Halitosis and Related Factors among Rural Residents (농촌지역 주민들의 구취실태와 유발요인)

  • This study was conducted through an interview process in which questionnaires were administered to 293 people. The questionnaires related to the behaviors of oral hygiene care, and disease history related to halitosis, and status of halitosis, halitosis measurement, oral examination, and caries activity tests such as the snyder test, Salivary flow rate test, and Salivary buffering capacity test. Our sample was taken from 293 rural residents within the period from 4th to 21st of January 2006. This was done in order to provide basic data to prepare both policies of halitosis prevention and a device to efficiently measure halitosis status and investigate the factors related therein. The major findings of this study results are as follows: 1. As for frequency of tooth brushing, twice a day occupied the greatest portion at 46.1 % Women exceeded men in frequency of tooth brushing. Tongue brushing everyday produced a 25.6 % result among subjects and The use of auxiliary oral hygiene devices occupied 9.2 %. 2. As for degree of usual self-awareness of halitosis: 62.5 %. This result also demonstrate that the severest time of self-awareness in regards to halitosis is wake up time in the morning. The time period produced the highest portion of 72.7 % in times of self-awareness. In terms of the area in which halitosis was observed, gum resulted in 23.0 %. As for types of halitosis, fetid smell was the most frequent at 37.2 %. 3. As for the result of halitosis measurement, values of OG less than 50 ppm occupied 54.3 % and $50{\sim}100ppm$ occupied 41.6 %. As for $NH_3$ values, $20{\sim}60ppm$ showed the highest value range of 52.6 %. 4. As for OG per disease history related to halitosis, values of OG were significantly high in the ranges of $50{\sim}100ppm$ within family history groups of food impaction by dental caries, diabetes mellitus and halitosis. As for values of $NH_3$, there showed a significant difference in respiratory system disease groups. 5 Value range of OG per ordinary halitosis self-awareness degree: values ranging less than 50 ppm were recorded at 55.9 % from the group realizing not aware of smell. 57.5 % from groups only realizing sometimes, while values range of $50{\sim}100ppm$ were recorded at 52.0 % from groups always aware of smell. 63.6 % from groups always strongly aware of smell. Meanwhile as for the values ranges of $NH_3$, $20{\sim}60ppm$. they occupied high portions for all groups of exams. 6. Values of OG per oral examination: the more pulp-exposed teeth and food impaction and the higher the tongue plaque index, values of OG increased within the range of $50{\sim}100ppm$. As for values of $NH_3$, the more prosthetic teeth and the higher the tongue plaque index, this value increased significantly, and the values increased up to no less than 60 ppm for groups of mandibular partial denture. 7. Within the realm of caries activity test: as for the Snyder test, high activity was highest by 43.0 % wherewith the higher the activity of acidogenic bacteria the higher the OG values. As for the salivary flow rate test, the number of cases below 8.0 ml showed the highest tendency by 62.5 %. The larger the salivary flow rate the more decreased OG values distribution. As for the salivary buffering capacity test, $6{\sim}10$ drops of 0.1N lactic acid showed the overwhelming trend by 58.7 % whereby the higher the salivary buffering capacity the greater distribution occupancy ratio of OG values below 50 ppm which is scentless to on ordinary person. 8. As for the correlation between oral environment and halitosis, OG showed the positive correlation with pulp exposed teeth, filled teeth, present teeth, tongue plaque index, and food impaction, while the negative correlation with salivary flow rate and prosthetic teeth. $NH_3$ showed a positive correlation with prosthetic teeth and frequency of tooth brushing, while decayed teeth was negative correlation. 9. As for the multiple regression analysis result, there have been selected female, pulp exposed teeth, prosthetic teeth, food impaction, salivary flow rate, tongue plaque index and severe activities in the Snyder test as factors affecting OG wherein explanatory power on it was 45.1 %. There have been selected females, pulp exposed teeth, tongue plaque index, and prosthetic teeth as factors affecting on $NH_3$ wherein explanatory power on it was 6.6 %. With the aforementioned results in mind, the status of halitosis among rural residents is considered to bare a close relation with oral environments and other factors related to halitosis such as the Snyder test from caries activity test, and salivary flow rate test. For the prevention of halitosis of residents in rural areas, we have to focus on correct tooth brushing methods and tongue brushing, with using auxiliary oral hygiene devices to remove fur of tongue plaque and food impaction. Also, when the cause and ingredients of halitosis are diverse and complex, in order to analyze exactly the factors of individual halitosis development, we need continuous and systematic study in order to provide rural residents with programs of oral hygiene education and encourage the use of dental hygienists in public health centers.

