치조제가 심하게 흡수된 하악 무치악환자의 나사-시멘트 유지형 임플란트 보철 수복증례 (Screw and cement retained implant prosthesis rehabilitation of mandibular edentulous patients with severely absorbed ridge)

  • Purpose: Prosthodontics for edentulous patients is a treatment technique using implant, which has impactful results in retention and support effects. Methods: As a retention technique, SCRP (screw and cement retained implant prosthesis) has reported in many studies as a beneficial method for both patients and curers, which can reduce errors in process of making abutment and top implant. Results: Prosthesis manufacturing, as polymerization method of hardened resin teeth with thermoplastic resin, is helpful for patients with aesthetic and financial situations regarding residual ridge and interocclusal relationship, also indicates reliable results in both retention and care. Conclusion: Using SCRP technique, we notably obtained a clinical and aesthetic outcome from five implants in anterior tooth, which are half fixable and detachable implants on screw of implant abutment by the technicians at anytime.

Effect of cement washout on loosening of abutment screws and vice versa in screw- and cement- retained implant-supported dental prosthesis

  • PURPOSE. The purpose of this study was to examine the abutment screw stability of screw- and cement-retained implant-supported dental prosthesis (SCP) after simulated cement washout as well as the stability of SCP cements after complete loosening of abutment screws. MATERIALS AND METHODS. Thirty-six titanium CAD/CAM-made implant prostheses were fabricated on two implants placed in the resin models. Each prosthesis is a two-unit SCP: one screw-retained and the other cemented. After evaluating the passive fit of each prosthesis, all implant prostheses were randomly divided into 3 groups: screwed and cemented SCP (Control), screwed and non-cemented SCP (Group 1), unscrewed and cemented SCP (Group 2). Each prosthesis in Control and Group 1 was screwed and/or cemented, and the preloading reverse torque value (RTV) was evaluated. SCP in Group 2 was screwed and cemented, and then unscrewed (RTV=0) after the cement was set. After cyclic loading was applied, the postloading RTV was measured. RTV loss and decementation ratios were calculated for statistical analysis. RESULTS. There was no significant difference in RTV loss ratio between Control and Group 1 (P=.16). No decemented prosthesis was found among Control and Group 2. CONCLUSION. Within the limits of this in vitro study, the stabilities of SCP abutment screws and cement were not significantly changed after simulated cement washout or screw loosening.

A novel retentive type of dental implant prosthesis: marginal fitness of the cementless double crown type implant prosthesis evaluated by bacterial penetration and viability

  • PURPOSE. This study aims to compare the marginal fitness of two types of implant-supported fixed dental prosthesis, i.e., cementless fixation (CL.F) system and cement-retained type. MATERIALS AND METHODS. In each group, ten specimens were assessed. Each specimen comprised implant lab analog, titanium abutment fabricated with a 2-degree tapered axial wall, and zirconia crown. The crown of the CL.F system was retained by frictional force between abutment and relined composite resin. In the cement-retained type, zinc oxide eugenol cement was used to set crown and abutment. All specimens were sterilized with ethylene oxide, immersed in Prevotella intermedia culture in a 50 mL tube, and incubated with rotation. After 48 h, the specimens were washed thoroughly before separating the crown and abutment. The bacteria that penetrated into the crown-abutment interface were collected by washing with 500 µL of sterile saline. The bacterial cell number was quantified using the agar plate count technique. The BacTiter-Glo Microbial Cell Viability Assay Kit was used to measure bacterial adenosine triphosphate (ATP)-bioluminescence, which reflects the bacterial viability. The t-test was performed, and the significance level was set at 5%. RESULTS. The number of penetrating bacterial cells assessed by colony-forming units was approximately 33% lower in the CL.F system than in the cement-retained type (P<.05). ATP-bioluminescence was approximately 41% lower in the CL.F system than in the cement-retained type (P<.05). CONCLUSION. The CL.F system is more resistant to bacterial penetration into the abutment-crown interface than the cement-retained type, thereby indicating a precise marginal fit.

Comparative study on stress distribution around internal tapered connection implants according to fit of cement- and screw-retained prostheses

  • PURPOSE. The aim of this study was to compare the passivity of implant superstructures by assessing the strain development around the internal tapered connection implants with strain gauges. MATERIALS AND METHODS. A polyurethane resin block in which two implants were embedded served as a measurement model. Two groups of implant restorations utilized cement-retained design and internal surface of the first group was adjusted until premature contact between the restoration and the abutment completely disappeared. In the second group, only nodules detectable to the naked eye were removed. The third group employed screw-retained design and specimens were generated by computer-aided design/computer-aided manufacturing system (n=10). Four strain gauges were fixed on the measurement model mesially and distally to the implants. The strains developed in each strain gauge were recorded during fixation of specimens. To compare the difference among groups, repeated measures 2-factor analysis was performed at a level of significance of ${\alpha}$=.05. RESULTS. The absolute strain values were measured to analyze the magnitude of strain. The mean absolute strain value ranged from 29.53 to 412.94 ${\mu}m/m$ at the different strain gauge locations. According to the result of overall comparison, the cement-retained prosthesis groups exhibited significant difference. No significant difference was detected between milled screw-retained prostheses group and cement-retained prosthesis groups. CONCLUSION. Within the limitations of the study, it was concluded that the cement-retained designs do not always exhibit lower levels of stress than screw-retained designs. The internal adjustment of a cement-retained implant restoration is essential to achieve passive fit.

