Objective : The authors tried to reveal some unique features of lipomeningomyelocele (LMMC), including clinical presentation, factors precipitating onset of symptoms, pathologic entities of LMMC associated with tethered cord syndrome, and surgical outcome in LMMC patients. Methods : Seventy-five patients with LMMC were enrolled in this study. Neuro-imaging and intraoperative findings allowed classification of LMMC into three Types. The patients were divided into two groups by age : A (51 patients), from birth to 3 years, and B (24 patients), from 3 to 24 years. For prevention of retethering of the cord, a mega-dural sac rebuilding procedure was performed in 15 patients. Results : During a mean postoperative follow-up period of 4 years, the surgical outcome was satisfactory in terms of improved pain and motor weakness, but disappointing with reference to the resolution of bowel and bladder dysfunction. Among these 75 patients with LMMC, preoperative deficits were improved after surgery in 29 (39%), remained stable in 28 (37%), changed slightly in 13 (17%), and worsened in 5 (7%). Patients in group A achieved better outcomes than those in group B. Depending on the type of lesion, patients with types I and II LMMC have better outcomes than those with type III LMMC. Finally, retethering of the cord with neurological deterioration occurred in 4 (5.3%) of the 75 patients, but no retethering was found in the 15 patients who were recently treated with a mega-dural sac rebuilding procedure. Conclusion : Our data continue to support the opinion that early diagnosis and optimal surgery are still essential for the treatment of patients with LMMC, since there is a high likelihood of residual neurological functions that can be preserved. Based on our surgical experience of untethering and decompression of lipomas, a mega-dural sac repair is useful to prevent retethering of the cord.

In this paper, we present an audio stream delivery using the AMR (Adaptive Multi-Rate) coder that was adopted by ETSI and 3GPP as a standard vocoder for next generation IMT-2000 service in which includes combined sender (FEC) and receiver reconstruction technique in the Internet. By use of the media-specific FEC scheme, the possibility to recover lost packets can be much increased due to the addition of repair data to a main data stream, by which the contents of lost packets can be recovered. The AMR codec is based on the code-excited linear predictive (CELP) coding model. So we use a frame erasure concealment for CELP-based coders. The proposed scheme is evaluated with ITU-T G.729 (CS-ACELP) coder and AMR - 12.2 kbit/s through the SNR (Signal to Noise Ratio) and the MOS (Mean Opinion Score) test. The proposed scheme provides 1.1 higher in Mean Opinion Score value and 5.61 dB higher than AMR - 12.2 kbit/s in terms of SNR in 10% packet loss, and maintains the communicab1e quality speech at frame erasure rates lop to 20%.
The strengthening of concrete structures in situ with externally bonded carbon fiber is increasingly being used for repair and rehabilitation of existing structures. Because carbon fiber is attractive for this application due to its good tensile strength, resistances to corrosion, and low weight. Generally bond strength and behavior between concrete and carbon fiber mesh(CFM) is very important, because of enhancing bond of CFM. Therefore if bond strength is sufficient, it will be expect to enhance reinforcement effect. Unless sufficient, expect not to enhance reinforcement effect, because of occuring bond failure between concrete and CFM. In this study, the bond strength and load-displacement response of CFM to the concrete by the direct pull-out test(the tensile-shear test method) were investigated using the experiment and the finite element method analysis with ABAQUS. The key variables of the experiment are the location of clip, number of clips and thickness of cover mortar. The general results indicate that the clip anchorage technique for increasing bond strength with CFM appear to be effective to maintain the good post-failure behavior.
