Disease Prevalence, Parent's Educational Needs, and Disease Management according to Severity of Respiratory Infections in Early Childhood (영유아 호흡기 감염 중증도에 따른 이환실태, 부모의 교육요구 및 관리수행정도)

  • Purpose: The purpose of this study was to investigate disease prevalence, parent's educational needs, and disease management according to severity of respiratory infections in early childhood. Methods: Participants for this study were 173 mothers whose child was admitted to I university hospital in Seoul and whose child was an infant or toddler. Data were collected from December, 17, 2014 to February, 15, 2015 using self-report structured questionnaires. Data were analyzed using IBM/SPSS 21.0 program. Results: There were significant differences in the severity of respiratory infection according to neonatal admission due to dyspnea, feeding type, atopic dermatitis in the infant or allergic disease in father and siblings. Parent's educational needs for the severe respiratory infection group were higher than for the non-severe group. Parent's disease management for the severe respiratory infection group was lower than the non-severe group. Conclusion: As important care factors in neonatal admissions include dyspnea, cow milk feeding, eczema, family history of allergies, parent's educational needs and disease management, they should be considered when caring for young children with respiratory infections and their parents.

  • In view of the hypothesis that the effects of Parkinson disease on voice production can be detected before pharmacological intervention, the voice differences of patients with Idiopathic Parkinson's disease and a healthy aging group were diagnostically analyzed with the long term object of establishing, for clinical purposes, early disease-progression biomarkers. Fifteen patients with Idopathic Parkinson's disease (prior to pharmacological intervention) and a healthy control group of 15 were selected and every voice was recorded three times using praat (ver. 5022) with a headset mic. Relevant parameters - acoustic measure of /a/ phonation, F0 related parameters, MPT related parameters, articulatory ratio, VOT - were then analyzed by MANOVA. Significant differences were found in the F0 related (low F0, high F0, F0 range) and MPT related parameters. There were also significant differences in acoustic measurements (intensity, shimmer, HNR, jitter), AMR (/$t{\Lambda}$/,/$k{\Lambda}$/) and VOT (/ta/), The findings indicated that the voice production of patients with Idiopathic Parkinson's disease have normal pitch but bad quality. In particular, with slow articulatory ratios and VOT values, the tongue tip functioning of patients was lower than for the healthy group.

  • Caroli's disease is a non-obstructive dilatation of the intrahepatic bile ducts. It is a rare congenital disorder that classically causes saccular ductal dilatation, which usually it segmental dilatation. Caroli's disease is associated with recurrent bacterial cholangitis and stone formation. A 57 years-old female inpatient suffering from cholangitis due to Caroli's disease was admitted at Wonkwang University Jeonju Oriental Medicine Hospital. The patient was treated with herbal medicine such as Yongdamsagan-tang and acupuncture. As result, there was improvement of symptoms. The process and content of treatment and the patient's recovery over general symptoms is described.

  • This study was designed to report treatment for Behcet's disease with arthritis We treated 2 Behcet's disease patients with arthritis. A 46-year-old female patient was treated with Cheongsimyeonja-tang. Another case, 44-year-old female patient was diagnosed as Sjogren's disease at first and diagnosed as The Behcet's diseases about 1 year later, treated with Cheonglijagam-tang. The progress was evaluated with the visual analogue scale for joint pain and hot flush, and length of sleeping time. There was improvement in the symptoms of visual analogue scale for joint pain and hot flush, also length of sleeping time. Cheongsimyeonja-tang, Cheonglijagam-tang can be considered as a treatment for patient diagnosed as Behcet's disease suffering from arthritis, especially.

  • Objectives The aim of this study was to examine significant improvement of motor and non-motor symptoms after treatment with Sasang constitutional medicine treatment on a Soyangin patient with early-stage Parkinson's disease. Methods The patient was treated with Soyangin's constitutional medications (herb and acupuncture) and scalp acupuncture. The progress was evaluated using the Unified Parkinson Disease Rating Scale (UPDRS) before and after treatment. The UPDRS was used to assess the overall function of patient. Results We observed a significant improvement in patient's motor and non-motor symptoms using UPDRS. The UPDRS score decreased in Part I, Part II, and Part III. Conclusions This study shows that Sasang constitutional medicine can be effective treatment for Soyangin patient with early-stage Parkinson's disease.

  • As the aging society continues in recent years, interest in dementia is increasing. Among them, Alzheimer's disease is a degenerative brain disease that accounts for the largest percentage of all dementia patients, with the medical community currently not offering clear prevention and treatment for Alzheimer's disease, and the importance of early treatment and early prevention is emphasized. In this paper, we intend to find the most efficient activation function by combining various activation functions centering on convolutional neural networks using MRI datasets of normal people and patients with Alzheimer's disease. In addition, it is intended to be used as a dementia classification modeling suitable for the medical field in the future through Alzheimer's dementia classification modeling.