주조체의 설계 변화에 따른 수지접착형 보철물의 접착강도에 관한 연구 (A STUDY ON THE BOND STRENGTH OF RESIN-RETAINED PROTHESIS WITH VARIOUS CAST RETAINER DESIGNS)

  • The purpose of this study was to evaluate the effect of some resistance form designs on the bond strength of resin-retained prosthesis. Six sub-groups are designed in natural teeth group and resin teeth group . The framework designs in natural teeth group: 1) no groove preparation 2) groove at the center of distal surface 3) groove at the distobuccal line angle 4) 45 degree lateral load with no groove 5) 45 degree lateral load with center groove 6) splint two teeth with no groove. The framework designs in resin teeth group: 1) no groove preparation 2) groove at the center of distal surface 3) groove at the distobuccal line angle 4) metal covered the 1/2 of distal surface 5) metal covered the 1/2 of mesial surface 6) metal extended over the 114 of buccal surface. Specimens were treated electrolytic etching by Oxy-Etch and cemented with Panavia EX. Failure load was measured by Instron. Another 30 specimens were carried out fatigue tests by MTS 810 fatigue testing machine for 5000 cycles at different load level. The following results were obtained from this study. 1. The failure load was significantly increased by resistance forms. 2. The failure load was not increased by increase of total surface area bonded with teeth. The distal surface area played an important role in failure load. 3. In 45 degree lateral load group, the failure load was decreased significantly than that of in vertical load group. 4. Bond failure modes between static test and fatigue test exhibited no differences.

Effects of cementless fixation of implant prosthesis: A finite element study

  • PURPOSE. A novel retentive type of implant prosthesis that does not require the use of cement or screw holes has been introduced; however, there are few reports examining the biomechanical aspects of this novel implant. This study aimed to evaluate the biomechanical features of cementless fixation (CLF) implant prostheses. MATERIALS AND METHODS. The test groups of three variations of CLF implant prostheses and a control group of conventional cement-retained (CR) prosthesis were designed three-dimensionally for finite element analysis. The test groups were divided according to the abutment shape and the relining strategy on the inner surface of the implant crown as follows; resin-air hole-full (RAF), resin-air hole (RA), and resin-no air hole (RNA). The von Mises stress and principal stress were used to evaluate the stress values and distributions of the implant components. Contact open values were calculated to analyze the gap formation of the contact surfaces at the abutment-resin and abutment-implant interfaces. The micro-strain values were evaluated for the surrounding bone. RESULTS. Values reflecting the maximum stress on the abutment were as follows (in MPa): RAF, 25.6; RA, 23.4; RNA, 20.0; and CR, 15.8. The value of gap formation was measured from 0.88 to 1.19 ㎛ at the abutment-resin interface and 24.4 to 24.7 ㎛ at the abutment-implant interface. The strain distribution was similar in all cases. CONCLUSION. CLF had no disadvantages in terms of the biomechanical features compared with conventional CR implant prosthesis and could be successfully applied for implant prosthesis.

Effect of repetitive firing on passive fit of metal substructure produced by the laser sintering in implant-supported fixed prosthesis

  • PURPOSE. The aim of the present study was to investigate the passive fit of metal substructure after repetitive firing processes in implant-supposed prosthesis. MATERIALS AND METHODS. Five implants (4 mm diameter and 10 mm length) were placed into the resin-based mandibular model and 1-piece of screw-retained metal substructure was produced with the direct metal laser sintering (DMSL) method using Co-Cr compound (n = 10). The distance between the marked points on the multiunit supports and the marginal end of the substructure was measured using a scanning electron microscope (SEM) at each stage (metal, opaque, dentin, and glaze). 15 measurements were taken from each prosthesis, and 150 measurements from 10 samples were obtained. In total, 600 measurements were carried out at 4 stages. One-way ANOVA test was used for statistical evaluation of the data. RESULTS. When the obtained marginal range values were examined, differences between groups were found to be statistically significant (P<.001). The lowest values were found in the metal stage (172.4 ± 76.5 ㎛) and the highest values (238.03 ± 118.92 ㎛) were determined after glaze application. When the interval values for groups are compared with pairs, the differences between metal with dentin, metal with glaze, opaque with dentin, opaque with glaze, and dentin with glaze were found to be significant (P<.05), whereas the difference between opaque with metal was found to be insignificant (P=.992). CONCLUSION. Passive fit of 1-piece designed implant-retained fixed prosthesis that is supported by multiple implants is negatively affected by repetitive firing processes.

