  • The reconstruction of major head and neck defects must be an integral part of the overall cancer treatment plan. The priorities of surgical treatment of head and neck tumor are 1) local tumor control, 2) relief of pain, 3) avoidance of difficult dressing, 4) provision of oral continence, and 5) ability to swallow and manage saliva. The recent advances in reconstructive surgery including the development of musculocutaneous flaps and microvascular free tissue transfer have allowed the surgical restoration of head and neck tumor defects that previously were not possible. These techniques have provided the opportunity to undertake larger, more aggressive resection while at the same time permitting functional rehabilitation. The timing of reconstruction demands on the nature of the resection, the ability of the ablative and reconstructive teams to coordinate efforts, the overall health of the patients, the patient's needs and wishes. So, we report to emphasize current methods for restoring major head and neck tumor defects after tumor ablation, reviewing for the reconstructive operations, postoperative complications, and postoperative sequelae etc, of patients from Jan, 1990 to Dec, 1993.

Adjustment of Middle-aged People with Hemiplegia after a Stroke (뇌졸중 후 편마비를 가진 중년기 환자의 적응과정)

  • Purpose: The purpose of this study was to understand the adjustment process of middle-aged people in Korea with hemiplegia after a stroke. Method: For this study, the grounded theory method was utilized. Results: After constant comparative analysis, the core category emerged as 'rebuilding the body which was ruined'. In addition, the causal conditions were 'restriction of physical function', and 'loss of roles'. The adjustment process consisted of the 'facing reality phase', 'overcoming depression phase', 'overcoming physical limits phase' and 'reestablishing roles phase' The main strategies in the facing reality phase were 'holding on to a glimmer of hope', 'getting away from the harsh reality' and 'facing up to the reality'. The main strategies in the overcoming depression phase were 'soothing oneself', 'Self-introspection' and 'restoring self-esteem'. In addition, the main strategies in the self-initiative overcoming physical limits phase was 'discovering personal strategies and striving to recover'. The main strategies in the reestablishing roles phase were 'reestablishing a parents' role', 'reestablishing a spouse's role' and 'reestablishing social roles'. Conclusion: In conclusion, even though many middle aged stroke patients remained in the depression phase, most of them who overcame depression strived to conquer physical limits on their own initiative.

RETROSPECTIVE STUDY OF SINTERED POROUS-SURFACED DENTAL IMPLANTS IN RESTORING THE POSTERIOR MAXILLA (상악 구치부에 식립된 엔도포아 임플란트의 후향적 연구)

  • Purpose: The purpose of this retrospective report was to analyze long-term survival rate of sintered porous-surfaced dental implant ($Endopore^{(R)}$ Dental Implant system, Innova Corporation, Toronto, ON, Canada). Methods: 61 partially edentulous patients were received a total of 127 Endopore dental implants in the maxilla. Of the 127 implants, 24 implants were restored with individual (ie, non-splinted) crowns, while 103 implants were splinted to other implants. Medical records and radiographs were evaluated and analyzed by the cumulative survival rate, location of implants, implants length and diameter, crown/implant ratio and whether the implant was splinted. Chi squire test was used statistically. Result: Of the 127 implants, 8 implants (6.3%) were removed and and cumulative survival rate was 93.7%. Conclusion: Endopore implants showed satisfactory results after up to 8 years function periods in the edentulous posterior maxilla.

Automatic Electronic Cleansing in Computed Tomography Colonography Images using Domain Knowledge

  • Electronic cleansing is an image post processing technique in which the tagged colonic content is subtracted from colon using CTC images. There are post processing artefacts, like: 1) soft tissue degradation; 2) incomplete cleansing; 3) misclassification of polyp due to pseudo enhanced voxels; and 4) pseudo soft tissue structures. The objective of the study was to subtract the tagged colonic content without losing the soft tissue structures. This paper proposes a novel adaptive method to solve the first three problems using a multi-step algorithm. It uses a new edge model-based method which involves colon segmentation, priori information of Hounsfield units (HU) of different colonic contents at specific tube voltages, subtracting the tagging materials, restoring the soft tissue structures based on selective HU, removing boundary between air-contrast, and applying a filter to clean minute particles due to improperly tagged endoluminal fluids which appear as noise. The main finding of the study was submerged soft tissue structures were absolutely preserved and the pseudo enhanced intensities were corrected without any artifact. The method was implemented with multithreading for parallel processing in a high performance computer. The technique was applied on a fecal tagged dataset (30 patients) where the tagging agent was not completely removed from colon. The results were then qualitatively validated by radiologists for any image processing artifacts.

  • This study is about the preservation of wooden buildings as cultural properties. I got the idea for preservation and repair of cultural properties of wooden buildings by watching the process of repairing them in Japan. The Japanese have been endeavored to restoring old wooden buildings to their original forms for more than a hundred years. The idea to change decayed materials into new materials or the new approach to the process of restoration used to be in conflict with the concept of 'Authenticity'--the criteria for evaluation of the value of cultural properties, based on the Western of thinking, established by UNESCO. Since 'Nara Conference on Authenticity', held in 1994 in Nara, however, the concept of 'Authenticity' has changed, and the peculiarity of certain cultural properties of the wooden buildings and their diversity of culture have now begun to be recognized by UNESCO. This is a crucial change for the study of preservation, repair and restoration of cultural properties of wooden buildings. The purpose of this study is, therefore, to look closely at the process of change of the concept 'Authenticity' and find a better method for preservation of cultural properties of wooden buildings.

