Surgical treatment of perianal fistula in Crohn's disease (크론병에서 복잡성 항문주위 샛길의 수술적 치료)

  • Perianal Crohn's disease is a major problem that impair quality of life. This article reviews the current surgical treatment of Crohn's perianal fistula. Fistulotomy and loose seton are commonly used surgical methods for treatment of perianal Crohn's disease. Mucosal advancement flap and fibrin glue are used in this treatment, despite a lake of controlled trials. Fecal diversion is disturbingly high in complicated complex perianal fistula in Crohn's disease. Ligation of intersphincteric fistula and autologous or allogenic stem cells are new surgical procedures for treatment of Crohn's disease that need further studies. Treatment success might be improved by multimodal treatment and new surgical and medical treatment options.

Effects of Ginseng Radix Plus Crataegi Fructus on the Mice Model of Alzheimer's Disease (인삼산사복합방(人蔘山査複合方)이 Alzheimer성 치매의 병태(病態) 모델에 미치는 영향)

  • This research was investigated the effect of the Ginseng Radix plus Crataegi Fructus (Gin-CF) on Alzheimer's disease. Specifically, the effects of the Gin-CF extract on $IL-1\beta,\;TNF-\alpha$ of BV2 microglia cell line treated with LPS. The Gin-CF extract suppressed the over-expression of $IL-1\beta$ protein, $TNF-\alpha$ protein, MDA, and CD68/CD11b, in the mice with Alzheimer's disease induced by ${\beta}A$. These results suggest that the Gin-CF extract may be effective for the prevention and treatment of Alzheimer's disease. Investigation into the clinical use of the Gin-CF extract for Alzheimer's disease is suggested for future research.

  • Leveling colostomy with a frozen-section biopsy in a Hirschsprung's disease is an important factor for a successful procedure. Two neonatal cases of Hirschsprung's disease in the descending colon are reported. In both cases, loop ileostomy was established because of the unavailability of frozen-section biopsy on an emergency basis. At the time of definitive procedure of the first case, transition zone at the splenic flexure was noted and was compatible with the frozen section biopsy. In the second case, an unexpected longer resection at a higher level than transition zone was required because of the poor vascularity after dissection. In conclusion, a leveling colostomy should be selected as a choice in long-segment Hirschsprung's disease. Confirming preservation of the marginal artery of Drummond is particularly important in case of Hirschsprung's disease in the descending colon.

A Case Report of Neuro-Behcet's Disease with Paraparesis (하지부전마비를 동반한 Neuro-Behcet 병 1례 보고)

  • Behcet's disease is a systemic disease affecting multiple organs including the central nervous system. Neuro-Behcet's disease was regarded as relatively rare, but thanks to the development of diagnostic tools, more and more cases are being reported. We are reporting a case of neuro-Behcet's disease in which the patient displayed paraparesis, dysarthria and involuntary tremor as neurologic symptoms. The patient's brain MRI showed cerebellar atrophy, and a spinal cord MRI failed to reveal any significant lesions. The patient experienced a couple of fever attacks during hospitalization, which were managed adequately by herbal medicines. Her main neurological symptoms such as paraparesis were, however, grossly unchanged at discharge.

A Case of Cerebral Aneurysmal Subarachnoid Hemorrhage in Fabry's Disease

  • We report an unusual case of cerebral aneurysmal subarachnoid hemorrage (SAH) with Fabry's disease. A 42-year-old woman presented with aneurysmal SAH originated from a saccular aneurysm of the right posterior communicating artery. The patient was treated by an endovascular coil embolization of aneurysm. Postoperatively the patient recovered favorably without any neurological deficit. During her admission, the patient had a sign of proteinuria in urine analysis. The pathologic findings of kidney needle biopsy implied nephrosialidosis (mucolipidosis of lysosomal stroage disease), which is consistent with a Fabry's disease. It is uncommon that Fabry's disease is presented with aneurysmal SAH, especially in middle-aged patients, but could be a clinical concern. Further investigations are needed to reveal risk factors, vascular anatomy, and causative mechanisms of Fabry's disease with aneurysmal SAH.

  • Objectives : This research was investigated the effect of the Ginseng Radix plus Chaenomelis Fructus on Alzheimer's disease. Methods : Specifically, the effects of the Ginseng Radix plus Chaenomelis Fructus extract on $IL-1{\beta}$, $TNF-{\alpha}$ of BV2 microglia cell line treated with lipopolysacchride. Results : The Ginseng Radix plus Chaenomclis Fructus extract suppressed the over-expression of $IL-1{\beta}$ protein, $TNF-{\alpha}$ protein, MDA, and CD68/CD11b, in the mice with Alzheimer's disease induced by ${\beta}$ amyloid peptide. Conclusion: These results suggest that the Ginseng Radix plus Chaenomelis Fructus extract may be effective for the prevention and treatment of Alzheimer's disease. Investigation into the clinical use of the Ginseng Radix plus Chaenomelis Fructus extract for Alzheimer's disease is suggested for future research.

  • PET of the cerebral metabolic rate of glucose is increasingly used to support the clinical diagnosis in the examination of patients with suspected major neurodegenerative disorders, such as Alzheimer's disease. $^{18}F-FDG$ PET has been reported to have high diagnostic performance, especially, very high sensitivity in the diagnosis and clinical assessment of therapeutic efficacy. According to clinical research data hitherto, $^{18}F-FDG$ PET is expected to be an effective diagnostic tool in early and differential diagnosis of Alzheimer's disease. Since 2004, Medicare covers $^{18}F-FDG$ PET scans for the differential diagnosis of fronto-temporal dementia (FTD) and Alzheimer's disease (AD) under specific requirements; or, its use in a CMS approved practical clinical trial focused on the utility of $^{18}F-FDG$ PET in the diagnosis or treatment of dementing neurodegenerative diseases.

  • The purpose of this study is to compare the voice differences of persons with Idiopathic Parkinson's disease and a normal group according to five vowels. Eight persons with Idiopathic Parkinson's disease and a healthy control group of 22 were selected and every voice analyzed by MDVP. The first result showed that jitter measurements between the two group showed a significant statistical difference according to all vowels. Second, the two groups' shimmer measurements showed a significant statistical difference according to nearly all vowels. Third, jitter measurements between the five vowels were more relatively closely correlated persons with Idiopathic Parkinson's disease than the normal group. Fourth, shimmer figures between the five vowels more relatively closely correlated persons with Idiopathic Parkinson's disease than the normal group.

  • This review describes the overall knowledges of parkinson's disease. There are involved the causes. chemical characteristics, clinical features including tremor. bradykinesia, and rigidity. evaluation of movement disorders. and physical therapy of parkinson's disease. People who are diagnosed with parkinson's disease experience movement disorders that, if not managed, can lead to considerable disability. Before treated with parkinson's disease, it is important to recognise the pathogenesis of that. Then we need to develop physiotherapeutic programs based on the pathophysiology of parkinson's disease.