하악 4전치 상실시 치료 계획 (Treatment plan for missing mandibular 4 incisors)

  • 상실한 하악 4전치에서의 치료 계획은 어느 다른 부위보다 수술의 성공률이나 보철치료시 유리한 환경을 지니고 있다고 흔히 생각되는 부위이다. 하지만 하악 전치들의 M-D 사이즈나 crowding으로 인한 공간적 제한, 임시치아 문제, 치조골 흡수로 인해 생기는 심미적인 문제 등 의외로 까다로운 경우가 종종 발생하는 부위기도 한다. 본문에서는 하악 4전치 상실 시의 치료 계확에 대해서 케이스를 통해 알아 보겠다. 하악 4전치 상실시 가능한 옵션들: 하나 혹은 4전치중 일부 상실의 경우나 잔존 치조골의 흡수가 거의 없어 임플란트 위치 관계가 심미적으로 중요하다고 판단되는 경우라면 직경이 작은 임플란트들을 원래 발치와 중앙에 식립(Narrow type 이나 One body mini implant type) 최종 보철물 형태가 Pink porcelain을 포함하는 이미 광범위한 치조골의 흡수가 일어난 경우 즉 임플란트 식립 위치가 덜 중요한 상황이라면 regular size의 임플란트들을 측절치와 중절치의 사이 Interseptal bone위치에 식립 4전치중 하나 혹은 일부가 상실됐고 남아 있는 전치들의 예후가 불량하다고 의심되나 환자가 당장 나머지 전치들의 발치에는 동의하지 않을 때 남아 있는 전치들이 발치 될 때까지의 Tentative restoration로서 Resin Bonded Bridge(Resin Retained Bridge/Resin Bonded Fixed Partial Denture)의 적용.

나사유지형 임플란트 고정성 보철물의 적합도와 캔틸레버가 지지골조직의 응력분산에 미치는 영향 (The Effects of Screw Retained Prosthesis Misfit & Cantilever on Stress Distribution in Bone Around the Implant)

  • 임플란트의 장기적인 성공을 위해 고정체의 형태, 외과적 술식, 골조직의 조건, 보철물 적합성, 주기적인 검사, 환자의 구강 위생 등에 많은 주의가 필요하다. 많은 연구에서 임플란트 지지 보철물의 적합도에 따른 임플란트의 예후에 관해 보고되었다. 보철물이 수동 적합되어야 임플란트의 상부구조및 하부구조에 해로운 응력을 야기하지 않는다고 보고되고 있으나 현재의 임플란트 보철물의 제작과정으로 진정한 수동 적합을 얻는 수 없다고 인정된다. 임상과정과 기공과정을 포함하여 임플란트 치료의 전 과정에서 오차가 발생하며, 이는 보철물을 변형을 야기하고 이는 임플란트 상부 보철물과 지대주 사이의 오차를 발생시킨다. 이러한 오차는 보철물 장착 후 보철물의 파절, 나사의 헐거움(screw loosening), 골소실, 골유착 실패와 같은 문제를 야기한다. 이런 오차에 의한 문제점은 cantilever의 존재, 과도한 교합력이 존재할 경우 더욱 증가된다고 보고되고 있다. 본 연구에서는 ITI 임플란트를 하악골의 견치후방의 무치악부에 3개를 식립하고 4-unit 캔틸레버 고정성 국소의치를 다양한 위치의 $100{\mu}m$ gap을 생성한 후 제작하고 gap을 생성하지 않은 고정성 국소의치와 30 lb의 하중하에서 광탄성 응력분석을 시행하여 응력분포 양상과상대적인 응력의 크기를 비교분석하였다.

보철물 조건에 따른 Periotest수치의 실험적 평가 (IN VITRO EVALUATION OF PERIOTEST VALUES UNDER VARIOUS CONDITIONS OF PROSTHESES)

  • Periotest(Siemens, Germany) has been used to test mobility of the implants clinically, however the effects of target materials and connection methods on the PTVs(Periotest Values) have not been evaluated. Periotest has been regarded as a reliable and objective tool to test implant and natural teeth mobility clinically, however this instrument showed different PTVs under various test conditions. This in vitro study was designed to compare PTVs of different veneering materials and prosthodontic designs (single and bridge restorations). To compare the effects of veneering materials on PTVs, 1 mm thickness of five different testing materials (porcelain, type III gold alloy, pure titanium, composite resin, acrylic resin) were placed on the resin block. Three full length of 13 mm Mark II implant fixtures were embedded into autopolymerizing resin block to fabricate single and bridge restorations. To evaluate effects of the connection method in single restorations, PTVs of screw retained(UCLA type) and cementation type(Cera-One system) were compared. Finally, to test reliability of PTVs of the final restorations, screw retained three unit short span PFM bridges were fabricated on the standard and Estheti-Cone abutments. All testing components were tightened with torque controller and PTVs of all specimens were measured 15 times for statistical analysis with SAS program. Following conclusions were made within the limit of this in vitro study. 1. PTVs of type III gold alloy, grade II titanium, composite resin veneering materials showed no significant differences, however acrylic resin and porcelain showed significant differences (P<0.05). 2. Single tooth restorations showed consistent PTVs as long as proper torque force was applied. 3. PTVs of bridge type prostheses was inconsistent regardless of abutment types. 4. PTVs of the prostheses showed higher scores and standard deviations than those of abutments regardless types of connection (P<0.05).