A study on the intraoral factor related to oral malodor (구취발생에 관여하는 구강내부요인에 관한 연구)

  • This study was peformed in order to find out the relationship between the causing factors and the production of each gas 01 oral malodor, to contribute the oral malodor control at dental clinic as well as to establish the effective application of malodor control project for public oral health program 127 patients from 20 to 40 years old who had been visited for preventive dental cares were participated for the study. Such items as caries status, periodontal status, salivary flow, viscosity, pH. Snyder test, plaque deposit and tongue plaque were checked through the oral examination in order to find out the contributing factors Hydrogen sulfide, Methyl mercaptan, Di-methyl sulfide and Ammonia gas components were checked with Oral-Chroma and Attain, the oral malodor check units. Not only the corelation coefficiencies but also the multi-way variance analysis were calculated between each causing factor and each component of oral malodor gases to estimate the contributing factors of the oral malodor. 1. There was no relationship between the caries status and each component pf the oral malodor such as sulfur compound or Ammonia, both in laboratory test and VAS test (pF0.05). It revealed negative relationship between Hydrogen sulfide and FT(rM-0.1904. pE0.05) as well as the VAS and FT (rM-0.210. pE0.05). S0, it was estimated that the less oral malador was recognized when caries state changed to filled state in Hydrogen sulfide laboratory test or VAS test 2. High relationship was showed between salivary flow and Hydrogen sulfide (rM-0.183, pM0.039), Methyl mercaptan(rM-0.234, p-0.008). Dimethyl sulfide(rM-0.234, pM0.008) and Ammonia(-0.361. pM0.001) gas(pE0.05). 3. There was a high relationship between M-PHP(Modified-Patient Hygiene Performance Index) and tong plaque all kinds of sulfide(rM0.249. pM0.005). Ammonia gas component(rM0.232, pM0.009). 4. It was found that considerable relationship was appeared between the periodontal status and Ammonia gas (rM0.274, pM0.002), so, it should be needed to control Ammonia. Such dental Cares as the prevention or early treatment of periodontal disease and the accelerating the salivary flow as well as reducing the amounts and activities of filament or spiral typed oral micro-organism were recommended for adults, not only for dental care program at the dental clinics but also for public health programs, in order to promote the oral health and quality of file for individual and community peoples.

The Effect of Pilocarpine-containing Chewing Gum on Anti-microbial Components in Whole Saliva of Xerostomic Patients (구강건조증 환자에서 필로카핀 함유 껌 사용에 따른 전타액내 항균성분의 변화)

  • The purpose of this study was to investigate the effect of pilocarpine containing chewing gum on anti-microbial components in whole saliva of xerostomic patients, The objective xerostomic patients were instructed to use 5mg-pilocarpine containing chewing gum for 20minutes three times per day, and the author measured the flow rates of unstimulated whole saliva and stimulated whole saliva at the beginning the treatment, 1,2,3, and 4 weeks after. The concentration and flow rate of anti-microbial components in whole saliva were quantitated by enzyme-linked immunosorbent assay(ELISA). The obtained results were as follows: 1. There were significant increase in the unstimulated and stimulated whole salivary flow rate after using pilocarpine-containing chewing gum in xerostomic patients. 2. The concentrations of IgA in the unstimulated and stimulated whole saliva showed increasing pattern but, no significant changes, arid the flow rates of IgA in the unstimulated and stimulated whole saliva showed significant increase after using pilocarpine-containing chewing gum in xerostomic patients. 3. The concentrations of IgM in the unstimulated and stimulated whole saliva showed increasing pattern but, no significant changes, and the flow rates of IgM in the unstimulated and stimulated whole saliva showed significant increase after using pilocarpine-containing chewing gum in xerostomic patients. 4. The concentrations of lactoferrin in the unstimulated and stimulated whole saliva showed no significant changes, and the flow rates of lactoferrin in the unstimulated and stimulated whole saliva showed significant increase after using pilocarpine-containing chewing gum in xerostomic patients. 5. The concentrations of lysozyme in the unstimulated and stimulated whole saliva showed no significant changes, and the flow rates of lysozyme in the unstimulated whole saliva showed significant increase, but in stimulated whole saliva showed no significant changes after using pilocarpine-containing chewing gum in xerostomic patients.

