Patterns of superficial veins in the cubital fossa and its clinical implications among southern Ethiopian population

  • Cubital fossa is the site where the venous accesses are frequently made. Superficial veins at this site display variations in their pattern among different populations. Knowledge of different venous pattern in the cubital fossa is important for diagnostic, surgical and therapeutic procedures. The purpose of this study was to report variations of the cubital superficial vein patterns in the southern Ethiopian subjects. An institution based cross-sectional study design was employed among 401 randomly selected patients presented at the triage room of Arba Minch General Hospital from January 15 to February 15, 2021. A questionnaire was used to collect socio-demographic data and images of the common and variant superficial venous patterns were recorded. Descriptive statistical analysis was performed. P<0.05 was considered as statistical significance. In the present study, a total of 802 cubital fossae from 401 study participants were examined. Five patterns of superficial veins were identified. Type 2 was the most common pattern and observed in 55.0% of cubital fossae (42.1% right and 67.8% left cubital fossae). The least common, type 5 variant was detected in 2.6% cubital fossae (2.7% right and 2.5% left). Statistically significant association based on sex and laterality was noted. The current study concluded that type 2 and type 3 patterns were more frequent superficial venous patterns in the cubital fossa and more common in males than female. Awareness of these uncommon cubital venous patterns and their incidence is very useful for those performing venipuncture or venisection especially under emergency conditions.

The effect of Myofascial Release and Infrared on the Range of Motion and Pain in Persons with Neck Pain (적외선치료와 근막이완술이 경부통증환자의 경추가동범위와 통증에 미치는 영향)

  • Purpose : The purpose of this study was to evaluate the Influence of Infra red(IR) and myofascial release(MRF) on the range of motion and pain in persons with neck pain. Methods : 24 subjects with neck pain participated in the experiment. All subject randomly assigned to the IR group and MFR group. Both groups receive 10minutes, 3 times per week during 3 weeks period. laser exercise(LEX) used to measure range of motion of neck and visual analog scale(VAS) used to measure pain. All measurements of each subject were measured at pre-treatment and post-treatment. Results : 1. The neck flexion, extension, right-sidebending, left-rotation, right-rotation range of motion of MFR group was significantly increased.(P<0.05). 2. The neck left-sidebending, left-rotation, right-rotation range of motion of IR group was significantly increased.(P<0.05). 3. As to compare two group, MFR group increases ROM more than IR group. Conclusion : These data suggests that MFR is more beneficial than IR.

Changes of the Hand Grip Strength accodring to Shoulder joint Angle (견관절의 각도에 따른 악력변화)

  • The shoulder joint permit ate greatest mobility of any joint area carries out the importment function of stabilization for hand use. So handgrip activity is important to evaluate while assessing shoulder load in manual work. There was an association between static handgrip and shoulder muscle activity. The purpose of this study was to find out the changes of the hand grip strength according to shouter an81e. One hundred (50 female, 50 male) college adult volunteers with no known shoulder dysfunction participated subject in three positions with elbow extension: (1) shoulder $0^{\circ}$ flexion (2) shoulder $90^{\circ}C$ flexion (3)shoulder $180^{\circ}C$ flexion. The paired t-test was used to determine the different in grip strength between right and left hand at shouter position change. All, there was significiant for all three position by right and left shoulder (p<0.001). In mon, the ANOVA results revealed not a significiant F-ratio fer all three position by right and left hand. In woman, revealed significiant (p<0.05).

The Analysis on Relation between Hamstring Length and Static Uprighting Balance (넙다리 뒤근육의 길이와 정적 기립균형과의 상관성 분석)

  • Purpose: The purpose of this study was to investigate hamstring length during static uprighting position using weight distribution ratio. Methods: Ninety-six volunteers(56 males) were participated. The active knee extension test(AKE) was measured 3 times on both legs. The mean score of AKE was obtained. Then, the measurement of weight distribution ratio was observed 3 times on both legs, and its mean value was also calculated. Subjects were divided four groups based on their hamstring length measurement; both hamstring shortening group, left hamstring shortening group, right hamstring shortening group and normal hamstring length group. One-way ANOVA was employed to analyse collected data. Pearson's correlation was also hired to see any relationship between the hamstring length and the weight distribution. Results: In anterior/left area and posterior/right area, both hamstring shortening group left hamstring shortening group were superior to right hamstring shortening group and normal hamstring length group. Significant difference was existed statistically(p<.05) between groups. The correlation analysis between hamstring length and weight distribution ratio has not been shown any significant findings(Pant/post=0.733, Plt/rt=0.135). Conclusion: The results of the study may be applied to therapeutic management in posture and movement skill in musculoskeletal physical therapy.

