Recently attention has been focused on the effects of early intervention, or its lack, on both normal and preterm infants. Particularly numerous studies suggest that premature infants are not necessarily understimulated but instead are subjected to inappropriate stimulation. Developmental support and sensory stimulation have become clinical opportunities in which nursing practice can impact on the neurobehavioral outcome of premature infants. Developmental care has been widely accepted and implemented in neonatal intensive care units across the country. Increasingly, attention and concern in caring for low-birth-weight infants and premature infants has led clinicians in the field to explore the effects of a complex of interventions designed to create and maintain a developmentally supportive environment; to provide age-appropriate sensory input; and to protect the infant from inappropriate, excessive and stressful stimulation. The components of developmental care include modifications of the macro-environment to reduce NICU light and sound levels, care clustering, nonnutritive sucking, and containment strategies, such as flexed positioning or swaddling. Sensory stimulation of the premature infants is presented to standardize the modification of a developmental intervention based on physiologic and behavioral cues. The most appropriate type of stimuli are those that are sensitive to infant cues. Evaluation of infant physiological and behavioral responds to specific intervention stimuli may help to identify more appropriate interventions based on infants' cues. A critical question confronting the clinician is that of determining when the evidence supporting a change in practice is sufficient to justify making that change. There are acknowledged limitations in the current studies. Many of the studies examined had small sample sizes; used nonprobability sampling; and used a phase lag design, which introduces the possibility of threats to internal validity and limits the generalizability of the results. Although many issues regarding the effects of developmental interventions remain unresolved, the available research base documents significant benefits of developmental care for LBW infants in consistent outcomes, without significant adverse effects. Particularly, although the individual studies vary somewhat in the definition of specific outcomes measured, instrumentation used, time and method of data collection, and preparaion of the care providers, in all studies, infants receiving the full protocol of individualized developmentally supportive care had improvements in some aspect of four areas of infant functioning: level of respiratory or oxygen support, the establishment of oral feeding; length of hospital stay, and infant behavioral regulation. In summary, based on the available literature, individualized developmental intervention should be incorporated into standard practice in neonatal intensive care. And this implementation needs to be coupled with ongoing research to evaluate the impact of an individualized developmental care programs on the short- and long-tenn health outcomes of LBW infants.
For years, physicians and anatomists have been interested in the heart that has one functioning ventricle. Various terms have been suggested for this entity including single ventricle, common ventricle, double-inlet left ventricle, cor biatriatum triloculare, and primitive ventricle. In this report, the term "single ventricle" is utilized as suggested by Van Praagh, and is defined as that congenital cardiac anomaly in which a common or separate atrioventricular valves open into a ventricular chamber from which both great arterial trunks emerge. An outlet chamber, or infundibulum, may or may not be present