Hiccups, also known as singultus, are spasmodic involuntary contractions of respiratory muscles that shorten respiration. The characteristic sound is caused by rapid closure of the glottis. In oriental medicine, the mechanism of hiccups is "reversed flow of Ki". Hiccups have been associated with neoplasm, infections, seizures, diabetes. renal failure, alcohol ingestion, various drugs, and ischemic events of the myocardium or central nervous system. We present a case of chronic hiccups after cerebral infarction that was successfully treated using moxibustion therapy.
The anomalous pulmonary venous return of the entire left lung was an extremely rare congenital anomaly. The reported surgical experience with correction of this disorder was limited. The 3-year-old female patient underwent an operation upon the unilateral total anomalous pulmonary venous return from the left lung, in which the left superior pulmonary vein drained into innominate vein and the left inferior pulmonary vein into the coronary sinus, in Yeungnam University Hospital. The symptoms were nonspecific except frequent upper respiratory infection. Cyanosis was not seen. On auscultatory findings, a grade 2/6 systolic ejection murmur was audible over left second intercostal space of left sternal border and second heart sound had an increased pulmonary component which was widely splitted. The electrocardiogram demonstrated a right ventricular hypertrophy and right axis deviation and chest X-ray showed slightly increased pulmonary vascularity and bulged pulmonary conus. The echocardiogram demonstrated increased right atrial, ventricular, and pulmonary arterial dimension, and also secundum atrial septal defect and enlarged coronary sinus. The cardiac catheterization confirmed the left-to-right with a Qp/Qs of 2.0: 1 and oxygen step-up was seen in pulmonary artery, right ventricle, right atrium, and left innominate vein, and the catheter was not been introduced into the left pulmonary vein. A median sternotomy incision was done. Left superior pulmonary vein was drained to the innominate vein through anomalous vertical vein and the left inferior pulmonary vein drained to right atrium through the coronary sinus. The diversion of the left inferior pulmonary vein to posterior wall of left atrium was done after division in the proximity of coronary sinus. The anomalous vertical vein was diverted to base of left atrial auricle and then a atrial septal defect was sutured directly. The postoperative course was uneventful and she was discharged on the eleventh postoperative day. In the postoperative follow-up-2 months, she has been well without specific problems.
The purpose of this study was to confirm the relevance of dental health capacity and pulmonary function status. The subjects of this study were adults aged 40 years and older, who participated in pulmonary function and oral examinations. The data used for this study were raw data from the sixth Korea National Health and Nutrition Examination Survey (2013~2015). Statistical analysis was performed by analysis of complex sample survey data. The results were as follows: First, restrictive and obstructive patterns were more prevalent among males than females. They were even worse in older subjects. Approximately 67% of smokers had obstructive pattern. Second, there was a relevance between dental health capacity and pulmonary function status. The functioning teeth index, number of present teeth, and number of sound teeth (ST), which are indicators of dental health, declined with decreased pulmonary function. The number of missing teeth (MT) increased as the pulmonary function deteriorated (p<0.05). Third, the relevance between dental health capacity and pulmonary function status after stratifying the life-long smoking experience was statistically significant in the tissue health (T-health) index and ST among non-smokers. The relevance between dental health capacity and pulmonary function status after stratifying the life-long smoking experience was statistically significant (p<0.05) for the T-health index, ST, and MT among smokers. However, there was no relevance between dental health capacity and pulmonary function status after stratifying the life-long smoking experience and adjusting confounding variables. The subjects were composed of 308 children who visited the department of pediatrics, ${\bigcirc}{\bigcirc}$ korean medicine hospital from January 2010 to May 2013. Results 1. Among 308 patients, there were 188 boys and 120 girls; their ratio was 1.57:1. The age distribution showed that 38.3% were in the age less than 1 years, 38.0% in 1 years, 7.8% in 2 years, 5.2% in 3 years, 3.3% in 4~5 years, 4.5% in 6~10 years, 2.9% in 19~21 years. 2. It was found that 36.7% of the sleep disorder was caused without motivation, 17.2% caused by negligent accidents, 13.9% by traffic accidents, 10.4% by the unfamiliar environment, 8.1% by separation from parents, 7.5% after suffering disease, and 6.2% by irritating sound. 3. The sleep onset insomnia accounts for 17.2% of sleep disorder, sleep maintenance insomnia for 67.5%, poor sleep quality 24.4%, and daytime sleep disorder takes 19.8%. The ratio of sleep onset insomnia in adolescence comprises larger proportion (44.4%) than it of any other age groups. 4. The symptoms complicated with sleep disorder are the respiratory infection which takes 25.3%, being easily startled 18.2%, anorexia 14.6%, soft stools frequency 13.0%, greenish stools 10.7%, and skin rash 10.7%. Conclusions The causes of sleep disorders, changes of sleeping patterns, and complicated diseases show diversity in children and adolescence. Further study of sleep disorders in children and adolescence should be progressed as well.
Background and Objectives : The most important two factors of the voice production are the respiratory function which is the power source of voice and the glottic closure that transform the air flow into sound signals. The purpose of this study was to investigate the differences between trained singers and untrained controls under varying intensity at a constant vocal pitch by simulataneous using the airway interruption method and electroglottography(EGG). Materials and Methods : Under two different intensity condition at a constant vocal pitch(/G/), 20(Male 10, Female 10) trained singers were studied. Mean flow rate(MFR), subglottic pressure(Psub) and intensity were measured with aerodynamic test using the Phonatory function analyzer. Closed quotients(CQ), jitter and shimmer were also investigated by electroglottography using Lx speech studio. These data were compared with that of normal controls. Results : MFR and Psub were increased on high intensity condition in all subject groups but there was no statistically significance. Statistically significant increasing of CQ. were observed in male trained singers on high intensity condition (untrained male : 51.31${\pm}$3.70%, trained male :55.52${\pm}$6.07%, p=.039). Shimmer percent, one of the phonatory stability parameters, was also decreased statistically in all subject groups(p<.001). Conclusion : The trained singers' phonation was more efficient than untrained singers. The result means that the trained singers can increase the loudness with little changing of mean flow rate, subglottic pressure but more increasing of glottic closed quotients.
