In this study, a failure mode analysis of CNC machining center is described. First, the system is classified through subsystems into components using part lists and drawings. The component failure rate and failure mode analysis are performed to identify the weak components of a machining center with reliability database. The failure probabilistic function of mechanical part is analyzed by Weibull distribution. The Kolmogorov-Smirnov test is also used to verify the goodness of fit.
Objectives : The aim of this study was to explore the academic failure experiences of Korean Medicine Students. The academic failure in a medical school is defined as the situation the student who have academic failure need to repeat their academic year again. Methods : Data were collected through in-depth individual interviews from April to May 2016 and analyzed through Colaizzi's phenomenological methodology. The participants were interviewed twice, for 30minutes to 60minutes per interview. Results : Eight theme-clusters of were identified from 21 themes. The theme-clusters were 'Dream of university life and freedom', 'Meeting Stranger academic field and growing study stress', 'Difficulty of self-management', 'Weak supporting system', 'Feeling of guilty, sense of isolation and decreasing in self-esteem', 'The experience during the vacation after the academic failure', 'The experience of returning to school' and 'Self growing through hard experience'. Conclusions : Understanding of academic failure experience of Korean Medicine Students is an important approach to counsel them and an important way to reach a good model of education at a medical college.
Storage system often applies erasure codes to protect against disk failure and ensure system reliability and availability. Liberation code that is a type of coding scheme has been widely used in many storage systems because its encoding and modifying operations are efficient. However, it cannot effectively achieve fast recovery from single disk failure in storage systems, and has great influence on recovery performance as well as response time of client requests. To solve this problem, in this paper, we present HRSF, a Hybrid Recovery method for solving Single disk Failure. We present the optimal algorithm to accelerate failure recovery process. Theoretical analysis proves that our scheme consumes approximately 25% less amount of data read than the conventional method. In the evaluation, we perform extensive experiments by setting different number of disks and chunk sizes. The results show that HRSF outperforms conventional method in terms of the amount of data read and failure recovery time.
In this paper, a modified rigid body spring model (RBSM) is proposed and used to analyze the damage and failure process of reinforced concrete (RC) structures. In the proposed model, the concrete is represented by an assembly of rigid blocks connected with a uniform distribution of normal and tangential springs to simulate the macroscopic mechanical behavior of concrete. Steel bars are evenly dispersed into rigid blocks as a kind of homogeneous axial material, and an additional uniform distribution of axial and dowel springs is defined to consider the axial stiffness and dowel action of steel bars. Perfect bond between the concrete and steel bars is assumed, and tension stiffening effect of steel bars is modeled by adjusting the constitutive relationship for the tensile reinforcement. Adjacent blocks are allowed to separate at the contact interface, which makes it convenient and easy to simulate the cracking process of concrete. The failure of the springs is determined by the Mohr-Coulomb type criterion with the tension and compression caps. The effectiveness of the proposed method is confirmed by elastic analyses of a cantilever beam under different loading conditions and failure analyses of a RC beam under two-point loading.
A long-span transmission tower-line system is indispensable for long-distance electricity transmission across a large river or valley; hence, the failure of this system, especially the collapse of the supporting towers, has serious impacts on power grids. To ensure the safety and reliability of transmission systems, this study experimentally and numerically investigates the collapse failure of a 220 kV long-span transmission tower-line system subjected to severe earthquakes. A 1:20 scale model of a transmission tower-line system is constructed in this research, and shaking table tests are carried out. Furthermore, numerical studies are conducted in ABAQUS by using the Tian-Ma-Qu material model, the results of which are compared with the experimental findings. Good agreement is found between the experimental and numerical results, showing that the numerical simulation based on the Tian-Ma-Qu material model is able to predict the weak points and collapse process of the long-span transmission tower-line system. The failure of diagonal members at weak points constitutes the collapse-inducing factor, and the ultimate capacity and weakest segment vary with different seismic wave excitations. This research can further enrich the database for the seismic performance of long-span transmission tower-line systems.
An ultra-high voltage (UHV) transmission system has the advantages of low circuitry loss, high bulk capacity and long-distance transmission capabilities over conventional transmission systems, but it is easier for this system to cross fault rupture zones and become damaged during earthquakes. This paper experimentally and numerically investigates the seismic responses and collapse failure of a UHV transmission tower-line system crossing a fault. A 1:25 reduced-scale model is constructed and tested by using shaking tables to evaluate the influence of the forward-directivity and fling-step effects on the responses of suspension-type towers. Furthermore, the collapse failure tests of the system under specific cross-fault scenarios are carried out. The corresponding finite element (FE) model is established in ABAQUS software and verified based on the Tian-Ma-Qu material model. The results reveal that the seismic responses of the transmission system under the cross-fault scenario are larger than those under the near-fault scenario, and the permanent ground displacements in the fling-step ground motions tend to magnify the seismic responses of the fault-crossing transmission system. The critical collapse peak ground acceleration (PGA), failure mode and weak position determined by the model experiment and numerical simulation are in relatively good agreement. The sequential failure of the members in Segments 4 and 5 leads to the collapse of the entire model, whereas other segments basically remain in the intact state.

