Factors Related to In-Hospital Death of Injured Patients by Patient Safety Accident : Using 2013-2017 Korean National Hospital Discharge In-depth Injury Survey (환자안전사고에 의한 손상환자의 병원내 사망 관련 요인 : 2013-2017 퇴원손상심층조사자료 활용)

  • This study aimed to analysis factors related to in-hospital death of injured patients by patient safety accident. A total of 1,529 inpatients were selected from Korea Centers for Disease Control and Prevention database(2013-2017). Frequency, Fisher's exact test, t-test, ANOVA, logistic regression analyses by using STATA 12.0 were performed. Analysis results show that the mortality rate was lower for female than male but the mortality rate was higher for the older age, the higher the CCI, head (or neck), multiple, systemic damage sites, internal and others, metropolitan cities based on Seoul and 300-499 based on the bed size of 100-299. Based on these findings, the possibility of using the in-depth investigation of discharge damage from the Korea Centers for Disease Control and Prevention as a data source for the patient safety survey conducted to understand the actual status of patient safety accident types, frequency, and trends should be reviewed. Also, it is necessary to prevent injury and minimize death by identifying factors that affect death after injury by patient safety accident.

  • Fire-alarm systems are safety equipment that facilitate rapid evacuation and early suppression in case of fire. It is highly desirable that fire-alarm systems have low false-alarm rates and are thus reliable. Until now, researchers have attempted to improve detector performance by applying new technologies such as IoT. To this end, IoT-based fire-detection systems have been developed. However, due to scarcity of large-scale operational data, researchers have barely studied malfunctioning in fire-alarm systems or attempted to reduce false-alarm rates in these systems. In this study, we analyzed false-alarm rates of smoke/temperature detectors and unwanted fire-alarm signal patterns at K institution, where Korea's largest IoT-based fire-detection system operates. After analyzing the fire alarm occurrences at the institution for five years, we inferred that the IoT-based fire-detection system showed lower false-alarm rates compared to the automatic fire-detection equipment. We analyzed the detection pattern by dividing it into two parts: normal operation and unwanted fire alarms. When a specific signal pattern was filtered out, the false-alarm rate was reduced to 66.9% in the smoke detector and to 46.9% in the temperature detector.

Analysis of Safety of Mahuang in Studies for Treatment of Obesity (비만 치료 연구에서 마황의 안전성 분석)

  • Objectives: The purpose of this study is to analyze the adverse events and safety of Mahuang used as anti-obesity treatment in Korean medicine. Methods: PubMed, EMBASE, Cochrane, KoreaMed, Oriental Medicine Advanced Searching Integrated System (OASIS) and SCIENCE-ON were used as searching for randomized controlled trials investigating the safety of Mahuang for treating obesity from January, 2000 to July, 2021. Results: Total 6 studies were included for analyzing. In most studies, heart rate was higher in experimental group than in placebo group with statistically significant difference. Both systolic and diastolic blood pressure had no significant differences between both groups, but systolic blood pressure got lower in experimental group. Most adverse events reported in experimental group were dry mouth, constipation, diarrhea, palpitation and insomnia, but serious adverse events were not reported. Conclusions: This study provides the evidence about the safety of Mahuang for treating obesity. However, increase of heart rate and minor adverse events could be occurred. Further study is needed to confirm the safety of Mahuang.

  • Current OSH system was analyzed in this paper to explain why high fatal incidents and disasters are continuously repeated for recent years in Korea. It was found that we have Dichotomous Perceptional Misconception of prevention before accident and compensation after accident and there is a significant lack of proper feed­back reward system for OSH performance. It was assumed that no reduction of accident rate and fatality rate have not been achieved recently despite of a great effort and increased resource allocations. Some statistics for proving weak punishment were analyzed. In the current system, the will of administrative agency would have been very limited particularly in the legal aspects. The Industrial Safety and Health Act is not suitable to after-injury punishment for employer and/or corporate since it is based on a framework for enforcement of prevention. Based on these analyses, it was concluded that there was a need to consider a special law for Corporate Accountability for Fatal Accidents. Because it is necessary to consider seriously for introduction of a new legal system for after injury punishment to repair the current system where it was found lack of proper feedback system. Also, there was no proper sanction measures for corporate with the current OSH legal system, and the most urgent problem in OSH area is the high fatality rate. it is necessary to consider seriously for introduction of a new legal system for after injury punishment. A Study of Smoke Exhaust Rate for the Transverse Ventilation with Oversized Exhaust Ports in Road Tunnel (횡류식 대배기구 방식을 적용한 도로터널에서 화재시 최적배연풍량 선정에 관한 연구)

  • Recently, the application of transverse ventilation system with oversized exhaust ports has been increased in bidirectional road tunnel in order to improve smoke exhaust ability. Therefore, in this study, for decision of the optimal smoke exhaust rates in the transverse ventilation system, several standards of nations are compared and numerical simulations with variations of exhaust flow rates are carried out in terms of smoke spread distance by FDS ver. 3.1. As results, in the case of no internal longitudinal air velocity in tunnel, the smoke exhaust rate of $80m^{3}/s$ (the smoke generation rate at HRR of 20MW) is sufficient enough to limit the smoke spread within 250m in 6 minutes after the fire. However, in the case of the internal longitudinal air velocity at 2.5m/s, the smoke exhaust rate should be increased $130m^{3}/s$.