The repair technique using surface impregnation of reactive compound is so effective for deteriorated concrete structures that many researches are recently focused on these works. Particularly, inorganic impregnant is regarded as ecofriendly material because there is no air-pollution during manufacturing process as well as field coating works. Furthermore, The delamination between old concrete and impregnated surface does not occur, resulting from different material characteristics. In order to evaluate the durability performance of surface-impregnated concrete, durability evaluation through the long-term exposure tests is significant, however, experiments are usually limited to the temporary and qualitative laboratorial scope. In this study, durability characteristics for inorganic and organic/inorganic impregnated concrete specimens are evaluated through longterm chloride exposure test. The specimens with 21MPa and 34MPa strength have been prepared and exposed to chloride attack in the atmospheric, tidal, and submerged conditions. Evaluation for compressive strength, chloride penetration, and electrical potential (half cell potential) for steel corrosion are performed for the specimens exposed for 2 years. From the results, no distinct strength gaining is observed but the resistance to chloride penetration and steel corrosion is evaluated to be improved through surface impregnation. The more improved resistance to chloride attack is measured in the inorganic impregnated concrete and the results from atmospheric condition show more improved resistance to chloride attack than those from submerged and tidal condition.
The purpose of this study is to evaluate the phenomenon of attachment and spreading of the cultured rat calvarial cell inoculated on their surface of different kinds of biodegradable membrane which had been used on tissue regeneration on periodontal defects by using scanning electron microscope. In this experiment 30 Sprague-Dawley male rats (mean BW 150gm) were used to harvest abundant number of cell in the short period. The rats were sacrificed by decapitatioan to obtain the calvaria for bone cell culture. Calvarial cells were cultured with Dulbecco's Modified Essential Medium contained with 10% Fetal Bovine Serum under the conventional conditions. Biodegradable barrier membrane were collected with collagen type, and were divided into 3 different kind of surface such as scattered, polarized and fine-net type as their surface texture. Microcover plate which usually used for cell culture was used as control for smooth surface. All the membrane were seeded with cultured calvarial cell on their surface. The number of cell inoculated on the membrane were $1{\times}10^6$ Cells/ml. After the culture as designed time, all the membrane were washed with 0.1 M Phosphate Buffered saline and fuxed with 2.5% Glutaraldehyde. And all specimen were treated with $OsO_4$, and Tannic acid before drying the cell for coating the cell with gold. Scanning Electron Microscope was used to observation. The following results were obtained. I. During the whole period of experiment, the phenomenon of cell attachment and spreading were revealed similar pattern to compare with smooth surface culture plate and ordinary culture dish. 2. The shape of cell attachment and spreading on the surface of barrier membrane were observed no remarked difference pattern between smooth surface culture plate and ordinary culture dish. 3. The cytoplasmic process of cultured calvaria cell extent to the deep portion of barrier membrane like as their own proper shape. 4. There were no remarkable relationships between the degree of cultured cell spreading and surface structure of barrier membrane. 5. Slight starified layer of cultured calvaria cell were observed on the scattered type of resorbable membrane, Conclusively, this study thus suggest that cultured bone cell inoculated onto the biodegradable barrier membrane may have an important role of carrier for many cell which could be used as new tissue regeneration, and those tissue engeering technique may become an new method in the approach to the repair of bone defects.

Background: Methylenetetrahydrofolate reductase (MTHFR) is involved in DNA synthesis and repair. We here aimed to investigate two common polymorphisms, C677T and A1298C, with genotype and haplotype frequencies in colorectal cancer (CRC) cases among Jordanian. Materials and Methods: 131 CRC cases were studied for MTHFR C677T and A1298C polymorphisms, compared to 117 controls taken from the general population, employing the PCR-RFLP technique. Results: We found the frequency of the three different genotypes of MTHFR C677T among Jordanians to be CC: 61.7%, CT: 35.2%, and TT 3.1% among CRC cases and 50.9%, 38.8% and 10.3% among controls. Carriers of the TT genotype were less likely to have CRC (OR=0.25; 95%CI: 0.076-0.811; p=0.021) as compared to those with the CC genotype. Genotype analysis of MTHFR A12987C revealed AA: 38.9%, AC: 45%, and CC 16% among CRC cases and 37.4%, 50.4% and 12.2% among controls. There was no significant association between genetic polymorphism at this site and CRC. Haplotype analysis of MTHFR polymorphism at the two loci showed differential distribution of the TA haplotype (677T-1298A) between cases and controls. The TA haplotype was associated with a decreased risk for colorectal cancer (OR=0.6; 95% CI: 0.4-0.9, p=0.03). Conclusions: The genetic polymorphism of MTHFR at 677 and the TA haplotype may modulate the risk for CRC development among the Jordanian population. Our findings may reflect an importance of genes involved in folate metabolism in cancer risk.

