치근 우식 수복에 사용되는 심미성 수복물의 변연누출에 관한 연구 (A STUDY ON THE MARGINAL LEAKAGE OF ESTHETIC RESTORATIVE MATERIALS ON ROOT CARIES RESTORATION)

  • The purpose of the study was to evaluate the degree of the marginal leakage of esthetic restorative materials on root caries restoration. 120 cavities were prepared to $90^{\circ}$ butt joint on all margins on the crown and root portion, and divided into 4 groups. The four groups of cavity were filled with Amalgam(Dongmuyung Dental Alloy Co., Ltd, KOREA), Silux$^{(R)}$(3M Co., USA)-Scotch Bond 2$^{(R)}$(3M Co., USA), Silux$^{(R)}$-All Bond$^{(R)}$(BISCO USA), and GC Fuji II$^{(R)}$(G-C Co., JAPAN) respectively. The apical margin of the preparation was finished to leave a flash of restorative material. The coronal margin of the preparation was finished not to leave a flash of restorative material. All specimens were sectioned longitudinally with Isomet Low speed saw(Buether Ltd, USA). The degree of dye penetration was evaluated as the parameter of marginal leakage under the stereoscope. The results were as follows. 1. At the enamel and dentin/cementum margins, the margin were finished to leave a flash of material showed less marginal leakage than that were finished not to leave a flash of material (P<0.001). 2. The enamel margins showed less marginal leakage than the dentin/cementum margins(P<0.001). 3. There was no significant difference in the degree of the marginal leakage between Silux$^{(R)}$-Scotch Bond 2$^{(R)}$ group and Silux$^{(R)}$-All Bond$^{(R)}$ group.

알카자이트 수복재의 색안정성 : 실험실적 연구 (Color Stability of Alkasite Restorative Material: in vitro Studies)

  • 이 연구의 목적은 알카자이트 수복재와 소아치과 영역에서 널리 사용되고 있는 수복재들의 색 안정성을 비교하고, 다양한 음료에서의 색상 변화를 비교하는 것이다. 글라스아이오노머, 레진강화형 글라스아이오노머, 알카자이트 수복재, 복합레진의 시편을 준비하였고, 열순환 후 색 안정성을 측정하였다. 침전 전과 7일, 14일, 21일, 28일 후 CIE L*a*b*값의 색상 변화를 분석하기 위해 각 수복재료의 시편을 5개의 군으로 나누어 각각 PBS, 커피, 녹차, 콜라, 오렌지주스 용액에 침전시켰다. 열순환 후 복합레진에서 가장 높은 색 안정성을 보였고, 알카자이트 수복재, 글라스아이오노머, 레진강화형 글라스아이오노머 순으로 높은 안정성을 보였다. 7일간 다양한 용액에 침전시킨 결과 모든 시편에서 색상 변화가 나타났으며, 알카자이트 수복재는 글라스아이오노머 계열보다 낮은 색상 변화를 보였고, 복합레진보다는 높은 색상 변화를 나타냈다. 알카자이트 수복재는 커피에서 가장 많은 색상 변화가 나타났으며 그 다음은 녹차, 오렌지주스 순이었으나, 콜라와 PBS 용액에서는 침전 28일 이후에도 색상 변화를 거의 보이지 않았다.

Criteria for clinical translucency evaluation of direct esthetic restorative materials

  • The purpose of this review was to suggest practical criteria for the clinical translucency evaluation of direct esthetic restorative materials, and to review the translucency with these criteria. For the evaluation of reported translucency values, measuring instrument and method, specimen thickness, background color, and illumination should be scrutinized. Translucency parameter (TP) of 15 to 19 could be regarded as the translucency of 1 mm thick human enamel. Visual perceptibility threshold for translucency difference in contrast ratio (${\Delta}CR$) of 0.07 could be transformed into ${\Delta}TP$ value of 2. Translucency differences between direct and indirect resin composites were perceivable (${\Delta}TP>2$). Universal and corresponding flowable resin composites did not show perceivable translucency differences in most products. Translucency differed significantly by the product within each shade group, and by the shade group within each product. Translucency of human enamel and perceptibility threshold for translucency difference may be used as criteria for the clinical evaluation of translucency of esthetic restorative materials.

