Regional Lymph Node Dissection as an Additional Treatment Option to Endoscopic Resection for Expanded Indications in Gastric Cancer: a Prospective Cohort Study

  • Purpose: Expanded indications for endoscopic submucosal dissection (ESD) in early gastric cancer (EGC) remain controversial due to the potential risk of undertreatment after adequate lymph node dissection (LND). Regional LND (RLND) is a novel technique used for limited lymphadenectomy to avoid gastrectomy. This study established the safety and effectiveness of RNLD as an additional treatment option after ESD for expanded indications. Materials and Methods: A total of 69 patients who met the expanded indications for ESD were prospectively enrolled from 2014 to 2017. The tumors were localized using intraoperative esophagogastroduodenoscopy (EGD) before RLND. All patients underwent RLND first, followed by conventional radical gastrectomy with LND. The locations of the preoperative and intraoperative EGD were compared. Pathologic findings of the primary lesion and the RLND status were analyzed. Results: The concordance rates of tumor location between the preoperative and intraoperative EGD were 79.7%, 76.8%, and 63.8% according to the longitudinal, circumferential, and regional locations, respectively. Of the 4 patients (5.7%) with metastatic LNs, 3 were pathologically classified as beyond the expanded indication for ESD and 1 had a single LN metastasis in the regional lymph node. Conclusions: RLND is a safe additional option for the treatment of EGC in patients meeting expanded indications after ESD.

Method for evaluating the safety performance and protection ability of the mobile steel protective wall during the high-explosive ammunition test (고폭탄 탄약시험 간 이동형 강재 방호벽의 안전성능 판단 및 유효 방호력 평가 방법)

  • In this study, a series of processes for evaluating the effective protection against barriers that should be equipped in institutions that perform reliability tests on high-risk ammunition, such as high-explosive ammunition, were introduced. The impact that high-explosive bombs can have on personnel includes damage to the eardrum and lungs caused by explosion overpressure and penetrating wounds that can be received by fragments generated simultaneously with the explosion. Therefore, a high-explosive with COMP B explosives as its contents were set up, and an explosion protection theory investigation to calculate the degree of damage, numerical calculations and simulations were performed to verify the protection power. A numerical calculation revealed the maximum explosion overpressure on the protective wall when the high-explosive exploded and the penetration force of the fragment against a 50 mm-thick protective wall to be 77.74 kPa and 41.34 mm, respectively. In the simulation verification using AUTODYN, the maximum explosion overpressures affecting the firewall and personnel were 56.68 kPa and 18.175 kPa, respectively, and the penetration of fragments was 35.56 mm. This figure is lower than the human damage limit, and it was judged that the protective power of the barrier would be effective.

  • Object detection in maritime means that the captain detects floating objects that has a risk of colliding with the ship using the computer automatically and as accurately as human eyes. In conventional ships, the presence and distance of objects are determined through radar waves. However, it cannot identify the shape and type. In contrast, with the development of AI, cameras help accurately identify obstacles on the sea route with excellent performance in detecting or recognizing objects. The computer must calculate high-volume pixels to analyze digital images. However, the CPU is specialized for sequential processing; the processing speed is very slow, and smooth service support or security is not guaranteed. Accordingly, this study developed maritime object detection software and implemented it with FPGA to accelerate the processing of large-scale computations. Additionally, the system implementation was improved through embedded boards and FPGA interface, achieving 30 times faster performance than the existing algorithm and a three-times faster entire system.

