A Case of Macular Edema Resulted from Branch Retinal Vein Occlusion (망막분지정맥폐쇄로 유발된 황반부종 치험 1례)

  • Objective : This study was performed to prove the effect of the oriental medical treatments like acupuncture, moxibustion, herbal medicine on macular edema resulted from branch retinal vein occlusion which could lead to terrible result like permanent visual loss at worst. Methods : The patient was treated 62 times, from July 15th, 2009 through May 12th, 2010. We used fundus photography and Dr. Hann's standard test chart for 5m to examinate the patient's eyesight. We used acupuncture, electropuncture, moxibustion and herbal medicine to treat the patient. We considered visual field, scotoma, metamorphopsia and vitreous floater to measure the effect of treatment. Results : After about 10 months of treatment, the eyesight of Rt eye was improved from 0.8 to 0.9, Lt. eye from zero to 0.3. The visual field turbidity were absolutely cleared. The scotomas in Lt. eye were reduced from 5 to 2. The metamorphopsia were improved. The vitreous floaters in Rt. eye were decreased from 5~6 to 1~2. Conclusion : These results show that the oriental medical treatment can be conservative measure to the macular edema resulted from branch retinal vein occlusion.

Abusive Head Trauma in Infants and Children in Japan

  • Subdural hematoma in infants can be caused by abuse, and is thought to be more likely if subdural hematoma is associated with retinal hemorrhage and cerebral edema. In Japan, few doctors disagree that cases of subdural hematoma with retinal hemorrhage and cerebral edema with multiple findings on the body are more likely to have been caused by abuse rather than by household accident. On the other hand, in cases where there are no other significant physical findings, only subdural hematoma and retinal hemorrhage, there is a difference of opinion as to whether the injury was caused by an accident or abuse. The reason for this is that neurosurgeons in Japan promoted the concept that infants can develop subdural hematomas and retinal hemorrages due to minor trauma at home before the concept of abusive head trauma became known. In addition, the age distribution of subdural hematomas in Japan differs from that in other countries, with peaks at around 8 months, and the reason for this remains unclear. Therefore, the etiology of infant subdural hematoma in Japan needs to be investigated in greater detail.

  • 1 . Experimental study Preventive effect of Korea Red Ginseng (KPG) on hypertensive retinal arteriolosis in rabbits was studied. The results as follows: Blood pressure: Hypertensive group (B) was obviously raised up in comparing with that in normal group(A) and in hypertension + KRG group(C). Ocular fundus:Changes in B group including the retinal arteriospasm, crossing arterioveous, exudation and edema. But C group showed lightly. Light microscope: HE stained vascular damage in retina including thickness hyalimisation, execdates and edema Electron microscope: The endothelial cells were arranged irregularly, different shape and showed cytoplasm loose and vacuole. Immunohistochemistry: Ginseng can regulating endothelin-1, angiotension-ll, endothelium grow factor expre,j,iion and secreation in retinal blood clrultion. 2. ClinicAl Study 66 of hypertensive patients (42 men, 23 women,48-68 years old)and 20 normal person (7 men, 13 women,47-68 years old) were administrated(p.0.) by HRG (3g per day for 6 weeks). The results showed that marked effective rate and total effective rate were 53cyo and 60.6alo respectively and no severe side effects were found. The above results suggest that Ginseng have a difinite hypotensive effect and a role of preventing hyperfine sloe arteriosclerosis.

  • Objective: Report of a case study on the resolution of central serous chorioretinopathy through cellular nutrition therapy (OCNT). Methods: A Korean male born in 1965 who has been diagnosed with central serous chorioretinopathy in the left eye since August 22, 2022. Results: Complete resolution of central serous chorioretinopathy following the implementation of nutritional therapy. Conclusion: Nutritional therapy may be beneficial in alleviating symptoms in patients with central serous chorioretinopathy

Validation Data Augmentation for Improving the Grading Accuracy of Diabetic Macular Edema using Deep Learning (딥러닝을 이용한 당뇨성황반부종 등급 분류의 정확도 개선을 위한 검증 데이터 증강 기법)