  • It has been known that saliva may affect the most of oral diseases. On the contrary, several systemic conditions may affect salivary flow and cause oral dryness and psychosocial stress especially may a crucial role in the etiology of hyposalivation and oral dryness. Many studies have focused on macroscopic effects of the stress on the salivary glands by autonomic respose, but on the other hand it has hardly been reported on cellular microscopic effects of the stress on the salivary glands. Therefore, this study was performed to examine clusterin, a antiapoptotic and cytoprotective protein, in the parotid glands under restraint stress condition. For this study, 10 rats were divided into 3 groups; 1) 2 rats of group I were selected as a normal control. 2) 2 rats of group II, as a experimental control were placed in the restraint cone for 2 hours 3) 6 rats of group III were placed in the restraint cone for 2 hours once a day. The rats were sacrificed immediately(group II, as a experimental control), 24, 48, and 72 hours after application of the stress and the parotid glands were excised. Western blotting and immunohistochemistry were performed. The finding were as follows: 1. In parotid glands, clusterin was mildly increased and clearly expressed in the ductal cell under restraint stress immediately after application of the stress. 2. In parotid glands, clusterin was significantly decreased and slightly stained in the ductal cell under restraint stress 24 and 48 hours after experiment. 3. In parotid glands, clusterin was prominently increased again and densely stained in the ductal cell under restraint stress 72 hours after experiment.

The Associated Factors with Whole Saliva Flow Rate and Xerostomia (전타액분비율과 구강건조증의 관련요인 분석)

  • The aim of this study was to analyze the factors related to whole saliva flow and the xerostomia for the ground use of the materials in developing oral health improvement program and life quality improvement program. So, this study performed a questionnaire survey, targeting at 160 adults, older than 40 years, living in Gyeonggi and Incheon provinces from January to February 2011 and measured unstimulated salivary flow rate and stimulated salivary flow rate. The results are as followings. The group of participants who took gums and candies to relieve oral dryness had low stimulated whole saliva flow. The group of participants who graduated from middle schools, who separated by death, who were not healthy with menopause, and who recognized periodontal disease symptoms and tongue burning sensation recognized more symptoms of dry mouth than others. In addition, the group of participants who were not satisfied with life so much, who couldn't feel the meaning of life, and who recognized negative feelings frequently recognized symptoms of dry mouth more. Women, the group of participants who graduated from middle schools, who suffered from depressive symptoms, and who recognized halitosis had low unstimulated whole saliva flow. The group of participants who graduated from middle schools, who were not satisfied with life so much, who couldn't feel the meaning of life, and who were not satisfied with themselves had low stimulated saliva flow.

  • Sjögren's syndrome (SS) is a chronic inflammatory autoimmune disorder that affects mainly salivary and lacrimal glands, but its cause remains largely unknown. Clinical data indicating that SS occurs in a substantial proportion of patients with lupus points to common pathogenic mechanisms underlying the two diseases. To address this idea, we asked whether SS develops in the lupus-prone mouse strain sanroque (SAN). Owing to hyper-activation of follicular helper T (Tfh) cells, female SAN mice developed lupus-like symptoms at approximately 20 wk of age but there were no signs of SS at that time. However, symptoms typical of SS were evident at approximately 40 wk of age, as judged by reduced saliva flow rate, sialadenitis, and IgG deposits in the salivary glands. Increases in serum titers of SS-related autoantibodies and numbers of autoantibody-secreting cells in cervical lymph nodes (LNs) preceded the pathologic manifestations of SS and were accompanied by expansion of Tfh cells and their downstream effector cells. Thus, our results suggest that chronic dysregulation of Tfh cells in salivary gland-draining LNs is sufficient to drive the development of SS in lupus-prone mice.

