칼슘 제제 복용 후 발생한 우유알칼리증후군 (Milk-alkali syndrome secondary to the intake of calcium supplements)

  • Milk-alkali syndrome (MAS), a triad of hypercalcemia, metabolic alkalosis, and renal failure, is associated with ingestion of large amounts of calcium and absorbable alkali. MAS is the third most common cause of hypercalcemia in hospital, after primary hyperparathyroidism and malignant neoplasm. MAS is not often reported in the Korean literature. We describe MAS secondary to intake of calcium citrate for the treatment of osteoporosis with thoracic spine compression fracture. A 70-year-old man presented to our hospital with a 1-week history of general weakness and lethargy. He was found with acute kidney injury (serum creatinine, 4.6 mg/dL), hypercalcemia (total calcium, 14.8 mg/dL), and alkalosis. Laboratory evaluation excluded both hyperparathyroidism and malignancy. Mental status and serum calcium level was normalized within a week after proper hydration and intravenous administration of furosemide. However, he developed aspiration pneumonia, pseudomembranous colitis, and sepsis with multi-organ failure. Despite intensive treatment including inotropics, mechanical ventilation, and renal replacement therapy, he expired with no signs of renal recovery on the 28th hospital day.

일차성 및 이차성 부갑상선기능항진증 환자에서 Double-Phase Tc-99m Sestamibi 스캔에 의한 병변의 국소화에 관한 비교 (Comparison Study of Lesion Localization in Patients with Primary and Secondary Hyperparathyroidism using Double-Phase Tc-99m Sestamibi Scintigraphy)

  • 목적: 일차성 및 이차성 부갑상선기능항진증 환자에서 Tc-99m-sestamibi double-phase 스캔이 병변의 국소화에 보이는 성적을 비교 분석하고자 하였다. 대상 및 방법: 수술로 확진된 14예의 단일 병변과 2예의 다발성 병변을 가지는 총 16명의 18개의 일차성 병변과 11명의 44개의 이차성 병변을 대상으로 Tc-99m-sestamibi 스캔 소견에서의 섭취 정도, 지연 스캔에서의 섭취 변화 등의 영상을 분석하고 진단율을 알아보았다. Tc-99m-sestamibi double-phase 스캔은 LEM (Seimens, Germany) 카메라를 사용하여 얻었으며 740 MBq을 순간 주사한 후 평행 그리고 바늘구멍 조준기를 사용하여 기저 및 지연 영상을 각각 얻었다. 남녀비는 일차성 2명 및 이차성 질환의 3명을 제외한 모든 증례가 여자였으며 (M:F=5 :22) 연령분포는 $29{\sim}65$세로 평균 49.1세였다. 결과, 일차성 병변의 경우는 총 18병변 중 13예는 선종, 5예는 부갑상선증식증이었으며 이차성 병변의 경우는 모두 부갑상선증식증이었다. 일차성 병변은 조직 소견상 선종으로 진단된 13예를 스캔 상에서 모두 발견한 반면 부갑상선증식증에서는 5예중 2예(40%)만이 스캔에서 발견되었다. 또한 일차성 병변 중 3예에서는 기저 스캔에 비하여 지연 스캔에서 섭취의 감소를 보였다. 일차성 병변에서 Double phase 스캔에 의한 민감도는 58.8% (10/17), 특이도 83.3% (10/12), 양성 예측률 83.3% (10/12) 및 정확도 75.9% (22/29)였으며 이차성 병변에서는 민감도 37.5% (15/40), 특이도 50% (2/4), 양성 예측률 88.2% (15/17) 그리고 정확도 38.6% (17/44)였다. 일차성 및 이차성 병변 모두에서의 전체적인 민감도는 43.9% (25/57), 특이도는 73% (12/16), 양성 예측률 86.2% (25/29) 및 정확도 53.4% (39/73) 였다. 그 외에 일차성 및 이차성 부갑상선 병변 간에 무게의 차이는 없었다(p>0.05). 결론: Tc-99m-sestamibi 스캔은 일차성 부갑상선 병변의 국소화에 높은 민감도 및 특이도를 보이지만 일부에서는 지연스캔상의 섭취가 감소하므로 기저 스캔 영상을 참조하는 것이 진단에 도움을 줄 수 있으며 이차성 병변의 경우, 전반적으로 낮은 예민도를 보이지만 이소성 병변을 수술 전에 검사함으로써 수술의 성공률을 높이는 데 도움을 줄 수 있으므로 계속된 연구가 필요하겠다.

어린 shepherd 개에서 요독증의 조직 병리학적 연구 (Histopathological study of uremia in a juvenile shepherd dog)

  • A 7 month old male shepherd was presented with anorexia, diarrhea, and salivation, and euthanized due to no clinical improvement. Grossly, ulcers were seen on ventral surface of tongue and stomach. The mineralization was obvious in the intercostal space, lung and inner surface of aorta. Kidney was pale, firm and irregular. Histopathology confirmed uremic pneumonitis, end stage kidney and hyperplasia of parathyroid. This case was the typical uremic condition of juvenile dog, which was characterized the diffuse soft tissue mineralization due to hyperparathyroidism secondary to chronic renal failure.

