근거기반 정맥주입요법 간호실무지침 실무적용 프로그램 개발 및 평가 - 중소병원을 대상으로 (Development and Effectiveness of Practice Application Program of Intravenous Infusion Evidence-Based Nursing Practice Guideline - for Small and Medium Sized Hospitals)

  • Purpose: This study was conducted to develop and test the effects of a program for practice application of intravenous infusion evidence based nursing practice (EBP) guidelines in small and medium-sized hospitals. Methods: A mixed method research design was used, combining non-equivalent control group pre-post test design with qualitative study analysis. The subjects consisted of 55 nurses. The practice application program was developed based on the Advancing Research and Clinical practice through close Collaboration (ARCC) model. Data were collected for analysis in the following areas: nurses' EBP organizational culture and readiness, EBP beliefs, EBP implementation, importance about intravenous infusion, and performance about intravenous infusion, with data assessed using valid and reliable instruments. Patient outcomes were collected from the hospital's medical records. Data were analyzed using t-test, χ2-test, and Shapiro-Wilk test, with qualitative content analysis used for interview data. Results: Following the intervention, nurses' EBP organizational culture and readiness, EBP beliefs, EBP implementation, and performance of intravenous infusion and perceptions of its importance showed significant improvement in the experimental group. Phlebitis rates decreased in the experimental group compared to the control group. Conclusion: This program is effective to improve nurse's perception and practice of evidence based nursing. Therefore we recommend to use this program at same levels of hospitals.

간호대학생의 신생아 시뮬레이션 실습교육 연구동향(2011년~2023년) (Research Trends in Neonatal Simulation Practice Education of Nursing Students)

  • 본 연구는 국내 간호대학생의 신생아 시뮬레이션 실습교육 관련 연구동향을 살펴보기 위한 서술적 조사연구이다. 문헌수집은 국내 6개 전자 데이터베이스에서 ('신생아 시뮬레이션')과 ('간호학생' OR '간호대학생' OR '학생간호사')로 조합하여 검색한 결과 총 17편을 선정하였다. 연구결과는 2011년~2015년 7편으로 시작하여 2016년~2020년 5편과 2021년~2023년 5편으로 다소 감소하는 것으로 나타났다. 연구설계는 양적연구 총 16편(94%)으로 중재연구 15편(88%), 서술적 조사연구 1편(6%)이고, 혼합연구는 1편(6%)이었다. 시뮬레이션 주요 사례 주제는 고위험 신생아 간호 9편(52%), 호흡곤란증후군 중환아 간호 3편(18%), 신생아 간호 3편(18%) 정상 신생아 간호 1편(6%), 신생아 응급 기도 간호 1편(6%)으로 나타났다. 주요 결과 변수 및 적용효과는 임상수행능력 5편(19.2%)이 가장 많았고 그 다음 순으로 실습만족도 3편(11.5%)이었고 적용 효과가 있는 것으로 나타났다. 결론적으로 간호대학생의 신생아 시뮬레이션 실습교육의 확대, 반복연구, 질적연구 등 다양한 연구방법이 요구된다.

상상연습이 편마비 환자의 근력증가에 미치는 영향 (Effects of Mental Practice on the Muscle Strengthening of the Hemiplegic Patients)

  • The purpose of this study was to determine the effect of mental practice on biceps brachii muscle strengthening in 3 poststroke hemiplegia patients along with multiple baseline design across subjects. The mental practice adopted for this experiment involved imagery training to practice elbow flexion, which last 12 to 16 sessions with 30 minutes each session. Maximal muscle activities was measured pre- and post-mental practice to evaluate the strength of biceps brachii. The strength of biceps brachii was measured by surface-EMG. The results were: In the subject 1, 73.92 mV (pre-) and 127.56 mV (post-); in the subject 2, 147.60 mV (pre-) and 202.85 mV (post-); and in the subject 3, 20.75 mV (pre-) and 27.92 mV (post-). The results indicate that a simple mental practice is a useful method to strengthen biceps brachii muscle in hemiplegic patients.

블렌디드 러닝을 활용한 컴퓨터 실습수업에서의 학습자 만족 연구: 콘텐츠 요인과 교수자 상호작용을 중심으로 (A Study on the Learner's Satisfaction of Computer Practice Classes by applying BL: Focusing on contents and instructor interactions)

  • BL(Blended Learning) has been presented as a promising alternative learning approach. BL is defined as a learning approach that combines e-learning and face-to-face classroom learning. The adoption of BL in computer practice class is necessary due to the characteristics of computer practice class itself. This study proposes a research model that examines the determinants of learner's satisfaction of computer practice classes in BL environment. Considering the characteristics of computer practices classes contents and instructor interaction were identified as the determinants. The research model is tested using a questionnaire survey of 141 participants. Confirmatory factor analysis (CFA) was performed to test the reliability and validity of the measurements. The partial least squares (PLS) method was used to validate the measurement and hypotheses. The empirical findings shows that contents easiness and contents constructs are the primary determinants of instructor interaction in BL. Instructor interaction was also found to be related to the learner's satisfaction resulting in re-using. The findings provide insight into the planning and utilizing BL in computer practice classes to enhance learner's satisfaction.

