Surgical Treatment of Ruptured Achilles Tendon - End-to-end suture & Paratendinous wrapping (아킬레스건 파열의 수술적치료 -단단 봉합술 및 건외막피복술-)

  • Purpose: To evaluate the results and prognosis of operative repair to acute rupture of achilles tend on associated sports injury. Materials and Methods: 21 cases were surgically treated and average follow-up period was 1 year and eight months. The forth decade was most common with $55\%$ and soccer was most common in sports with 5 cases. End-to-end suture of ruptured achilles tendon was performed, and paratendinous structure was wrapped sufficiently. Postoperatively. ankle was plantarflexed for 6 weeks with longleg cast. And then 2 weeks interval, short leg cast with equinous position was conversed to functional position. About 10 weeks after operation, ankle was recovered to right angle. Hooker scale was used to evaluate the results. Results: Compared to normal side, heel-floor distance of ruptures side was decreased 0.7 cm in average, and 0.8 cm was deceased after 20 times weight loaded dorsiflexion. Mid-calf circumference was deceased 0.3 cm, and active dorsiflexion and plantar flexion of ankle was decreased each 3 and 5degree. 16 cases showed ‘excellent’result and 5 cases showed ‘satisfactory’. There was no complication, such as re-rupture or infection at operation site. Conclusion: After end-to-end operative repair to achilles tendon, sufficient wrapping of paratendinous structure is efficient for healing and prevention of postoperative adhesion. And serial dorsiflex-ion cast change is considered to be a successful treatment for preventing residual equinus deformity.

Clinical Results of Distal Femoral Osteotomy for Treatment of Grade 4 Medial Patella Luxation with Concurrent Distal Femoral Varus in Small Breeds Dogs: 13 Cases

  • The purpose of this study was to determine the outcome of distal femoral osteotomy for distal femoral varus and medial patellar luxation (MPL) grade 4 in small-breed dogs. Radiographs and medical records were reviewed to collect data and plan the surgery in small-breed dogs with MPL grade 4. Computed tomography (CT) imaging was also performed in cases of severe bone deformities. Signalment, weight, medial patellar luxation and lameness grade, radiographic bone union, complications, pre- and postoperative femoral varus angle, passive range of motion, static weight bearing distribution and visual analogue scale scores were recorded. Thirteen corrective distal femoral osteotomies were performed with ancillary and additional procedures in 9 dogs; 4 dogs had staged bilateral procedures; and four stifles were suspected to have partial or complete rupture of the cranial cruciate ligament. One stifle underwent patellar groove replacement. The mean ± SD pre- and postoperative femoral varus angles were 109.15°± 3.71° and 96.30°± 2.97°, respectively. Significant improvements in passive range of motion, thigh circumference and visual analogue scale (VAS) scores were observed. There was no reluxation of the patella. This study suggests that distal femoral osteotomy with traditional and additional procedures provided satisfactory outcomes in patient healing and functional recovery in small-breed dogs with excessive femoral varus angles.

Effect of 1,6-Hexamethylenediamine Content on the Properties/Adhesive Strength of EVA/Itaconated EPDM Blend Foams (I) (헥사메텔렌 디아민이 EVA/Itaconated EPDM 블렌드 발포체의 물성 및 접착강도에 미치는 영향 (I))

  • Simplification of the manufacturing process in shoe making is essential to improve productivity and reduce production costs. To improve the adhesion of EVA foam used as a midsole, EVA/itaconated EPDM(EPDM-g-IA)(80/20wt%) blend was prepared using Torque Rheometer-Plasti-Corder, and 1,6-hexamethylenediamine/crosslinking agent/foaming agent/additive were mixed, followed by amidation reaction and foaming to prepare EVA/EPDM-g-IA foam for shoe midsole. In this study, we investigate the effect of the content of 1,6-hexamethylenediamine(0, 0.5, 1.0, 2.0, 3.0) on the mechanical properties, water-contact angle and adhesion of EVA/itaconated EPDM foam. As the content of 1,6-hexamethylenediamine increased, mechanical properties such as tensile strength, tear strength, tensile elastic modulus, hardness, and water-contact angle were lowered, but elongation at break and compression set(%) were increased. Both normal type and non-UV type adhesive strength increased with increasing diamine content. In particular, it was found that the adhesion strength of the non-UV type adhesion increased sharply with increasing diamine content. As a result, an adherend rupture occurs in a foam sample having a content of 1,6-hexamethylenediamine of 3phr. From this, it can be seen that the EVA/itaconated EPDM foam for shoe midsoles, which can be used for non-UV adhesion without primer and UV treatments, have been developed.