Quality Control Tests and Acceptance Criteria of Diagnostic Radiopharmaceuticals (진단용 방사성의약품의 품질관리시험 및 기준)

  • Radiopharmaceuticals are drugs that contain radioisotopes and are used in the diagnosis, treatment, or investigation of diseases. Radiopharmaceuticals must be manufactured in compliance with good manufacturing practice regulations and subjected to quality control before they are administered to patients to ensure the safety of the drug. Radiopharmaceuticals for administration to humans need to be sterile and pyrogen-free. Hence, sterility tests and membrane filter integrity tests are carried out to confirm the asepticity of the finished drug product, and a bacterial endotoxin test conducted to assess contamination, if any, by pyrogens. The physical appearance and the absence of foreign insoluble substances should be confirmed by a visual inspection. The chemical purity, residual solvents, and pH should be evaluated because residual by-products and impurities in the finished product can be harmful to patients. The half-life, radiochemical purity, radionuclidic purity, and strength need to be assessed by analyzing the radiation emitted from radiopharmaceuticals to verify that the radioisotope contents are properly labeled on pharmaceuticals. Radiopharmaceuticals always carry the risk of radiation exposure. Therefore, the time taken for quality control tests should be minimized and care should be taken to prevent radiation exposure during handling. This review discusses the quality control procedures and acceptance criteria for a diagnostic radiopharmaceutical.

What Is Cultured Meat? (배양육이란 무엇인가?)

  • By 2050, 70% more food will be needed to fulfill the demands of a growing population. Among the solutions, cultured meat or clean meat is presented as a sustainable alternative for consumers. Scientists have begun to leverage knowledge and tools accumulated in the fields of stem cell and tissue engineering in efforts aimed at the development of cell-based meat. Cultured meat has to recreate the complex structure of livestock muscles with a few cells. Cells start to divide after they are cultured in a culture medium, which provides nutrients, hormones, and growth factors. An initial problem with this type of culture is the serum used, as in vitro meat aims to be slaughter free. Thus, it is contradictory to use a medium made from the blood of dead calves. The serum is expensive and affects to a large extent the production cost of the meat. A positive aspect related to the safety of cultured meat is that it is not produced from animals raised in confined spaces and slaughtered in inhumane conditions. Thus, the risk of an outbreak is eliminated, and there is no need for vaccinations and animal welfare issues. The production of cultured meat is presented as environmentally friendly, as it is supposed to produce less greenhouse gas, consume less water, and use less land in comparison to conventional meat production.

  • The objective of this study is to develop a two-dimensional (2D) flood model that can perform accurate flood analysis with simple input data. The 2D flood inundation models currently used to create flood forecast maps require complex input data and grid generation tools. This sometimes requires a lot of time and effort for flood modeling, and there may be difficulties in constructing input data depending on the situation. In order to compensate for these shortcomings, in this study, a grid-based model that can derive accurate and rapid flood analysis by reflecting correct topography as simple input data was developed. The calculation efficiency was improved by extending the existing 2×2 sub-grid model to a 5×5. In order to examine the accuracy and applicability of the model, it was applied to the Gamcheon Basin where both urban and river flooding occurred due to Typhoon Rusa. For efficient flood analysis according to user's selection, flood wave propagation patterns, accuracy and execution time according to grid size and number of sub-grids were investigated. The developed model is expected to be highly useful for flood disaster mapping as it can present the results of flooding analysis for various situations, from the flood inundation map showing accurate flooding to the flood risk map showing only approximate flooding.

The Effect of Communication Distance and Number of Peripheral on Data Error Rate When Transmitting Medical Data Based on Bluetooth Low Energy (저 전력 블루투스 기반으로 의료데이터 전송 시 통신 거리와 연동 장치의 수가 데이터 손실률에 미치는 영향)

  • Recently, the market for personal health care and medical devices based on Bluetooth Low Energy(BLE) has grown rapidly. BLE is being used in various medical data communication devices based on low power consumption and universal compatibility. However, since data errors occurring in the transmission of medical data can lead to medical accidents, it is necessary to analyze the causes of errors and study methods to reduce data error. In this paper, the minimum communication speed to be used in medical devices was set to at least 800 byte/sec based on the wireless electrocardiography regulations of the Ministry of Food and Drug Safety. And the data loss rate was tested when data was transmitted at a speed higher than 800 byte/sec. The factors that cause communication data error were classified, and the relationship between each factor and the data error rate was analyzed through experiments. When there were two or more activated peripherals connected to the central, data error occurred due to channel hopping and bottleneck, and the data error rate increased in proportion to the communication distance and the number of activated peripherals. Through this experiment, when the BLE is used in a medical device that intermittently transmits biosignal data, the risk of a medical accident is predicted to be low if the number of peripherals is 3 or less. But, it was determined that BLE would not be suitable for the development of a biosignal measuring device that must be continuously transmitted in real time, such as an electrocardiogram.

