Clinical study of endocardial cushion defect: 37 cases report (심내막상 결손증에 대한 임상고)

  • Thirty-seven patients had undergone repair of a endocardial cushion defect between 1977 and Aug. 1983 in Seoul National University Hospital. Twenty eight had a partial defect, one intermediate defect and eight complete endocardial cushion defect. Tricuspid cleft was found in 4 cases and mitral cleft was in all p-ECD. Seven patients were of type C anatomy in c-ECD. Four patients had associated major anomalies, including three TOF in c-ECD, one coarctation in p- ECD. In p-ECD patients, the septal defect was closed with patch in all cases and the atrioventricular valvular insufficiency was corrected with MVR in 4 cases, TVR in 1 case and simple interrupted sutures in remainders. In c-ECD patients the septal defect was closed with single patch except one case. The atrioventricular valve was repaired with simple interrupted sutures except one MVR and TVR case. The operative mortality was 14.2% in p-ECD, 44.4% in c-ECD, but recent 3 years [1980-1983] mortality was 8.7% in p-ECD, 20% in c-ECD. More than grade III systolic regurgitant murmur was oted postoperatively in 4 cases of c-ECD and 3 cases of p-ECD. The operative risk factors were preoperative NYHA classification, cyanosis, Rp/Rs, systolic pressure of main pulmonary artery and the degree of regurgitation of atrioventricular valves. The causes of death were low cardiac output syndromes, pulmonary complications and arrhythmias.

Analysis of various composite patches effect on mechanical properties of notched Al-Mg plate

  • In this study, the effect of various adhesively bonded composite patches on mechanical properties of notched Al-Mg alloy plates was analyzed. For this purpose firstly, the un-notched and notched specimens were fabricated from 5086 Al-Mg alloys which have been used in armor-plated military vehicles. The surface notches as a flaw were machined with circular cutting tool to form notch aspect ratio a/c=0.15 and notch-to-thickness ratios a/t=0.5 in the radial direction on the test specimens. Then, various composite patches which reinforced by glass, carbon and Kevlar fibers were bonded adhesively at elliptically surface notches. Finally, experimental measurements conducted by applying tensile static loading. The experimental results showed that repairing with composite patches with order of carbon, glass and Kevlar fibers have remarkable effect on tensile strength of the notched plate. Also the finite element models were developed using Abaqus/Explicit code to predict the tensile strength and elongation of unrepaired notched specimen and specimen repaired by carbon fiber composite patch. The comparison between numerical and experimental results showed good agreement between them and proved the accuracy of numerical modeling.

  • This paper addresses the development of VP curing equipment for the substitution of autoclave which has several limits in application. This equipment adopt vacuuming and pressurizing processes similarly to the autoclave, but has flexible pressurizing and movable system. The experiments were pursued to determine and to compare specific properties of the specimens produced by three kinds of processes; autoclave, hotpress, and VP. The results show that the VP curing equipment can produce the useful composite material with a little inferior quality to the autoclave, but better than the hotpress. Considering the movability of VP curing equipment, it can be expanded its application to the areas of composite patch-repaired structure in large objects.

  • Perforated esophagus is a surgical emergency; it is the most serious and frequently the most rapidly lethal perforation of the gastrointestinal tract. Contamination of the mediastinum and often a pleural cavity with corrosive fluids, food material and bacteria leads to cardiorespiratory embarrassment, shock, major fluid losses and fulminating infection. Despite the improved diagnostic and treatment modalities, a perforation or leak from the esophagus remains a major source of morbidity and mortality. When esophageal perforation occurs, a successful outcome can be expected only when it is diagnosed soon after the event and repaired early. Esophageal perforations have a poor prognosis after delayed surgical treatment. With the use of the Thai onlay gastric patch method, a case of postemetic spontaneous perforation of the esophagus was successfully managed 6days after the event.

