The Fabrication of Accurate Removable Partial Denture by Analyzing the Failed Cases (실패 증례 분석을 통한 국소의치 제작 시 주의사항)

  • Removable partial denture for oral function recovery of partial edentulous patients is a complicated treatment because the dentist should consider both hard tissue and soft tissue. Removable partial denture produced without consideration of precise design for each component and rotation of denture may cause fracture, deformation or loss of abutment teeth. In case of failure to achieve a proper denture, the reason of failure should be analyzed and the result must be applied to later RPD cases. Finding the solutions through the failed denture cases will help patients use their dentures more comfortably and keep their sound residual teeth and tissue longer.

  • When planning oral rehabilitation for maxillary edentulous patients, fixed prosthetic restoration using implants, complete denture restoration or overdentures using implants can be considered as treatment methods. In the case of complete denture restoration, it does not require additional surgery and is relatively economical. In the case of implant-supported fixed prostheses, the functional part is generally superior to that of complete denture restoration, but there are cases in which implant placement is clinically difficult. Recently in consideration of the patient's needs and the condition of the remaining alveolar bone, after partial implant placement, a method of restoring with a removable partial denture using implant-supported surveyed crown is also being attempted. This case is a case of performing a removable partial denture restoration using implant-supported surveyed crown in the anterior maxilla, and showing satisfactory esthetic and functional results.

  • When making conventional removable partial denture for the remaining teeth where the remaining teeth are only on one side, rotation of the denture occurs -in function- on the axis of the connected remaining teeth. If the edentulous portion is long, it becomes harder to obtain retention and stability for the abutment as the importance of the mucous membrane support is elevated. Such movements of denture decrease denture retention and stability, put excessive stress on the abutment, and give bad influence on periodontal health. Therefore, additional implant placement can be of a good choice in gaining additional retention and stability for partial denture. Thus hereby we report this clinical case as successful results were obtained by placing implants symmetrical to the remaining teeth and putting implant-supported surveyed crowns on the implants, allowing the partial denture to be designed to accept varying components and to have suitable path of insertion.

  • Although not all kinds of deep bite occlusions are considered pathological states, they can cause persistent wear and eruption of the anterior mandibular teeth if the anterior occlusal contact is unstable or accompanied by an occlusal disharmony. The patient was a 72-year-old man with no occlusal contact of the remaining anterior teeth in the centric relation position because of a skeletal class II malocclusion and loss of occlusal support. If the vertical dimension needs to be increased for prosthetic restoration in a patient with a deep bite, the amount of vertical dimension increase must be determined through precise diagnosis. This is because the overjet may increase and cause an unstable occlusion. In this review, we report the case of a patient with a stable occlusion and good treatment results achieved by using a removable partial denture prosthesis and applying the Dahl concept.

  • Surgical management of oral cancer results in compromised masticatory and swallowing function which affects patient in social and psychological aspects due to reduced phonetic ability and facial deformity, thus, it is imperative to provide applicable prosthetic treatment to overcome such complications. This clinical study describes rehabilitation of a patient with squamous cell carcinoma treated with marginal mandibulectomy and implantation on preserved posterior portion of mandible to provide stability and support for subsequent denture treatment. Kennedy class IV removable partial denture has provided satisfactory results in esthetics and function. Bone level stability around implants was reported to be maintained during eight months of clinical observation.

Subtractive versus additive indirect manufacturing techniques of digitally designed partial dentures

  • PURPOSE. The purpose of this in vitro study was to evaluate the accuracy of digitally designed removable partial denture (RPD) frameworks, constructed by additive and subtractive methods castable resin patterns, using comparative 3D analysis. MATERIALS AND METHODS. A Kennedy class III mod. 1 educational maxillary model was used in this study. The cast was scanned after modification, and a removable partial denture framework was digitally designed. Twelve frameworks were constructed. Two groups were defined: Group A: six frameworks were milled with castable resin, then casted by the lost wax technique into Co-Cr frameworks; Group B: six frameworks were printed with castable resin, then casted by the lost wax technique into Co-Cr frameworks. Comparative 3D analysis was used to measure the accuracy of the fabricated frameworks using Geomagic Control X software. Student's t-test was used for comparing data. P value ≤ .05 was considered statistically significant. RESULTS. Regarding the accuracy of the occlusal rests, group A (milled) (0.1417 ± 0.0224) showed significantly higher accuracy than group B (printed) (0.02347 ± 0.0221). The same results were found regarding the 3D comparison of the overall accuracy, in which group A (0.1501 ± 0.0205) was significantly more accurate than group B (0.179 ± 0.0137). CONCLUSION. In indirect fabrication techniques, subtractive manufacturing yields more accurate RPDs than additive manufacturing.

