Verification of a computer-aided replica technique for evaluating prosthesis adaptation using statistical agreement analysis

  • PURPOSE. The purpose of this study was to evaluate the reliability of computer-aided replica technique (CART) by calculating its agreement with the replica technique (RT), using statistical agreement analysis. MATERIALS AND METHODS. A prepared metal die and a metal crown were fabricated. The gap between the restoration and abutment was replicated using silicone indicator paste (n = 25). Gap measurements differed in the control (RT) and experimental (CART) groups. In the RT group, the silicone replica was manually sectioned, and the marginal and occlusal gaps were measured using a microscope. In the CART group, the gap was digitized using optical scanning and image superimposition, and the gaps were measured using a software program. The agreement between the measurement techniques was evaluated by using the 95% Bland-Altman limits of agreement and concordance correlation coefficients (CCC). The least acceptable CCC was 0.90. RESULTS. The RT and CART groups showed linear association, with a strong positive correlation in gap measurements, but without significant differences. The 95% limits of agreement between the paired gap measurements were 3.84% and 7.08% of the mean. The lower 95% confidence limits of CCC were 0.9676 and 0.9188 for the marginal and occlusal gap measurements, respectively, and the values were greater than the allowed limit. CONCLUSION. The CART is a reliable digital approach for evaluating the fit accuracy of fixed dental prostheses.

Early cicatricial lagophthalmos release with pentagonal wedge resection of the scar, fat redistribution, and full-thickness skin grafting

  • The specialized structure of the upper eyelid ensures complete closure of the eye and eyeball sealing. An upper eyelid injury can cause various symptoms associated with eyeball trauma, not just scar formation or eyelid deformity. In this report, we describe a case of lagophthalmos observed after wound repair in a patient with a crushing injury caused by a grinder. Several surgical techniques are used to treat lagophthalmos or scar contracture. In most cases, a releasing procedure is performed after 6 months of initial repair. However, if the patient has severe symptoms that are not relieved by conservative care, early revision is inevitable. We describe a case of early lagophthalmos successfully resolved with pentagonal wedge resection, fat redistribution, and full-thickness skin grafting. After the revisional surgery, we observed that the patient regained the ability to completely close the injured eyelid, with restoration of function and favorable cosmetic outcomes. Pentagonal wedge resection to release a retracted structure, fat redistribution to prevent readhesion, and full-thickness skin grafting for enough amount of skin to regain upper eyelid function is useful for scar release and lagophthalmos following crushing injuries of the upper eyelid.

Creation Techniques of Ecological Park to Consider Urban Forest Ecosystem in Woomyeonsan Urban Nature Park, Seoul (서울시 우면산 도시자연공원내 도시림의 생태적 특성을 고려한 생태공원 조성기법)

  • This study is to suggest the technique of ecological park to compatible mountain ecosystem near the urban in Woomyeonsan Urban Nature Park. It was proposed to differential technique from established ecological park focused on changing the cognition of nature, wetland ecosystem protection and restoration, and studying by served sound leisure program(nature observation and experience, etc.), in compatible mountain and urban forest near the Urban. Firstly, to achieve this aim, it was carried out delicated research and analysis that were the angle of inclination, trail, fauna, actual vegetation, bird and insect, and then to selected nature trail course and analysis capability through it.. In the basic concept and plan, firstly to make park aid mark and foundation mark, and then to establish the detail programs, nature observation program, management and organization program. This site where valley and mountain coexist was highly capable but whole site was very steep, so usable area is very narrow. To make Woomyeonsan nature ecological park escape from the established wetland type, ecological park and the park of event have to use nature material and introduce visitor. To establish it as the Nature Culture observation park and reflect on forest, valley of Woomyeonsan was divided into three display places; Nature ecosystem Zone, Meditation Zone, Nature Culture experience Zone and Nature Culture observation Center. For satisfying with ecological park aid mark and foundation mark, it was proposed to settle management plan for volunteer control and organization composition.