  • This study was conducted to investigate the main factors that contain a rotating velocity ratio between wheel and conveyor belt, a tilt angle of conveyor belt and a rotating velocity of a dick cutter for mechanization of Chinese leek harvest. In the survey on the cultivation of Chinese leek, row spacing of 350 m and cutting height of 10 mm from the ground were set up for field tests. Test equipment was designed to cut, pick up and convey Chinese leek one row by one row. From the results of material tests, pick-up height of conveyor belt was set up at $60\~90m$ from the bottom, and the strain and stress at rupture of Chinese leek was 0.487 m/m and 0.01078 MPa. An elastic coefficient of the rubber (Neoprene) of conveyor belts was 1.1077 under the strain of 0.3 nym. from the results of field tests, the tilt angle of conveyor belt was the range of $25^{\circ}\~30^{\circ}$ under consideration far space of container, the velocity ratio between vehicle and conveying belt was 1 to 2.4~1.7 at 0.1~0.3 m/s of vehicle, and optimum rotating velocity of the disk cutter was 34.8 m/s or more under consideration for soil friction.

Measurement of the Tibial Plateau Angle in Normal Small Breed Dogs (정상 소형 견에서 경골 고평부각의 측정)

  • This study measured the tibial plateau angle (TPA), a value that predicts cranial cruciate ligament rupture (CCLR) and a prerequisite for dynamic stabilization, in small breed dogs. The cadavers of 32 skeletally mature small breed dogs without CCLR were used to measure TPA. Differences were evaluated between males and females, left and right limbs, and among breeds. The mean TPA of all dogs was $26.13^{\circ}{\pm}2.33^{\circ}$. The mean TPAs of left and right limbs were $26.00^{\circ}{\pm}2.23^{\circ}$ and $26.26^{\circ}{\pm}2.45^{\circ}$, respectively, which was not a significant difference. The mean TPAs of male and female dogs were $26.01^{\circ}{\pm}1.57^{\circ}$ and $26^{\circ}.22{\pm}2.80^{\circ}$, respectively, which was also not significant difference. The mean TPAs in Maltese, Poodle, Shih Tzu, and Yorkshire terrier were $26.06^{\circ}{\pm}2.96^{\circ}$, $25.21^{\circ}{\pm}1.38^{\circ}$, $26.65^{\circ}{\pm}2.96^{\circ}$, and $26.27^{\circ}{\pm}1.61^{\circ}$, respectively. Differences in these means were not statistically significant. The TPA measured from this study could provide important information for further investigation of the pathogenesis of and surgical techniques for CCLR in small breed dogs.

Development and Strength Evaluation of Ring Projection Welding Process of the Microminiature Tube and Tubesheet (초소형 튜브와 튜브판의 링 프로젝션 용접 공정개발 및 강도 평가)

  • Microminiature heat exchanger has been applied to the gas turbine in order to increase energy efficiency. During the production of microminiature heat exchanger, however, it is very difficult to weld tube to tubesheet. In this study, therefore, welding process of resistance ring projection was used, and weld tensile tests were performed. Sound weld joint was obtained as a result of applying resistance ring projection welding to microminiature heat exchanger to tubesheet. Cold weld occurred at under 1600A. Even though tensile strength was increased with increasing current, splash occurred and tensile strength decreased at 2000A due to the excessive current. Therefore it was determine that the optimal current is 1900A. As result of tensile tests based on ASME code for tube to tubesheet weldment, rupture position was weldment due to Fs(Fractured section) of nugget, which was smaller than tube thickness (t), and it was proven as a partial strength welding because of the average joint efficiency fr = 0.90.