A clinical case of hybrid telescopic double crown using friction pin with an isolated few remaining teeth (고립된 소수 잔존치 환자에서 프릭션핀을 이용한 하이브리드 텔레스코픽 이중관 국소의치의 임상증례)

  • A few authors have reported good clinical results using double crown removable partial denture (RPD) with a few remaining teeth. Hybrid telescopic double crown is a good indication for a patient with poor periodontal condition and/or few remaining teeth after extraction, especially located in cross-arch position. In this case, there was a poor periodontal condition with teeth mobility. Several teeth with poor prognosis were extracted. Remaining anterior teeth was restored with fixed prostheses and edentulism was restored with Kennedy class I removable partial denture in maxilla. In mandible, it was hard to restore with clasp removable partial prostheses because of bilaterally isolated remaining teeth so that hybrid double crown removable partial denture with friction pin was suitable for this case. The objective of this report is to discuss the characteristics and the utility of hybrid double crown prostheses using a few remaining teeth in mandible.

  • A rationale and compendium of treatment for a specific situation of cases have been presented. It consists of a cast metal bar called a truss bar joint removable denture which is soldered to crowns and dowel core on a few remaining teeth, i.e., cuspids or bicuspids. This system gives additional support and retention to the individual teeth and makes possible a more stable removable denture. It is my opinion that the prognosis of the chosen cases may be obtained Successful results when applied on the teeth of less favorable crown-root ratio.

  • Treatment options for partially edentulous patients are fixed partial denture, removable partial denture and implant supported fixed partial denture. In case of a patient with a few remaining teeth, removable partial denture and implant supported fixed prosthesis are available. For implant fixed prothesis, enough implant fixtures are required and the patient's general condition, local factors and economic status must be considered. When the condition of the abutments and the residual ridge is favorable and the prosthesis is well designed, removable partial denture can be an option. In removable partial denture, the bilateral support is important. If the teeth remain unilateral, harmful stress is put on the abutments by the fulcrum line. In this situation, strategic implantation and implant-retained or assisted removable partial denture is beneficial to the retention and support of the denture. And this can be cost-effective, functional and esthetic choice of treatment. This article describes the prosthodontic rehabilitation of Maxillary Kennedy class I partially edentulous patients. In these two cases, the patients had a small number of teeth and they were restored by the combination of a removable partial denture and dental implants.

Clinical outcomes of rigid and non-rigid telescopic double-crown-retained removable dental prostheses: An analytical review

  • PURPOSE. The objective of this literature review was to analyze the cumulative survival rates (CSRs) of rigid and non-rigid double-crown-retained removable dental prostheses. MATERIALS AND METHODS. Screening of the literature published from January 1995 to December 2019 was performed by using electronic data base (Pubmed) and manual search. The CSRs of rigid and non-rigid double crown removable dental prostheses were investigated. RESULTS. A total of 403 articles were reviewed and 56 relevant articles of them were selected. Subsequently, 25 articles were included for data extraction. These articles were classified according to rigid and non-rigid type double crowns and further subdivided into teeth, implants, and teeth-implant combination types. The CSRs of rigid type double crown ranged from 68.9% to 95.1% of 5 to 10 years in tooth abutments, 94.02% to 100% over a 3-year mean observation periods in implant abutments, and 81.8% to 97.6% in tooth-implant combination. Non-rigid type double crowns had various CSR ranges from 34% to 94% maximum during 10 years observation in teeth abutment. The CSRs of non-rigid type had over 98% in implant abutments, and ranged from 85% to 100% in tooth-implant combination. CONCLUSION. The CSRs of double crowns varies according to types. With accurate evaluation of the remaining teeth and plan of the strategic implant placement, it could be successful treatment alternatives for partially or completely edentulous patients.
