One Case Treated Cerebral Infarction with Rheumatic Mitral Stenosis (류마티스성 승모판 협착증을 동반한 뇌경색 환자의 치험 1예)

  • Valvular heart disease is one of the main current of cardiac problems and has many problems must be solved by sequelae and complications, etc. Rheumatic Mitral Stenosis is mainly attacked by rheumatic fever and developed by process of treatment of mitral valve or formation of trace. The purpose of this study is to examine the efficacy of oriental treatment for Cerebral Infarction with Rheumatic Mitral Stenosis. At the time of visiting ER, he was shown the symptoms of dull mentality, Rt. hemiplegia, global aphasia, dysphagia, chest discomfort, insomnia, dyspnea, etc, It showed that Atrial fibrillation in EKG monitoring, Atrial fibrillation, Rheumatic Mitral Stenosis, Ejection-Fraction slope 60% in Cardiac echography, Lt. atrial hypertrophy, Rt. atrial hypertrophy, Rt. ventricular hypertrophy with pulmonary congestion in chest X-ray. From the view of oriental diagnostic criteria. We classified the patient's clinical conditions and treated accordingly. As a result of treatment, symptoms were markedly improved and he was discharged. Further elaboration of oriental diagnostic classification could possibly lead to the fundamental treatment.

A case with GRIN2A mutation and its non-neurological manifestations

  • In epilepsy-aphasia spectrum (EAS) disorders, mutations in the glutamate receptor ionotropic N-methyl-D-aspartate type subunit 2A (GRIN2A) have become important for screening the disease. Research into the phenotypic variability of several types of neurologic impairment involving these mutations is in progress. However, the non-neurological problems related to these mutations are poorly understood. EAS disorders usually have epileptic, cognitive, or behavioral manifestations. In this case report, we present a female patient with epilepsy, delay in expressive language and social development, and intellectual disability with low intelligence quotient and memory quotient, but normal motor development. Through genetic analysis, she was found to have a missense and a nonsense mutation in GRIN2A (c.1770A>C; p.Lys509Asn and c.3187G>T; p.Glu1063∗, respectively) and we consider the nonsense mutation as 'pathogenic variant'. She was also discovered to have congenital hypothyroidism, growth hormone deficiency and Rathke's cleft cyst in the brain, which were previously unknown features of GRIN2A mutation. Our findings should widen understanding of the spectrum of GRIN2A phenotypes, and emphasize the need for more research into the association between GRIN2A mutations and non-neurologic clinical presentations.

Conservative treatment of corpus callosum hemorrhage due to a falling coconut in Indonesia: a case report

  • The potential for traumatic brain injury resulting from falling coconuts is frequently overlooked. These incidents can cause focal lesions in the form of brain hemorrhage. Corpus callosum hemorrhage due to blunt trauma from a falling object is rare and typically associated with poor prognosis. The purpose of this report is to detail a case of corpus callosum hemorrhage caused by a coconut fall and to discuss the conservative management approach employed. We report the case of a 54-year-old woman who was admitted to the hospital with symptoms of unconsciousness, headache, and expressive aphasia after being struck by a falling coconut. Notably, hemorrhage was detected within the body of the corpus callosum, as revealed by imaging findings. The patient received intensive monitoring and treatment in the intensive care unit, including oxygen therapy, saline infusion, an osmotic diuretic, analgesics, and medication to prevent stress ulcers. The patient demonstrated marked clinical improvement while undergoing conservative treatment. Despite the typically unfavorable prognosis of these rare injuries, our patient exhibited meaningful clinical improvement with conservative treatment. Timely diagnosis and appropriate interventions were crucial in managing the patient's condition. This report emphasizes the importance of considering traumatic brain injury caused by falling coconuts and highlights the need for further research and awareness in this area.