Perception of discrepancy in the upper midline position in conjunction with the gingival display according to various occupations in Iran

  • Objective: This study evaluated the influence of various gingival displays on the esthetic perception in the presence of upper dental midline discrepancy. Methods: A smiling image of a male subject was altered digitally to produce five image series: normal smile (series A), decreased tooth show (series B), increased gingival show (series C), maxillary cant (series D), and asymmetric upper lip elevation (series E). In each image series, the midline was deviated to the right and left incrementally. A total of 210 raters (four professional groups and laypersons, n = 42 in each group) determined the midline deviation threshold and the attractiveness of midline position in each series. Results: The right and left thresholds were statistically similar for the symmetrical series (A, B, and C), while for series D, the right threshold was significantly lower. In most rater groups, the mean threshold order was: B > A > E > C > D. In all the series, the raters selected the coincident midline as the most attractive series except for series D, for which 1-2-mm deviations to the left were selected as the most attractive by almost all the groups. Conclusions: It is crucial to establish the coincident midline position in a symmetrical smile, especially when a gummy smile exists. In the asymmetrical gingival show, a coincident midline might not be the most esthetic midline position.

A Study on Women's Level of Educational Need & Knowledge about Routine Episiotomy and the Degree of Discomforts and Pain after Episiotomy (일상화된 회음절개술에 대한 여성의 지식, 교육요구, 불편감 및 통증정도에 관한 일 연구)

  • The purpose of this study was to find out women's need and level of knowledge about episiotomy, pain and discomfort related to episiotomy on a cross-sectional survey design. The subjects were 102 postpartal women agreed on oral consent. 34 postpartal women admitted at obstetric ward of H university hospital, 34 postpartal women admitted at 2 Sanhujoriwons, and 34 women within one year afterbirth. They were selected in Seoul, Korea. Data were collected from July, 1 to September 30, 2000, by a structured questionnaire. The instrument used for this study was a questionaire consisted of 5 items of general characteristics, 12 items of obstetric characteristics, 10 items of level of knowledge (Chronbach $\alpha$ .8176), 8 items of need of education(Chronbach $\alpha$ .8836), 3 items of pain (Chronbach $\alpha$ .9252), and 3 items of discomfort (Chronbach $\alpha$ .8092). The data were analyzed by the SPSS/PC+ program using t-test, ANOVA and Scheffe test as a post hoc and Pearson Correlation Coefficient. The results of the study were as follows; 1. 63.2% of respondents had right answer on 6-8 items among 10 items. Only 4.4% of women got right answer on 10 items all. 2. The need of education was high(4.45%) on all items and the range of score was $4.25{\sim}4.64$. 3. The strength of pain was the highest within one week afterbirth(5.93/10) and became lower in 8-14 days afterbirth(2.55). And after 15days of postpartum, the pain level became to the lowest level(1.08). However, pain was delayed until more than one month afterbirth. 4. The level of discomfort was the highest one week afterbirth(6.88/10) and became lower in 8-14 days afterbirth(4.20). And after 15days of postpartum, the discomfort level became to the lowest level(2.47). Universally, the degree of discomfort was higher than pain. 5. There was a strong positive correlation between discomforts and pain ($r=.752^{**}$) and weak positive correlation between discomforts and the level of educational need($r=.308^*$). In conclusion, women have a right to choose whether she will have episiotomy or not according to her decision making based on the comprehensive knowledge of episiotomy before they get episiotomy with consent process and explanation in detail. Women health care providers like nurses have a responsibility to do conscious raising and empowerment for women so that they could lead themselves to choose given medical treatments for women's health and wellbeing and the quality of life in her life cycle.

The Effect of Hand Acupuncture Therapy and Moxibustion Heat Therapy on Dysmenorrhea Women (수지침과 뜸요법이 월경곤란증에 미치는 효과)

  • In order to find out whether Hand Acupuncture Therapy and Moxibustion Heat Therapy is effective to relieve dysmenorrhea syndrome, we performed a Quasi-experiment on a group of fourty females. The experiment was carried out during the period from April 20 to August 20, 1999. The group was divided into two sub-groups called "a Hand Acupuncture Therapy sub-group" and "a Moxibustion Heat Therapy sub-group" consisting of 20 females respectively. Hand Acupuncture Therapy and Moxibustion Heat Therapy were performed four times a week. Especially, in case of Moxibustion Heat Therapy, subjects were treated twice a times. The data analyzed by an SAS program. The results are as follows : 1) Hand Acupuncture Therapy and Moxibustion Heat Therapy are very effective to relieve dysmenorrhea syndrome. This study shows that in case of Hand Acupuncture Therapy sub-group, supposing that mean score of Menorrhalgia before treatment was 7.85, it became low to 4.50 when. subjects suffered the first menstruation and it was 2.50 at the second menstruation, and 1.60 at the third menstruation. In the mean score of Moxibustion Heat Therapy sub-group, Menorrhalgia before treatment was 7.85, it was 5.90 at the first menstruation(p<.05), and 3.00 at the second, and 1.85 at the third menstruation. 2) Among Hand Acupuncture Therapy subgroup, 9 subjects could hardly be relieved from the pain of dysmenorrhea at the first menstruation. So, they were treated additionally with the method of tonification and sedation of abdominal diagnosis of three constitution and became completely relieved at second menstruation. Meanwhile, 7 subjects among Moxibustion Heat Therapy also faced the same situation. So they were treated with Moxibustion on dorsum of hand and got effectiveness at the third menstruation after taking therapy. 3) Odinary dysmenorrhea syndrome are constipation, dizziness, anorexia, abdominal pain, lumbago, breast engorgement, abdominal distention, dysconcentration, nervousness, diarrhea, nausea & vomitting, apathy, restlessness, fatigue, aggression, leg pain, edema. After taking therapy, all of subjects were relieved from these dysmenorrhea syndrome at third menstruation. 4) All subjects were classified into five types of physical constitution with abdominal diagnosis of three constitution as follows: 18 cases of left kidney right yang excess, 8 cases of left yin right yang excess, 7 cases of left right kidney excess, 5 cases of left right yang excess, 2 cases of left yang right kidney excess.

