일반 성인들의 심폐소생술 자기효능감에 미치는 영향 요인 (Factors Affecting Self-efficacy of Cardiopulmonary Resuscitation(CPR) in Adults)

  • 이 연구는 일반 성인들을 대상으로 심폐소생술 자기효능감에 미치는 영향 요인들을 포괄적으로 탐색함으로써 일반인의 심폐소생술 시행률을 향상시킬 뿐만 아니라 궁극적으로 급성 심정지 환자의 생존율을 향상시키기 위한 다양한 전략 개발에 필요한 근거 자료를 마련하고자 수행하였다. 연구 자료는 질병관리본부가 매년 수행하는 2016년 지역사회 건강조사의 원시자료를 이용하였고, 연구대상은 성인(만 19-64세) 총 164,165명, 조사방법은 가구방문 1:1 면접 조사, 조사기간은 2016년 8월 16일부터 10월 31일까지이었다. 조사내용은 심폐소생술 자기효능감 보유여부, 심폐소생술 용어 인지여부, 심폐소생술 교육 경험여부, 인구사회학적 특성, 의료 특성, 건강증진 행위, 안전 행위 실천여부를 파악하였다. 일반 성인들의 심폐소생술 자기효능감 보유율은 60.6%로 다소 낮은 수준이었다. 로지스틱 회귀분석 결과, 심폐소생술 자기효능감 영향 요인으로 고연령 일수록, 남자가, 읍면지역 거주자가, 고학력일수록, 직업이 있을 때와 특히 군인에서, 미혼보다는 배우자가 없을 때 심폐소생술 자기효능감이 있을 가능성이 높았다. 평생 심폐소생술 교육 경험이 있을 때, 최근 2년간 심폐소생술 교육 경험이 있을 때, 최근 2년간 마네킨을 이용한 실습 교육 경험이 있을 때 심폐소생술 자기효능감이 있을 가능성이 높았다. 1년간 사고나 중독으로 응급실 방문이 있을 때, 신체활동 실천을 할 때, 자전거 운전을 할 때, 다른 사람 운전차량 뒷좌석 탑승 시 안전벨트를 착용할 때, 버스 탑승 시 안전벨트 착용할 때에 심폐소생술 자기효능감이 있을 가능성이 높았다. 결론적으로 일반 성인들을 대상으로 심폐소생술 자기효능감의 향상을 도모하는 전략 수립 시 영향요인으로 밝혀진 최근 시기의 심폐소생술 교육 경험률 향상, 건강증진 행위 실천 향상, 안전 행위 실천을 향상 시킬 수 있는 교육 프로그램이나 전 국민 대상 홍보 전략 등이 마련되어야 하겠다. 일반 성인들의 심폐소생술 자기효능감 향상은 실제 목격자 심폐소생술 시행률을 높이며 궁극적으로는 심정지 환자의 생존율 향상에 기여할 수 있을 것이다.

25인승 버스 전복사고의 탑승자 손상 분석 (Injury Analysis of a 25-passenger Bus Left-quarter Turn Rollover Accident)

  • Purpose: Rollover motor vehicle crashes have a higher injury severity and fatality than other motor vehicle crash types. From a left-quarter turn rollover accident of 25-passenger bus, we intend to assess the injury of the occupant and the injury severities according to the occupants' position. Methods: We carried out the 3 steps investigation of occupants' interview, visiting the repair shop and using the police report. We analyzed injuries sustained by occupants, and compared injury severities considering column, row in occupant's position and passenger interaction Results: The rollover involved 14 passengers in the bus who were all old women except a man driver. The most common injury was in the upper extremity, with six occurrences being a left clavicle fracture. Major injuries including hemothorax and pneumothorax were diagnosed at left side of the occupant. In the comparison of injury severity among driver's column (left side), mid column and passengercolumn, ISS of passenger column was the highest ($9.9{\pm}7.4$, $8.8{\pm}5.5$, and $10.3{\pm}4.0$, respectively, p>0.05). The injury severity of multiple occupants by row was higher than that of single occupant (10.8 vs. 3, p<0.05). Conclusion: An occupant must fasten their seat belt to prevent an injury by passenger interaction in the left-quarter turn rollover accident of a bus.

