Validation of the Korean Functional Gait Assessment in Patients With Stroke (뇌졸중 환자를 대상으로 실시한 한글판 기능적 보행평가의 타당도)

  • Background: The Functional Gait Assessment (FGA) was developed to measure of gait-related activities. The FGA was translated in Korean but only a few psychometric characteristics had been studied. Objects: The purpose of this study was to evaluate the validity and reliability of the Korean version of FGA scale using Rasch analysis. Methods: The study included 120 patients with stroke (age range=30~83 years; mean${\pm}$standard deviation=$58.3{\pm}11.1$). The FGA and Berg Balance Scale were performed, and were analysed for dimensionality of the scale, item difficulty, scale reliability and separation, and item-person map using Rasch analysis. Results: The 4 rating scale categories of FGA were satisfied with optimal rating scale criteria. The most items of the FGA showed sound item psychometric properties except 2 items ('gait with the horizontal head turns', and 'gait with narrow base of support'), and the 2 misfit items were excluded for all further analyses. The 8 items were arranged in order of difficulty. The most difficult item was 'gait with eyes closed', the middle difficult item was 'gait level surface', and the easiest item was 'gait with vertical head turns.' A person separation reliability was .93 and the person separation index was 3.57. Conclusion: This study suggests that the 8-item Korean FGA are valid measure of assess the gait-related balance performance, and to set the goal of rehabilitation plan in patient with stroke.

Analgesic Effect of Electroacupuncture after Femoral Head Osteotomy in Dogs (개에서 대퇴골두 제거술 후 전침 자극이 통증완화에 미치는 영향)

  • This experiment was designed to investigate the analgesic effect of non-invasive electroacupuncture treatment on femoral head osteotomy in dogs. Twelve dogs were divided into 3 groups in this experiment. Electroacupuncture group was non-invasively electrostimulated in Huan Tiao (GB-30) acupoint for 10 minutes by 4.5V, 5Hz for 10 days. Medication group was treated with carprofen (4mg/kg, P.O.) for 10 days. Control group was not treated. Each group was pre-operatively and post-operatively scored by using VAS (Visual Analogue Scale), SDS (Simple Descriptive Scale), NRS (Numerical Rating Scale), and measured serum cortisol levels. The results of VAS, SDS and NRS in electroacupuncture and medication groups were better than those of control group. And, the results of electroacupuncture group were better than those of medication group since 6th day after osteotomy in pain management(p<0.05). But, there were no significances in serum cortisol level among 3 groups. In conclusion, non-invasive electroacupuncture treatment showed sufficient analgesic effect on postoperative pain management after femoral head osteotomy, and VAS, SDS, and NRS scoring system would be adapted in evaluation of pain management.

Item Goodness-of-fit and Difficulty of Childhood Autism Rating Scale(CARS) - Application of Rasch Model - (아동기 자폐증 평정척도(CARS)의 문항 적합도 및 난이도 -Rasch 모형의 적용-)

  • The purpose of this study was to investigate item goodness-of-fit of Childhood Autism Rating Scale(CARS), Rasch rating scale model was applied to 15 items of the CARS in a sample of pervasive development disorder(n=238). An assumption to test Rasch Model, which is satisfaction of unidimensionality, is regarded through PCAR analysis, and jMetrik 4.03 program is used to test the goodness-of-fit of items. The results of this study were: First, 5-point rating scale was appropriate for the CARS rather than 7-point original rating scale. Second, the result of examining the CARS questions goodness-of-fit, there was a overfitting or misfitting items according to the classified groups. Only in particular Q11 item in diagnosis subject of integration population of autism has become inappropriate. Therefore, it is necessary to provide education for the CARS more systematically. Thirdly, the result of comparing the personal attributes score and difficulty of a CARS question, Q2, Q3, Q10, Q11 items are necessary to distinguish conceptually defined in more detail. Fourth, the results of investigating the difficulty of CARS question, it was found to exhibit a verbal communication is most serious problem for the population of autism.

A Rating Tabulation System of Amenity / Disamenity Elements in Rural Villages (농촌마을 수준에서의 어메니티 강화 및 저해요소 평가등급체계 구축)

  • This study, a sub-one of comprehensive research work titled under "A Evaluation System of Amenity / Disamenity Elements in Rural Villages", tried to propose a rational methodology for development of rating tabulation for evaluation of amenity / disamenity elements in rural villages. The system developed in the study has the formalized rating grades between 2 and 5 for convenience/simplification of analysis work. For each evaluation element in the lowest order of the system, 5 types of rating technique were arranged, which were based on statistical analysis using mean and standard deviation values, its existence or not, relative significance of its holding volume and quality, synthesized weight scale of its quality and quantity, and the others. And, standardized 4-steps procedure for rating tabulation was proposed. Finally, through case studies of 4 sample villages representing the flat-plain, upland, seashore and periurban rural areas, the applicability of this rating tabulation system was verified.

Maternal Separation Anxiety on Quality of Day Care and Mother's Self-Esteem (탁아의 질과 어머니 자아존중감에 따른 어머니 격리불안)

  • The purpose of this study was to examine the maternal separation anxiety depending on the quality of day care and mother's self-esteem. subjects consisted of 187 mothers who had children from 60 t 36 months of age. the instruments used were the questionnaires which consisted of the Maternal separation anxiety scale(Cho and park, 1992), Self-Esteem Inventory(Coopersmith, 1967), and the checklist which consisted of the early childhood environment Rating scale(Harms & Clifford, 1980) and the Family Day Care Rating scale(Harms & Clifford, 1989). The datas were analyzed by the statistical method based o frequency, percentile, t-test, and one-way ANOVA. The main results were as follows : First, there was a significant difference in maternal separation anxiety by mothers' age and not by their educational levels. Second, there were no significant differences in maternal separation anxiety by children's age, sex, and girth order. Third, there was no significant difference in maternal separation anxiety by quality of day 2 care, size of group, and adult-child ratio. Forth, there was a significant differences in maternal separation anxiety by mother's self-esteem.

