  • A 3-month-old male domestic shorthair cat was presented for evaluation of stranguria and hematuria and a 4-year-old male domestic shorthair cat was presented for evaluation of stranguria. For cat 1, a swollen prepuce, erythematous preputial orifice, and urine pooling from the preputial orifice were noted. The preputial orifice was a pinpoint opening and penis was not extruded from the prepuce. For cat 2, a necrotic skin around the preputial orifice, small preputial orifice, and severe urine pooling from the preputial orifice were noted. The round-shaped resection of the tip of the prepuce was performed to increase the diameter of the preputial orifice and remove erythematous fragile skin or a necrotic skin around the preputial orifice and the edematous, redundant preputial mucosa in these 2 cats. The preputial mucosa was then apposed to the ipsilateral incised skin edge. There was no recurrence of clinical signs over 22- and 24-month follow-up period, respectively. The round-shaped resection technique provided adequate enlargement of the preputial orifice to allow extrusion of penis in these 2 cats with a phimosis. This technique could be helpful in cases with skin and mucosa necrosis around the preputial orifice that require round-shaped resection.

  • Synovial sarcoma us a rare malignant neoplasm of the soft tissue arising in the lower extremity, inguinal area, and upper arm. The majority occurs in patients between the age of 15 and 40 years. The histologic diagnosis is based on the classical biphasic type with the distinct epithelial and spindle cell components. We have recently encountered a case of metastatic synovial sarcoma of the lung diagnosed by fine needle aspiration cytology. A 34-year-old man was admitted because of a palpable mass on the antero-lateral side of the right tibia for 3 years. On admission, a well demarcated metastatic pulmonary nodule, measuring 5 cm in diameter, was also identified in the simple chest X-ray. Resection of the lower leg mass revealed typical histologic features of biphasic synovial sarcoma. Aspiration cytology of the pulmonary nodule revealed numerous clusters of spindle cells admixed with groups of epithelial cells. The epithelial cells had moderate-sized, round to oval shaped, and hyperchromatic nuclei. The cytoplasm was clear, but not distinctive. Interspersed tell elements were fibroblast-like spindle cells having elongated hyperchromatic nuclei.

  • A 6-year-old spayed female American Cocker Spaniel presented with episcleritis in the right and then the left eye (OS) at eight month interval. Repeated intralesional triamcinolone acetonide was administered subconjunctivally to both eyes (OU). During this period, scleral ectasia was revealed on ocular funduscopy OS and then confirmed on ultrasonography and computed tomography. A year later, conjunctival hyperemia occurred around remnant triamcinolone particles and was treated by resection of these particles in the OU. A recurrence of episcleritis, which did not regress, required repeated triamcinolone subconjunctival injections four months later in the OU. Four months after these injections OU, the dog was presented with bilateral conjunctival mass, which had developed over the previous month. The round-shaped masses with diameters of 1 cm were surgically resected from exposed scleral ectasia lesion of thin and bulging scleral surface in the OU. The cross-section of both masses showed a white-colored accumulation at the center and triamcinolone induced granulomas enclosing necrotic tissue were confirmed by impression cytology and histopathological examination.

  • Background In 2012, a new anatomic breast implant of form-stable silicone gel was introduced onto the Korean market. The intended use of this implant is in the area of aesthetic breast surgery, and many reports are promising. Thus far, however, there have been no reports on the use of this implant for breast reconstruction in Korea. We used this breast implant in breast reconstruction surgery and report our early experience. Methods From November 2012 to April 2013, the Natrelle Style 410 form-stable anatomically shaped cohesive silicone gel-filled breast implant was used in 31 breasts of 30 patients for implant breast reconstruction with an acellular dermal matrix. Patients were treated with skin-sparing mastectomies followed by immediate breast reconstruction. Results The mean breast resection volume was 240 mL (range, 83-540 mL). The mean size of the breast implants was 217 mL (range, 125-395 mL). Breast shape outcomes were considered acceptable. Infection and skin thinning occurred in one patient each, and hematoma and seroma did not occur. Three cases of wound dehiscence occurred, one requiring surgical intervention, while the others healed with conservative treatment in one month. Rippling did not occur. So far, complications such as capsular contracture and malrotation of breast implant have not yet arisen. Conclusions By using anatomic breast implants in breast reconstruction, we achieved satisfactory results with aesthetics better than those obtained with round breast implants. Therefore, we concluded that the anatomical implant is suitable for breast reconstruction.