  • This study aims to analyze the construction of Kyungsang KAMYOUNG in the late Chosun dynasty by restoring its constructive system and relating it with the office organization. followings are the summary of this study. 1. Government office of Daegu consists of inn(KAEGSA), Kyungsang KAMYOUNG, and Daegu city hall(BUA), and JINYOUNG. 2. The layout of virtual placement is shown in the in the text. Even though it had been newly constructed due to three times of fire, the fundamental scheme of space is found out not to have been changed since mid-eighteen century at the latest. 3. Followings are the result of analyzing the space of KAMYOUNG, after dividing its space into entry area, main business area and police military business area in the aspect of business. First, entry area consists of 2 door system with external door(UOISAMMUN) and internal door(NAESAMMUN). Second, main business area consists of facilities for inspector, public officials, and low grade employee respectively This space is divided into SUNHWADANG area and JINGCHONGKAK area, and the former consists of active business facilities, and the latter consists of passive business facilities focusing on the rest facilities. Third, business support area consists of facilities for low grade employee and minor assistant respectively, which are placed to the left and right of POJONGMUN and KWANPUNGRU. Fourth, police military business space is in middle(JUNGYOUNG) and forms separate areas to the east of SUNHWADANG. Accordingly, It is certain that the space of Kyungsang KAMYOUNG had been placed in accordance with the nature of business per position.

  • Improving quality of life can be achieved by rehabilitating one socially and restoring their sexual life. Holistic care for persons with disabilities means the rehabilitation nurse intervenes when problems of sexual function are discovered, observed, or expressed. However, most people with chronic illnesses and disabilities must confront prevalent myths about their sexual nature and abilities. These myths often have their basis in the notion, common in our society, that the only people who are sexual are those who are young and beautiful. People who are obese, old, or do not conform to current standards of sexual attractiveness are usually deemed as asexual or undesirable. These narrow sterotypes have been particularly damaging to physically or mentally disabled people. People with chronic illnesses and disabilities often have special needs regarding their sexual behavior. Most of us will confront chronic illness or disability in our own lives to some degree. In addition, illnss or disability can occur at any age. In this study, I discuss some of the special needs of people with chronic illnesses and disabilities and the methods of coping with those needs. I also discuss some of the sexual concerns that people with chronic illnesses and disabilities have in common, and the assessment of sexual function and nursing strategies through literature review.

Regularized Iterative Image Restoration by using Method of Conjugate Gradient (공액경사법을 이용한 정칙화 반복 복원 방법)

  • This paper proposes a regularized iterative image restoration using method of conjugate gradient considering a priori information. Compared with conventional regularized method of conjugate gradient, this method has merits to prevent the artifacts by ringing effects and the partial magnification of the noise in the course of restoring the image degraded by blur and additive noise. Proposed method applies the constraints to accelerate the convergence ratio near the edge portions, and the regularized parameter suppresses the magnification of the noise. As experimental results, I show the superior convergence ratio and the suppression by the artifacts of the proposed method compared with conventional methods.

The evaluation of maximum bite force in the occlusal rehabilitation of patient with Angle Class III malocclusion: a case report

  • The case report describes the occlusal rehabilitation of a male patient with Angle Class III malocclusion and its effect on maximum bite force. The main complaints of patient were masticatory difficulty and poor esthetic. The patient's expectations from the treatment were a good esthetic and function with a less invasive and relatively promptly way. Therefore, increasing of the occlusal vertical dimension (OVD) and then restoring the maxillary and mandibular teeth was chosen by the patient among the treatment options. At the beginning of treatment maximum bite force of patient was measured. Then an occlusal splint was provided to evaluate the adaptation of the patient to the altered OVD. Full mouth rehabilitation with metal ceramic restorations was made. After the completion of full mouth restoration, bite force measurement was repeated and patient exhibited increased maximum bite force. Full mouth restorative treatment in a patient with Class III malocclusion could be an effective treatment approach to resolve esthetic concern and to improve masticatory function related to maximum bite force.

Visual Biofeedback to Monitor Serratus Anterior and Upper Trapezius Muscle Activities During Arm Lowering (팔 내리기 동작 시 시각적 바이오피드백 훈련이 전거근과 상부 승모근의 근활성도에 미치는 영향)

  • To reduce winging scapula, various exercise protocols have been widely used by clinicians. Selective serratus anterior strengthening, and restoring balanced function, are especially recommended to reduce winging scapula. The purpose of this study was to investigate visual biofeedback using a real time video camera display system for monitoring scapular winging during arm lowering. For this study, 13 males with winging scapular were recruited during arm lowering. Electromyography (EMG) activity was recorded from the serratus anterior (SA) and upper trapezius (UT) of the right side and compared with normal EMG activity using a paired t-test. The study showed, through visual biofeedback, that EMG activity significantly increased in the SA and significantly decreased in the UT (p<.05). These results suggest that visual biofeedback can be recommended as an effective method for scapular eccentric control, to prevent scapular winging during arm lowering.
