksite received the intervention, which consisted of nine different antitobacco messages. Baseline and follow-up surveys were conducted at each worksite to assess change in knowledge and behavior; the data were compared across the two worksites. Results: Two hundred twenty-two and 243 participants completed baseline and follow-up surveys at the intervention and comparison sites, respectively. A statistically significant difference was seen over time between the worksites on knowledge of the dangers of tobacco (p < 0.0001); the mean knowledge score increased at the intervention site, but remained unchanged at the comparison site. In general, non-smokers at both worksites appeared to try to decrease exposure to secondhand smoke over the follow-up period. Repeated measures analysis indicated that there were no differences in motivation to quit (p = 0.81), interest in quitting (p = 0.40), thinking about quitting (p = 0.53), or several tobacco-use behaviors over time among smokers at the intervention and comparison worksites. There were slight increases over time in the proportion of smokers who do not allow smoking in their homes/vehicles at the intervention worksite, although not statistically significant. Conclusion: Participants at the intervention worksite increased their knowledge regarding the dangers of tobacco use and secondhand smoke exposure. Among current tobacco users, the intervention appeared to increase family rules regarding secondhand smoke exposure in their homes and vehicles.
Objectives: We examined the prevalence of tobacco use and exposure to secondhand smoke among middle-school students in Korea using the Global Youth Tobacco Survey (GYTS) in 2013. Methods: The GYTS in Korea was conducted between July and August 2013 by the Korea Centers for Disease Control and Prevention. Data were collected using a self-administered anonymous questionnaire from a nationally representative sample of middle-school students aged 13-15 years in sampled classrooms. Results: The GYTS in Korea was completed by 4235 students aged 13-15 years in 43 middle schools. Approximately one in five of the students (17.8%) reported that they had tried cigarettes in the past, while 5.2% reported currently being cigarette smokers. Current cigarette smoking was higher in boys (7.5%) than in girls (2.6%). Of the students, 29.7% had been exposed to secondhand smoke at home, 47.4% inside enclosed public places, and 53.9% in outdoor public places. Of the current cigarette smokers, 25.7% bought their cigarettes from a store despite a law prohibiting this. Additionally, 58.0% of students noticed point-of-sale tobacco advertisements or promotions, 66.8% of current cigarette smokers wanted to stop smoking, and 70.9% of students had been taught about the dangers of tobacco use in school. Conclusions: These findings provide an opportunity to develop, implement, and evaluate a comprehensive tobacco control policy. The results suggest that youth have relatively easy access to cigarettes and are regularly exposed to secondhand smoke in public places, as well as to point-of-sale tobacco advertisements and promotions. Strict enforcement of the ban on tobacco sales to youth, expanding smoke-free areas, and advertising bans are needed to reduce tobacco use among youth.
Background: Increasing numbers of young people go to clubs. In Korea, however, no studies have been conducted regarding the exposure of club patrons to secondhand smoke. The present study was conducted to evaluate the degree of club customers' exposure to secondhand smoke. Methods: The study subjects included 10 male and 12 female non-smokers. The investigational site was a club located in Daegu. Urine samples were collected before exposure to secondhand smoke in the club and 6 hours after a 3-hour exposure. The urine cotinine levels were measured via the LC -MS/MS method. A survey was conducted to collect data regarding the subjects' smoking experiences and the degree of exposure to secondhand smoke in their daily lives. Results: The average urine cotinine level increased from 1.09 ${\mu}g/L$ to 5.55 ${\mu}g/L$ ($p$<0.05). No significant difference existed in the change in urine cotinine level between the male and female subjects. In addition, there was no significant difference in the change in urine cotinine level by the degree of exposure to secondhand smoke in daily life. Conclusions: The average urine cotinine level in all the subjects significantly increased after exposure to secondhand smoke. This is the first study on exposure to secondhand smoke in clubs; these results can be used to craft measures that reduce exposure to secondhand smoke in public places, such as clubs.
Nicotine is the main component of environmental tobacco smoke, and its presence in indoor air is widely used as a secondhand-smoke indicator. Environmental tobacco smoke is a major source of indoor air pollution, but sufficient investigation of the uncertainty of its measurement, which mirrors the reliability of nicotine measurement, has not been performed. We calculated the uncertainty of measurement of indoor air nicotine concentration at low, medium, and high concentrations of 11.3798, 10.1977, $98.3768{\mu}g/m^3$, respectively, and we employed the Guide to the Expression of Uncertainty in Measurements (GUM), proposed by the International Organization for Standardization (ISO). The factors considered in determining the uncertainty were uncertainty of the calibration curve (calibration curve and repeated measurements), desorption efficiency, extraction volume, and sampling airflow (accuracy and acceptable limits of flowmeter). The measurement uncertainty was highest at low concentrations; the expanded measurement uncertainty is $0.9435{\mu}g/m^3$ and is represented as a relative uncertainty of 63.38%. At medium and high (concentrations, the relative uncertainty was 13.1% and 9.1%, respectively. The uncertainty of the calibration curve was largest for low indoor nicotine concentrations. To increase reliability of measurement in assessing the effect of secondhand smoke, measures such as increasing the sample injection rate ($1{\mu}L$ or more), increasing sampling volume to increase collected nicotine, and using gas chromatography-mass spectrometry (GC/MS) or GC/MS/MS, which has a lower quantitation threshold, rather than gas chromatography with nitrogen phosphorous detector, should be considered.

