강원도 보건진료소 고혈압 환자의 원격관리시스템 적용 효과 (The Effect of the Telemedicine Service System Application for the Patients with Hypertension at Community Health Practitioner Posts in Gangwon Province)

  • Objectives: This study is to identify the effects of utilizing a telemedicine service system on patients with hypertension at home in rural areas. Methods: The study was designed to be a retrospective case-control study. The subjects of this study were 152patients with hypertension who were managed by community health practitioner posts; using telemedicine service system group (n=76), usual care group (n=76). The data was collected through EMR (electric medical record) from September to December, 2013, and analyzed by descriptive statistics of $x^2$/t-test and ANCOVA. Results: The analysis showed that telemedicine service system was an effective way to deal with body weight (F=4.723, p=.031) and BMI (F=5.225, p=.024). Conclusions: This study provides evidence for utilizing the telemedicine service system based on information technology as intervention method in the hypertension management.

임신성 당뇨 과거력을 가진 여성의 2형 당뇨진단 양상과 관련요인 (Patterns of Diagnosis and Risk Factors for Type 2 Diabetes in Women with a History of Gestational Diabetes Mellitus)

  • Purpose: This study aimed to identify patterns of diagnosis and to explore risk factors for type 2 diabetes beyond the postpartum period in women with a previous history of gestational diabetes, and to identify differences in such risk factors between early and late-onset (aged <45 and ${\geq}45$). Methods: Using epidemiological data from the Korean Genome and Epidemiology Study, a retrospective analysis of 175 women with various timings of type 2 diabetes diagnosis was performed. Results: The average age ($42.6{\pm}10.6$) at type 2 diabetes diagnosis was earlier than the general population, and obesity was prevalent with marked weight gains around 35 years old. Longer duration of breastfeeding was observed in women with late-onset of type 2 diabetes. Conclusion: For prevention of type 2 diabetes, early intervention is required, and modifiable factors such as weight control and breastfeeding should be taken into consideration for intervention strategies.

Melanoma in Iran: a Retrospective 10-Year Study

  • Background: Melanoma, the most life-threatening type of skin cancer, is a malignant tumor initiating in melanocytes that rapidly metastasizes and causes death. Materials and Methods: In this retrospective study, samples were selected from patients' information files in our Cancer Institute in Tehran with a designed checklist. A total of 322 files were found from 2003 until 2012. Then the raw data were transferred to Statistical Package for Social Sciences (SPSS) software version 16 and additional analysis was performed by Students t-test. The important variables were considered according to the available information from history of pathology including age, gender, occupation, stage and location of tumor. Results: Our data showed that incidence of melanoma has been different in the studied 10-year period according to age. Also, incidence of melanoma was higher in men than women. It was more common in lower limbs. More commonly housewives among women and farmers among men were affected by melanoma. Conclusions: Taken together the descriptive data clarified general aspects of this disease for further screening and interventions.

Impact of Socioeconomic Status on 30-Day and 1-Year Mortalities after Intensive Care Unit Admission in South Korea: A Retrospective Cohort Study

  • Background: Socioeconomic status (SES) is closely associated with health outcomes, including mortality in critically ill patients admitted to intensive care unit (ICU). However, research regarding this issue is lacking, especially in countries where the National Health Insurance System is mainly responsible for health care. This study aimed to investigate how the SES of ICU patients in South Korea is associated with mortality. Methods: This was a retrospective observational study of adult patients aged ${\geq}20$ years admitted to ICU. Associations between SES-related factors recorded at the time of ICU admission and 30-day and 1-year mortalities were analyzed using univariable and multivariable Cox regression analyses. Results: A total of 6,008 patients were included. Of these, 394 (6.6%) died within 30 days of ICU admission, and 1,125 (18.7%) died within 1 year. Multivariable Cox regression analysis found no significant associations between 30-day mortality after ICU admission and SES factors (P>0.05). However, occupation was significantly associated with 1-year mortality after ICU admission. Conclusions: Our study shows that 30-day mortality after ICU admission is not associated with SES in the National Health Insurance coverage setting. However, occupation was associated with 1-year mortality after ICU admission.