A Study on the Morbidity and the Types of Accident among the Workers of Limestone Industry (석회석 가공 산업 근로자들의 상병 상태 및 사고 유형)

  • The workers of limestone industry have relatively higher rate of accident and injury than other industry workers. This survey was conducted to provide the informations for the planning of safety and health educations to prevent the accident and injury. The purpose of this study is to identify the morbidity rate per month and to determine the related health factors, and to find accident experiences of the limestone workers and to analyze the reasons and types of that accidents. The study design was a descriptive survey. Self reporting questionnaires developed by researcher were used for data collection. The subjects of this study were 225 workers in limestone industries in Jecheon city. The statistical methods utilized for data analysis were frequency analysis, $x^2$-test with SPSS-pc(ver.9.0). The results are as follows, 1. The workers morbidity rate per month was 22%, and morbid factors were Flu., G-I trouble, hypertension, orthopedic trauma, external wound, burn, etc., and the related health factors are self-assessment for health, exercise, accessability for medical facilities. 2. 40% of the subjects have experienced the accidents during last 6 months. The types of the accidents were safety violations, traffic accidents and fire accidents. Injury types were external wounds, fracture/amputation and burn. The workers of limestone industry have higher morbidity rate and experiences of accidents. Therefore the more increased safety and health educations than now for both the workers and the managers are needed. And this study could be helpful to increase the quality and the quantity of the educations needed.

Modeling and Discussing the Accident Rate Model of Rotary and Roundabout by Type of Land Use (토지이용별 로터리 및 회전교차로 사고율 모형개발 및 논의)

  • Rotary that causes traffic delays and safety issues by high-speed entry vehicles is currently being improved to roundabout. The operational difference between rotary and roundabout can cause driver's confusion and traffic accident. The purpose of this study is to develop the accident rate models which explain the factors related to the accidents by land use and intersection type. The main results are as follows. First, the null hypotheses that the type of land use and two intersections do not affect the accident rate are rejected. Second, the conflicting factors such as the number of crosswalk and bicycle lane should be carefully considered to reduce traffic accident at rotary. In the case of roundabout, greater than 3.5 m in circulatory lane width and two circulatory lane are analyzed to be important to prevent the accidents. Finally, the commercial and mixed areas are evaluated to be weak to traffic accidents than residential area.

Estimated Exposure Population to Hazardous Workplace Noise among Korean Workers (델파이 조사를 통한 직업적 소음 노출 규모의 추정)

  • Objective: The objective of this study was to estimate the numbers of employees occupationally exposed to noise according to their industry and size. Methods: A Delphi panel consisting of 15 occupational health experts estimated the exposure prevalence of noise in workplace. Data on Industrial Accident Compensation Insurance provided from Occupational Safety and Health Research Institute were combined to produce the exposure population. Results: In Korea, 16.0% of employees, 2,539,890 out of 15,838,926, was estimated to exposed to noise occupationally. The rate was 32.7% and 10.3% in manufacturing sector and non-manufacturing sector, respectively. The highest rate, 52.5%, was found in manufacturing industries of wood and of products of wood and cork(except furniture) and of other transport equipment. Sorted by their size of business, the rate was higher as the number of employee was larger in manufacturing sector. Conclusions: Compared to the same rate estimated in the US, 17.2%, the result of this study seems to be in a resonable range.

  • Background: Firefighters may experience high environmental temperatures or carry out intensive physical tasks, or both, which leads to increased core body temperature and risk of fatalities. Hence there is a need to remotely and non-invasively monitor core body temperature. Methods: Estimated (heart rate algorithm) and actual core body temperature (ingested telemetric pill) measures were collected simultaneously for comparison during training exercises on 44 firefighter volunteers. Results: Prediction of core body temperature varied, with no specific identifiable pattern between the algorithm values and directly measured body core temperatures. Group agreement of Lin's Concordance of 0.74 (95% Upper 0.75, lower CI 0.73), was deemed poor. Conclusion: From individual agreement data Lin's Concordance was variable (Min 0.11, CI 0.13-0.01; Max 0.83, CI 0.86-0.80), indicating that the heart rate algorithm approach was not suitable for core body temperature monitoring in this population group, especially at the higher more critical core body temperatures seen.