and give rise to the origin of either of the great arteries. This definition excludes the entity of mitral and tricuspid atresia. An 11 year old cyanotic boy was admitted chief complaints of exertional dyspnea and frequent upper respiratory infection since 2 weeks after birth. He was diagnosed as inoperable cyanotic congenital heart disease, and remained without any corrective treatment up to his age of 11 year when he suffered from aggravation of symptoms and signs of congestive heart failure for 2 months before this admission. On 22nd of May 1979, he was admitted for total corrective operation under the impression of tricuspid atresia suggested by a pediatrician. Physical check revealed deep cyanosis with finger and toe clubbing, and grade V systolic ejection murmur with single second heart sound was audible at the left 3rd intercostal space. Development was moderate in height [135 cm] and weight[28Kg]. Routine lab findings were normal except increased hemoglobin [21.1gm%], hematocrit [64 %], and left axis deviation with left ventricular hypertrophy on EKG. Cardiac catheterization and angiography revealed 1-transposition of aorta, pulmonic valvular stenosis, double inlet of a single ventricle with d-loop, and normal atriovisceral relationship [Type III C solitus according to the classification of Van Praagh]. At operation, longitudinal incision at the outflow tract of right ventricle in between the right coronary artery and its branch [LAD from RCA] revealed high far anterior aortic valve which had fibrous continuity with mitral annulus, and pulmonic valve was stenotic up to 4 mm in diameter positioned posterolaterally to the aorta. Ventricular septum was totally defective, and one markedly hypertrophied moderator band originated from crista supraventricularis was connected down to the imaginary septum of the ventricular cavity as a pseudoseptum of the ventricle. Size of the defect was 3X3 cm2 in total. Patch closure of the defect with a Teflon felt of 3.5 x 4 cm2 was done with interrupted multiple sutures after cut off of the moderator band, which was resutured to the artificial septum after reconstruction of the ventricular septum. Pulmonic valvotomy was done from 4 mm to 11 mm in diameter thru another pulmonary arteriotomy incision, and right ventriculotomy wound was closed reconstructing the right ventricular outflow tract with pericardial autograft of 3 x 4 cm2. Atrial septal defect of 2 cm in diameter was closed with 3-0 Erdeck suture, and atrial wall was sutured also when rectal temperature reached from 24`C to 35.5`C. Complete A-V block was managed with temporary external pacemaker with a pacing rate of 110/min. thru myocardial wire, and arterial blood pressure of 80/50 mmHg was maintained with Isuprel or Dopamine dripping under the CVP of 25-cm saline. Consciousness was recovered one hour after the operation when his blood pressure reached 100 /70 mmHg, but vital signs were not stable, and bleeding from the pericardial drainage and complete anuria were persisted until his heart could not capture the pacemaker impulse, and patient died of low output syndrome 320 min after the operation.

수면 무호흡증은 고혈압, 뇌졸중, 심근경색, 우울증 및 인지기능 장애 등 다양한 2차 질환을 일으키며 심혈관 및 뇌혈관 질환의 발생 원인이 되기에 수면 무호흡의 조기 발견 및 지속적인 관리가 절실히 필요하다. 본 연구에서는 수면 중 호흡을 모니터링하기 위해 목의 기관 부위에서 호흡에 의한 미세한 진동을 검출할 수 있는 PVDF 필름 진동 센서를 이용하여 정상호흡과 수면 무호흡을 판단하는 웨어러블 디바이스를 개발하였다. 검출된 호흡음 데이터를 기반으로 호흡수, 무호흡 등의 변수들을 추출하였고 잡음 제거 알고리즘을 수립하여 잡음 신호가 있을 시에도 영향을 최소화하는 방법을 적용하였다. 또한 변동 문턱치 알고리즘을 수립하여 불규칙적인 호흡 패턴에서도 분석이 가능함을 확인하였다. 그 결과 분당 호흡수의 정확도는 98.7%이고 무호흡 횟수와 무호흡 지속 시간에 대한 개발기기와 수면다원검사의 결과를 비교한 결과 무호흡 횟수 측정 정확도는 92.6%, 무호흡 지속 시간의 정확도는 94.0%임을 확인하였다. 본 연구의 결과는 일상생활 중 가정에서 간편하고 정확하게 수면 정보를 모니터링하고 치료 경과를 확인할 수 있는 웨어러블 디바이스의 제품화로 이어져 수면장애의 관리에 큰 도움이 될 것이다.