The Clinical Study of Two Severe Menopausal Disorder Patients Treated by Gamisoyo-san (가미소요산(加味逍遙散)으로 호전된 중증 갱년기 장애 환자 2례에 대한 증례보고)

  • Objectives : The purpose of this study is to evaluate the effects of Gamisoyo-san on severe menopausal disorder patients. Methods : We recruited 2 severe menopausal disorder patients who have been suffering from hot flush and heat sensation. The patients had been administrated Gamisoyo-san. The patients of severe menopausal disorder had been estimated with Kupperman's index(KI), Menopause rating scale(MRS), Beck's depression Inventory(BDI), Visual analogue scale(VAS) and Heart rate variability (HRV). Kupperman's index, Menopause rating scale and Beck's depression inventory for every seven days. We measured VAS everyday. Heart rate variability was estimated twice, before and after treatment. Results : After the treatments, symptoms of menopausal disorder were decreased. Conclusions : This study suggests that Gamisoyo-san is significantly effective on severe menopausal disorder patients with hot flush and heat sensation.

The Clinical Case Report by Acupuncture Treatment for Dizziness after Acoustic Neuroma Surgery (청신경초종 수술 이후 어지럼증에 대한 침구 치료 1례)

  • Objectives : The objective of this clinical case report is to describe an acupuncture treatment for dizziness after acoustic neuroma surgery. Methods : The patient was treated with acupuncture treatment for 4 months, 3 times per week. Duration for every treatment were 30 min by manual acupuncture with De Qi, which was designed to treat the dizziness and accompanied symptoms. Side-lying test was also used to evaluate dizziness and Hamilton Rating Scale for Depression(HDRS) to evaluate depression. We evaluated the clinical result of the treatment by observing the patients symptoms. Results : After acupuncture treatments for 4 months, patient's Side-lying test reduced to 10 second, and Hamilton Rating Scale for Depression reduced 4 points. Conclusions : The clinical case treatment report may suggest that the acupuncture treatment can be a meaningful option for the intractable dizziness after acoustic neuroma surgery, and also able to reduced depression scale.

The Development of a Scale for the Measurement of Children's Temperament (아동용 기질 측정도구의 개발연구)

  • The purpose of this study was to develop a rating scale for measuring children's temperament. The subjects were 622(324 boys and 298 girls) 3-to 7-year-old children. The temperament rating scale was composed of 50 items(4-point Liken scales) was based on mother's interview, RITQ and PTQ. The raters were the subjects' mothers. The data were analysed on dimensions of validity, reliability, and item discrimination. Statistical methods were factor analysis, ${\chi}^2$-test, and calculations of Pearson's r Cramer's V and Cronbach's $\alpha$. On validity 5 factors were extracted, which explained 84% of the total common variance. The 5 factors were named 'malleability', 'activity', 'sensitive reactivity', 'eating habits & physical constraints', 'physiological rhythmicity'. Regarding reliability, Cronbach's $\alpha$ appeared between .66 and .81 among the 5 factors. Regarding item discrimination, the degree of discrimination on all 50 items was significant and acceptable.

Maternal Parenting Behaviors and Preschoolers' Peer Competence : Mediating Effects of Preschoolers' Internal Representations (어머니의 양육행동과 유아의 또래 유능성 : 유아 내적 표상의 매개 효과 검증)

  • Data were collected from 110 preschoolers, 59 boys, 51 girls (ages 4-5) and their mothers. Peer competence was assessed by the Child Behavior Scale (Birsh & Ladd, 1998) and the Peer Rating Scale (Asher et al., 1979). Children's internal representations were measured by the MacArthur Story-Stem Battery (2004) and coded by the MacArthur Narrative Coding Manual (2004). Maternal parenting behaviors were observed during mother-child interaction at home and analyzed with the Teaching Strategies Rating Scale (Erickson, Sroufe, & Egeland, 1985). Data were analyzed by structural equation modeling analysis. Results confirmed the pathway from maternal parenting behaviors via children's internal representations to peer competence showing a significantly good model fit.

Relationships Between Learning-Related Social Skills and Literacy Development of Young Children (유아의 학습관련 사회적 기술과 문식성 발달과의 관계)

  • To examine relationships between learning-related social skills(LRSS) and literacy development a sample of 167 children aged 5-6 years were selected from 3 preschools in Seoul. Instruments for measuring LRSS were the cooperation, assertion, and self-control scales of the Social Skills Rating System (Gresham & Elliott, 1990) and the mastery behaviors scales of the Child Behavior Rating Scale (Bronson, et al. 1990). Literacy development was measured by the Concepts about Print(Woon, 1999), the Learning Readiness Scale(Korean Educational Development Institution 1998), and the Writing Development Scale(Lee, 1997). Teachers rated children's LRSS. Data were analyzed by mean, standard deviation, t-test, bivariate correlation, and regression analysis. LRSS correlated with children's literacy development. Mastery behaviors were the best predictor of literacy development.