3D Object Restoration and Data Compression Based on Adaptive Simplex-Mesh Technique (적응 Simplex-Mesh 기술에 기반한 3차원 물체 복원과 자료 압축)

  • Most of the 3D object reconstruction techniques divide the object into multiplane and approximate the surfaces of the object. The Marching Cubes Algorithm which initializes the mesh structure using a given isovalue. and Delaunay Tetrahedrisation are widely used. Deformable models are well-suited for general object reconstruction because they make little assumptions about the shape to recover and they can reconstruct objects *om various types of datasets. Now, many researchers are studying the reconstruction systems based on a deformable model. In this paper, we propose a novel method for reconstruction of 3D objects. This method, for a 3D object composed of curved planes, compresses the 3D object based on the adaptive simplexmesh technique. It changes the pre-defined mesh structure, so that it may approach to the original object. Also, we redefine the geometric characteristics such as curvatures. As results of simulations, we show reconstruction of the original object with high compression and concentration of vertices towards parts of high curvature in order to optimize the shape description.

Effects of Diluents and Cryoprotectants on Sperm Cryopreservation in Starry Flounder (Platichthys stellatus) (강도다리(Platichthys stellatus) 정자의 냉동보존에 미치는 희석액과 동해방지제의 영향)

  • An experiment was performed to obtain cryopreservation techniques of starry flounder (Platichthys stellatus) sperm. Milt obtained from 24 males were cryopreserved using two diluents, artificial seminal plasma (ASP) and Stein's solution (SS) with three cryoprotectants, dimethyl sulfoxide (DMSO), methanol, and glycerol concentrated from 5% to 20%. Post-thaw sperm activity (motility and/or speed) revealed the highest in 10% DMSO and 15% methanol in ASP and SS as diluent. Motility and speed of cryopreserved sperm were decreased according to increase of glycerol concentration. To conclude, DMSO was a better cryoprotectant than methanol or glycerol for cryopreservation of starry flounder sperm. Glycerol was incongruent cryoprotectant because of toxic to starry flounder sperm. Most cryopreserved spermatozoa without cryoprotectant showed the enlarged head with granulated chromatin and ruptured plasma membrane by freezing and thawing injuries compared with unfrozen normal spermatozoa.

  • 무심천에는 2002년부터 자연형 하천정비사업에 착수하면서 저수로, 저수로 하안, 고수부지 등 하도내에 여러 자연형 하천 공법과 함께 시설들을 설치하였다. 본 모니터링 조사사업은 '무심천 살리기'의 일환으로 이루어진 하천종합조사사업으로써 무심천의 수리/수환경 변화와 생태계의 변화를 조사를 통해 생태 수환경 수리특성 등 생태복원에 필요한 기초자료를 축적하고, 필요한 공법 등 향후 추진될 무심천 하천정비 및 관리에 반영하기 위하여 저수량, 홍수량, 생태계 및 하천공법 등에 대한 조사 및 분석을 전문가, 시민참여 그리고 공무원이 함께 수행하고 있다. 본 연구에서는 무심천을 친자연형 하천으로 복원하기 위하여 실시한 친자연형공법에 대하여 모니터링을 실시하고 얻은 수리/공학적 그리고 생태적 성과물을 자연형하천정비사업에 피드백시키는 자연형 하천관리방안에 대하여 논하였다. 무심천의 친자연형하천관리를 위하여 하천의 3대기능과 관련한 항목을 이용하여 평가지표를 설정하고, 평가를 하고 자연형 하천공법 등급을 I-V로 구분하여 하천관리를 위한 기준으로 사용하였다. 평 저수시 무심천 본류와 지류에 대한 유량측정을 실시하고, 하천유지유량을 산정하였다. 저수/고수호안, 하상, 둔치에 대한 조사분석을 실시하고 관리방안을 제시하여 무심천을 점차 자연형하천으로 변화시키고 있다. 또한 생태계의 적응관리로 하천식생을 유지하기 위한 관리를 실시하고, 유해식물과 외래식물을 제거하고, 하도내 직경이 10(m) 이상되는 버드나무 수목에 밑둥치기를 실시하여 치수기능의 저해를 방지하였다.