In vitro wear behavior between enamel cusp and three aesthetic restorative materials: Zirconia, porcelain, and composite resin

  • PURPOSE. The aim of this study was to identify the effects of three aesthetic restorative materials on the wear between tooth and restoration by a pin-on-disk manner. MATERIALS AND METHODS. Six aesthetic restorative materials were used to prepare disk specimens for wear test, which were Lava Zirconia as zirconia group, Vintage MP and Cerabien ZR as veneering porcelain group, Gradia Direct microhybrid composite containing prepolymerized fillers, Filtek Z250 microhybrid composite containing zirconia glass and colloidal silica particles, and Filtek Z350 nanocomposite as composite resin group. Vertical loss of the worn cusp, change of the surface roughness of the restoration materials, and the surface topography were investigated after wear test under 9.8-N contact load. RESULTS. The porcelain groups (Vintage MP and Cerabien ZR) caused the largest vertical loss of teeth when compared with those of the composite resin and zirconia groups, and Filtek Z250 microhybrid composite results in the second-largest vertical loss of teeth. The surface of Filtek Z350 nanocomposite was deeply worn out, but visible wear on the surface of the zirconia and Gradia Direct microhybrid composite was not observed. When the zirconia surface was roughened by sand-blasting, vertical loss of teeth considerably increased when compared with that in the case of fine polished zirconia. CONCLUSION. It was identified that microhybrid composite resin containing a prepolymerized filler and zirconia with reduced surface roughness by polishing were the most desirable restorative materials among the tested materials to prevent the two-body wear between aesthetic restorative material and tooth.

Translucency changes of direct esthetic restorative materials after curing, aging and treatment

  • The purpose of this article was to review the changes in translucency of direct esthetic restorative materials after curing, aging and treatment. As a criterion for the evaluation of clinical translucency changes, visual perceptibility threshold in translucency parameter difference (${\Delta}TP$) of 2 was used. Translucency changes after curing were perceivable depending on experimental methods and products (largest ${\Delta}TP$ in resin composites = 15.9). Translucency changes after aging were reported as either relatively stable or showed perceivable changes by aging protocols (largest ${\Delta}TP$ in resin composites = -3.8). Translucency changes after curing, aging and treatment were perceivable in several products and experimental methods. Therefore, shade matching of direct esthetic materials should be performed considering these instabilities of translucency in direct esthetic materials.

Comparative study of the radiopacity of resin cements used in aesthetic dentistry

  • PURPOSE. The aim of this study was to compare the radiopacity of 6 modern resin cements with that of human enamel and dentine using the Digora digital radiography system, to verify whether they meet the requirements of ANSI/ADA specification no. 27/1993 and the ISO 4049/2000 standard and assess whether their radiopacity is influenced by the thickness of the cement employed. MATERIALS AND METHODS. Three 3-thickness samples (0.5, 1 and 1.5 mm) were fabricated for each material. The individual cement samples were radiographed on the CCD sensor next to the aluminium wedge and the tooth samples. Five radiographs were made of each sample and therefore five readings of radiographic density were taken for each thickness of the materials. The radiopacity was measured in pixels using Digora 2.6 software. The calibration curve obtained from the mean values of each step of the wedge made it possible to obtain the equivalent in mm of aluminium for each mm of the luting material. RESULTS. With the exception of Variolink Veneer Medium Value 0, all the cements studied were more radiopaque than enamel and dentin (P<.05) and complied with the ISO and ANSI/ADA requirements (P<.001). The radiopacity of all the cements examined depended on their thickness: the thicker the material, the greater its radiopacity. CONCLUSION. All materials except Variolink Veneer Medium Value 0 yielded radiopacity values that complied with the recommendations of the ISO and ANSI/ADA. Variolink Veneer Medium Value 0 showed less radiopacity than enamel and dentin.

Endodontic micro-resurgery and guided tissue regeneration of a periapical cyst associated to recurrent root perforation: a case report

  • Although the success rates of microsurgery and micro-resurgery are very high, the influence of a recurrent perforation combined with radicular cyst remains unclear. A 21-year-old white female patient had a history of root perforation in a previously treated right maxillary lateral incisor. Analysis using cone-beam computed tomography (CBCT) revealed an extensive and well-defined periapical radiolucency, involving the buccal and palatal bone plate. The perforation was sealed with bioceramic material (Biodentine) in the pre-surgical phase. In the surgical phase, guided tissue regeneration (GTR) was performed by combining xenograft (lyophilized bovine bone) and autologous platelet-rich fibrin applied to the bone defect. The root-end preparation was done using an ultrasonic tip. The retrograde filling was performed using a bioceramic material (Biodentine). Histopathological analysis confirmed a radicular cyst. The patient returned to her referring practitioner to continue the restorative procedures. CBCT analysis after 1-year recall revealed another perforation in the same place as the first intervention, ultimately treated by micro-resurgery using the same protocol with GTR, and a bioceramic material (MTA Angelus). The 2-year recall showed healing and bone neoformation. In conclusion, endodontic micro-resurgery with GTR showed long-term favorable results when a radicular cyst and a recurrent perforation compromised the success.

