A Study on the Chief Complaints among Farmers in the Rural Community (농촌주민의 주소증에 대한 조사연구)

  • Objectives and Methods : This study was conducted to investigate chief complaints among famers. The subjects of this study were 640 famers who live in the rural area in Wanju-gun Jeonbuk province. They were received medical treatment by oriental doctors who visited each village from July 1996 to June 1999. The data of this study were collected from their medical charts . Results and Conclusions : The number of female patients was 449(70.16%) and the number of male patients was 191(29.84%). The group of patients above 70 years was 32.81%, 60-69 years 32.03%, 50-59 years 20.15%, 40-49 years 8.90%, and bellow 39 years 6.09%. The most frequent complaint in farmers was low back pain(27.16%) the second was knee joint pain(16.14%), the third was shoulder Pain(12.06%), and the fourth was headache-dizziness(9.59%). The frequency of musculoskeletal disease(64.48%) was most high, and cardiovascular disease(11.33%) was the second, gstrointestinal disease(10.73%) was the third, respiratory disease(3.42%) was the fourth. It is necessary to pay attention to musculoskeletal disease-especially low back pain and knee joint pain- in study and treatment of farmer's disease.

A Case Study of a Taeeumin Patient with Advanced Parkinson's Disease Diagnosed as Dry-heat Symptomatic Pattern (태음인(太陰人) 조열증(燥熱證)으로 진단한 진행된 파킨슨병 환자 치험례)

  • Objectives This case study was about a Taeeumin patient with advanced Parkinson's disease diagnosed as Dry-heat symptomatic pattern. In this study, we reported significant improvement of motor and non-motor symptoms of this patient after Sasang constitutional medicine treatment. Methods The patient was diagnosed with Taeeumin Dry-heat symptomatic pattern and treated with Taeeumin's constitutional medications and acupuncture. The primary outcome measure for this study is Unified Parkinson Disease Rating Scale(UPDRS) to assess the overall function of patient. Secondary outcome assessments include Global Assessment Scale(GAS), change of patient's sleep, stool and urine. Results and Conclusions The patient treated with Taeeumin's constitutional medications and acupuncture had reduction in UPDRS (specifically Part II, Part III and Part IV) and GAS of motor symptoms. Furthermore, patient's sleep, stool and urine were reported to be normal after treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeeumin patient with Advanced Parkinson's disease diagnosed as Dry-heat symptomatic pattern.

  • Purpose: The purpose of this study was to describe the health care experiences among patients with Parkinson's disease. Methods: Of the qualitative research methods, Colaizzi's phenomenological method was used in this study. A total of nine patients, who were diagnosed with Parkinson's disease and receiving outpatient treatment, were selected as the subjects of this study. Subsequently, data were collected through individual in-depth interview. Results: The four categories obtained as a result were 'strenuous efforts to control my own body,' 'subject of health that no one can replace,' 'focus on the current while expecting a breakthrough in health management,' and 'human dignity that cannot be lost to the end.' Conclusion: The study results are expected to help health care providers deeply understand the experiences in health care among patients with Parkinson's disease and to provide source data for nursing intervention development that can be helpful in managing the health status of patients with Parkinson's disease.

  • This study was investigated so that reliability of disease mechanism diagnosis would be examined, the estimation about disease mechanism item of Questionnaires and the relations of disease mechanism would be inquired about 'health diagnosis program' Questionnaires which were used for the object diagnosis of Oriental medicine in the department of Oriental OB&GY, Oriental Medical hospital of Dong-Eui University. We analyzed the results of Questionnaires for 3354 outpatients who had OB & GY disease in the Oriental Medical hospital of Dong-Eui University from April 2000 to March 2004. The diagnosis Questionnaires(after DSOM (r) S.1.1) was the figures 188, the health diagnosis Questionnaires (after DSOM (r) S.1.1) was the figures 137. phiegm deficiency of qi was used in DSOM (r) R.1.1 as it is. The reliability of DSOM (r) S.1.1 was usually higher than DSOM (r) R.1.1 in deficiency of qi blood stasis insufficiency of Yang heat syndrom damp, 5 case disease mechanism. The reliability of DSOM (r) S.1.1 was usually lower than DSOM (r) R.1.1 in blood deficiency stagnation of qi coldness damp dryness liver heart spleen kidney, 8 case disease mechanism. but the great difference wasn't seen, therefore both DSOM (r) S.1.1 and DSOM (r) R.1.1 had similar result. A meeting point both DSOM (r) S.1.1 and DSOM (r) R.1.1 was above 90% in liver spleen blood stasis blood deficiency, 4 case disease mechanism with the exception of phlegm deficiency of Yim nothing of fluctuations of question. A meeting point of coldness that was 82.47% was lowest, A meeting point of the rest disease mechanism was above 85%. The effect that contributed in producing disease mechanism result and in which pure question was over relevance calculation 0.9, insufficiency of Yang damp phlegm that contributed in producing disease mechanism result was lower comparatively in DSOM (r) R.1.1. But the effect that contributed in producing disease mechanism result and in which pure question was over relevance calculation 0.9 except spleen kidney phlegm in DSOM (r) S.1.1