Acute eosinophilic pneumonia (AEP) is a disease characterized by an acute febrile onset, eosinophilia in bronchoalveolar lavage fluid, and a dramatic response to corticosteroids. Although many studies have reported a close relationship between direct cigarette smoking and AEP, few studies have identified an association between passive smoking and AEP. Here, we report a case of AEP in a 19-year-old female with cough, fever, and dyspnea after 4 weeks of intense exposure to secondhand smoke for 6 to 8 hours a day in an enclosed area.
Since mid-1960s the reports from the Surgeon General, the World Health Organization, and other health experts state that there is no risk-free level exposure to smoking and secondhand smoke. Tobacco smoke is made up of more than 7,000 chemicals. Hundreds are toxic, and at least 70 are carcinogens. The chemicals in tobacco smoke reach smoker's lungs quickly every time smoker inhale causing damages immediately. Tobacco litigation has been an important tool in tobacco control strategies aimed at limiting the activities of tobacco companies and providing redress to people who have become ill as a result of their use of tobacco products. Tobacco litigation is a kind of tort litigation. Quite often, as in the asbestos and other mass tort litigation episodes, tobacco litigation can play an educational role, warning the public about the magnitude of health risks that might otherwise be less clearly perceived. Tobacco litigation allows smokers, their families or other victims of smoking to sue tobacco companies in order to be compensated for the harm they have suffered. Potential benefits of tobacco litigation include compensation for smoking-related damages, strengthening regulatory activity, publicity, documents disclosure and changing tobacco industry behavior. And also tobacco litigation can limit the political activities of tobacco industry, protect human rights of smokers and non-smokers, increase burden to tobacco price-up and enhance the effects of law and politics in public health.
Objectives: Outdoor tobacco smoke can penetrate into the indoor environment through cracks in the building envelope. This study aimed to characterize the particle size distribution of infiltrated secondhand smoke (SHS) through the gap in a single glazed and a secondary glazed window according to pressure differences in a chamber. Methods: Two polyvinyl chloride sliding windows were evaluated for infiltration, one with a glazed window and the other with a secondary glazed window. Each window was mounted and sealed in a polycarbonate chamber. The air in the chamber was discharged to the outside to establish pressure differences in the chamber (${\Delta}P$). Outdoor smoking sources were simulated at a one-meter distance from the window side of the chamber. The particle size distribution of the infiltrated SHS was measured in the chamber using a portable aerosol spectrometer. The particle size distribution of SHS inside the chamber was normalized by the outdoor peak for fine particles. Results: The particle size distribution of SHS inside the chamber was similar regardless of window type and ${\Delta}P$. It peaked at $0.2-0.3{\mu}m$. Increases in particulate matter (PM) concentrations from SHS infiltration were higher with the glazed window than with the secondary glazed window. PM concentrations of less than $1{\mu}m$ increased as ${\Delta}P$ was increased inside the chamber. Conclusions: The majority of infiltrated SHS particles through window gap was $0.2-0.3{\mu}m$ in size. Outdoor SHS particles infiltrated more with a glazed window than with a secondary glazed window. Particle sizes of less than $1{\mu}m$ were associated with ${\Delta}P$. These findings can be a reference for further research on the measurement of infiltrated SHS in buildings.
Objectives: In Indonesia, 61 million adults smoked in 2018, and 59 million were exposed to secondhand smoke at offices or restaurants in 2011. The Presidential Decree 109/2012 encouraged local governments to implement a smoke-free policy (SFP), and the city of Jayapura enacted a local bill (1/2015) to that effect in 2015. This study aimed to evaluate compliance with this bill and to explore challenges in implementing it. Methods: We conducted a mixed-methods study. Quantitatively, we assessed compliance of facilities with 6 criteria (per the bill): the presence of signage, the lack of smoking activity, the lack of sale of tobacco, the lack of tobacco advertisements, the lack of cigarette smoke, and the lack of ashtrays. We surveyed 192 facilities, including health facilities, educational facilities, places of worship, government offices, and indoor and outdoor public facilities. Qualitatively, we explored challenges in implementation by interviewing 19 informants (government officers, students, and community members). Results: The rate of compliance with all 6 criteria was 17% overall, ranging from 0% at outdoor public facilities to 50% at health facilities. Spatial patterning was absent, as shown by similar compliance rates for SFP facilities within a 1-km boundary around the provincial and city health offices compared to those outside the boundary. Implementation challenges included (1) a limited budget for enforcement, (2) a lack of support from local non-governmental organizations and universities, (3) a lack of public awareness at the facilities themselves, and (4) a lack of examples set by local leaders. Conclusions: Overall compliance was low in Jayapura due to many challenges. This information provides lessons regarding tobacco control policy in underdeveloped areas far from the central government.
Background: The Korean government has established Health Plan and reinforced tobacco control policies step by step according to Framework Convention on Tobacco Control (FCTC). This study aims to investigate yearly smoking and secondhand smoking (SHS) exposure rates adjusted by demographic and socioeconomic factors. Methods: Multiple logistic regression analysis was conducted about the smoking experience, current smoking, and exposure to SHS at home, at work, and in public places using data from the 6th to 8th Korea National Health and Nutrition Examination Survey (2015-2020). Results: Comparing 2015 with other years, smoking experience rates significantly decreased from 2015 to 2018 in men and significantly increased from 2018 to 2020 in women. Compared to 2015, current smoking rates significantly decreased only in 2020 for men and significantly increased in 2018 and 2019 for women. The rate of exposure to SHS at home significantly decreased until 2018. Rates of exposure to SHS in the workplace and public places were significant all year. Conclusion: There were a slowly decreasing trend in men and an increasing trend in women at current smoking rates. It was found that there were significant decreasing trends at exposure to SHS. There is a need to set policies that reduce current smoking rates and especially to set a customized program to lower women's smoking rates.