Safety belt effectiveness versus crash types

  • Based on Fatal Accident Reporting System (FARS) data, safety belt effect- tiveness in preventing fatalities is investigated for the following five types of crashes: frontal, left, rear, right, and rollover. Passenger cars containing two occupants, a driver and a right front passenger, are included in this analysis. For each crash type, these cars containing the two occupants are classified into four categories according to the safety belt usage categories for the two front seat occupants, namely, both belted, both unbelted, and either one was belted but not both. Relative risks of driver and right front passenger fatalities are compared among these four cases. For each crash type, two independent estimates of safety belt effectiveness are obtained for drivers and for right front passengers. The weighted average of the two estimates is calculated for drivers and for right front passengers for the five crash types. Using FARS data starting 1978 throught 1983, safety belts are more effective in rollover accidents than in frontal collisions. In rollover accidents, safety belt effectiveness estimate for drivers is $68%{\pm} 6% $ and that for right front passengers is $71%{\pm}6% $ , in which the error limits indicate one standard error. Sfety belt effectiveness estimates for drivers and right front passengers involved in frontal collisions are $41%{\pm} 9% $ and $37%{\pm} 10% $ , respectively. For left and right sided collisions and for both drivers and right-front-passengers, none of the four estimates are significantly different from 0%, statistically : however, when left and right sided collisions are combined with far sided occupants(drivers involved in right sided collisions and right front passengers involved in left sided collisions) safety belt effectiveness is significant, $38%{\pm} 12% $ . For rear collisions, the estimate for drivers shows statistically significant positive effect, $60%{\pm}23% $ . while for right-front-passengers the estimate is not significantly different from 0%. These results show that a safety belt is an effective restraint system not only in frontal crashes but also in a variety of crashes.

구급차 내부 디자인 개선에 관한 연구 (A study on improving the interior design of ambulance)

  • Purpose: This research project addressed the need to designing more safe and efficient interior of the future ambulance in Korea. Methods: The study sample contained 760 paramedics in 4 districts. Data was collected by using a revised and complemented questionnaire based on literature review. Results: In relation to the efficacy and safety of work, answers related to storage closet showed to be the highest, and the most difficult part of paramedic work in an ambulance was lurching. CPR is the most frequently used emergency care inside an ambulance, but 66% of the paramedics responded that accurate CPR is not possible during vehicle transfer. Safety belts are not worn for 82.8% of the time, because of discomfort (51.3%). 13.8% of the paramedics responded that stretchers are unstable, 29.5% had an experience of having patients fall off the stretcher inside an ambulance. There were comments on installing equipments to prevent noise, and assist communication. Conclusion: The suggested practical layout contains five main modifications 1. Developing specially designed belt is needed for paramedic safety & efficient work. 2. The seats are molded to be ergonomically friendly. 3. Equipments to secure the body and safety devices for CPR are needed. 4. System improvement for communication between the driver seat and paramedics is needed. 5. The stretchers are molded to be maximize efficiency and minimize injury.

Use of child safety seats during transportation of newborns

  • Purpose: Child safety seats (CSS) are critical for the protection of children, in case of motor vehicle accidents. Although the national legislation mandates that all newborns must be placed in an appropriately installed CSS during transportation, people often do not perceive the importance of CSS and do not use it as recommended. The purpose of this survey was to understand the use of CSS for the safe transport of newborns from hospital to home. Methods: We interviewed parents of newborn infants, using a structured questionnaire, at the time of their discharge from Cheil General Hospital & Women's Health Care Center, between May 2014 and July 2014. Results: A total of 403 participants were interviewed. The rate of CSS use was only 14.9%. Overall, 76.4% of the families interviewed were not aware about the recommendations on CSS use for newborns when travelling in a car. The provision of education on using CSS significantly influenced their rate of use. Parents who were educated about mounting the CSS in a car used it more as compared with others (25.7% vs. 12.2%) (P=0.002). Furthermore, if parents had heard about the importance or necessity of CSS, they used it more than others did (19.5% vs. 10.6%, P=0.032). Conclusion: Despite the legal regulation, most parents transport their newborn infants without a CSS while traveling from hospital to their home. The rate of CSS use was influenced by parental education and their knowledge about its necessity. Education programs for parents must be reinforced to increase the CSS use.