The Effects of Crataegus prinnatiflora BGE. var. major N.E. BR(CPVM) Extract on the the Alzheimer's Disease Model Induced by CT105 (산사육(山査肉)이 CT105로 과도(課導)된 Alzheimer's Disease 병태(病態) 모델에 미치는 영향(影響))

  • This research investigates the effect of the Crataegus pinnatifida BGE. var. major N.E. BR(CPVM) on Alzheimer's disease. Specifically, the effects of the DYHT extract on (1) $IL-1{\beta}$, IL-6, amyloid precursor proteins(APP), acetylcholinesterase(AChE), and glial fibrillary acidic protein(GFAP) mRNA of PC-12 cells treated with CTI05; (2) the AChE activity and the APP production of PC-12 cell treated with CT105; (3) the behavior; and (4) expression of $IL-1{\beta}$, $TNF-{\alpha}$, reactive oxygen species(ROS), nitrite oxide(NO); and (5) the infarction area of the hippocampus, and brain tissue injury in Alzheimer's diseased mice induced with CT105 were investigated. The results are as follow. 1. The CPVM extract suppressed the expression of $IL-1{\beta}$, IL-6, APP, AChE, and GFAP mRNA in PC-12 cells treated with CT105. 2. The CPVM extract suppressed the AChE activity and the production of APP significantly in PC-12 cells treated with CT105. 3. The CPVM extract group showed a significant inhibitory effect on the memory deficit for the mice with Alzheimer's disease induced by CT105 in the Morris water maze experiment. 4. The CPVM extract suppressed the over-expression of $IL-1{\beta}$, $TNF-{\alpha}$, ROS and NO in the mice with Alzheimer's disease induced by CT105. 5. The CPVM extract reduced the infarction area of hippocampus, and controlled the injury of brain tissue in the mice with Alzheimer's disease induced by CT105. These results suggest that the CPVM extract may be effective for the prevention and treatment of Alzheimer's disease.

The Study on the Effect of Acupuncture on UPDRS and Heart Rate Variability in the Patients with Idiopathic Parkinson's Disease (태충(太衝) 양릉천(陽陵泉)의 침(針)자극이 특발성 파킨슨 환자의 UPDRS 및 HRV Parameter에 미치는 영향)

  • Introduction : This study was designed to evaluate the effect of acupuncture on Unified Parkinson's Disease Rating Scale (UPDRS) and Heart Rate Variability (HRV) of patients with Parkinson's disease Methods: Subjects were voluntarily recruited through newspaper and internet advertisement. All the subjects were confirmed as idiopathic parkinson's disease by a neurologist. Acupuncture was applied 2 times a week for four weeks by oriental medical doctor at Kyung-hee University hospital. Acupuncture points used were GB34 and LR3. The patient's symptoms were assessed before and after 4 weeks of treatment by UPDRS and HRV. HRV was measured for 5 minutes before and after 4 weeks of treatment. Results : The results were as follows; 1. In both groups, UPDRS scores were significantly improved after 4 weeks compared to the pre-treatment 2. After 4 weeks of treatment UPDRS score differences between the two groups were insignificant 3. After 4 weeks of treatment HRV parameter scores showed statistically insignificant differences between the two groups. SDNN parameters of the Experimental group were significantly increased. Conclusion : This study suggests that acupuncture treatments can be applicable to improve symptoms in the patients with idiopathic Parkinson's disease. Further study on the acupuncture and HRV in the patients with Parkinson's disease is recommended.