Developmental procedures for the clinical practice guidelines for conscious sedation in dentistry for the Korean Academy of Dental Sciences

  • Background: Evidence-based clinical practice guidelines (CPGs) are defined as "statements that are scientifically reviewed about evidence and systematically developed to assist in the doctors' and patients' decision making in certain clinical situations." This recommendation aims to promote good clinical practice for the provision of safe and effective practices of conscious sedation in dentistry. Method: The development of this clinical practice guideline was conducted by performing a systematic search of the literature for evidence-based CPGs. Existing guidelines, relevant systematic reviews, policy documents, legislation, or other recommendations were reviewed and appraised. To supplement this information, key questions were formulated by the Guideline Development Group and used as the basis for designing systematic literature search strategies to identify literature that may address these questions. Guideline documents were evaluated through a review of domestic and international databases for the development of a renewing of existing conscious sedation guidelines for dentistry. Clinical practice guidelines were critically appraised for their methodologies using Appraisal of guidelines for research and evaluation (AGREE) II. Results: A total of 12 existing CPGs were included and 13 recommendations were made in a range of general, adult, and pediatric areas. Conclusion: The clinical practice guidelines for conscious sedation will be reviewed in 5 years' time for further updates to reflect significant changes in the field.

코로나19 팬데믹 시기에 간호대학생의 웹 기반 시뮬레이션 실습을 포함한 임상 실습 경험 (Clinical Practice Experience including Web-based Simulation Practice of Nursing Students during the COVID-19 Pandemic)

  • 본 연구의 목적은 코로나 팬데믹 시기에 웹기반 시뮬레이션 실습을 포함한 임상 실습 경험의 의미를 파악하는 것이다. 웹기반 시뮬레이션 실습과 임상 실습 경험을 포커스그룹 인터뷰를 통하여 자료를 수집하고 내용분석 방법에 따라 분석하였다. 포커스 그룹에 의한 심충 면담내용을 분석한 결과, 2개의 구성요소와 7개의 주제 묶음, 18개의 주제로 나타났다. 첫 번째 구성요소로, 임상 실습은 4개의 주제 묶음으로 '팬데믹 상황에서의 불안한 실습의 출발', '다양한 사례에 의한 직접 경험', 미래 간호사로서 준비하는 훈련 기회', ' 수행의 부담감 및 제한적 경험'이었다. 두 번째 구성요소인 웹기반 시뮬레이션 실습은 3개의 주제 묶음으로 '미완성의 간호실습', '가상공간에서의 임상 간호에 대한 간접경험', '통합 실습 모델의 요구'로 분류되었다. 간호교육과정에서 임상 실습은 매우 중요한 부분이나 학생들이 실제로 현장에서 수행할 수 있는 간호는 매우 제한적이다. 따라서 관찰 위주의 임상 실습의 단점을 보완하고, 실습 교육의 질적 수준을 높이기 위하여, 웹기반 시뮬레이션 실습을 포함한 혼합 실습 모델 등을 고려할 필요가 있다.

한의 임상 연구에 대한 국내외 제언 고찰 - 미래 임상 연구 전략 II - (Study on the Proposals for Clinical Research in Korean Medicine Worldwide - Future Clinical Research Strategy II -)

  • The purpose of this paper is to explore the strategy of future Korean medicine(KM) clinical research through the study on the proposals for KM clinical research worldwide. In this study, the papers published in English through Pubmed were investigated mainly. Among them, we analyzed the methodological proposals from the clinical research papers that were published in the KM related fields. Various proposals for improving the problems in KM clinical studies are as follows. First, KM clinical research should be designed based on understanding for the theory, backgrounds, paradigms and worldviews of KM. In addition, considering the model validity, KM clinical research model should include the diagnosis, interventions and outcomes measurement methods reflecting the characteristics and real practice in KM. The internal validity and external validity should be also taken into account. One of the most important thing is to identify the contents about various and complex 'real practice' in KM. A prospective observational study was suitable for the purpose of this study. Finally, we suggested a few improvement directions for RCTs studies in KM. First, we would be able to improve the quality and the internal validity in KM clinical research using the checklists of CONSORT(Consolidated Standards for Reporting Trials) Statement and STRICTA(Standards for Reporting Interventions in Clinical Trials of Acupuncture). Second, we could use various clinical research methods and the modified research of RCTs such as PCT(pragmatic clinical trial) to reflect the characteristics of actual KM practice. Consequently, we would be able to improve the external validity. Third, the KM diagnosis and outcomes measurement methods should be developed based on an actual KM practice and it should reflect a real practice. The 'pattern identification(辨證)' is the core to KM diagnosis. But in order to be applied to the clinical research, the pattern identification(辨證) should be objectified and standardized. Future KM clinical research model should reflect the characteristics and a real practice in KM. In addition, it should include the advantage of rigorous RCTs research.Specially, the diagnosis, interventions and outcomes measurement methods in KM clinical research should reflect this view.