A retrospective study of mandibular fractures in children

  • Objectives: The incidence and patterns of mandibular fractures vary by country and population age. This retrospective study evaluated the etiologies and patterns of mandibular fractures in children. Materials and Methods: The clinical records of 89 children (45 males and 44 females) aged 0 to 12 years who presented with mandibular fractures from July 2012 to June 2016 were retrospectively reviewed. The sex, patient age, site of fracture, etiology of trauma, and monthly variations of the fractures were recorded. Descriptive statistics, the z-test and chi-square test were used for statistical analysis and the P-value was set at <0.05. Results: Eighty-nine children (male-to-female ratio 1.02:1) sustained 131 mandibular fractures. Within the study sample, the 6 to 9 year age group most frequently experienced fractures (47.3%). Falls and road traffic accidents (RTA) were the two most common etiological factors that accounted for 44.9% and 24.7% of cases. The condylar fracture was the most common anatomical location (38.9%) followed by the angle (20.6%), parasymphysis (18.3%), body (15.3%), and symphysis (5.3%). A single fracture (51.7%) was more common than multiple fractures (48.3%). The month-wise distribution of mandibular fractures was fairly constant. Conclusion: The condylar region was the most common anatomic site for fractures; in addition, a fall and RTA were the major etiological factors for mandibular fractures. A single fracture was observed in 51.7% of patients while multiple fracture lines accounted for 48.3% of cases.

회전근개 파열 수술 후 환자에 대한 한의학적 치료 효과 : 후향적 관찰연구 (Effects of Korean Medicine on Post-rotator Cuff Surgery Patients : A Retrospective Observational Study)

  • The aim of this study was to investigate the effect of Korean medicine treatments on pain reduction and functional improvement of patients after rotator cuff surgery. We studied 10 patients who had admitted to Dunsan Korean Medical hospital after have being diagnosed as rotator cuff tear and got a surgery at other hospital from April 30, 2014 to April 30, 2019. This study was conducted as retrospective study which analyzes patient's medical records. Statistical analysis was performed using the IBM SPSS statistics 24 program. We used Numeric Rating Scale to evaluate pain reduction, Range of Motion to evaluate shoulder functional improvement and Five-point likert scale to evaluate the patient's satisfaction. The result showed that Korean medicine treatments statistically significantly reduced pain on the patients after rotator cuff surgery. Although there were improvement of Range of Motion, it wasn't statistically significant. In conclusion, we found that the Korean medicine treatments showed a positive effect on pain reduction of patients after rotator cuff surgery and got a high satisfaction.

Clinical features and management of snake bites in 70 dogs in Korea

  • Background: Snakebites remain a devastating and life-threatening environmental hazard. While the management of snakebites has been well described in humans, few clinical data and guidelines exist for dogs, especially in Korea. Objectives: This retrospective study evaluated the clinical features of 70 dogs with snakebite wounds in Korea. Methods: The medical records of 72 dogs that presented to three animal hospitals from June 2008 to July 2021 were reviewed; among these, 70 dogs that met the inclusion criteria were enrolled. Their signalment, history, clinical signs, physical examination, blood analysis, treatment, and prognosis were also evaluated. Results: Of 70 dog owners, 35 (50%) witnessed the bite, with a mean time between bite and hospital presentation of 9.7 ± 4.1 h in 58 dogs. Blood smears were evaluated in 45 dogs, of which 28 (62%) showed echinocytosis. Anemia and acute kidney injury were found in 21 (29%) and 2 dogs (3%), respectively. A total of 37 dogs (53%) were hospitalized, 5 (7%) of which died. Conclusions: The most significant finding was the high prevalence of echinocytosis. The data from this retrospective study could inform the management of dogs bitten by snakes in Korea.