  • This paper proposed a method of validation data augmentation for improving the grading accuracy of diabetic macular edema (DME) using deep learning. The data augmentation technique is basically applied in order to secure diversity of data by transforming one image to several images through random translation, rotation, scaling and reflection in preparation of input data of the deep neural network (DNN). In this paper, we apply this technique in the validation process of the trained DNN, and improve the grading accuracy by combining the classification results of the augmented images. To verify the effectiveness, 1,200 retinal images of Messidor dataset was divided into training and validation data at the ratio 7:3. By applying random augmentation to 359 validation data, $1.61{\pm}0.55%$ accuracy improvement was achieved in the case of six times augmentation (N=6). This simple method has shown that the accuracy can be improved in the N range from 2 to 6 with the correlation coefficient of 0.5667. Therefore, it is expected to help improve the diagnostic accuracy of DME with the grading information provided by the proposed DNN.

Temporary blindness caused by corneal edema after a local anesthetic injection in the eyebrow region: a case report

  • Plastic surgery around the eyes is usually performed under local anesthesia, using a mixture of lidocaine and epinephrine. Blindness is a rare but devastating complication after the injection of local anesthesia in this region. Most cases reported to date have been caused by occlusion of the ophthalmic artery or central retinal artery. In this case report, however, we present a highly unusual case of blindness caused by corneal edema after a local anesthetic injection. A patient visited the emergency room with a laceration on the eyebrow, and local anesthesia was injected before suturing. Immediately after the injection, severe corneal edema developed, making it impossible to observe the structures in the anterior chamber in detail or check the light reflex and visual acuity of the naked eye. An antibiotic (moxifloxacin hydrochloride) and high-concentration steroid eyedrops were promptly applied. High-concentration steroids were also administered orally. On day 13 post-injury, the visual acuity of the naked eye improved to 1.0, and no recurrence of corneal lesions was observed. Although the cause of corneal edema after the local injection could not be conclusively identified, we hope that this report will help raise clinicians' awareness of this complication and appropriate treatment methods.

  • Even though phototherapy is an effective means of treating hyperbilirubinemia, adequate binocular shields are essential to prevent light hazard of retina during phototherapy. The phototherapy to young albino rabbits was performed under various conditions; exposured for 12, 24, and 48 hours, continuity or intermittence of irradiation, and use of shields. Although there was no definite pathologic change in indirect ophthalmoscopic examination, severe damage was seen at electron microscopic level. In all experimental groups, swelling and edema of photoreceptor were seen at right after phototherapy. Futhermore, more severe damage was seen in case of intermittent irradiated group and prolong exposed group. In mydriatic eyes, severity of damage was similiar to non mydriatic eyes, but area of retinal damage was more extensive than non-mydriatic eyes. In control group which was protected by binocular shields during phototherapy, no significant abnormality was found.

Smartphone Fundus Photography in an Infant with Abusive Head Trauma (학대뇌손상 영아에서 스마트폰으로 촬영한 안저소견)

  • Purpose: To report fundus photography using a smartphone in an infant with abusive head trauma. Case summary: An 8-month-old male infant presented to the emergency room with decreased consciousness and epileptic seizures that the parents attributed to a fall from a chair. He had no external wounds or fractures to the skull or elsewhere. However, computerized tomography of the brain revealed an acute subdural hematoma in the right cranial convexity and diffuse cerebral edema, leading to a midline shift to the left and effacement of the right lateral ventricle and basal cistern. The attending neurosurgeon promptly administered a decompressive craniectomy. Immediately after the emergency surgery, a fundus examination revealed numerous multi-layered retinal hemorrhages in the posterior pole extending to the periphery in each eye. He also had white retinal ridges with cherry hemorrhages in both eyes. We acquired retinal photographs using the native camera of a smartphone in video mode. The photographer held the smartphone with one hand, facing the patient's eye at 15-20 cm, and held a 20 diopter condensing lens at 5 cm from the eye in the other hand. Our documentation using a smartphone led to a diagnosis of abusive head trauma and to obtain the criminal's confession, because the findings were specific for repetitive acceleration-deceleration forces to an infant's eye with a strong vitreoretinal attachment. Conclusions: This ophthalmic finding had a key role in the diagnosis of abusive head trauma. This case presented the diagnostic use of a smartphone for fundus photography in this important medicolegal case.

