The Correlation of Serum Osteoprotegerin with Non-Traditional Cardiovascular Risk Factors and Arterial Stiffness in Patients with Pre-Dialysis Chronic Kidney Disease: Results from the KNOW-CKD Study

  • Background: Osteoprotegerin (OPG) plays protective roles against the development of vascular calcification (VC) which greatly contributes to the increased cardiovascular events in patients with chronic kidney disease (CKD). The present study aimed to find the non-traditional, kidney-related cardiovascular risk factors correlated to serum OPG and the effect of serum OPG on the arterial stiffness measured by brachial ankle pulse wave velocity (baPWV) in patients with the pre-dialysis CKD. Methods: We cross-sectionally analyzed the data from the patients in whom baPWV and the serum OPG were measured at the time of enrollment in a prospective pre-dialysis CKD cohort study in Korea. Results: Along with traditional cardiovascular risk factors such as age, diabetes mellitus, pulse pressure, and baPWV, non-traditional, kidney-related factors such as albuminuria, plasma level of hemoglobin, total $CO_2$ content, alkaline phosphatase, and corrected calcium were independent variables for serum OPG in multivariate linear regression. Reciprocally, the serum OPG was positively associated with baPWV in multivariate linear regression. The baPWV in the 3rd and 4th quartile groups of serum OPG were higher than that in the 1st quartile group after adjustments by age, sex and other significant factors for baPWV in linear mixed model. Conclusion: Non-traditional, kidney-related cardiovascular risk factors in addition to traditional cardiovascular risk factors were related to serum level of OPG in CKD. Serum OPG level was significantly related to baPWV. Our study suggests that kidney-related factors involved in CKD-specific pathways for VC play a role in the increased secretion of OPG into circulation in patients with CKD.

The Association of Employment Status, Workplace Environment, and Hazard Exposure with Health Outcome in the Adult Korean Population according to KNHANES IV (제 4기 국민건강영양조사 자료를 이용한 근로자들의 근로형태, 작업환경 및 유해요인 노출과 건강검진결과의 관련성)

  • Objectives: This study was designed to investigate the current status of the association of job-related information such as employment status, workplace environment, and hazard material exposures with health examination outcomes. Methods: The study used data from KNHANES 2007-2009 representing the three years of 2007-2009, which was conducted annually using a rolling sampling design that involved a complex, stratified, multistage, probability-cluster survey of a representative sample of the non-institutionalized civilian population in Korea. The final analytical sample consisted of 17,240 participants. Information on age, education, smoking history and alcohol intake was collected during the health interview. Job related information consisted of employment status, workplace environment, and hazardous material exposure. The selected indices of health examination were blood pressure, fasting glucose, blood cholesterol, HDL, SGOT, SGPT, and BUN. Results: In multiple logistic regression analysis using hypertension and pre-hypertension as dependent variables and job related categories as independent variables after covariate adjustments, the odds of hypertension and pre-hypertension were significantly lower in those with responsibility and power in their job activities. Interestingly, low odds for hypertension were observed among those who reported that their jobs were fast-paced. Conclusions: This study confirmed that some job-related categories in employment status, workplace environment, and hazardous material exposure had an association with health outcome status. It is worthwhile to comment that high responsibility and power in job activities were revealed as one of the important favorable factors to improve health condition of workers.

Comparison of Gesture Characteristics of Career Teachers and Novice Teachers in Elementary Science Class - Focused on the 5th Grade Unit of the Function and Structure of Our Body - (초등과학 수업에서 경력교사와 초보교사의 제스처 특징 비교 - 우리 몸의 구조와 기능 단원을 중심으로 -)