Tests on Failure of Steel Angles due to Very Low-Cycle Fatigue of Loading (극저사이클 재하하에서 앵글 강부재의 파괴실험)

  • The objective of this study is to identify the quantitative relationships among the important physical factors associated with failure of steel members under strong seismic excitations through very low-cycle fatigue tests. Very low-cycle fatigue is meant to be structural fatigue causing cracks and rupture in about 5~30 cycle ranges. The angle specimen was subjected to repeated axial Ioad after undergoing inelastic buckling. The test results reveal that the energy absorption capacities vary heavily with the history of loading and the failure mode. The maximum values of residual local strain at the initiation of a visible crack due to the very low-cycle fatigue were of the order of 25~40%, regardless of loading patterns, deflection modes, and width-to-thickness ratios.

Gait Recovery Characteristic According to the Injury Aspect of Descending Motor Pathway in a Chronic Stroke Patient: a Case Study

  • Purpose: The stroke patients have gait dysfunction due to impaired neural tracts; corticospinal tract (CST), corticoreticular pathway (CRP), and vestibulospinal tract (VST). In this study, we investigated characteristics of gait pattern according to the injury aspect of the neural track in a stroke patient. Methods: One patient and six control subjects of similar age participated. A 19-year-old male patient with spontaneous intracerebral hemorrhage on right basal ganglia, thalamus, corona radiata and cerebral cortex due to arteriovenous malformation rupture. Diffusion tensor imaging (DTI) data was acquired 21 months after the stroke. Kinematic and spatio-temporal parameters of gait were collected using a three-dimensional gait analysis system. Results: On 21 months DTI, the CST and CRP in affected hemisphere showed severe injury, in contrast, the VST in affected hemisphere showed intact integrity. Result of gait analysis, walking distance and speed were significantly decreased in a patient. The stance rate of unaffected lower limb, the swing rate of affected lower limb and the duration of double stance significantly increased compared with normal control. The knee and hip joint angle were significantly decreased in a patient. Conclusion: We found recovered independent gait ability may be associated with unimpaired VST in a patient with severe injury in CST and CRP.

  • When open reduction of maxilla fractures is postponed due to concurrent life-threatening injuries, delayed union may result with malunion or nonunion. If delayed malunion is occurred, significant facial deformity may result, including a dished-out face, irregular retromaxillism with Angle's class III malocclusion, open anterior bite, nasal collapse, telecanthus and malar flattening. The treatment planning for this problem includes cephalometric evaluation anterior and lateral tomograms, dental casts, orthodontic planning, dental planning and use of impression tray to rupture the fibrous tissue casts, orthodontic planning, dental planning and use of impression tray to rupture the fibrous tissue attachment at the fracture site. In this paper, one case presented a 58-year-old female patient with maxilla retrusion after comminuted fracture, who was treated with orthodontic methods of maxillary protraction headgear and Plaster headcap, whereas the other two cases were about male patients who were treated principally with surgically open reduction or Le Fort I-controlled transverse osteotomy with iliac bone graft.

Comparison between Chronic Ulnar and Radial Collateral Ligament Repairs in the Metacarpophalangeal Joint of the Thumb (무지 중수지 관절의 만성 파열된 척측, 요측 측부 인대 봉합술 후 결과 비교)

  • Purpose: Favorable results have been reported after the direct repair of chronic ulnar collateral ligament ruptures of the thumb metacarpophalangeal (MP) joint, but the results for radial ligament seem rather controversial. The purpose of this study is to compare the results of ligament reattachment between chronic rupture of the ulnar and the radial collateral ligament (RCL) of the joint. Methods: We reviewed retrospectively the radiologic and clinical results of ligament reattachment with suture anchors for chronic (more than 6 weeks) rupture of the collateral ligament of the thumb MP joint with averaged 22-month follow-up. The data between 6 radial and 8 ulnar ligament repairs were compared statistically. Results: The average of postoperative ulnar deviation angle was $13.3^{\circ}$ in radial ligament and $2.0^{\circ}$ in ulnar ligament (p=0.020) in the last follow-up plain X-ray. Postoperative ligament instability was positive in 4 out of the 6 radial repairs and no case with instability was observed in ulnar ligament. In postoperative follow-up, sustained joint subluxation was observed only in 2 out of the 6 radial repairs. Conclusion: The delayed repair of the RCL of the thumb MP joint resulted in less favorable outcomes and ligament instability was observed postoperatively in more than half of the cases.