  • Introduction : The Aim of the Study Is to Investigate the Characteristics of Yangdorak m the Patients with Idiopathic Parkinson's Disease. Methods : Subjects Were Voluntarily Recruited by Newspapers And Internet. All the Subjects Were Confirmed as Idiopathic Parkinson's Disease by a Neurologist. The Patient's Symptoms Were Assessed by Unified Parkinson's Disease Rating Scale (UPDRS), Hoehn-Yahr (H-Y) Stage, Schwab & England Activity of Daily Living And Freezing of Gait Questionnaire(FOGQ). Sasang Constitution Was Differentiated by QSCCII. Yangdorak Was Measured after 15 minutes' Bed Rest Results : The Results Were as Follows; 1. The Average Value of Yangdorak m 50 Patients with Parkinson's Disease Was $39.01{\pm}17.97$. 2. The Mean Values of Fl, F2, F3, F4, F5 and F6 were under 40. 3. The Number of Pyesaek Was Significantly Increased According to H-Y Stage. 4. The Yangdorak Value Had No Significant Correlation between UPDRS, H-Y Stage and Duration. 5. There Was No Significant Difference in the Value of Yangdorak among Sasang Constitutions. Conclusion : This Study Suggests that Yangdorak Can Be Used as a Assistant Tool to Investigate the Patients of Parkinson's Disease. Further Study on the Yangdorak And Parkinson's Disease Is Recommended.

Gender Differences in Paediatric Patients of the Swiss Inflammatory Bowel Disease Cohort Study

  • Purpose: Gender differences in paediatric patients with inflammatory bowel disease (IBD) are frequently reported as a secondary outcome and the results are divergent. To assess gender differences by analysing data collected within the Swiss IBD cohort study database since 2008, related to children with IBD, using the Montreal classification for a systematic approach. Methods: Data on gender, age, anthropometrics, disease location at diagnosis, disease behaviour, and therapy of 196 patients, 105 with Crohn's disease (CD) and 91 with ulcerative or indeterminate colitis (UC/IC) were retrieved and analysed. Results: The crude gender ratio (male : female) of patients with CD diagnosed at <10 years of age was 2.57, the adjusted ratio was 2.42, and in patients with UC/IC it was 0.68 and 0.64 respectively. The non-adjusted gender ratio of patients diagnosed at ${\geq}10$ years was 1.58 for CD and 0.88 for UC/IC. Boys with UC/IC diagnosed <10 years of age had a longer diagnostic delay, and in girls diagnosed with UC/IC >10 years a more important use of azathioprine was observed. No other gender difference was found after analysis of age, disease location and behaviour at diagnosis, duration of disease, familial occurrence of IBD, prevalence of extra-intestinal manifestations, complications, and requirement for surgery. Conclusion: CD in children <10 years affects predominantly boys with a sex ratio of 2.57; the impact of sex-hormones on the development of CD in pre-pubertal male patients should be investigated.

  • De Quervain's disease is caused by stenosing tenosynovitis of the first dorsal compartment of the wrist. The first dorsal compartment at the wrist includes the tendons of the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB). Patients with De Quervain's usually report pain at the dorsolateral aspect of the wrist with referral of pain toward the thumb and/or lateral forearm. This condition responds well to nonsurgical treatment. Objective : The aim of this study was to find out the therapeutic effect of the Cervus elaphus Herbal-acupuncture in the treatment of patients with De Quervain's Disease. Design : A prospective study of Cervus elaphus Herbal-acupuncture was conducted. Setting : The study was conducted in the Kang-nam Oriental Hosp. of Dong-Guk University. Patients : The study comprised 4 patients with De Quervain's Disease. Methods : We investigated 4 cases patients the De Quervain's Disease. We evaluated wrist function that before and after Cervus elaphus Herbal-acupuncture treatment by VAS score, tenderness and Finkelstein test. the treatment is operated every other day. Results : Pain reduction(VAS) was found out in the case treated with Cervus elaphus Herbal-acupuncture. Conclusions : Clinical results show that Cervus elaphus Herbal-acupuncture has a good analgesic effect in De Quervain's Disease.

The Effect of Oriental Medical Treatment on Crohn's disease; 1 Case Report (항통을 주소로 내원(來院)한 크론병(Chron's disease) 호전 1례)

  • Objective: This study evaluated the effects of Oriental treatments on one patient who was diagnosed to Crohn's disease. Methods: One patient was diagnosed as Crohn's disease by physician. We used acupuncture and moxibustion at 曲池(LI11), 天樞(S25), 足三里(ST36), 合谷(LI4) and (GL25), 2 times per week about one month. We analyzed the change symptom by Crohn's disease activity index(Harvey-Bradshaw index) before and after treatment. Results: After treating acupuncture and moxibustion in the case, We found out that the Crohn's disease activity index score was significantly improved after treatment. First the number of liquid or very soft stool was decreased to 2 times a day and the stools became more solid condition. The abdominal pain and use of anti-diarrhea medication score came to none. The well being score set slightly below poor condition. Conclusions: These result suggest acupuncture, moxibustion were effective to Crohn's disease clinical symptom. These could help patient ordinary life by reducing abdominal pain and diarrhea.
