  • Traumatic injuries of the inferior vena cava (IVC) are the most challenging lesions in abdominal vascular injuries and are associated with a high mortality rate. Although endovascular treatment has been addressed in the management of vascular trauma, surgery is the mainstay in the treatment for IVC injury as an endovascular technique for the venous system has not been developed. We report a case of successful surgical repair of an extensive IVC laceration following a fall.

  • For high-risk patients, endovascular aortic aneurysm repair (EVAR) is a good option but may lead to serious complications, which should be addressed immediately. A 75-year-old man with a history of abdominal surgery underwent EVAR for an aneurysm of the abdominal aorta and iliac arteries. During EVAR, iliac artery rupture and graft limb occlusion occurred, and they were successfully managed by the additional deployment of an iliac stent graft and balloon thrombectomy, respectively. We, herein, report a rare case of the simultaneous development of the two fatal complications treated by the endovascular technique.

A Simulation Study of Navy Drydocks (해군 건선거 모의실험 연구)

  • A simulation study was conducted to determine optimum capacity of Navy drydock facility using GASP-IV, an advanced FORTRAN-based simulation language, under demands of regular overhauls and emergency repairs by ships of an hypothetical fleet composition. Three year dock usage data was analyzed to produce probability distributions underlying drydock repair demands. The present facility size of two drydocks was simulated and was found to be somewhat short of adequate service capability, showing excessive average waiting time and average queue length. The simulation model was then modified to include an additional drydock of similar size as the other two and a year's simulation was again conducted. All repair needs were quite satisfactorily met and all docks showed very high utilization factor (0.98). This contributed to an increase in the fleet's ship availability from 0.95 to 0.99. This study illustrates the usefulness of simulation technique as a tool for analyzing policy alternatives in military long-term investment areas.

  • This research addresses study on thermal residual stress of a composite patch repair of the edge cracked aluminium panel of aging aircraft. Composite patch repair is an efficient and economical technique to improve the damage tolerance of cracked metallic structures. These are thermal residual stresses due to the mismatch of coefficient of thermal expansion, and these are affected by the curing cycle of patch specimen. In this study, three curing cycles were selected for F.E. analysis. This study features the effect on composite patch and aluminum by thermal residual stress during crack propagation in aluminum plate.

Repair of an oroantral communication by a human amniotic membrane: a novel technique

  • The amniotic membrane is the innermost layer of fetal membrane and is attached to the chorion in the placenta. This membrane has been used for nearly a century in varied fields such as ophthalmology, reconstructive surgery, and burn treatment. In this case report, we used a human amniotic membrane to repair an iatrogenic oroantral communication that occurred during the extraction of the patient's right upper second molar. A splint was given after the perforation was covered with human amniotic membrane and healing was clinically evaluated at various intervals. The outcome of the study revealed that the human amniotic membrane was an efficient graft material for repairing the defect caused by an iatrogenic oroantral communication following tooth extraction.

  • HVDC(High Voltage Direct Current) is an underwater cable between Jeju Island and Haenam in main land and supplies approximately 50% of electrical usage in Jeju Island. If there is any power failure due to HVDC, it will cost approximately 50,000 US dollars per day including Thermal Electrical Generation. Therefore it is absolutely necessary to recover the problem in rapid timely basis. In conclusion, evaluation methode of HVDC cable is needed urgently to upgrade current HVDC underwater cable repair technique in Korea to minimize the cost and time factors.

  • The selection of an appropriate surgical technique for repair of aneurysm of the ascending aortia with aortic insufficiency is unsettled. The etiology of the disease process has been the best indicator for the type of repair. Placement of a supracoronary graft[seperate graft and valve] is a compromise if the coronary ostia are displaced cephalad by the aneurysm, where as insertion of a valved conduit is difficult and unnecessary if the coronary ostia are normally placed. A 53 year old female patient underwent primary repaiar of proximal dissected layer and aortic valve replacement with 24mm carbomedics, The operative findings consisted of a supravalvular intimal tear, cicumferential dissection, dilated aortic annulus and normal position of coronary ostia. She is good physical activity now llmonths posoperatively.

  • Agent-based modeling (ABM), as a relatively new simulation technique, has recently gained in popularity in the civil engineering domain due to its uniquely advantageous features. Among many civil engineering applications, ABM has been applied to facility operation and management, such as energy consumption management, as well as the enhancement of maintenance and repair processes. The former studies used ABM to manage energy consumption through simulating human energy-related behaviors and their interactions with facilities, as well as electrical, heating, and cooling systems and appliances, while the latter used ABM to enhance maintenance process through facilitating coordination, negotiation, and decision making between facility managers, service providers, and repair workers. The present study aims to provide a short qualitative review on the most recent applications of ABM in the above-mentioned areas. Based on the review and follow-up analysis, the study identifies the advantages, disadvantages, and limitations of ABM applications to facility operation and management, and offers several potential future research topics in the hope of filling the existing literature gaps.

  • Bilateral transverse thoracosternotomy, or "clamshell" thoracotomy, can be complicated by dehiscence. A 65-year-old male underwent lung transplantation via clamshell thoracotomy, with subsequent sternal dehiscence on postoperative day 11. Upon repair, the previous sternal wires had pulled through, so a Sternal Talon connected to a Recon Talon was utilized to re-approximate the inferior sternum. On follow-up at 3 months, the patient recovered well. Use of the Sternal Talon provides an effective technique for repairing transverse sternal dehiscence.

Outcomes of Primary Unilateral Cheiloplasty in Same-Day Surgical Settings

  • Background Financial, clinical, and psychological considerations have made same-day surgery an attractive option for a variety of procedures. This article aimed to analyse the postoperative results of same-day primary unilateral cleft nasolabial repair. Methods This study was performed from 2011 to 2014. Unilateral cleft lip patients fulfilling the inclusion criteria were preoperatively classified as mild, moderate, and severe. All patients underwent same-day surgery and were discharged after satisfying the appropriate clinical criteria, receiving thorough counselling, and the establishment of a means of communication by phone. Postoperative outcomes were assessed and stratified according to preoperative severity and the type of repair. Results A total of 423 primary unilateral cleft lip patients were included. Fisher's anatomical subunit approximation technique was the most common procedure, followed by Noordhoff's technique. The postoperative outcome was good in 89.8% of cases, fair in 9.9% of cases, and poor in 0.2% of cases. The complication rate was 1.18% (n=5), and no instances of mortality were observed. The average hospital stay was 7.5 hours, leading to a cost reduction of 19% in comparison with patients who stayed overnight for observation. Conclusions Mild unilateral cleft lip was the most common deformity for which Fisher's anatomical subunit approximation technique was performed in most of the cases, with satisfactory postoperative outcomes. Refinements in the cleft rhinoplasty techniques over the course of the study improved the results regarding cleft nasal symmetry. Single-day primary unilateral cleft cheiloplasty was found to be a cost-effective procedure that did not pose an additional risk of complications.