전방경추융합술을 시행한 후 역설상기도폐쇄와 중추성 무호흡이 발생하였던 증례를 경험하여 이를 보고하고자 한다. 환자는 48세 남자로 본원으로 전원되기 9개월 전에 5번과 6번 경추사이의 추간판탈출증으로 수술을 시행받았으며, 내원 2개월 전에는 가성관절증이 발생하여 자가골이식과 함께 전방접근법으로 5번과 6번 경추의 경추융합술을 시행받았다. 이 수술을 시행 후 환자는 코골이와 과도한 주간 졸음증, 불면증 등의 증세가 생겼으며 호흡시에 상기도부위에서 이상한 소리가 발생하였다. 코인두경 및 자기공명영상으로 호기시 코인두부위의 역설적 협착소견을 관찰할 수 있었고, 철야 수면다원검사에서 무호흡지수는 8.7/시간 (중추성 무호흡, 7.0/시간; 폐쇄성 무호흡, 1.7/시간)이었다. 경비적 지속성 기도양압치료를 시작하였으나 환자가 압력을 못견뎌하여 레이저목젖입천장성형술을 시행하였다. 수술 2개월 경과 후 무호흡과 더불어 임상증세는 현격히 호전되었다. 저자들은 이러한 역설상기도폐쇄가 비록 그 기전이 명확하지는 않지만 전방경추융합술과 관련되어 있다고 추정한다. 또한 이러한 상기도 폐쇄가 중추성 수면 무호흡을 유발할 수 있다는 것을 보여주는 증례라고 생각한다.
연구배경 : 기관지 결핵은 폐결핵의 특이형태 또는 일종의 합병증으로 알려져 있으며 진단 및 치료에 많은 문제점이 있어왔으나 전체적인 발생율은 명확히 알려져 있지않다. 저자들은 활동성 폐결핵 환자들을 대상으로 전향적으로 기관지 내시경을 시행하여 기관지 결핵의 빈도 및 임상상을 연구하였다. 방법 : 1995년 3월 부터 1996년 2월까지 충남대학교병원 내과에 내원하여 활동성 폐결핵으로 진단받은 환자 103명을 대상으로 폐결핵 진단후 2주 이내에 기관지 내시경을 시행하였고 이들을 대상으로 임상 증상과 흉부 방사선 검사 및 기관지 세척액 항산균 검사, 폐기능검사 등을 시행하였다. 결과 : 활동성 폐결핵 환자 103명 중 기관지 결핵환자는 55명으로 기관지 결핵의 동반율은 53.4% 였고 이중 남자는 32명으로 43.8%의 발병율을, 여지는 23명으로 76.7%의 발병율을 보였다. 가장 흔한 증상은 기침, 객담, 발열 등의 비특이적 증상이었고 진찰소견상 수포음, 감소된 호흡음 및 천명이 청진되었다. 기관지경상의 기관지결핵 아형은 7가지 모두가 관찰되었으며 부종-충혈형이 가장 흔히(32.7%) 관찰되었고, 만성 비특이성 기관지염형 및 섬유성 협착형의 순으로 관찰되었다. 이들 아형과 흉부 방사선 소견 및 결핵의 진행정도와는 무관하였고, 우폐의 침범율이 좌폐보다 높았으며, 좌상엽이 가장 호발하는 부위였고, 우상엽, 기관의 순으로 침범된 소견을 보였다. 또한 폐기능검사 소견상 특이한 점은 없었고, 폐결핵만 존재한 경우와 차이가 없었다. 이들에서 객담 및 기관지 세척액 항산균 도말 검사 양성은 72.7% 이었다. 결론 : 이상의 결과로서 폐결핵환자에서 기관지 결핵의 병발은 이전에 알려진 것보다 훨씬 높으며, 페결핵의 진단시 특히 항산균 도말양성시 기관지 내시경 검사의 시행이 고려되어야 할 것으로 생각된다.