Minimally Invasive Treatment for Benign Parathyroid Lesions: Treatment Efficacy and Safety Based on Nodule Characteristics

  • Objective: This study aimed to evaluate the efficacy and safety of ultrasound (US)-guided minimally invasive treatment in patients with parathyroid lesions. Materials and Methods: This study included 27 patients who had undergone US-guided radiofrequency ablation (RFA) or ethanol ablation (EA) for parathyroid lesions between January 2010 and 2018. RFA was performed in 19 patients with primary hyperparathyroidism (PHPT, n = 11) or secondary hyperparathyroidism (SHPT, n = 8), and EA was performed in eight patients with symptomatic nonfunctioning parathyroid cysts (SNPCs). Nodule size, volume, serum parathyroid hormone (PTH) and calcium levels were recorded before and after treatment. Complications were evaluated during and after treatment. Results: In patients with PHPT, significant reductions in size and volume were noted after RFA at 6- and 12-month follow-up (all, p < 0.05). Seven nodules nearly completely disappeared (residual volume < 0.1 mL); serum PTH and calcium levels were reduced to normal ranges (7/11, 63.6%). Four patients experienced partial reductions of serum PTH and calcium levels (4/11, 36.4%). In patients with SHPT, three experienced therapeutic response of serum PTH (3/8, 37.5%), while five showed persistent hyperparathyroidism (5/8, 62.5%) within 6 months after RFA. In patients with SNPCs, EA resulted in significant reductions in cyst size and volume (all, p < 0.05) at the last follow-up. A total of four complications (two transient hypocalcemia [RFA], one permanent [RFA], and one transient [EA] hoarseness) were observed. Conclusion: Minimally invasive treatments, such as RFA and EA, may serve as therapeutic alternatives for patients with PHPT or SNPCs; they may have limited usefulness in patients with SHPT.

만성신부전 모델쥐에서 인의 함량은 같고 단백질 수준이 다른 식이가 신기능과 골격 대사에 미치는 영향 (Effect of Dietary Protein Level on Renal Function and Bone Metabolism in Rats with Reduced Renal Mass When Dietary P Level is Equal)

  • Studies were carried out to explore the influence of dietary protein level on bone metabolism in uninephrectomized rat (experimental renal failure model) when dietary Ca and P contents were equal. Male rats were uninephrectomized or sham operated and fed 8%, 15% and 40% casein diets for 24 weeks. Ca and P contents of the all diet were 0.4% and 0.6% respectively. The results are summarized as follows. We did not found any significant difference in PTH and Ca level of the serum, Ca intake and Ca excretion among the experimental groups. There was significant positive correlation between the PTH and phosphate level. There was significant inverse correlation between serum Ca and creatinine level. The effect of the dietary protein level and renal mass loss on density and Ca contents of the bone were small and different according to the kinds of the bone. Low protein diet was associated with a significant enhancement of scapular density. Femur and vertebra density, however, were not influenced by dietary protein level and uninephrectomy. Light microscopic examination showed several calcified foci in the kidney in all experimental groups. Low protein diets have been used for a long time in the conservative management of chronic renal failure as they have a beneficial effect in preventing the appearance of symptoms. This study elucidated that part of beneficial effects of the low protein diet related to the suppression of the hyperphosphatemia. And these results, even though uninephrectomized rats fed high protein diet, the secondary hyperparathyroidism is supressed by the regulation of the P level. Therefore this study emphasized the need to pay more attention to the regulation of dietary P level as well as dietary protein content in chronic renal failure. 제주경주마의 영양성이차성 상피소체 기능항진증 (Nutritional Secondary Hyperparathyroidism in Cheju Pony Racehorces)

  • This study was peformed to investigate the morbidity of nutritional secondary hyperparathyroidism(NSH) caused by imbalance of Ca and p, and related athletic disease in Cheju pony racehorse. The seventeen horses with clinical signs among 33 NSH affected, administered CaCO$_3$(34 g) and Vita-rinka1(120 g) respectively for 40 days. The results were asd follows; Morbidity of NSR was 33 among 47 horses, and it was caused by the deficiency of Ca in 32 horses. In a case, level of Ca was norm질 although P was high. There was no case of Ca deficiency with P excess. Among 33 NSH affected horses, 13 were subclinical and 20 were clinical types with severe lameness in 6 and transient lameness in 14. Although there was no difference in bone density between transient lameness and normal horses on radiography, among six horses wlth severe lameness two showed hyperplasia at periosteum, one had low density of phalanges and metacarpal bones, and thin cortex. and there with fracture at carpus, nivicular bone and proximal sesamoids. The levels of FECa and FEP were recovered after CaCO$_3$ administration in 2 horses among ten, and after Vita-rinkal in all of seven. The clinical signs were disappeared in slx horses among ten CaCO$_3$ treated, and in five among seven Vita-rinkal treated. There were no differences on radiography in bone density and thickness of cortex on 14 horses with transient lameness. Three horses with severe signs were recovered to normal bone density and thickness of cortex, and there was no significant difference between two groups. In summary, the morbidity of NSH in Cheju pony racehorses was relatively high because of deficiency of Ca. Constant admistration of Ca supplements is desirable to treat and prevent athletic disease development in Cheju racehorses.