12인승 밴 전복사고의 상해 분석 (Injury Analysis of a 12-passenger Van Rollover Accident)

  • The fatality of rollover accidents in motor vehicle crashes is high despite their low incidence. Through the investigation of a 12-passenger van rollover accident in which 10 passengers were involved, we intend to analyze the correlation between the severity of the injury and the position of the occupants. We collected accident information from medical records, interviews, photo-images of the damaged van, field surveys, and the results of the Korean New Car Assessment Program (KNCAP). Based on the occupants' position, we classified injury sites and estimated injury severity. Passenger injury severity was evaluated by trauma score calculation. The initiation type of the rollover accident was passenger side 'fall-over' and the Collision Deformation Classification (CDC) code for the damaged van was 00TDZO3. The crash of the van involved 10 passengers, with an average age of $16.3{\pm}4.2years$. Few of the occupants had fastened seat belts at the time of the incident, and there was no airbag installed. One patient sustained severe liver injury and another was diagnosed with a fracture of the right humerus. The most common injuries were at the upper extremities and the neck. The average of Injury Severity Score (ISS) was $4.8{\pm}5.9$, and the average ISS of right-seated, mid-seated and left-seated occupants was $7.5{\pm}9.3$, $1.5{\pm}0.7$, and $3.3{\pm}2.1$ respectively (p>0.05). In the rollover (to-passenger side) accident of occupant unfastened, the average ISS of right-seated occupants (near side) was higher, but there was no statistically significant difference.

서울 경기 지역 교사의 건강행동과 건강상태 (School Teachers' Health Behaviors and Health Status in Seoul and Gyeonggi-do)

  • Objectives: Few studies have focused on health behaviors and health status among teachers even though these are one of principles and values of WHO's Health Promoting Schools. In this study, we described health behaviors and health status among teachers and explored the differences of these by personal and school characteristics. Methods: A stratified and random cluster sampling design was used to obtain a cross-sectional sample. A total of 59 elementary, middle and high schools in Seoul and Gyeonggi areas were included in which 333 teachers (males 40.6%, females 59.4%) participated in this survey. Chi-square tests were conducted with SPSS 17.1. Results: Overall, health behaviors and health status among teachers were better than the general public according to Korea National Health and Nutrition Examination Survey. Male and middle/high school teachers had higher prevalence of alcohol dependence and of driving under the influence of alcohol. Also, their rate of wearing seat belts were lower compare to female. In other hand, female, health and nutrition teachers, rural school teachers had lower prevalence of meeting recommended levels of physical activity. Underweight were more prevalent to them in contrast to male with higher prevalence of obesity. In addition, teachers of health promoting schools were not likely to ride on a car with a drunk driver. They also have a lower prevalence of skipping breakfast and have higher positive perception of health for themselves. Conclusion: Teachers' health should be given higher concerns as a key component of school based health promotion. Gender and age specific programs should be considered to develop health promoting programs for them.