Immunoelectron Microscopic Study on the Nitric Oxide Synthase in Rat Salivary Glands (흰쥐 침샘의 Nitric Oxide Synthase에 관한 면역전자현미경적 연구)

  • Endogenous nitric oxide (NO) has been known to regulate many physiological and pathological processes, especially the glandular secretion and blood flow. However, nitric oxide synthase (NOS) responsible for NO synthesis has not been well studied ultrastructurally in rat salivary gland. The present study was performed to investigate the distribution of nitric Oxide synthase isoforms (endothelial. neuronal, and inducible NOS). Immunoelectron microscopic study, using monoclonal mouse anti-endothelial NOS, anti-neuronal NOS, and anti-inducible NOS, was performed in the salivary gland of rat. Endothelial NOS (eNOS)-positive immunoreactivities were most prominent in the secretory granules of serous cells of the salivary gland of the rat. Immunoreactivities were well concentrated on serous secretory granules in the serous cells. However, weak eNOS-positive immunoreactivity was observed in the mucous secretory granules of the mucous cells. Positive endothelial NOS (eNOS) immunoreactivities were most prominent in the secretory granules of intralobular ducts. Ductal secretory granules and acinar serous secretory granules have a similar pattern of labeling as eNOS suggestings. Neural NOS (nNOS)-positive immunoreactivity was not detected in duct systems or in acinar cells. Inducible NOS (iNOS)-positive immunoreactivity was not seen in acinar and ductal cells. These results reveal the presence of eNOS in the salivary gland of the rat, which may be related with regulation of the glandular secretion and blood flow through the gland.

EFFECT OF OCTANOL, THE GAP JUNCTION BLOCKER, ON THE REGULATION OF FLUID SECRETION AND INTRACELLULAR CALCIUM CONCENTRATION IN SALIVARY ACINAR CELLS (흰쥐 악하선 세포에서 gap junction 봉쇄제인 octanol이 타액분비 및 세포내 $Ca^{2+}$ 농도 조절에 미치는 영향)

  • From bacteria to mammalian cells, one of the most important mediators of intracellular signal transduction mechanisms which regulate a variety of intracellular processes is free calcium. In salivary acinar cells, elevation of intracellular calcium concentration ($[Ca^{2+}]_i$) is essential for the salivary secretion induced by parasympathetic stimulation. However, in addition to $[Ca^{2+}]_i$, gap junctions which couple individual cells electrically and chemically have also been reported to regulate enzyme secretion in pancreatic acinar cells. Since the plasma membrane of salivary acinar cells has a high density of gap junctions, and these cells are electrically and chemically coupled with each other, gap junctions may modulate the secretory function of salivary glands. In this respect, I planned to investigate the role of gap junctions in the modulation of salivary secretion and $[Ca^{2+}]_i$, using mandibular salivary glands of rats. In order to measure the salivary flow rate, fluid was collected from the cannulated duct of the isolated perfused rat mandibular glands at 2 min intervals. $[Ca^{2+}]_i$, was measured from the cells loaded with fura-2 by spectrofluorometry. The results obtained were as follows: 1. CCh-induced salivary secretion was reversibly inhibited by 1 mM octanol, a gap junction blocker. 2. CCh-induced increase in $[Ca^{2+}]_i$, was also reversed by the application of 1 mM octanol. 3. Octanol did not block the initial increase in $[Ca^{2+}]_i$ caused by CCh, which suggested that the reduction of $[Ca^{2+}]_i$, caused by gap junction blockade was not resulted from the inhibition of $Ca^{2+}$ release from intracellular $Ca^{2+}$ stores. 4. Addition of octanol during stimulation with $1{\mu}M$ thapsigargin, a potent microsomal ATPase inhibitor, reduced $[Ca^{2+}]_i$, to the basal level. This suggested that inhibition of gap junction permeability closed plasma membrane $Ca^{2+}$ channels. 5. 2,5-di-tert-butyl-1,4-benzohydroquinone (TBQ) generated $[Ca^{2+}]_i$ oscillations resulting from periodic influx of $Ca^{2+}$ via plasma membrane. The TBQ-induced $[Ca^{2+}]_i$ oscillations were stopped by the application of 1mM octanol which implicated that gap junctions modulate the permeability of plasma membrane $Ca^{2+}$ channels. 6. Glycyrrhetinic acid, another well known gap junction blocker, also inhibited CCh-induced salivary secretion from rat mandibular glands. These results suggested that gap junctions play an important role in the modulation of fluid secretion from the rat mandibular glands and this was probably due to the inhibition of $Ca^{2+}$ influx through the plasma membrane $Ca^{2+}$ channels.