Sevelamer 인결합제와 투석환자의 Chronic Kidney Disease-Mineral Bone Disorder 관리 (Management of Chronic Kidney Disease-Mineral Bone Disorder with Sevelamer Hcl Phosphate Binder in Korean Patients with Dialysis)

  • Background: Sevelamer is associated with reduced complications of chronic kidney disease-mineral bone disorder (CKD-MBD) resulted from hyperphosphatemia, which may contribute mortality, in CKD patients with dialysis. So far clinical outcomes of sevelamer on mortality and risk of cardiovascular mortality related to CKD-MBD are debating. Purpose of this study was to evaluate the effectiveness of sevelamer HCl on mortality of secondary hyperparathyroidism (SHPT), risk of cardiovascular mortality and, frequency of osteopathy in end stage renal disease (ESRD) patients with dialysis. Methods: We retrospectively reviewed the electronic medical records of 536 patients with ESRD, who were admitted for moderate to severe SHPT, for 36 months. 75 patients who met inclusion criteria were evaluated for the efficacy of sevelamer (mean serum iPTH = 487.5 pg/mL). Results: Sevelamer intervention was not associated with increased three-year survival time compared with non-sevelamers group [average survival month: 30.4 months in sevelamer group, 26.8 months in non-sevelamer group, p = 0.463]. Sevelamer intervention was not associated with significant mortality benefit and cardiovascular mortality benefit as compared to non-sevelamer group [sevelamer group: non-sevelamer group, all-cause mortality (iPTH > 600 pg/mL): 14.3% (1/34): 20% (1/41) p = 0.962, OR = 0.935, 95% CI, 0.058-14.98, heart disease mortality: 6.67% (2/30): 0% (0/32) p = 0.138]. Sevelamer was not associated with significantly lower cumulative incidence of osteopathy compared to non-sevelamer group (sevelamer group: non-sevelamer group, 5.9% (2/34):9.8% (4/41); p = 0.538; OR = 0.578; 95% CI, 0.099-3.367). Conclusion: Sevelamer was not associated with decreased all-cause mortality and risk of cardiovascular mortality compared to non-sevelamer group in ESRD patients with SHPT.

X-선학적 진단에 의한 대사성 골질환 예 (Radiographical Diagnosis of Metabolic Bone Disease)

  • Seven patients with signs of metabolic bone disease clinically seen at the Veterinary Teaching Hospital of Seoul National University from May 1990 to February 1992 were evaluated retrospectively. 1. Clinical signs include bowed leg, deformity of spine, digestive disturbance, constipation, retarded growth, lameness and hindlimb paralysis. 2. Radiographical sings include generalized skeletal demineralization, decreased radiological contrast between skeleton and soft tissues, thinning of bone cortex and pathological fractures. The diagnosis of nutritional secondary hyperparathyroidism resulting in metabolic bone disease caused by mineral imbalance was made based upon history, clinical signs, physical examination and radiographical signs.

Late side effects of radiation treatment for head and neck cancer

  • Patients undergoing radiation therapy for head and neck cancer (HNC) experience significant early and long-term side effects. The likelihood and severity of complications depends on a number of factors, including the total dose of radiation delivered, over what time it was delivered and what parts of the head and neck received radiation. Late side effects include: permanent loss of saliva; osteoradionecrosis; radiation recall myositis, pharyngoesophageal stenosis; dental caries; oral cavity necrosis; fibrosis; impaired wound healing; skin changes and skin cancer; lymphedema; hypothyroidism, hyperparathyroidism, lightheadedness, dizziness and headaches; secondary cancer; and eye, ear, neurological and neck structures damage. Patients who undergo radiotherapy for nasopharyngeal carcinoma tend to suffer from chronic sinusitis. These side effects present difficult challenges to the patients and their caregivers and require life-long strategies to alleviate their deleterious effect on basic life functions and on the quality of life. This review presents these side effects and their management.

Tumoral calcinosis and calciphylaxis treated with subtotal parathyroidectomy and sodium thiosulphate

  • Tumoral calcinosis (TC) is a condition resulting from extensive calcium phosphate precipitation, primarily in the periarticular tissues around major joints. Calciphylaxis is a fatal ischemic vasculopathy mainly affecting dermal blood vessels and subcutaneous fat. This syndrome is rare and predominantly occurs in patients with end-stage renal disease. Here, we report on a rare case involving a patient with TC complicated with calciphylaxis. Our patient was a 31-year-old man undergoing hemodialysis who presented with masses on both shoulders and necrotic cutaneous ulcers, which were associated with secondary hyperparathyroidism, on his lower legs. He underwent subtotal parathyroidectomy, and sodium thiosulfate (STS) was administered for 27 weeks. Twenty months after beginning the STS treatment course, he experienced dramatic relief of his TC and calciphylaxis.