  • The purpose of this study is to analyze the characteristics and differences of gesture between career teachers and novice teachers in elementary science class. In order to analyze the gesture of elementary science teachers, gesture analysis framework was developed. The teachers who participated in the experiment were 2 beginner teachers and 2 career teachers. We analyzed 'bones and muscles', 'digestion', 'breathing', and 'excretion' of 'body' section in the second semester of 5th grade. The video recording of the class scene with the camcorder was recorded and analyzed by Observer XT. The results of this study are summarized as follows. First, the career teacher lessens unnecessary gestures than the novice teacher. During the class, the career teachers lessened the gestures not related to the context of the class. These differences were more prominent in the activities of the group with many unexpected situations than those of the teachers who prepared the class contents. Second, career teachers have more communication control act than novice teachers. Career teachers have often made adjustments to induce learner presentations or control unnecessary utterances. Third, career teachers efficiently interacted with learners using gestures that can enhance communication.

  • Purpose - The current study will provide a cause-effect explanation that will reinforce the existing perception that effective communication fosters organizational communication. The findings will also empower organizations with information on why they must polish their communication strategies. Research design, Data, and methodology - This study has collected numerous textual datasets through 'Qualitative Content Approach' (QCA). The author of this research had a responsibility to develop comprehensible methods for categorizing and drawing inferences from raw data in addition to data collecting tactics that may effectively solicit representations. Result - The current study ultimately concludes that effective communication is the most cost-effective tool for motivating employees. Two-way organization communication allows employees to communicate the hardships they encounter in their job. The result of this is adjustments that are geared towards eliminating work-related challenges. Total four suggestions exist for practitioners in the main texts on 'Results' section. Conclusion - According to theoretical research, organizational commitment is anchored on the ability of an organization's leadership to communicate the organization's values to lower-level employees. Moreover, the findings reiterate the importance of creating leadership systems that embody an organization's core values. Communication systems that engender this ensure that management discloses the rationale for their business and corporate strategies to employees.

Children's YouTube Watching Behaviors and Their Adjustments: Mediation Effects of Parental Attitude, Self Esteem, and School Satisfaction (초등학생의 유튜브 이용과 심리적 적응 : 부모양육태도, 자아존중감, 학교생활만족도의 매개효과)

  • The present study was designed to examine how much Korean elementary school children used the YouTube, and tested whether the overuse of YouTube might be related to adjustment problems such as low parental warmth, low self-esteem, and low school satisfaction. Two hundred and thirty-eight elementary school students answered a questionnaire enquiring about their YouTube use, parental practice, self-esteem, and school satisfaction. Results indicated that more than half of the participants watched the YouTube at least one hour per day, and 15.6% of them watched the YouTube more than 3 hours per day. Negative relationships between YouTube watching time and parental warmth, self-esteem, and school satisfaction were found. YouTube watching time also partially mediated between parental warmth and adjustment problems. Parental rejection and overprotection were not related to children's YouTube watching time. These study results can be used as seminal data for developing children's YouTube watching guidelines.

Effects of Adversities during Childhood on Anxiety Symptoms in Children and Adolescents: Comparison of Typically Developing Children and Attention-Deficit/Hyperactivity Disorder Group

  • Objectives: Childhood adversity is a risk factor for anxiety symptoms, but it affects anxiety symptoms in attention-deficit/hyperactivity disorder (ADHD). The current study aimed to examine the association between childhood adversity and anxiety symptoms in participants with and without ADHD. Methods: Data were obtained from a school-based epidemiological study of 1017 randomly selected children and adolescents. The ADHD and non-ADHD groups were divided using the Diagnostic Interview Schedule for Children Predictive Scale (DPS). The DPS was also used to assess comorbidities such as anxiety and mood disorders. The childhood adversities were assessed using the Early Trauma Inventory Self Report-Short Form, and the anxiety symptoms were assessed using the Screen for Child Anxiety Related Disorders. Linear and logistic regression models were used to investigate the association between childhood adversity and anxiety in the ADHD and non-ADHD groups with adjustments for age and sex. Results: This study found that the ADHD group did not show any significant association between anxiety symptoms and childhood adversities, whereas the non-ADHD group always showed a significant association. In a subgroup analysis of the non-ADHD group, the normal group without any psychiatric disorders assessed with DPS demonstrated a statistically significant association between childhood adversities and anxiety symptoms. These results were consistent with the association between childhood adversities and anxiety disorders assessed using DPS, as shown by logistic regression. Conclusion: The association between anxiety symptoms and childhood adversities statistically disappears in ADHD; ADHD may mask or block the association. Further longitudinal research is necessary to investigate this relationship.