  • A Literature study on the language disturbance, the results are as follows; 1. Utterance was closely concerned not only the vocal organs(pharynx, larynx, epiglottis, lips, tongue, vocal cord etc,) but also five viscera{especially heart, lung, kidney etc.) in The Yellow Emperor's Canon of Internal Medicine. It is very like the vocal mechanism in Medical science. 2. In the language disturbance, It is classified with dysarthria and dysphasia in Medical science. But in Oriental medicine, it is expressed the language disturbance as coma-speech lessness, stiff tongue-speechlessness, frightening-speechlessness etc. Especially in Oriental medicine, Non-utterance is called aphasia in literature study. 3. In the concern of the language disturbance and five viscera, $Heart{\cdot}Lung{\cdot}Kidney$ are counted of first importence. In differential diagnosis, It is divided sthenia-syndrome and asthenia-syndrome. Sthenia-syndrome is classified with wind-cold, fire-evil, adverseness of vital energy, stagnation of phlegm, is easy to cure. Asthenia-syndrome is classified with sexual desire, anxiety-meditation, fear, is hard to cure. 4. The pathogenesis of dysphasia originated from two factors; The first internal damages are consumption of body fluid caused by lung-dryness and yin-dificiency of lung & kidney. The second disease caused by exogenous evjls is sluggishness of lung-energy. 5. In many using points of acupuncture of the language disturbance, the order is LI-4(合谷), H-7(神門), K-l(湧泉), L-3(太衝), K-3(太谿), S-6(三陰交), H-5(通里), G-15(아門), C-23(廉泉), S-40(豊降), K-6(照海), L-7(列缺), S-36(足三里) etc.

  • Objective: There were few reports on the treatment of Wolff-Parkinson-White syndrome with Hypoxic ischemic encephalopathy. We treated a Wolff-Parkinson-White syndrome with Hypoxic ischemic encephalopathic patient with Oriental medical approach, and got a significant result. This treatment shows the possibility of healing Wolff-Parkinson-White syndrome with Hypoxic ischemic encephalopathy. So we are reporting this case. Method : Acupuncture treatment, herb medication, rehabilitation therapy were applied for treating patient's chief symptom.(involuntary movement, dystonia, aphasia, fever, perspiration) Results : Consciousness loss in acute stage of Hypoxic ischemic encephalopathy can be considered as Mental Confusion due to Phlegm(Dammisimgyu) in veiw point of Oriental medicine. Wolff-Parkinson-White syndrome can be considered as Sudden Palpitation(Gyounggye) in veiw point of Oriental medicine. After oriental medical treatment, patient's chief symptoms were improved. Conclusion: More study about oriental medical treatment on Wolff-Parkinson-White syndrome with Hypoxic ischemic encephalopathy is needed.

A Systematic Review on the Treatment of Post-Stroke Patients Based on Sasang Constitutional Medicine (사상의학적 중풍 후유증 치료에 대한 체계적 문헌 고찰)

  • Objectives The purpose of this study was to review and evaluate the clinical evidence of the efficacy and safety of treatment based on Sasang constitutional medicine (SCM) for post-stroke patients by systematic review and meta-analysis. Methods Randomized controlled trials (RCTs), published in 10 electronic databases up to December 2020, were searched. For the included studies, Cochrane's risk of bias assessment was performed to analyze the methodological quality. The strength of evidence was evaluated using the grading of recommendations assessment, development, and evaluation system based on the results of analyses. All review processes were performed by two independent researchers. Results Five RCTs were finally included. All included RCTs were conducted for one month on post-stroke patients in 60-80s, four studies on Tae-Eum patients and one study on So-Yang patients. Four types of constitution-specific herbal medicine (Chungpyesagan-tang, Cheongsimsanyak-tang, Yeoldahanso-tang, and Yangkyuksanhwa-tang) and constitution-specific acupuncture therapy were identified as interventions. More than half of the included studies were evaluated as low quality due to the high-risk of bias in selection, performance, and detection. The combination of constitution-specific herbal medicine, acupuncture, and conventional treatment was more effective in improving the patients' motor impairment, dysphagia, aphasia, and depression than conventional treatment alone. No serious adverse events by SCM treatment were reported. Conclusions SCM treatment may improve the sequelae of post-stroke patients safely in combination with conventional treatment. Since the quality of clinical evidence included in this study was low, higher quality clinical evidence obtained in well-designed clinical studies will be needed.

  • Tongue acupuncture is not only a body acupuncture treatment for the tongue but also a new microacupuncture treatment. Tongue acupuncture is a very promising treatment from a diagnostic and therapeutic point of view. According to several studies, Guan's tongue acupuncture and tri-tongue acupuncture are frequently used. Tongue acupuncture can be used for dysphagia, dysarthria, aphasia, insomnia, autism spectrum disorder, burning mouth syndrome, and cerebral palsy.