Six volunteers (mean $age=25.7{\pm}1.7$, $height=173{\pm}1.9$ and $weight=63.4{\pm}2.3{\;}kg$) participated in a graded exercise test and one hour of basic form of ChunDoSunBup (CDSB) Qi-training to investigate the cardiorespiratory responses and exercise intensity of Qi-training, a Korean traditional psychosomatic training. In the maximal exercise, the trainee showed $96.2{\pm}8.89{\;}l/min$ in ventilation (VE), $46.0{\pm}4.4$ in breath frequency (BF), $1.31{\pm}0.05$ in respiratory exchange ratio (RER), $180.7{\pm}3.0$ in heart rate (HR), and $2.6{\pm}1.1{\;}l/min$ or $40.7{\pm}2.3{\;}ml/kg/ml$ in oxygen consumption $(VO_2)$. Qi-training induced significant changes in BF, RER, HR, and $VO_2$. The exercise intensity of Qi-training were 42.3%, 46.9% and 38.7% of $HR_{max}$ during the sound exercise, slow motion (haeng-gong) and meditation respectively and the average was 46.2% of $HR_{max}$. We conclude that Qi-training is an aerobic exercise of a light (mild) intensity exercise, and it leads to decrease the metabolic rate in the trainee by breathing efficiently and relaxing them. In addition, Qi-training may affect cardiorespiratory function of BF, RER, HR and $VO_{2max}$ in trainees.
In this paper, we proposed a non-contact respiration measurement system with ultrasonic proximity sensor. Ultrasonic proximity sensor approach of respiration measurement which respiration signatures and rates can be derived in real-time for long-term monitoring is presented. 240 kHz ultrasonic sensor has been applied for the proposed measurement system. The time of flight of sound wave between the transmitted signal and received signal have been used for a respiration measurement from abdominal area. Respiration rates measured with the ultrasonic proximity sensor were compared with those measured with standard techniques on 5 human subjects. Accurate measurement of respiration rate is shown from the 50 cm measurement distance. The data from the method comparison study is used to confirm the performance of the proposed measurement system. The current version of respiratory rate detection system using ultrasonic can successfully measure respiration rate. The proposed measurement method could be used for monitoring unconscious persons from a relatively close range, avoiding the need to apply electrodes or other sensors in the correct position and to wire the subject to the monitor. Monitoring respiration using ultrasonic sensor offers a promising possibility of non-contact measurement of respiration rates. Especially, this technology offers a potentially inexpensive implementation that could extend applications to consumer home-healthcare and mobile-healthcare products. Further advances in the sensor design, system design and signal processing can increase the range of the measurement and quality of the rate-finding for broadening the potential application areas of this technology.
우리의 일상생활에서 양질의 수면은 행복 지수와 밀접한 관계가 있다. 사람들은 수면 장애를 만성 질환으로 인식하든 아니든 많은 어려움을 호소하고 있으며 일상생활에서 수면 중에 호흡 곤란을 경험하는 경우가 종종 발생한다. 수면 중에 호흡 관련 장애를 자동으로 인식하는 것은 매우 중요하나 현실적으로 매우 어렵다. 본 논문은 이러한 문제를 해결하기 위해 가정에서 건강관리를 위해 모바일 기반의 비접촉 수면 모니터링을 제안한다. 수면 중 호흡 신호는 스마트 폰의 소리 센서를 이용하여 호흡 신호를 수집하고, 신호의 특징을 추출, 호흡의 주파수, 진폭, 호흡의 주기, 호흡의 패턴을 분석한다. 모바일 건강이 모든 문제를 해결하지는 못하나 개인의 건강 상태의 조기 발견과 지속적인 관리를 목적으로 하고, 일반 가정의 침실에서 스마트 폰으로 추가 센서 없이 수면 중에 호흡과 같은 생리학적 데이터 모니터링의 가능성을 보여준다.
Purpose: This study was to develop a factor-type patient classification system for general nursing unit based on nursing needs (KPCS; Korean patient classification system for nurses). Method: We reviewed workload management system for nurses(WMSN) of Walter Reed Medical Center, Korean patient classification system for ICU, and nursing activities in nursing records and developed the first version of KPCS. The final version KPCS was evaluated via validity and reliability verifications based on panel discussions and data from 800 patient classifications. Content validity was performed by Delphi method and concurrent validity was verified by the correlation of two tools (r=.71). Construct validity was also tested by medical department (p<.001), patient type (p<.001), and nurse intuition (p<.001). These verifications were performed from April to October, 2008. Results: The KPCS has 75 items in classifying 50 nursing activities, and categorized into 12 different nursing area (measuring vital sign, monitoring, respiratory treatment, hygiene, diet, excretion, movement, examination, medication, treatment, special treatment, and education/emotional support). Conclusion: The findings of the study showed sound reliability and validity of KPCS based on nursing needs. Further study is mandated to refine the system and to develop index score to estimate the necessary number of nurses for adequate care.