Electrical Characterization of Ultrathin Film Electrolytes for Micro-SOFCs

  • The reliability of solid oxide fuel cells (SOFCs) particularly depends on the high quality of solid oxide electrolytes. The application of thinner electrolytes and multi electrolyte layers requires a more reliable characterization method. Most of the investigations on thin film solid electrolytes have been made for the parallel transport along the interface, which is not however directly related to the fuel cell performance of those electrolytes. In this work an array of ion-blocking metallic Ti/Au microelectrodes with about a $160{\mu}m$ diameter was applied on top of an ultrathin ($1{\mu}m$) yttria-stabilized-zirconia/gadolinium-doped-ceria (YSZ/GDC) heterolayer solid electrolyte in a micro-SOFC prepared by PLD as well as an 8-${\mu}m$ thick YSZ layer by screen printing, to study the transport characteristics in the perpendicular direction relevant for fuel cell operation. While the capacitance variation in the electrode area supported the working principle of the measurement technique, other local variations could be related to the quality of the electrolyte layers and deposited electrode points. While the small electrode size and low temperature measurements increaseed the electrolyte resistances enough for the reliable estimation, the impedance spectra appeared to consist of only a large electrode polarization. Modulus representation distinguished two high frequency responses with resistance magnitude differing by orders of magnitude, which can be ascribed to the gadolinium-doped ceria buffer electrolyte layer with a 200 nm thickness and yttria-stabilized zirconia layer of about $1{\mu}m$. The major impedance response was attributed to the resistance due to electron hole conduction in GDC due to the ion-blocking top electrodes with activation energy of 0.7 eV. The respective conductivity values were obtained by model analysis using empirical Havriliak-Negami elements and by temperature adjustments with respect to the conductivity of the YSZ layers.

Association of Soy Foods With Gastric Cancer Considering Helicobacter pylori: A Multi-Center Case-Control Study

  • Purpose: This study aims to explore the relationship between soy food consumption and gastric cancer (GC) risk, accounting for Helicobacter pylori infection status. Materials and Methods: We analyzed data from patients with GC and healthy individuals prospectively enrolled by 6 hospitals between 2016 and 2018. Dietary intake was evaluated using questionnaires that categorized seven dietary habits and 19 food groups. Multivariate logistic regression models were applied to examine associations. Model I adjusted for various epidemiological factors, while Model II included further adjustments for H. pylori infection. Primary exposures examined were consumption frequencies of nonfermented, unsalted soy foods (soybean/tofu) and fermented, salty soy foods (soybean paste stew). Results: A total of 5,535 participants were included, with 1,629 diagnosed with GC. In Model I, the frequency of soybean/tofu consumption was inversely related to GC risk; adjusted odd ratios (aORs) were 0.62 (95% confidence interval [CI], 0.48-0.8), 0.38 (95% CI, 0.3-0.49), 0.42 (95% CI, 0.33-0.53), and 0.33 (95% CI, 0.27-0.42) for 1 time/week, 2 times/week, 3 times/week, and ≥4 times/week. Consumption of 2 servings/week of soybean paste stew showed the lowest GC association, forming a V-shaped curve. Both low (aOR, 4.03; 95% CI, 3.09-5.26) and high serving frequencies of soybean paste stew (aOR, 2.23; 95% CI, 1.76-2.82) were associated with GC. The association between soy foods and GC in Model II was similar to that in Model I. The soy food-GC associations were consistent across sexes in Model I. Nonetheless, the positive correlation between frequent consumption of soybean paste stew (≥5 times/week) and GC was more pronounced in women (aOR, 7.58; 95% CI, 3.20-17.99) compared to men (aOR, 3.03; 95% CI, 1.61-5.88) in Model II. Subgroup analyses by H. pylori status and salty diet revealed a consistent inverse relationship between soybean/tofu and GC risk. In contrast, soybean paste stew showed a V-shaped relationship in H. pylori-positive or salty diet groups and no significant association in the H. pylori-negative group. Conclusions: Soybean/tofu intake is consistently associated with a decreased risk of GC. However, the relationship between soybean paste stew consumption and GC risk varies, depending on H. pylori infection status and dietary salt intake.