Development and Evaluation of Health Literacy Instrument for Alzheimer's Disease: Case of Older Adults in Rural Areas (알츠하이머병에 관한 건강정보 이해력 측정도구 개발 및 평가: 농촌 노인을 대상으로)

  • Purpose: This study develops a health literacy instrument for Alzheimer's disease. Methods: Items were drawn from The Korean version of the Alzheimer's Disease Knowledge Scale (ADKS) and the Korean version of European Health Literacy Survey Questionnaire (HLS-EU-Q47 & HLS-EU-Q16). Content validity was tested by experts. To further refine the questionnaires and test their reliability and validity, data were collected from 324 older adults in the community. Results: Five significant items in the two subscales were derived from the factor analysis. The subscales were named access and understanding. Reliability was good at Cronbach's ⍺ .79, and validity through exploratory factor analysis was KMO .897, p<.001, which was found to be high and significant. Conclusion: The instrument demonstrated high reliability and validity. Therefore, this instrument can contribute to the evaluation of health literacy for Alzheimer's Disease in older adults and to any subsequent intervention, as well as to develop a theory for health literacy for Alzheimer's Disease.

A case of tremor in Parkinson's disease treated with Korean medicine (파킨슨병으로 유발된 진전이 한약 치료를 통하여 호전된 치험 1례)

  • As society ages, the seriousness of Parkinson's disease in the elderly increases. This study was performed in order to evaluate the effect of Korean medical treatments on Parkinson's symptoms. An elderly patient was diagnosed with Parkinson's disease by clinical symptoms, which were resting tremor, pill-rolling tremor and mild bradykinesia. We treated her disease with Korean medical treatment, including herbal medicine and acupuncture therapy and excluding western medication. For the herbal medicine, Insamyangyeong-tang (人參養榮湯), Bobi-tang (補脾湯), Dokhwaljihwang-tang (獨活地黃湯) and Sipimijihwang-tang (十二味地黃湯) were primarily used. In result, the clinical symptoms, measured by unified Parkinson's disease rating scale (UPDRS) and modified Hoehn and Yahr stage (H-Y stage), of Parkinson's disease in this patient had improved after the Korean medical treatment. This study suggests that Korean medical treatments can be applicable to improve symptoms in patients with Parkinson's disease.

Case report of Kimura's disease Treated by Radical resection and Post Operative Steroid therapy (기무라 병 환자에서 수술적 치료 및 술 후 스테로이드 병합요법의 치험2례)

  • Purpose: Kimura's disease is a relatively rare head and neck tumor that frequently occurs in young orientals accompanied with eosinophila. We shared our experience of two cases of Kimura's disease, treated by radical resection and post operative steroid therapy, so we report the correlation of eosinophil counts and disease progression. Methods: A 25 years old male came to the clinic with a mass localized to the right cheek inferior to the right auricle. We could not resect the mass totally. During the follow up period, we checked the eosinophil counts, and steroid therapy was started 7 months after the surgery. A 34 year old female came to the clinic with a mass localized inferior to the left auricle reaching from the posterior portion of the auricle to the left temporal portion. We tried to remove as much tumor as possible, save the temporal region, in regard to the impairment of blood supply to the auricle. After operation, steroid therapy was started. Results: In the first case, the tumor was easily approachable, and radical resection of the tumor with post operative steroid therapy was an effective treatment. In tumors located at difficult regions to remove, as in the second case, optimal debulking and post operative steroid therapy was also effective in treating Kimura's. There were no recurrences in both cases. Eosinophil counts were always reduced after surgery and steroid therapy, and during the period with low eosinophil counts, there was no recurrence of Kimura's disease. Conclusion: Surgery and post operative steroid therapy were effective in treatment of Kimura's disease, and we could assume eosinophil counts as a good indicator for evaluation of the prognosis of Kimura's disease during the follow up period.

Parkinson's disease diagnosis using speech signal and deep residual gated recurrent neural network (음성 신호와 심층 잔류 순환 신경망을 이용한 파킨슨병 진단)

  • Parkinson's disease, one of the three major diseases in old age, has more than 70 % of patients with speech disorders, and recently, diagnostic methods of Parkinson's disease through speech signals have been devised. In this paper, we propose a method of diagnosis of Parkinson's disease based on deep residual gated recurrent neural network using speech features. In the proposed method, the speech features for diagnosing Parkinson's disease are selected and applied to the deep residual gated recurrent neural network to classify Parkinson's disease patients. The proposed deep residual gated recurrent neural network, an algorithm combining residual learning with deep gated recurrent neural network, has a higher recognition rate than the traditional method in Parkinson's disease diagnosis.
