Repeat Colonoscopy Every 10 Years or Single Colonoscopy for Colorectal Neoplasm Screening in Average-risk Chinese: A Cost-effectiveness Analysis

  • Background: The appropriate interval between negative colonoscopy screenings is uncertain, but the numbers of advanced neoplasms 10 years after a negative result are generally low. We aimed to evaluate the cost-effectiveness of colorectal neoplasm screening and management based on repeat screening colonoscopy every 10 years or single colonoscopy, compared with no screening in the general population. Methods and materials: A state-transition Markov model simulated 100,000 individuals aged 50-80 years accepting repeat screening colonoscopy every 10 years or single colonoscopy, offered to every subject. Colorectal adenomas found during colonoscopy were removed by polypectomy, and the subjects were followed with surveillance every three years. For subjects with a normal result, colonoscopy was resumed within ten years in the repeat screening strategy. In single screening strategy, screening process was terminated. Direct costs such as screening tests, cancer treatment and costs of complications were included. Indirect costs were excluded from the model. The incremental cost-effectiveness ratio was used to evaluate the cost-effectiveness of the different screening strategies. Results: Assuming a first-time compliance rate of 90%, repeat screening colonoscopy and single colonoscopy can reduce the incidence of colorectal cancer by 65.8% and 67.2% respectively. The incremental cost-effectiveness ratio for single colonoscopy (49 Renminbi Yuan [RMB]) was much lower than that for repeat screening colonoscopy (474 RMB). Single colonoscopy was a more cost-effective strategy, which was not sensitive to the compliance rate of colonoscopy and the cost of advanced colorectal cancer. Conclusion: Single colonoscopy is suggested to be the more cost-effective strategy for screening and management of colorectal neoplasms and may be recommended in China clinical practice.

  • Epithelial-to-mesenchymal transition (EMT) is a collection of events that allows the conversion of adherent epithelial cells, tightly bound to each other within an organized tissue, into independent fibroblastic cells possessing migratory properties and the ability to invade the extracellular matrix. EMT contributes to the complex architecture of the embryo by permitting the progression of embryogenesis from a simple single-cell layer epithelium to a complex three-dimensional organism composed of both epithelial and mesenchymal cells. However, in most tissues EMT is a developmentally restricted process and fully differentiated epithelia typically maintain their epithelial phenotype. Recently, elements of EMT, specially the loss of epithelial markers and the gain of mesenchymal markers, have been observed in pathological states, including epithelial cancers. Increasing evidence has confirmed its presence in human colon during colorectal carcinogenesis. In general, chronic inflammation is considered to be one of the causes of many human cancers including colorectal cancer(CRC). Accordingly, epidemiologic and clinical studies indicate that patients affected by ulcerative colitis and Crohn's disease, the two major forms of inflammatory bowel disease, have an increased risk of developing CRC. A large body of evidence supports roles for the SMAD/STAT3 signaling pathway, the NF-kB pathway, the Ras-mitogenactivated protein kinase/Snail/Slug and microRNAs in the development of colorectal cancers via epithelial-tomesenchymal transition. Thus, EMT appears to be closely involved in the pathogenesis of colorectal cancer, and analysis refered to it can yield novel targets for therapy.

MDM2 Expression in Serous and Mucinous Epithelial Tumours of the Ovary

  • Background: Different types of cancer exhibit abnormalities in cell cycle regulators. The murine double minute-2(MDM2) cell cycle regulator is a proto-oncogene that negatively regulates the P53 tumour suppressor gene. Surface epithelial tumours constitute approximately two thirds of ovarian neoplasms. Each histologic type can be classified as benign, borderline and malignant. This study aimed to examine immunohistochemical expression of the MDM2 protein in ovarian serous and mucinous epithelial tumours (benign, borderline and malignant). Materials and Methods: This study included forty five ovarian tumours, subdivided into fifteen cystadenomas (5 serous and 10 mucinous), fifteen borderline tumours (11 serous and 4 mucinous) and fifteen cystadenocarcinomas (9 serous and 6 mucinous). Paraffin sections were stained with haematoxylin and eosin for histopathologic study, and with mouse monoclonal anti-MDM2 antibody for immunohistochemistry. Results: MDM2 positivity was detected in 28.9% of the studied ovarian tumours. All benign tumours were negative and positivity was significantly higher in malignant than borderline tumours (P value of chi-square test =0.000). Significantly, all MDM2 positive mucinous tumours were malignant with no positive mucinous borderline tumours. Malignant tumours showed positive MDM2 expression in 83.3% of mucinous type and in 55.6% of serous type. Borderline serous tumours showed negative MDM2 in 72.7% of cases (P value of Z test =0.04). Conclusions: Alterations in the expression of the cell cycle regulator (MDM2) occur early in the process of tumourigenesis in serous and mucinous ovarian tumours. We suggest that MDM2 may be used in those tumours as a marker for risk stratification and identification of cases with cancer development and progression. We recommend further studies on MDM2 immunohistochemistry, in conjunction with adjuvant methods as DNA ploidy and FISH gene amplification, focusing on the mucinous tumours and differentiating between the three tumour categories, benign, borderline and malignant.