The Comparative Study between the Transformations(傳變症) of Sogal(消渴) and the Complications of Diabetes Mellitus (소갈(消渴)의 전변증(傳變症)과 당뇨병(糖尿病)의 만성합병증(慢性合倂症)에 대한 비교고찰(比較考察))

  • Acute complications of diabetes mellitus were diminished after Banting and Best discovered insulin. But chronic complications of diabetes mellitus have been increased. The main complications of diabetes mellitus are diabetic retinopathy, diabetic nephropathy, diabetic neuropathy, diabetic foot lesion and macrovascular complication. These complications can result in renal failure, loss of sight, cerebral infarction and myocardial infarction. So it is very difficult to treat the complications of diabetes mellitus. In oriental medicine, the transformations(傳變症) of Sogal(消渴) are edema, carbuncle, loss of sight and so on. The comparative study between the trcmsformations(傳變症) of SogaI(消渴) and the complications of diabetes mellitus has come to the following conclusions. 1. In oriental medicine, diabetic retinopathy was expessed as loss of sight and the treament of diabetic retinopathy should be started at an early stage, to prevent vitreous hemorrhage and traction retinal detachment. 2. In oriental medicine. diabetic nephropathy was expressed as edema and the treatment should be started at an early stage of renal injury when the protein comes from urine.3. Symmetrical distal polyneuropathy is the main part of diabetic neuropathy and it was expressed as weakness of the lower limbs and pain of joints in the symptoms of Haso(下消). In Oriental medicine, acupuncture and herb medicine which effect is SopungHwalHyul can treat polyneuropathy. 4. Chief macrovascular complications are coronary artery disease and cerebrovascular disease, The cause of macrovascular complication is atherosclerosis. So the method of treating atherosclerosis should be studied in oriental medicine. 5. Diabetic foot were expressed as carbuncle and its main causes are decreasing perfusion of fool, diabetic neuropathy and infection. So these causes should be studied in oriental medicine. 6. The complications of diabetes mellitus afe very similar to the transfonnatiuns of Sogal(消渴).The control of blood glucose is indispensable to prevent and delay the complication of diabetes mellitus.

Ocular Lesions Induced Experimentally by Very Virulent Strain of Marek′s Disease Virus in Chickens (닭의 마렉병 바이러스 강독주 실험접종에 의한 안구병변)

  • Ocular lesions induced in 40 specific-pathogen-free Marek's disease (MD) resistant chicks by inoculation at 1 day of age with very virulent strain of Marek's disease virus (WV) were pathologically examined. Grossly,24/40 (60%) chicks had white gel-like materials in the vitreous body, whereas thickening and discoloration of iris (gray eye) were not observed. Microscopically, characteristic ocular MD lesions were observed in choroid (27/40), ciliary (30/40) and iris (23/40) in which small focal inflammatory to diffuse neoplastic Iymphoid cells were infiltrated. Five out of 40 MDV-inoculated birds revealed necrotizing Iymphomas in choroid. These lesions consisted of necrotic and degenerating Iymphoblasts accompanied by intranuclear inclusion body. There was retinal atrophy and necrosis with inclusion body detected in necrotic ganglion, inner or outer nuclear and infiltrated Iymphoblast cells. Conjunctiva showed lymphoid cell infiltration in 29/40 chicks inoculated with MDV, Vitreous body exhibited mild to severe exudation of eosinophilic proteinaceous material in 24/40 chicks. These lesions were associated with Iymphoid cell infutration, edema and fibrosis of choroid. Pecten (7/40) and optic nerve (13/40) were infiltrated usually mildly with Iymphoid cells. From these results, very virulent strain, Md/5 of MDV caused high incidence of ocular lesions in MD resistant chicks. In addition, Md/5 induced exudation of proteinaceous material into the vitreous body and fibrosis of choroid. Necrotizing ocular Iymphoma lesions in choroid is the first report in the MD literature.