Effect of Tongue Scraping, ZnCl2 Mouth Rinse, and Periodontal Treatment on the Reduction of Oral Malodor (수종의 구취처치법의 구치감소효과에 관한 연구)

  • This study was performed to investigate the effect of three modalities commonly used for reduction of oral malodor and to review the contributing factors for halitosis. For this study, 45 subjects of good general health in their third decades were selected, and they were divided into three groups by each modality, that is, tongue scraping, $ZnCl_2$ mouth rinse, and periodontal treatment. Subjective sense of halitosis, diets, Oral hygiene index based on tooth brushing and tongue brushing habit, resting salivary flow rate, gingival index, degree of tongue coating were recored in the Halito-Chart designed by the author. Oral malodor converted from volatile sulfur compound was measured by the Halimeter$^{(R)}$(Interscan Co., USA), and the correlation between the oral factors and pre-treatment halimeter score were analysed. Data obtained were analysed by the SPSS windows program and the results of this study were as follows : 1. There were no significant correlation between oral factors such as oral hygiene index, salivary flow rate, gingival index, degree of tongue coating and pre-treatment halimeter score. 2. Difference between pre- and post-treatment halimeter score by the three modalities were significant, but correlation between pre- and post-treatment score within group was most significant in the periodontal treatment group followed by the tongue scraping group and lowest in the $ZnCl_2$ mouth rinse group. 3. Reduction of halimeter score was significant only in subjects with relatively more tongue coating in the tongue scraping group, but in the $ZnCl_2$ mouth rinse group and in the periodontal treatment group, the modality applied to each group had same significant effects without regard to the degree of oral hygiene index or gingival index. 4. Men had more tongue coating than women, and reduction of halimeter score was significant in men, but not in women.

The Effect of Traditional Korean Medicine Treatment on Patients with Taste Disorders: A Case Series of Five Patients (미각 장애 환자 5례를 대상으로 한 대한 한방 치료의 효과)

  • Objectives: The purpose of this study was to report the effect of traditional Korean medical therapy such as acupuncture, electroacupuncture and herbal medicine in taste disorder patients. Methods: We surveyed 5 taste disorder patients visiting the Oral Diseases Clinic in the Kyung Hee University Oriental Medicine Hospital from January, 2014 to June, 2014. Before starting traditional Korean medical therapy such as acupuncture, electroacupuncture and herbal medicine, the subjects were evaluated on severity of discomfort using visual analogue scale (VAS), salivary flow rate (SFR), quality of life about oral health (based on the 14-item Oral Health Impact Profile-14) and qi-stagnation condition (based on the 23-item qi-stagnation Questionnaire). Visual analogue scale was re-evaluated during the treatment period. Results: There was no relationship between diminished SFR and severity of discomfort. Also change or loss of taste did not influence the quality of life about oral health. However, stress which refers to qi-stagnation could be one of the reasons taste disorder occurs. After receiving traditional Korean medical therapy, all 5 patients' visual analogue scale score decreased. Conclusions: Traditional Korean medical therapy may potentially be an option for taste disorder. Further evaluations including pre-post comparison with larger number of cases will be needed in the future.