Nurses' Experience of Middle East Respiratory Syndrome Patients Care (메르스 환자 간호에 대한 간호사의 경험)

  • This study was conducted to investigate the practical experience of nurses in MERS patients care. Nine nurses in one general hospital in S city underwent in-depth interviews from August 2015 to March 2016. The collected data were analyzed according to the procedure of the phenomenological method of Colaizzi. The following five categories were derived: '1. Anxiety and burden due to the risk of transmission of new infectious diseases', '2. Social isolation because of being the nurse in charge of MERS patients', '3. Faced with problems due to an unprepared treatment environment', '4. Overcome the burdensome MERS patient care process', '5. Reflect on the steps for preparing against new infectious diseases'. These results suggest that it is important to draw positive experiences as well as the negative effects of nurses who are involved in treatment of various infectious diseases, including MERS. Based on this, it can be used as a basis for reducing negative psychological and social impacts and improving positive adaptation. Additionally, it is necessary for healthcare workers including nurses to establish the necessary workforces, departments and guidelines for nursing new infectious disease patients.

  • One of the important applications in Enterprise Resource Planning(ERP) systems is the Manufacturing Resource Planning(MRPII) system using Bill of Material(BOM). The manufacturing resource planning determines the quantity and timing of the production or purchase of subassemblies and raw materials needed to support the Master Production Scheduling(MPS). The bill of material is the recipe, a list of the materials needed to make a product. This paper intends to suggest a component-based materials management system using Unified Modeling Language(UML), as an application system for automobile part industry. Applying component based materials management systems designed with UML methodology, we analyzed the workflow and the document on materials management process from production planning to inventory management, and implemented a prototype of efficient materials management system, as a surrogate of existing material requirement planning(MRPI) system. To produce many other assemblies for a automobile part firm, component parts are assembled into subassemblies that are joined to assemble the finished product. Through the system suggested in this study, the level of inventory has cut down and the cost of inventory management has decreased. Also, the development method using UML makes the analysis and design phase to shorten in implementation period of MRPII system. The implementation of materials management system using CBD shows the ease of use in software reuse and the interoperability with corporate Internal information system. The result of applying object-oriented CBD technique is to minimize the risk of life cycle and facilitate the reuse of software as mentioned to limitation of information engineering methodology.

Lateral approach for maxillary sinus membrane elevation without bone materials in maxillary mucous retention cyst with immediate or delayed implant rehabilitation: case reports

  • This case series study demonstrates the possibility of successful implant rehabilitation without bone augmentation in the atrophic posterior maxilla with cystic lesion in the sinus. Sinus lift without bone graft using the lateral approach was performed. In one patient, the cyst was aspirated and simultaneous implantation under local anesthesia was performed, whereas the other cyst was removed under general anesthesia, and the sinus membrane was elevated in a second process, followed by implantation. In both cases, tapered 11.5-mm-long implants were utilized. With all of the implants, good stability and appropriate bone height were achieved. The mean bone level gain was 5.73 mm; adequate bone augmentation around the implants was shown, the sinus floor was moved apically, and the cyst was no longer radiologically detected. Completion of all of the treatments required an average of 12.5 months. The present study showed that sufficient bone formation and stable implantation in a maxilla of insufficient bone volume are possible through sinus lift without bone materials. The results serve to demonstrate, moreover, that surgical treatment of mucous retention cyst can facilitate rehabilitation. These techniques can reduce the risk of complications related to bone grafts, save money, and successfully treat antral cyst.