  • Ozone, an atmospheric pollutant, can damage similar UV and X-rays DNA and its components. It is possible then that the KNA damage produced by this gas are similar, to some extent, to those of radiations and that they could be repaired by the same DNA repair mechanisms. It has been observed in Escherichia coli that radiosensitive strains such as lex A, rec A and pol A, all deficient to some extent for DNA repair, are more sensitive to ozone than a wild type strain. We have thendetermined the ozone resistance and host-cell reactivation of ozone-damaged T3 phages for the E. coli double mutants pol A, lex A, uvr B, lex A, uvr A, rec A and rec A lox A. According to the results, the DNA polymerase 1 plays a key role in ozone resistance and Type 11 mechanism and/or shory patch excision repair are the most important for it. The interactions between the different DNA repair mechanisms are secondary. There is a strong correlation between ozone resistance and the capacity to reactivate T3 phages damaged by ozone.

Surgical experience in Criss-Cross heart: report of 4 cases (Criss-Cross heart 를 동반한 심장기형의 수술 치험 4례 보)

  • The criss-cross, a term first used by Anderson and Ando , is a rare cardiac malformation in which the systemic and pulmonary blood streams cross at the atrioventricular level, without mixing. We have surgically experienced four cases of crossed atrioventricular connection, three concordant and one discordant, at Seoul National University Hospital from July 1982 to March 1984. The atrial situs, the atrioventriculoarterial connection, the spatial position of the ventricles, and associated cardiac anomalies were analyzed. We have performed right Blalock-Taussig shunt for case 1, modified Fontan operation for case 2, patch closure of VSD for case 3, and septation of common ventricle with primary closure of ASD for case 4. The relatively good postoperative results in these patients were gratifying and suggest that cardiac anomalies associated with this rare malformation can be successfully repaired without further risk. To our knowledge, this is the first report of surgical experience for criss-cross heart in the Korean literature.

  • There are several methods of repairing in aortic periprosthetic leakage after aortic valve replacement. We present a case in with the aortic periprosthetic leakage with pseudoaneurysm was repaired with dacron patch.

Interrupted Aortic Arch(Type A) associated with PDA, VSD, Mitral Regurgitation and Single Coronary Artery (대동맥궁 결손증의 완전교정 치험 1예)

  • We experienced a case of interrupted aortic arch[Type A] associated with PDA, VSD, mitral regurgitation and single coronary artery. The patient was 7 years old boy, who showed congestive heart failure[NYHA functional class III]. One stage total correction was performed under profound hypothermia with total circulatory arrest. Aortic continuity was established using PDA with anterior wall of main pulmonary artery flap. VSD was closed with Dacron patch and mitral regurgitation repaired by Reed`s annuloplasty method. The postoperative cardiac catheterization revealed no pressure gradient between ascending aorta and descending aorta, decreased pulmonary artery pressure and trivial residual shunt[Qp/Qs: 1.28]. The aortogram showed good continuity of the aorta without narrowing of the anastomotic site. During the period of 1 year follow up, heart failure symptoms were nearly subsided.

  • Atrial septal defect is the most common of the congenital heart disease in the adult. Since the first description of atrial septal defect by Rokitansky in 1857, this anomaly has been studied by many workers in past one century. In 1953, Lewis had first corrected the atrial septal defect under direct vision with deep hypothermia, and Gibbon [1954] had first done the atrial septal defect under direct vision with extracorporeal circulation. In our college [May 2’ 1977], we have first repaired the A.S.D. under direct vision with artificial heart-lung machine and, the defect was 4x5cm in size which was closed by Dacron patch. This patient was 12 year old girl who is well now. [postoperative 13 days]

Traumatic ventricular septal defect - One case report - (외상성 심실중격결손증 1례 보)

  • We have experienced a rare case of traumatic ventricular septal defect caused by fall down injury. The patient suffered from congestive heart failure after trauma and represented new developing typical murmur. Cardiac catheterization revealed a large left-to-right shunt at the ventricular level and a 4.4:1 ratio of pulmonary to systemic flow. Left ventricular angiogram also showed ventricular septal defect low in the muscular septum. At operation, the right ventricle and pulmonary artery were found to be moderately dilated. A marked systolic thrill was palpable over the right ventricle near the apex. Through a small left ventriculotomy an oval defect, 2x.3 cm with a firm scarred margin, was found in the lower muscular ventricular septum. This defect was repaired using Dacron patch and ventriculotomy was closed with long strip of Teflon felt. Postoperative course was uneventful and discharged in good condition.