수복재료에 대합되는 의치용 레진치의 마모저항성 비교 (COMPARISON OF WEAR RESISTANCE AMONG RESIN DENTURE TEETH OPPOSING VAR10US RESTORATIVE MATERIALS)

  • The aim of this study was to compare wear resistance of resin denture teeth opposing various restorative materials. The wear resistance of conventional acrylic resin teeth(Trubyte Biotone) and three high-strength resin teeth(Bioform IPN, Endura, SR-Orthosit-PE) opposing different restorative materials(gold alloys, dental porcelain, composite resin) was compared. Wear tests were conducted with a sliding-induced wear testing apparatus which applied 100,000 strokes to the specimen in a mesio-distal direction under conditions of 100 stroke/min and constant loading of 1Kgf/tooth. Wear resistance of the resin denture teeth was evaluated by the following criteria : 1) wear depth, 2) weight loss, and 3) SEM observation. Results were as follows. 1. When opposed to gold alloys and composite resin, high-strength resin teeth showed superior wear resistance compared to acrylic resin teeth. But, in cases opposing dental porcelain, differences between the wear of the high-strength and acrylic resin teeth were not statistically significant (p<0.05). 2. When comparing wear resistance among high-strength resin teeth, opposing gold alloys, Endura was slightly more resistant and while in cases opposing dental porcelain, SR-Orthosit-PE was showed to be slightly resistant(p<0.05). 3. The wear of high-strength resin teeth was greater by 5 to 7 times when opposing porcelain and 2 to 3 times when opposing composite resin compared to gold alloys(p<0.05). 4. SEM observations of the wear surface showed that wear of resin teeth opposing gold alloys is a fatigue type of wear and wear of resin teeth opposing dental porcelain is fatigue and abrasion type of wear. Trubyte Biotone showed more severe fatigue type of wear than high-strength resin teeth. In conclusion, the use of dental porcelain should seriously be considered as restorative material in cases opposing resin denture teeth and improvement seems to be needed on resin teeth in the areas of wear resistance.

Radiopacity of restorative composites by conventional radiograph and digital images with different resolutions

  • Purpose: This study was performed to evaluate and compare the radiopacity of dentin, enamel, and 8 restorative composites on conventional radiograph and digital images with different resolutions. Materials and Methods: Specimens were fabricated from 8 materials and human molars were longitudinally sectioned 1.0 mm thick to include both enamel and dentin. The specimens and tooth sections were imaged by conventional radiograph using #4 sized intraoral film and digital images were taken in high speed and high resolution modes using a phosphor storage plate. Densitometric evaluation of the enamel, dentin, restorative materials, a lead sheet, and an aluminum step wedge was performed on the radiographic images. For the evaluation, the Al equivalent (mm) for each material was calculated. The data were analyzed using one-way ANOVA and Tukey's test (p<0.05), considering the material factor and then the radiographic method factor, individually. Results: The high speed mode allowed the highest radiopacity, while the high resolution mode generated the lowest values. Furthermore, the high resolution mode was the most efficient method for radiographic differentiation between restorative composites and dentin. The conventional radiograph was the most effective in enabling differentiation between enamel and composites. The high speed mode was the least effective in enabling radiographic differentiation between the dental tissues and restorative composites. Conclusion: The high speed mode of digital imaging was not effective for differentiation between enamel and composites. This made it less effective than the high resolution mode and conventional radiographs. All of the composites evaluated showed radiopacity values that fit the ISO 4049 recommendations.

Influence of different surface treatments on bond strength of novel CAD/CAM restorative materials to resin cement

  • PURPOSE. To evaluate the effects of different surface treatments on the bond strength of novel CAD/CAM restorative materials to resin cement by four point bending test. MATERIALS AND METHODS. The CAD/CAM materials under investigation were e.max CAD, Mark II, Lava Ultimate, and Enamic. A total of 400 bar specimens ($4{\times}1.2{\times}12mm$) (n=10) milled from the CAD/CAM blocks underwent various pretreatments (no pretreatment (C), hydrofluoric acid (A), hydrofluoric acid + universal adhesive (Scotchbond) (AS), sandblasting (Sb), and sandblasting + universal adhesive (SbS)). The bars were luted end-to-end on the prepared surfaces with a dual curing adhesive resin cement (Variolink N, Ivoclar Vivadent) on the custom-made stainless steel mold. Ten test specimens for each treatment and material combination were performed with four point bending test method. Data were analyzed using ANOVA and Tukey's test. RESULTS. The surface treatment and type of CAD/CAM restorative material showed a significant effect on the four point bending strength (FPBS) (P<.001). For LDC, AS surface treatment showed the highest FPBS results ($100.31{\pm}10.7MPa$) and the lowest values were obtained in RNC ($23.63{\pm}9.0MPa$) for control group. SEM analyses showed that the surface topography of CAD/CAM restorative materials was modified after treatments. CONCLUSION. The surface treatment of sandblasting or HF acid etching in combination with a universal adhesive containing MDP can be suggested for the adhesive cementation of the novel CAD/CAM restorative materials.