영과잉포아송회귀분석을 활용한 안정병동에 입원한 정신질환자의 공격행동 예측요인 (Predictors for Aggressive Behavior of Patients with Mental Illness in a Closed Psychiatric Ward using Zero-Inflated Poisson Regression: A Retrospective Study)

  • Purpose: This study was conducted to identify predictors related to aggressive behavior of patients with mental illness admitted to a closed psychiatric ward. Methods: This study adopted a retrospective design which analyzed the hospital medical records of 363 patients with mental illness admitted to the psychiatric closed ward of a university hospital in Seoul, Korea. The collected data were analyzed using SPSS IBM 20.0 and STATA 12.0 SE. ZIP (Zero-Inflated Poisson) and count data analysis were used for the factor influencing the occurrence and frequency of aggressive behavior. Results: The results of ZIP model showed that the factors influencing non-probability of aggressive behavior were anxiety, non-adherence, and frustration. In addition, the factors influencing frequency of aggressive behavior were bipolar disorder and personality disorder trait. Conclusion: We found that bipolar disorder, frustration, and non-adherence are more likely to increase the likelihood of aggressive behavior in patients with mental illness. In particular, patients diagnosed with bipolar disorder were 1.95 times more likely to engage in repetitive aggressive behavior compared to those without a diagnose. However, since the results were different form previous studies, further studies on the traits of anxiety and personality disorders are needed.

Prediction of medication-related osteonecrosis of the jaw (MRONJ) using automated machine learning in patients with osteoporosis associated with dental extraction and implantation: a retrospective study

  • Objectives: This study aimed to develop and validate machine learning (ML) models using H2O-AutoML, an automated ML program, for predicting medication-related osteonecrosis of the jaw (MRONJ) in patients with osteoporosis undergoing tooth extraction or implantation. Patients and Methods: We conducted a retrospective chart review of 340 patients who visited Dankook University Dental Hospital between January 2019 and June 2022 who met the following inclusion criteria: female, age ≥55 years, osteoporosis treated with antiresorptive therapy, and recent dental extraction or implantation. We considered medication administration and duration, demographics, and systemic factors (age and medical history). Local factors, such as surgical method, number of operated teeth, and operation area, were also included. Six algorithms were used to generate the MRONJ prediction model. Results: Gradient boosting demonstrated the best diagnostic accuracy, with an area under the receiver operating characteristic curve (AUC) of 0.8283. Validation with the test dataset yielded a stable AUC of 0.7526. Variable importance analysis identified duration of medication as the most important variable, followed by age, number of teeth operated, and operation site. Conclusion: ML models can help predict MRONJ occurrence in patients with osteoporosis undergoing tooth extraction or implantation based on questionnaire data acquired at the first visit.

Weight status in survivors of childhood acute lymphocytic leukemia in South Korea: a retrospective descriptive study

  • Purpose: This study investigated weight status in survivors of childhood acute lymphocytic leukemia (ALL) and identified related factors. Methods: A retrospective review of the electronic medical records of survivors of childhood ALL (n=230) was conducted. We analyzed the survivors' characteristics, including sex, age, weight status at diagnosis, central nervous system involvement, risk classification, length of treatment, radiation therapy, and hematopoietic stem cell transplantation. Analysis of variance and the chi-squared test were applied to investigate influencing factors. Results: The weight status distribution was as follows: 23 individuals (10.0%) were classified as underweight, 151 individuals (65.7%) were healthy weight, and 56 individuals (24.3%) were overweight/obese. Age at diagnosis (F=10.03, p<.001), weight status at diagnosis (x2=43.41, p<.001), and risk classification (F=10.98, p=0.027) showed significant differences among the weight status groups. Survivors who were older at diagnosis and those in the very high-risk category had a higher likelihood of experiencing underweight status during their survivorship, while survivors who were overweight/obese at diagnosis were more likely to remain overweight/obese at the time of survival. Conclusion: Considering the potential health implications related to an unhealthy weight status in survivors of ALL, it is imperative to undertake early identification and implement interventions for at-risk individuals.