자율주행자동차 정면충돌평가방안 마련을 위한 국내 정면충돌사고 심층분석 연구 (An In-depth Analysis of Head-on Collision Accidents for Frontal Crash Tests of Automated Driving Vehicles)

  • The seating postures of passengers in the automated driving vehicle are possible in atypical forms such as rear-facing and lying down. It is necessary to improve devices such as airbags and seat belts to protect occupants from injury in accidents of the automated driving vehicle, and collision safety evaluation tests must be newly developed. The purpose of this study is to define representative types of head-on collision accidents to develop collision standards for autonomous vehicles that take into account changes in driving behavior and occupants' postures. 150 frontal collision cases remained by filtering (accident videos, images, AIS 2+, passenger car, etc…) and random sampling from approximately 320,000 accidents claimed by a major insurance company over the past 5 years. The most frequent accident type is a head-on collision between a vehicle going straight and a vehicle turning left from the opposite side, accounting for 54.7% of all accidents, and most of these accidents occur in permissive left turns. The next most common frontal collision is the center-lane violation by drowsy driving and careless driving, accounting for 21.3% of the total. For the two types above, data such as vehicle speed, contact point/area, and PDOF at the moment of impact are obtained through accident reconstruction using PC-Crash. As a result, two types of autonomous vehicle crash safety test scenarios are proposed: (1) a frontal oblique collision test based on the accident types between a straight vehicle and a left-turning vehicle, and (2) a small overlap collision test based on the head-on accidents of center-lane violation.

건강증진과 관련된 행태에 영향을 미치는 인구사회학적 특성 (Association of Health-related Behaviors with Socio-demographic Characteristics)

  • 인구사회학적 변수가 건강생활양식 실천도와 건강증진관련 행위의 실천율에 미치는 영향을 파악하기 위하여 1993년 6월 1일부터 2개월간 경주시에 거주하는 주민 1,903명을 대상으로 사전에 훈련된 조사원이 가구방문을 통하여 면접조사표에 의거 직접 면접식 설문조사를 실시하였다. 설문조사 내용은 인구학적 및 사회경제적 특성, 상병 및 의료이용양상, 그리고 건강증진과 관련된 행위(남자 24개, 여자 26개 항목)이었다. 조사대상자의 사회인구학적 특성별로 건강생활양식 실천도가 높은 경우한 남자의 고연령군, 유배우 기혼자와 사별한 자, 종교거 없는 사람, 아파트 및 연립주택 거주자, 남자의 저소득층과 여자의 고소득층, 공무원 및 사립학교 교직원 의료보험 가입자, 고학력자, 전문관리직 종사자, 만성질환 이환자 및 처체증자 등이었다. 건강증진 관련행위의 실천율에 있어 남자는 흡연, 음주, 청량음료음용, 육류섭취, 식염섭취 및 건강 진단율이 여자에 비해서 높았으며 여자는 과일 채소섭취율, 양치질수행률 및 우유음용률이 남자보다 높았다. 연령이 증가할수록 실천율이 높아지는 건강증진 관련행태로는 과일 채소섭취, 규칙적 식사, 아침식사, 식염섭취, 의사방문, 혈압측정 및 단골의사 보유였으며, 연령이 증가할수록 실천율이 낮아지는 것으로는 안전벨트착용, 양치질수행, 커피음용, 차음용, 청량음료음용, 우유음용 및 간염예방 접종이었다. 월평균 가구소득이 증가할수록 실천율이 높아지는 건강관련 행태로는 규칙적인 운동, 안전벨트 착용, 양치질수행, 커피음용, 차음용, 우유음용, 과일 채소섭취, 육류섭취, 건강진단, 간염예방접종 및 자궁암 검진이었으며, 반대로 소득이 낮을수록 실천율이 높은 것으로는 흡연을, 규칙적인 식사, 아침식사 및 유방암자가검진이었다. 생산직 및 농 어 축산업에 종사하는 사람은 다른 작업군에 비해 흡연율, 음주율, 의사방문율 및 단골의사 보유율이 높았고 안전벨트착용률, 양치질수행률 및 간염예방접종률은 낮았다. 판매 서비스직종의 경우는 커피음용률, 육류섭취율, 식염섭취율, 자궁암 검진율이 다른 직업군에 비해 높았던 반면, 충분한 수면을, 규칙적인 식사율, 아침식사율, 혈압측정률, 건강진단율은 낮았다. 사무직 및 전문 관리직이 다른 직업군에 비해 규칙적인 운동, 맨손체조, 안전벨트착용, 양치질수행, 차음용, 청량음료음용, 우유음용, 혈압측정, 건강진단, 간염예방접종 실천율은 높았으며, 식염섭취율과 자궁암 검진율은 낮았다. 교육수준이 높을수록 규칙적인 운동, 맨손체조, 안전벨트착용, 양치질수행, 커피음용, 차음용, 우유음용, 육류섭취, 간염예방접종 및 보조제 복용행위의 실천율이 높았으며, 음주율, 아침식사율 및 식염섭취율은 낮았다. 초졸 이하 군에서는 흡연율, 규칙적인 식사율, 의사방문율, 혈압측정률 및 단골 의사보유율이 높았다. 만성질환에 이환된 사람은 흡연, 음주, 규칙적인 운동, 맨손체조, 규칙적인 식사, 아침식사, 육류 섭취, 식염섭취, 의사방문, 혈압측정, 건강진단, 단골의사보유, 보조제 복용 등의 실천율이 높았으며, 만성질환에 이환되지 않은 사람은 안전벨트착용, 양치질수행, 충분한 수면, 커피음용, 차음용, 청량음료음용, 우유음용, 과일 채소섭취율, 간염예방접종, 자궁암 검진, 유방암 자가검진 등의 실천율이 높았다. 정상체중군에서는 흡연, 음주, 안전벨트착용 등의 실천율이 높았고, 과체중군에서는 규칙적인 운동, 맨손체조, 육류섭취, 식염섭취, 혈압측정, 건강진단, 단골의사보유율이 높았으며, 저체중군에서는 양치질수행, 우유음용, 보조제 복용률이 높았다. 결론적으로 인구사회학적 특성에 따라 건강생활양식 실천도와 건강증진 관련행위의 실천양상이 다양하게 나타났다. 따라서 건강증진 프로그램에 주민의 참여를 촉진시키고 건강관련행위의 실천을 권장하는 전략으로는 주민의 사회경제적인 특성을 고려한 건강증진 프로그램이 전개되어야 한다.