  • As the Seated Iron Buddha Statues, Vairocana Buddha of Dopiansa Temple in Cheolwon, Nosana Buddha of Samhwasa Temple in Donghae and Sakyamuni of Mangisa Temple in Pyeongtaek were made during Unified Silla to Koryo Dynasty. These are damaged degradation which are crack, break-out, peel off and various pollutant. As a result of deterioration evaluation using radiography, crack, gap, break-out, pore space and restoration material are confirmed inside in the Buddha Statues. Based on iron strength, the Buddha Statues will be maintain current state as long as a high external impact is not applied. Also, iron core and nails used for fixing of internal and external framework were observed in the Buddha Statues. According to prominent line of surface, embossed inscription, hands cast separately and combined, the Buddha Statues were made by using division casting.

Fabrication of Tissue Engineered Intervertebral Disc Using Enable 3D bio-printing and Scaffod-Free technologies (3D 바이오프린팅과 무지지체 조직공학 기술 기반 추간판 복합 조직 제작)

  • Intervertebral disc(IVD) mainly consists of Annulus fibrosus(AF) and Nucleus pulposus(NP), playing a role of distributing a mechanical load on vertebral body. IVD tissue engineering has been developed the methods to achieve anatomic morphology and restoration of biological function. The goal of present study is to identify the possibilities for creating a substitute of IVD the morphology and biological functions are the same as undamaged complete IVD. To fabricate the AF and NP combine biphasic IVD tissue, AF tissue scaffolds have been printed by 3D bio-printing system with natural biomaterials and NP tissues have been prepared by scaffold-free culture system. We evaluated whether the combined structure of 3D printed AF scaffold and scaffold-free NP tissue construct could support the architecture and cell functions as IVD tissue. 3D printed AF scaffolds were printed with 60 degree angle stripe patterned lamella structure(the inner-diameter is 5mm, outer-diameter is 10 mm and height is 3 mm). In the cytotoxicity test, the 3D printed AF scaffold showed good cell compatibility. The results of histological and immunohistochemical staining also showed the newly synthesized collagens and glycosaminoglycans, which are specific makers of AF tissue. And scaffold-free NP tissue actively synthesized glycosaminoglycans and type 2 collagen, which are the major components of NP tissue. When we combined two engineered tissues to realize the IVD, combined biphasic tissues showed a good integration between the two tissues. In conclusion, this study describes the fabrication of Engineered biphasic IVD tissue by using enable techniques of tissue engineering. This fabricated biphasic tissue would be used as a model system for the study of the native IVD tissue. In the future, it may have the potential to replace the damaged IVD in the future.

Slot Correction by the Frechet Flaps in Hair Restoration Surgery (두발재건 환자에서 Frechet 피판술을 이용한 선상 반흔제거)

  • Purpose: A common side effect of the scalp reduction is a creation of a 'slot' with the hair growing in the opposite directions away from the scar. Overcoming the unnatural appearance of the slot has been a vexing problem in the scalp reduction surgery. None of the conventional corrective surgical techniques provides a complete and satisfactory aesthetic result. The Frechet flap is a triple transposition flap used for the correction of the slot defect secondary to scalp reduction surgery, seldom needing further scar revision. The Frechet technique provides a solution to the problem of the central slot concealment that is unattainable by other means, such as; Z-plasty and mini-graft. Methods: Authors applied the Frechet technique to Asian patients who had undergone scalp reduction and operated on 4 patients from March, 2000 to January, 2001. Average follow-up period was 13 months. Patients with long scars passing through the temporoparietoccipital zone were excluded. All the undermining was performed in the subgaleal plane, reaching the upper auricular sulcus and stopping just above the nuchal ridge. Results: None of the patients experienced infection, hematoma, nor any permanent hair loss. Transient telogen effluvium at the distal end of flap 2 and 3 was noticeable in one case. Conclusion: In conclusion, the results are aesthetically satisfactory without any significant complications.

  • Purpose: The purpose of the present article is to help orthopedic surgeons better understand the function and performance of reverse total shoulder arthroplasty, and also to help them perform the most proper surgical technique for reconstruction. Materials and methods: In this article, the specific technical aspects and pitfalls of reverse total shoulder arthroplasty were reviewed in depth. Additionally, the current issues relevant to the reverse total shoulder arthroplasty such as scapular notching and restoration of active external rotation were discussed. Results and conclusion: An understanding of the biomechanics of reverse total shoulder arthroplasty and the technical details and pitfalls of its implantation are critical in order to provide the best functional outcome without increasing the risk of complications.