Study on the Process to Decrease the molecular Weight of $\beta$-[1,6]-branched $\beta$-[1,3]-D-Glucans (분지 베타 글루칸의 저분자화 기술 연구)

  • ${\beta}$-(1,6)-Branched ${\beta}$-(1,3)-D-glucans are known to enhance the immune system in human body, and in most cases have higher molecular weights over 1 MDa. In order to enhance the efficacy of glucans by decreasing their molecular weights, sonication, acid treatment, and enzymatic hydrolysis were tested and compared in this work. Treatment of sonication was effective to decrease the molecular weight to the extent of several dozens of kilo-daltons, but have a risk to disorder the triple helical structure of the glucans. Acid treatment was also an effective method to degrade polysaccharides, but ${\beta}$-(1,6)-branched of the glucan molecules was found to be also hydrolyzed. Treatment of ${\beta}$-(1,3)-glucanase was an effective method to decrease the molecular weight in mild conditions, but could not hydrolyse the highly ${\beta}$-(1,6)-branched ${\beta}$-(1,3)-glucans efficiently.

  • The analogue radio broadcasting was proven to be a successful and efficient media of delivering information because of free content and wide availability of cheap receivers. But the competition was more dropped than any other media due to the limited amount of radio spectrum that is available. The risk of doing nothing is to miss the opportunity and to loss existing audiences in the long term, or to be locked into inefficient proprietary systems. That is the reason why it is important to act now. It is time for regulators and market participants to hone their strategies, by clearly establishing factors to adopt the digital radio and to facilitate its diffusion. The purpose of this study is to find adoption factors that affected to potential users of digital radio broadcasting. Former research based on diffusion of innovation theory showed that adoption of innovation is affected by demographic variables, media usage and consumer's attitude of innovation. In this study based on a consumer survey, we examined those factors and characteristics of each steps that suggested in diffusion of innovation theory. The findings of this study have implications to explore the suitable technologies and services of digital radio broadcasting.

  • Tremendous progress has been made over the past quarter-century studying the genetics of gametogenesis and the resulting gametes and embryos. Studies merging molecular techniques and conventional cytogenetics are now beginning to bridge the gap between what we have learned about the meiotic process in males and females and what we know of the mitotic chromosomes of zygotes. Numerical abnormalities in sperm, oocytes and embryo can now diagnosed by fluorescence in situ hybridization (FISH). "At risk" couples can, therefore, have only unaffected embryos replaced in the sterus and avoid the possibility of terminating a pregnancy that might only be diagnosed as affected later gestation. Single-cell genetic analysis has also provided powerful tools for studying genetic defects arising during early human development. Recent studies of sperms, oocytes and cleavage-stage human embryos have revealed an unexpectedly high incidence. These genetic abnormalities are likely to contribute to early pregnancy loss and have important implications for improving pregnancy rates in infertile couples by assisted reproduction. The widespread use of preimplantation genetic diagnosis (PGD) awaits further documentatio of safety and accuracy. Other issues also must be addressed. First, the ethical issues regarding germ cell and embryo screening must be addressed including what diseases are serious enough to warrant the procedure. Another concern is the use of this technology for non-genetic disorders such as gender selection. Finally, the experimental nature of these procedure must continually be discussed with patients, and long-term follow-up studies must be undertaken. Development of more accurate and less expensive assays coupled with improved assisted reproductive technology success rates may make PGD a more widely use clinical tool. The future awaits these development.velopment.

A Stochastic Analysis of VOC Emissions from the Distribution Process of the Gasoline

  • Estimating the emission rate of VOCs from a gasoline industry at national level can be a challenging take even though the estimation is mean-based. However, using the procedures in the US EPA AP-42 guidelines, it is possible to approximate the mean industry emission rate once enough data are available. However, this estimate can be misled in the sense that there exist many stochastic factors in the EPA\\`s estimation procedures and also throughout the marketing channels of gasoline industry. Addressing the stochasticity problem in EPA\\`s procedure is hard to tackle because the detailed data needed to execute the estimation are not usually available even from refiners. Instead, this research tries to stay focused on the second type of stochasticity issue, raised from the mean0based metrological and marketing practice data collected from the 4 major refiners. To do so emission raters from each marketing channels (8 marketing points by 3 transportation types and by storage facilities of 4 refiners) are estimated monthly, following AP-42 procedures and using Tank 4.0. Once these estimates are acquired, the distribution of VOC emission rate for each marketing channel of all 4 refiners is estimated through simulation method using @Risk. The mean-based emission rates are weighted by company quantities to estimate the emission rate from the whole gasoline industry. Simple economic implication is provided, based on the result. This study found that, on the mean-bases, about 0.66% of gasoline marketed are evaporated into air. Considering the stochasticity in the estimation, about 90% of simulation results fell into the range of 0.65 to 0.68%. For 90% chance, the estimated economic loss is $54.65 million to $57.17 million, not counting the cost caused by air quality degradation and associated health impact.