옴니 휠 드라이브를 이용한 1인승 탑승 로봇의 사용성 평가 연구 (Research of Usability Test on Single-Seater Rider Robot using Omni Wheel Drive)

  • 본 연구는 옴니휠을 이용한 1인승 탑승 로봇개발을 통해 장애인 및 비장애인 탑승자의 거동특성과 안정성을 고려한 사용자의 불편요소와 개선방안을 파악하여 총 6명의 사용성 평가를 실시하였다. 본 연구 결과를 보면 다음과 같다. 첫째, 대구대학교 신애5호관 기숙사에서 실험대상자들에게 옴니휠을 이용한 1인승 로봇을 사용하여 불편요소를 파악하는데 있는 승하차의 경우 비장애인이 만족도 4점보다 장애인이 만족도 1.6점으로 가장 큰 불편함을 느꼈다. 이는 휠체어 높이보다 제품의 시트높이가 10cm 더 높아 이동하기 어려웠던 것으로 분석된다. 둘째, 1인승 로봇의 주행와 안전을 위해 장착된 시트, 등받이, 암레스트, 풋레스트, 안전벨트 등 장애인과 비장애인 모다 3점 평균이하의 각각 2.33점과 2.62점의 만족도를 보였으나, 가변형설계 및 설계디자인의 경우 장애인과 비장애인 모두 3.5점의 평균 이상 만족도를 나타내었다. 셋째, 1인승 로봇의 가격의 경우 6~7백만원으로 상당히 비싸다는 의견이 많아 정부 보조금 지원이 가능하다면 전동휠체어 대용으로 구입하겠다는 의견을 나타내었다. 따라서 장애인 사용성 평가에서 나타난 불편사항과 개선방안을 토대로 추가적인 연구개발이 이루어져 교통약자들에게 스스로 탑승 및 이동하여 간단한 일상생활 하는데 있어 삶의 질을 높일 필요성이 있다.