A Study on Efficient Methods of Pesticide Control Using Agricultural Unmanned Aerial Vehicles (농업용 무인항공기를 활용한 농약방제 효율성 방안에 관한 연구)

  • In the agricultural environment, pesticide control requires a high risk of work and a high labor force for farmers. The effectiveness of pesticide control using unmanned aerial vehicles varies according to climate, land type, and characteristics of unmanned aerial vehicles. Therefore, an effective method for pesticide control by unmanned aerial vehicles considering the spraying conditions and environmental conditions is required. In this paper, we propose an efficient pesticide control system based on agricultural unmanned aerial vehicles considering the application conditions and environmental information for each crop. The effectiveness of the proposed model was demonstrated by measuring the drop uniformity of pesticides according to the change in altitude and speed after attaching the sensory paper and measuring the penetration rate of the drug inside the canopy according to the change in crop growth conditions. Experiment result, the closer the height of the UAV is to the ground, the more evenly the crops are sprayed, but for safety reasons, 2m more is suitable, and on average a speed of 2m/s is most suitable for control. The proposed control system is expected to help develop intelligent services based on the use of various unmanned aerial vehicles in agricultural environments.

A Study on the Method for Managing Hazard Factors to Support Operation of Automated Driving Vehicles on Road Infrastructure (자율주행시스템 운행지원을 위한 도로 인프라 측면의 위험 요소 관리 방안)

  • As the competition among the autonomous vehicle (AV, here after) developers are getting fierce, Korean government has been supporting developers by deregulating safety standards and providing financial subsidies. Recently, some OEMs announced their plans to market Lv3 and Lv4 automated driving systems. However, these market changes raised concern among public road management sectors for monitoring road conditions and alleviating hazardous conditions for AVs and human drivers. In this regards, the authors proposed a methodology for monitoring road infrastructure to identify hazardous factors for AVs and categorizing the hazards based on their level of impact. To evaluate the degrees of the harm on AVs, the authors suggested a methodology for managing road hazard factors based on vehicle performance features including vehicle body, sensors, and algorithms. Furthermore, they proposed a method providing AVs and road management authorities with potential risk information on road by delivering them on the monitoring map with node and link structure.

Hepatic and Renal Safety of Concurrent Use of Conventional and Herbal Medications for Musculoskeletal Disorders: A Retrospective Observational Study (근골격계 질환에 대한 양약 및 한약 병용의 간과 신장에 대한 안전성: 후향적 관찰 연구)

  • Objectives This study aimed to investigate whether the concurrent use of conventional and herbal medications affects liver and kidney function, by examining blood test data. Methods We retrospectively reviewed the electronic medical records of 590 inpatients with musculoskeletal diseases between 2013 and 2017. We investigated cases of drug-induced liver injury (DILI) according to the Roussel Uclaf Causality Assessment Method criteria and cases of drug-induced kidney injury (DIKI) based on the Kidney Disease Improving Global Outcomes definition. Results One case (0.17%) of DILI and one case (0.17%) of DIKI were identified. Significant improvements in serum laboratory data were observed after the concurrent use of both types of medications (p<0.05). The kappa coefficients ranged from 0.26 to 0.72, indicating that the values after the concurrent use of conventional and herbal medications showed a fair similarity to the baseline values of the patients. The linear regression test showed that female sex and high body mass index (BMI) were risk factors for an increase in the serum blood levels of liver function parameters. Conclusions The concurrent use of conventional and herbal medications for musculoskeletal disorders is relatively safe; however, clinicians should exercise caution when prescribing these medications to female patients and patients with a high BMI because of their potential effect on hepatic function.