RAM of elements to support weapon systems is conducted at the initial development phase and standard is suggested to accomplish strategy requirement performance from a design spec. Operational availability is a key point of the military's ability to ensure combat readiness and to win the battle. In the weapon system development phase, operational availability is used as a development standard. The military provides ALDT, operation and standby time, which are elements of operational availability. ALDT is a key element of operational availability that must be maintained for combat readiness, as it depends on the aging of a weapon system, maintenance policies and geographical conditions. Operational Availability to be set at the development phase has many differences from the operational availability that is analyzed in the actual operational phase because ALDT is applied as a simple assumption. In the paper, we analyzed ALDT applying the decision tree method through failure data acquired from initial operation. Through this study, we have devised the optimal ALDT setting method to achieve operational availability about operation when the weapons system is unstable.
Purpose: In the finger, there are three major palmar arches in the arterial system. The location of this arches are constant. The middle and distal transverse arches are consistently large (almost 1 mm) and may be used for arterial vessel repairs either proximally or distally, depending on the length and direction needed. This paper describes our experiences in reconstruction and replantation of the finger using rerouting the transverse digital palmar arch. Methods: 31 patients with injuries according to our classification were treated from March of 2005 to October of 2008. In this study the authors subdivided injuries into those with amputation distal to the insertion of the flexor digitorum profundus (Class I, 31 fingers); those with amputation distal to the insertion of the flexor digitorum superficialis (Class II, 4 fingers). Replantation was performed using the artery-only technique with neither vein nor nerve repair. Because the artery has been damaged, it is still possible to make a direct suture by transposing the arterial arch in an inverted Y to I arterial configuration or converting the arch. Venous drainage was provided by an external bleeding method with partial nail excision, medical leech, and repaired margin. Results: The success rate was 87% (n=27) in class I and 75% (n=4) in class II. The authors conclude that crushing and complete avulsion injuries & amputations are salvageable, with acceptable functional results in select patients, especially those with amputation distal to the insertion of the flexor digitorum superficialis. Conclusion: We performed replantation and reconstruction with only-arterial transposing anastomosis successfully, resulting in good recovery of aesthetic and functional outcome. Three major digital palmar arches, especially distal two branches, give us additional treatment options. In the finger replantation and reconstructive techniques using rerouting healthy the transverse digital palmar arch increase the survival rate of the finger.
With an increasing awareness of the limitations of both mechanical prostheses and bioprostheses, aortic valvuloplasty has gained attention as an alternative procedure for aortic valve disease. Material and Method: Eight consecutive patients underwent aortic valvuloplasty caused by leaflet prolapse between June 1799 to June 2000. Mean age of the patients was 18.4$\pm$12.6 year. Four paitents(50%) were male. Six patients had tricuspid valves and ventricular septal defect and two patients had bicuspid valves. The extent of aortic insufficiency was 3.5$\pm$0.5 by preoperative Doppler echocardiography. The technique involved triangular resection of the free edge of the prolapsed leaflet, annular plication at the commissure, and resection of a raphe when present in bicuspid valves. Result: There was no in-hospital mortality or morbidity. Mean follow-up was complete at 11.973.6months. There was no late mortality or morbidity. The amount of the severity of aortic insufficiency, as assessed by echocardiography preoperatively, postoperatively and at late follow-up was 3.5$\pm$0.5, 0.6$\pm$0.5 and 0.8$\pm$0.6, respectively(p value : 0.01). There was one patient with grade 2/4 aortic insufficiency and in the other patients, grade 1/2 or trivial aortic insufficiency were detected with late echocardiograms. Conclusion: Triangular resection in the patients with aortic leaflet prolapse offers a good early clinical result, but long-term follow-up is necessary.