원발성 쇼그렌 증후군 (Primary Sj$\ddot{o}$gren's syndrome) 환자에서 폐에 발생하는 Bronchus-associated lymphoid tissue(BALT) 림프종은 드문 질환이다. 본 환자는 49세 여자 환자로 5년 전부터 안구 건조증 및 구강 건조증이 점차 진행하였으나 그냥 지내던 중 2년 전부터 운동시 호흡곤란 및 전신 무력감이 계속 진행하여 본원에 입원하였다. 흉부청진상 호흡음이 거칠게 들렸으며 양측 폐야 전체에 걸쳐 흡기시 악 설음이 관찰되었다. 형광항핵항체시험 (FANA)이 양성으로 "speckled pattern"을 보였으며 anti-SSA(Ro) Ab 및 anti-SSB(La) Ab는 강양성 소견을 보였다. 단순 흉부 방사선 사진 상 양측 폐에서 미만성의 망상 결절성 음영 증가가 관찰되었으며 고해상도 흉부 컴퓨터 단층촬영상에서 양측 폐에 걸쳐 미만성 분포를 보이는 모자이크 양상의 비균질성 감쇠 (mosaic pattern of inhomogeneous attenuation) 소견이 관찰되었다. 경기관지 폐 조직 생검상 림프구의 침윤이 관찰되었고 개흉 폐 생검술을 이용하여 얻은 폐조직의 병리 소견에서 'low grade marginal zone B cell lymphoma of BALT(bronchus associated lymphoid tissue) type'에 합당한 소견을 얻어 원발성 쇼그렌 증후군 (Primary Sj$\ddot{o}$gren's syndrome) 환자에서 폐에 발생한 Bronchus-associated lymphaid tissue(BALT) 림프종으로 확진되었다.
본 연구는 대한소아치과학회 산하 진정법 교육연구위원회가 진정법에 대한 가이드라인의 제정과 향후 정기적인 진정법교육 프로그램을 만들기 위해 시행하였다. 진정법을 사용하는 소아치과 회원 215명에게 2014년 2월 메일로 설문을 보내어 한달간 응답한 111명의 자료를 수집하여 분석하였다. 진정법은 어린이의 행동조절을 위해 선택되어, 3~4세 어린이(78%)에게 주로 사용되었다. 치료시간은 1~2시간(74.8%)이 많으며, 전악 치료(76.6%)가 주로 이루어지고 있었다. Chloral hydrate(CH) 50~70 mg/kg, hydorxyzine(Hx) 1~2 mg/kg, midazolam 근주(Mida IM) 0.1~0.2 mg/kg가 선호하는 진정법약제의 용량이었다. 진정법 시 선호하는 진정법 약제의 조합은 CH+Hx+$N_2O/O_2$(67.6%), CH+Hx+Mida 점막하투여(SM)+$N_2O/O_2$(29.7%), Mida IM+$N_2O/O_2$(23.4%) 등으로, 추가투여는 48%에서 행해지며 주로 Midazolam을 사용하고 있었다. 진정법 시 부작용을 경험한 응답자는 87.5%로, 주로 오심과 구토, 과흥분, 호흡저하 및 호흡곤란, 비틀거림 등이며, 약 20%만 정기적인 응급처치 교육을 받고 있었다. 진정법 후 환자 퇴원기준에 대해 응답자들은 명확한 퇴원기준의 부족과 추천하는 퇴원기준을 따르고 있지 않았다. 응답자의 86%가 진정법 코스에 대해 흥미를 가지고 있었으며, 진정법 코스에서 배우고 싶은 내용은 주로 응급 시 대처법과 안전한 약물용량 등 이었다. 소아치과에서의 진정법 사용은 환자안전을 최우선으로 고려하여야 하며, 의료사고예방을 위한 evidence-based 진료가 필요하다. 따라서, 대한소아치과학회는 진정법 가이드라인 및 응급처치에 대한 술기교육의 체계를 갖추어 소아치과의사들에게 제공할 필요가 있다.

제 5 장 계약 해지 및 이용 제한

② 상기 이용제한 규정에 따라 서비스를 이용하는 회원에게 서비스 이용에 대하여 별도 공지 없이 서비스 이용의
일시정지, 이용계약 해지 할 수 있습니다.
제 17 조 (전자우편주소 수집 금지)
회원은 전자우편주소 추출기 등을 이용하여 전자우편주소를 수집 또는 제3자에게 제공할 수 없습니다.
제 6 장 손해배상 및 기타사항
제 18 조 (손해배상)
당 사이트는 무료로 제공되는 서비스와 관련하여 회원에게 어떠한 손해가 발생하더라도 당 사이트가 고의 또는 과실로 인한 손해발생을 제외하고는 이에 대하여 책임을 부담하지 아니합니다.
제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.
[부 칙]
1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
