Resorbable blasting media 및 산처리한 임플란트의 제거회전력에 생리식염수를 적시는 것이 미치는 영향 (On the effect of saline immersion to the removal torque for resorbable blasting media and acid treated implants)

  • 목적: Resorbable blasting media (RBM) 표면처리 후 산처리한 티타늄 임플란트와 동일한 표면처리 후 생리식염수에 적신 티타늄 임플란트에서 생리식염수에 적신 경우 초기 골유착에 미치는 영향을 제거회전력 및 표면분석을 통해서 알아보고자 하였다. 연구 재료 및 방법: 대조군은 RBM 표면처리 후 산처리된 임플란트(RBM + HCl)이고, 실험군은 대조군과 동일한 표면처리 후 생리식염수에 2주간 적신 임플란트(RBM + HCl + Sal)이다. 토끼 10마리의 좌우측 경골에 각각 대조군 및 실험군 임플란트를 식립하고, 동시에 식립회전력(ITQ)을 측정하였다. 10일 후 임플란트 식립부위를 노출시켜 제거회전력(RTQ)을 측정하였다. 실험에 사용된 임플란트 시편의 표면분석을 위해 field emission-scanning electron microscopy (FESEM), energy dispersive X-ray Spectroscopy (EDS), 표면거칠기 측정 및 Raman 분광분석을 시행하였다. 결과: RBM 표면처리 및 산처리하여 생리식염수에 적신 티타늄 임플란트에서 대조군에 비해 높은 제거회전력을 나타냈으며, 통계적으로 유의미한 값을 보였다(P = 0.014 < 0.05). 표면거칠기는 실험군에서 더 높은 거칠기를 나타냈다. 결론: 티타늄 임플란트에 RBM 표면처리 및 산처리 후 생리식염수에 적신 것이 생리식염수를 적시지 않은 것 보다 초기 골유착을 향상시키는 것으로 생각된다.

A Study of mechanical properties of oxide layer removed Co-Cr-Mo abutments

  • PURPOSE: The aim of this study was to evaluate the influence of the oxide layer removal process in the Co-Cr-Mo (CCM) abutment after casting procedure on the prosthesis settlement and screw stability. MATERIALS AND METHODS: CCM abutments of four different interface conditions (CCM-M; machined, CCM-O; oxide layer formed, CCM-B; blasted, CCM-P; polished after blasted) and gold abutment (Gold-C; Cast with type III Gold alloy) were used. The initial settling values of abutments were evaluated according to the difference of implant-abutment length when the tightening torques were applied at 5 Ncm and 30 Ncm, and the settling values of abutments caused by loading were evaluated according to the difference of implant-abutment length before and after loading with 250 N, 100000 cycle. The loss ratios of removal torque for abutment screws were evaluated according to the difference in value of removal torques under 30 Ncm tightening torque applied before and after cyclic loading. RESULTS: The CCM-P and CCM-B group showed a higher initial settling value compared with the Gold-C group (P<.05), while the Gold-C group showed the highest settling values caused by loading (P<.05) and no significant differences were observed for between CCM groups (P>.05). The loss ratio of removal torque values for the CCM-B, CCM-P groups did not differ significantly from that of the Gold-C group (P>.05). CONCLUSION: Even though the oxide layer was removed by different methods, CCM abutment with internal conical connection structure showed lower abutment settling and similar screw loosening after cyclic loading compared with gold abutment.

Correlation between microleakage and screw loosening at implant-abutment connection

  • PURPOSE. This study aimed to evaluate the correlation between microleakage and screw loosening at different types of implant-abutment connections and/or geometries measuring the torque values before and after the leakage tests. MATERIALS AND METHODS. Three different abutment types (Intenal hex titanium, internal hex zirconium, morse tapered titanium) with different geometries were connected to its own implant fixture. All the abutments were tightened with a standard torque value then the composition was connected to the modified fluid filtration system. After the measurements of leakage removal torque values were re-measured. Kruskal-wallis test was performed for non-parametric and one-way ANOVA was performed for parametric data. The correlation was evaluated using Spearman Correlation Test (${\alpha}=0.05$). RESULTS. Significantly higher microleakage was found at the connection of implant-internal hex zirconium abutment. Observed mean torque value loss was also significantly higher than other connection geometries. Spearman tests revealed a significant correlation between microleakage and screw loosening. CONCLUSION. Microleakage may provoke screw loosening. Removing torque values rationally decrease with the increase of microleakage.