The Study of Anticipatory Postural Adjustments in Voluntary Arm and Leg Movement (수의적인 상·하지 움직임 동안의 체간의 선행적 자세조절 연구)

  • Anticipatory postural adjustments are pre-planned by the central nervous system (CNS) before the activation of agonist muscles in the limbs, and minimize postural sway. Most previous studies on this topic have focused on upper-limb movement, and little research has been conducted on lower-limb movement. The purpose of this study was to investigate the recruitment order of left and right trunk muscles during limb movement. Fifteen healthy subjects (10 male, 5 female) were enrolled. Electro-myographic signals were recorded on the muscles of: (1) deltoid, lumbar erector spinae, latissimus dorsi and internal oblique during shoulder flexion, (2) rectus femoris, rectus abdominis, external oblique and internal oblique during hip flexion. During right upper limb flexion, the onset of left erector spinae muscle and left internal oblique muscle activity preceded the onset of right deltoid by 8.09 ms and 19.83 ms, respectively. But these differences were not significant (p>.05). A similar sequence of activation occurred with lower limb flexion. The onset of left internal oblique muscle activity preceded the onset of right rectus femoris muscle by 28.29 ms (p<.05). The onset of right internal oblique muscle activity preceded the onset of left rectus femoris muscles by 23.24 ms (p<.05). The internal oblique muscle was the first activated during limb movement. Our study established the recruitment order of trunk muscles during limb movement, and explained the postural control strategy of the trunk muscles in healthy people. We expect that this study will be used to evaluate patients with an asymmetric recruitment order of muscle activation due to impaired CNS.

Neural Tract Injuries by Penetration of Foreign Body: a Diffusion Tensor Tractography Study

  • We presented with a patient who showed injury of the cingulum and fornix by penetration of a foreign body into the brain on diffusion tensor tractography (DTT). A 63-year-old man suffered a brain injury by a part of a power saw blade that was suddenly detached from a power saw during work. A part of the power saw blade penetrated his right frontal skull and advanced to the right posterior horn of the lateral ventricle. This penetration caused traumatic intracerebral hemorrhage in the right frontal lobe and intraventricular hemorrhage in the lateral ventricle. He underwent craniotomy and removal of intracranial foreign bodies (bony pieces and saw blade). The patient's Memory Assessment Scale scores were 74 (4%ile) for global memory, 78 (7%ile) for verbal memory, and 80 (9%ile) for visual memory. DTTs showed disruptions in the anterior portion of the fornical body, right fornical crus, the anterior portion of the right cingulum, and the middle portion of the left cingulum, compared to the control. It seems that the sustained memory impairment of this patient might be related to injury of the cingulum and fornix.

Agenda and Alternatives of Home Health Care Nursing Policy based on Health Care Law and Regulations (가정간호사업을 위한 의료법 제정의 의미와 과제)

  • This article was trying to suggest an agenda and alternatives of home health care policy for the future home care growth and development and examine the significance of new health care law and regulations. In addition. It was to analyze and drive the home care system problems. of which was made an announcement on the 11th of April. that home health care project must be centered from the nationwide general hospital. As we have learned from the developed countries, the home health care have been closely related health care policy among the field of nursing area. Therefore. we need to understand our national health care policy and need to predict the future direction and goal of our home health care policy in order to enhance the growth and activation of health care system. Additionally. we also need to have a vision and ability to develop under our own health care policy with systematic and rational home care business escaping from past perspective and standpoint. We must create a master plan of home care system to fulfill one part of system of the function and an important role in order to pursue an advanced health care plan under our system. For instance. in the 21th century as we establish a master plan for the growth of our country home care plan with improving plan systematically and also we need to produce many highly qualified researching and good personal who can develop and maintain the system efficiently. Specially. based on the unique characteristics of our health care system and the direction of development of plan, we need to find and correct the problems which we have faced the present time, so that we can provide and reach the goal of advanced health care system which our government want to pursue. Finally. we have to strive our best effort to make our home health care system can be positioned and stand the right direction to have the benefit for every individual citizen in our country.