Study on the Correlation between Dietary Vitamin B Intakes and Clinical Indices of Type 2 Diabetes Patients (제2형 당뇨병 환자의 비타민 B 섭취와 임상지표의 상관관계에 대한 연구)

  • The objective of this study was to investigate the effects of dietary vitamin B intake on biomarkers related to lipid metabolism, inflammation and blood glucose control, that are important in the development of type 2 diabetes and its complications. Seventy-six adults (42 males, 34 females) were recruited from a group of diabetes patients who had visited the medical center for treatment. Data on anthropometric characteristics and dietary intake of thiamine, riboflavin, niacin, vitamin B6 and folate were collected using 24-hour diet recall and the CAN Pro 4.0 program. Also, data on clinical indices such as serum lipids, blood pressure, high-sensitivity C-reactive protein (hs-CRP), hemoglobin A1c (HbA1c) and homeostasis model assessment 2-insulin resistance (HOMA2-IR) were collected and analyzed for correlation with dietary vitamin B intake. Results from the dietary intake survey showed that riboflavin and folate intake (in males) and folate intake (in females) were below the Dietary Reference Intake for Koreans. Statistical analysis revealed a negative correlation between hs-CRP and dietary intake of B vitamins. Riboflavin intake was inversely associated with systolic blood pressure after adjustments for age, BMI, smoking, alcohol consumption, exercise, ingestion of diabetes mellitus medication and energy intake (p<0.05). Our results suggest that dietary vitamin B may influence inflammation and consequently may help in better management of type 2 diabetes.

Level of Nutrition Knowledge, Diet Practice and Education Demands in Dialysis Patients with Chronic Renal Failure (투석치료 중인 만성신부전 환자의 영양지식 수준과 식사요법 실천 정도 및 교육요구도)

  • The number of patients is increasing and their mean age is also increasing. Proper dietary adjustments are necessary to prevent protein-calorie malnutrition or complications but it is difficult for dialysis patients to adapt to diet therapy due to stress or anorexia. Education does not consider the individual characteristics, knowledge, dietary inhabit education demands, and initial education. The purpose of this study was to identify dialysis patient's nutrition knowledge and, dietary practice and compare those with nutrition education or counseling demands for providing basic data of desirable nutrition management. The data were collected by a survey consisting of the general characteristics, disease related characteristics, nutrition education and counsel characteristics, level of nutrition knowledge, diet therapy, and nutrition education and counsel demands from the 28th March to 22th July 2017. The total number of subjects were 33 patients among dialysis patients at two tertiary medical institutions and an artificial kidney room at a private hospital in Incheon Gyeonggi. The data collected were analyzed statistically using the SPSS program 23.0, followed by further analyses using frequency analysis, one-way ANOVA, cross analysis, and correlation analysis. The results of the dialysis patients showed that younger (P<0.05), female (P<0.05), abnormal high school diploma (P<0.001) groups had high nutrition scores. In addition, dietary practice and nutrition education and counsel demands showed a positive correlation (P<0.05, P<0.01). In particular, females were higher than males in nutrition knowledge, dietary practice, nutrition education, and counseling demand scores.