30, 40대 여성의 유방자가검진 수행에 영향을 미치는 요인 (Factors Influencing the Practice of Breast Self-Examination Among Women in Their 30s and 40s)

  • Purpose: The purpose of this study was to investigate the level of knowledge, health belief, and self-efficacy affecting on breast self-examination (BSE), and to identify factors influencing the practice of BSE of women in their 30's and 40's. Methods: With a correlation survey design, 194 women in their 30's and 40's were recruited at 4 culture centers in Busan via convenience sampling. Measures were included with knowledge, health belief, and self-efficacy for breast self-examination and level of BSE practice. Results: Level of knowledge, health belief and self-efficacy for BSE were greater than medium level. BSE practice had positive correlations with subscales of health belief, except barrier and self-efficacy, but no relationship with knowledge. Self-efficacy, sensitivity, and benefit among the entered variables were significant factors influencing the practice of BSE, and explanatory power of these variables was 32.8%. Conclusion: Based on this study, studies are required to compare through repeated research according to age, education, occupation, environmental characteristics. In addition, to maximize the educational effect, development of the educational program in conjunction with the local community to increase breast self-examination practice and research about the effectiveness are needed.

치위생과 학생의 전문직 자아개념과 임상실습 만족도와의 관계 연구 (A study on the professional self-concept of dental hygiene students and their satisfaction level with clinical practice)

  • The purpose of this study was to examine the professional self-concept of dental hygiene students, their satisfaction level with clinical practice and the correlation of the two factors. The subjects in this study were 129 dental hygiene seniors at a college located in the city of J. The selected students had been engaged in a 16-week clinical practice. The findings of the study were as follows: 1. The dental hygiene students investigated got a mean of 3.34 out of possible five points on professional self-concept. As for the subfactors of professional self-concept, they got 3.75, 3.38 and 2.87 respectively on communication, professional work handling and satisfaction level. 2. They got a mean of 3.29 out of possible five points on satisfaction level with clinical practice. As to the subfactors of the satisfaction level, they gave 3.60, 3.50, 3.46, 3.04 and 2.83 to environments for practice, the content of practice, supervision, the length of practice and evaluation respectively. 3. In regard to the relationship of general characteristics to professional self-concept, satisfaction level with the major and social perception of dental hygienist made significant differences to professional self-concept. 4. As for connections between the general characteristics and satisfaction level with clinical practice, satisfaction with clinical practice was significantly different according to satisfaction with the major, satisfaction with the amount of practice and satisfaction with social perception of dental hygienist. 5. There was a significantly positive correlation between professional self-concept and satisfaction level with clinical practice(r=.383, p=.001). In conclusion, how much dental hygienists who are trained to be a dental hygienist are gratified with clinical practice is one of integral factors to affect their development of professional self-concept. Therefore prolonged research efforts should be directed into determining in which way clinical practice should be implemented to develop the professional self-concept of would-be dental hygienists and how to raise their satisfaction with clinical practice.

플립러닝 적용 기본간호학실습 수업에 대한 실행연구 (An Action Research on Flipped Learning for Fundamental Nursing Practice Courses)

  • Purpose: This study was conducted to design and implement a fundamental nursing practice based on flipped learning and to examine the effects. Methods: Participants were 57 students who were taking the fundamental nursing practice course at D university in N city. The study included processes of instructional design, action/effects and reflection. Data were analyzed using paired t-test with the SPSS/WIN 23.0. Results: In the instructional design stage, the class consisted of 3 parts: outside class (pre-learning), inside class (assessment, collaborative practice, peer review, reflection), after-class (self-directed practice, feedback). In the action/effects stage, the flipped learning was applied for 15 weeks according to the instructional design and then the effects of flipped learning were evaluated. Students showed a significant improvement in self-directed learning ability (t=-3.56, p=.001) and critical thinking disposition after the class (t=-3.72, p<.001). Finally, in the reflection stage, the researchers examined whether the four pillars of flipped learning occurred. Conclusion: Findings indicate that flipped learning applied in fundamental nursing practice is effective in improving self-directed learning ability and critical thinking disposition. The action research method was a useful way to foster professor's educational competency as well as to verify effects of a new nursing education method.