  • Objective : There were few reports on the treatment of Adult-onset Still's disease and Diffuse cerebral dysfunction. This oriental medical treatment shows the possibility of healing Adult-onset Still's disease and Diffuse cerebral dysfunction, therefore we announce this. Methods : The acupuncture, herbal medicine, herbal acupucture(Jahageo, Nockyong) therapies were applied for treating this patient's chief symptom(chest discomfort, palpitation, rigidity, mentality change, global aphasia, dysphagia, coughing and sputum) Results : 1. Adult-onset Still's disease is a rare systemic inflammatory disorder of unknown etiology, characterised by salmon-colored rash, intermittent spiking high fever, arthralgia and variety of systemic features. This is one type of Juvenile arthritis. 2. Diffuse cerebral dysfunction is an anoxic-ischemic encephalopathy, and most cause of this is the hypoxia caused by hypotention or respiratory distress. 3. After oriental medical treatments, his chief symptoms were improved. Conclusion : The more study about oriental medical treatment on Adult-onset Still's disease and Diffuse cerebral dysfunction is needed.

A Study on the Activities of Daily Living, Self-efficacy and the Health Promoting Behavior in Stroke Patients (뇌졸중 환자의 일상생활동작 수행능력, 자기효능감 및 건강증진행위에 관한 연구)

  • Purpose: The purpose of this study to analyze the relationship the activities of daily living(ADL), self-efficacy, and health promoting behaviors. Method: The research was a descriptive survey study. The subjects of this study were 115 hospitalized stroke patients to two General hospitals and one Oriental treatment hospital in G. Metropolitan. Data was collected through questionnaires from December 1st, 2003 to January 30th, 2004. The survey instruments used in the study Shah, Vanclay and Cooper's MBI(11 items), and Sherer and Maddux's self-efficacy(10 items), Walker, Sechrist, and Pender's HPLP(23 items) was developed by the researcher. The obtain data were analyzed with an SPSS 10.0 program using frequency, percentage, Cronbach's alpha, t-test, ANOVA, Duncan test, and Pearson's correlation coefficient. Result: The results were as follows; The level of ADL the score was 50.5, self-efficacy 40.4, and health promoting behavior 54.6. There were significant differences of ADL by general and disease related characteristics as follows : for monthly income(F=2.72, p= .048), duration of stroke(F=7.98, p= .001), number of attack(F=5.21, p= .007), operation (F=17.88, p= .000). There were significant differences of self-efficacy by general and disease related characteristics as follows : for monthly income(F=4.83, p= .003), number of attack (F= 6.20, p=. 003), operation(F=4.03, p= .047). There were significant differences of health promoti ng behaviors by general and disease-related characteristics as follows : for aphasia(F=4.24, p=.042). There were significant correlated between ADL and self-efficacy(r= .698, p=.000), self- efficacy and health promoting behavior(r=.398, p= .000), ADL and health promoting behavior(r= .235, p= .011). Conclusion: As a result, There was a significant correlation between ADL, self-efficacy and health promoting behaviors. There will be considered useful nursing intervention effect to progress, support health promoting behavior of stroke patients.

Effects of breathing training in melodic intonation therapy on articulation intelligibility of aphasics: pilot study (멜로디 억양 치료에서 실어증 환자의 조음 명료도에 대한 호흡 훈련 효과: 초기 실험)

  • The present study was to test if breathing training in melodic intonation therapy (MIT) ameliorated the articulation intelligibility of Broca's aphasics or not. The experimental group did breathing training (2 stages) that preceded the MIT. In order to evaluate the efficacy of the MIT intervention, the VOT (Voice Onset Time), the TD (Total Delay), the voice sound intensity and the expiratory volume of the subjects, closely associated with articulation intelligibility were measured before and after the intervention. It was shown that, in the experimental group after the MIT intervention, the VOT and TD were increased on bilabial/p/, alveolar consonant /t/, and soft palatal /k/(p < 0.05), but no significant differences were found on affricate /c/ and fricative /s/(p > 0.05). In the control group, no significant increases in the VOT and TD were observed on all articulation points(p > 0.05). The voice sound intensity which influences the verbal articulation increased in the experimental group after the intervention(p < 0.05), whereas no significant changes were observed in the control group. In conclusion, the breathing training in the MIT was found to result in improving the articulation intelligibility of Broca's aphasiacs.