교정력의 즉시 부하가 미니스크류의 제거 회전력에 미치는 영향의 평가 (Influence of immediate loading on the removal torque value of mini-screws)

  • 교정치료에서 고정원의 조절은 교정 장치의 선택과 적용에서 가장 중요한 요소 중 하나이다. 이 연구의 목적은 교정용 미니스크류의 식립 즉시 교정력을 적용한 군과 교정력을 적용하지 않은 군의 제거회전력 측정과 조직학적 분석을 통해 즉시 교정력을 가한 미니스크류의 고정원으로서의 안정성을 확인하여 식립 후 교정력을 적용 수 있는 시기의 선택에 도움을 주고자 하는 것이다. 외경 1.6 mm. 길이 8 mm인 총 40개의 타이타늄 미니스크류를 10마리 가토의 경골에 식립하였다. 20개의 미니스크류 실험군은 식립 직후부터 6주 동안 지속적인 200g의 교정력을 적용하였고 나머지 20개의 미니스크류 대조군은 교정력을 적용하지 않았다. 제거 회전력은 디지털 토크 게이지를 이용하여 측정하였으며 t-test를 이용하여 통계처리 하였고, 조직표본은 미니스크류를 제거한 후 H-E 염색하여 조직 소견을 관찰하였다. 제거회전력은 실험군과 대조군에서 통계학적으로 유의성 있는 차이를 보이지 않았으며, 조직학적으로 두 군간의 차이는 없었다. 본 연구의 결과는 교정용 미니스크류에 즉시 교정력을 가하더라도 안정적인 고정원으로 활용 할 수 있다는 점을 제시한다.

초기 골 접촉이 없는 순수 티타늄 임프란트 주위 골의 치유반응 (Healing of the bone around pure titanium implants without primary bone contact)

  • Primary fixation is one of the most important factor in establishing adequate osseointegration between implant and bone. To evaluate the initial healing response of bone around implants without primary bone contact, this study was designed to create considerable space between implant and bone in 5 mongrel dogs, about 1-year old. After 3 holes of 6.0mm in diameter were prepared at the femur neck of the dogs, commercially pure titanium thread type implants(STERI-$OSS^{(R)}$), 8mm in length and 3.8mm, 5.0mm and 6.0mm in diameter, were inserted. Implants were supported by only nonresorbable membrane($Teflon^{(R)}$), and the penetration of upper soft tissue into the gap was inhibited by it. The each implant was positioned in the center of the drilled hole. 9 implants with different diameters were inserted in 3 dogs for histologic observation, and 12 were inserted in 2 dogs for mobility test and removal torque test.Fluorescent dyes were injected in order of Doxycycline, Alizarin Red S, and Calcein at intervals of 2 weeks. At 4-, 8-, and 12-week after placement, 3 dogs were sacrificed for histologic observation, and at 8- and 12-week after placement, 2 dogs were sacrificed for mobility test using $Periotest^{(R)}$ (Simens AG, Bensheim, Germany) and torque test using Autograph AGS-1000D $series^{(R)}$(Japan). The result were as follows: 1. The wider the gap between bone and implant was, the less bone maturity was, and the later osseointegration was occurred. Trabecular direction of new bone around implant was changed from parallel to perpendicular to the implant, and the gap was filled with new bone, over time. 2. There was a decreasing tendency over time in the mobility of all implants, but the wider gap between bone and implant was, the smaller decrease of the mobility was. 3. There was a increasing tendency over time in the removal torque gauge of all implants, and the wider gap was, the smaller increase of the removal torque gauge was. The results suggest that osseointegration in case of implant without primary bone contact may be obtained by guided bone regeneration technique with prolonged healing period, but the time of second surgery should be considered carefully.