As earthquakes have increased in Korea recently, people are paying attention to the seismic performance of buildings built in the past. Many school buildings in Korea were built based on standard drawings before the seismic design was applied. However, since school buildings are often designated as emergency evacuation facilities in case of disasters such as earthquakes, seismic evaluation and retrofit must be done quickly. This study investigated the failure modes among structural components (beams, columns, and joints), focusing on 1980s standard drawings for school buildings. The effects of column axial force, partial masonry infills, and different material strengths for concrete and rebar were considered for detailed evaluation. As a result, most of the joints were found to be the weakest among structural components. Column axial forces tended to make the joints more vulnerable, and partial masonry infills increased the possibility of joint failure and shear failure in columns.

Background: Free tissue transfer is the preferred method of reconstructing head and neck defects, with a success rate of approximately 95%. Although flap failure is uncommon, it has a major impact on patient morbidity and diminishes quality of life, making it is important to investigate the causes of flap failure. Methods: This retrospective chart review analyzed patients who underwent free tissue transfer during head and neck reconstruction at a single institution between 2016 and 2021. Results: During the study period, 58 patients underwent 60 free flap procedures. Revision surgery was needed in 14 patients. Subsequent free flap surgery was performed in one patient, and three free flaps (5%) could not be salvaged. Cardiovascular disease was significantly associated with flap failure, and venous congestion (thrombosis) was the most common reason for revision surgery. Conclusion: Cardiovascular disease clearly emerged as a factor related to the failure of free flap surgery, and this issue warrants particular attention in patients for whom free tissue transfer is planned.

This paper presents an experimental study on the behavior of composite floor slab comprised by a new steel sheet and concrete slab. The strength of composite slabs depends mainly on the strength of the connection between the steel sheet and concrete, which is denoted by longitudinal shear strength. The composite slabs have three main failures modes, failure by bending, vertical shear failure and longitudinal shear failure. These modes are based on the load versus deflection curves that are obtained in bending tests. The longitudinal shear failure is brittle due to the mechanical connection was not capable of transferring the shear force until the failure by bending occurs. The vertical shear failure is observed in slabs with short span, large heights and high concentrated loads subjected near the supports. In order to analyze the behavior of the composite slab with a new steel sheet, six bending tests were undertaken aiming to provide information on their longitudinal shear strength, and to assess the failure mechanisms of the proposed connections. Two groups of slabs were tested, one with 3000 mm in length and other with 1500 mm in length. The tested composite slabs showed satisfactory composite behavior and longitudinal shear resistance, as good as well, the analysis confirmed that the developed sheet is suitable for use in composite structures without damage to the global behavior.

In deep braced excavations, struts and walers play an essential role in the whole supporting system. For multi-level strut systems, accidental strut failure is possible. Once a single strut fails, it is possible for the loads carried from the previous failed strut to be transferred to the adjacent struts and therefore cause one or more struts to fail. Consequently, progressive collapse may occur and cause the whole excavation system to fail. One of the reasons for the Nicoll Highway Collapse was attributed to the failure of the struts and walers. Consequently, for the design of braced excavation systems in Singapore, one of the requirements by the building authorities is to perform one-strut failure analyses, in order to ensure that there is no progressive collapse when one strut was damaged due to a construction accident. Therefore, plane strain 2D and three-dimensional (3D) finite element analyses of one-strut failure of the braced excavation system were carried out in this study to investigate the effects of one-strut failure on the adjacent struts.
